I am reporting a safety issue involving my 2025 Tesla Model 3 Performance. My steering wheel began shaking while driving, and the vehicle felt unstable, especially at speed. I brought the vehicle to Tesla Service in Las Vegas. Their inspection found that the outer tie rod nut on the passenger front side was loose, which created steering play and caused the steering wheel shake (documented in Tesla invoice #XXX). A loose tie rod nut is a serious steering safety defect that can result in loss of steering control. Tesla performed a four-wheel alignment and tightened the components but stated the issue is “maintenance” and not covered under warranty, despite the vehicle having only 12,890 miles. Tesla also stated the steering rack had “no issues,” but they did not perform any repair on the steering system beyond tightening the loose tie rod nut and charging me for alignment. They also did not explain how a nearly new vehicle could develop a loose steering component. A loose outer tie rod nut should never occur on a new car unless there is a defect or improper assembly from the factory. This poses a significant safety hazard, especially at highway speeds. Tesla did not treat this as a steering safety defect and only attributed it to alignment. They declined warranty coverage and did not fully address the root cause. I am filing this complaint because steering components should not loosen on a 2025 model vehicle, and the situation could have resulted in loss of control or an accident. I want NHTSA to be aware in case this issue appears in other vehicles of the same model year.

The forward-facing camera on my vehicle repeatedly fogs up inside the sealed camera housing, creating a light to severe haze across the camera lens. This internal fogging obstructs the camera’s view and affects the vehicle’s safety systems that rely on it, including forward collision warning, automatic emergency braking, lane keeping, and adaptive cruise control. The components are available for inspection upon request. This obstruction puts my safety at risk because these systems either disable themselves or operate unpredictably when the camera view is blocked. The issue occurred again this year under normal outdoor temperatures, and the fogging cannot be cleared by the driver or by using defrost or cabin heat. The problem has been confirmed previously by a Tesla service center, where technicians inspected the camera area and told me my vehicle had one of the worst cases they had seen and told me this is a common issue. They attempted a repair, but the issue has returned, making it a recurring defect. No police or insurance representatives have inspected it. During the first occurrence, the vehicle displayed messages indicating that features like Autopilot, cruise control, and lane assist were disabled due to camera obstruction. The current recurrence produced visible internal haze over the camera lens and a warning message appeared which I do have that Tesla acknowledges this is an issue and that rather than fixing it, you can bring your car in for expedited cleaning. I have read others stating Tesla changed their windshield design and it doesn't heat up this area properly any longer. This issue has happened in multiple winter seasons and directly affects the proper operation of safety-critical driver assistance systems.

I am the owner of a 2021 Tesla Model 3 and am filing this complaint due to a significant safety concern: my vehicle does not have manual or mechanical releases for the rear doors; only electronic mechanisms are present. In the event of a total power failure, accident, fire, or submersion, rear seat passengers would be unable to exit the vehicle through the rear doors, as there is no physical lever or alternative escape route directly available inside the rear seats. This design poses a serious risk to occupant safety in emergencies, especially for families with children or passengers unable to access the front seat manual releases or trunk escape features. The lack of a rear mechanical release is inconsistent with standard automotive safety practices and could lead to injury or loss of life if electronic door access is disabled. It’s important to note that newer Tesla models now include manual rear door release mechanisms, demonstrating that Tesla recognizes the safety benefit and necessity of this feature. Owners of earlier models, like mine, are left without this critical safety upgrade, and I believe this should be addressed for all affected vehicles. I strongly urge NHTSA to investigate this issue and consider whether a recall or mandatory modification is warranted for the affected Tesla Model 3 vehicles. Tesla full self driving software malfunctioned and turned into oncoming traffic lanes instead of crossing over to the correct opposite side of the roadway. This occurred after getting off a on ramp in Ukiah, California around 2am Nov 15, 2025. This could have put others in serious danger if there were cars coming in the oncoming lanes heading in the different direction. The problem has not been reproduced by a dealer or other entity. Yet This has happened to other tesla drivers as well using FSD as you are probably already aware. No inspection and it was sudden and unexpected.

The Tesla FSD performed in a dangerous way. It came into a left lane when a speeding truck was in the lane and nearly hit my car. This occurred on Nov 12 around 5:30 pm near floor and decor at Kolb and Broadway Intersection in Tucson AZ. I engaged FSD to return home in Oro Valley. First the car did not follow straight path going from the parking store lot to make a right trun and enter Kolb road. It made right turn and left turn and then right to Kolb road to turn right. It was a busy time in the evening. When an opening was found it entered Kolb road and then rapidly zig zagged to change lanes to try to enter left turn lane only on Kolb to turn on Broadway. In the left most lane, it did not detect a fast approaching truck. The truck honked, and I took control of the car and sped through the intersection to avoid collision.

Compliance arm bolt securing it to the subframe is fractured. Tesla has dismissed my ongoing concerns of vibration and instability while driving. Stating they performed an in depth inspection of all suspension and subframe components and found nothing of concern during last service appointment. I did my own inspection and found the fractured bolt. Sent photo to service and they are trying to play it off as normal and safe to drive. Contacted several manufacturers and mechanics stating otherwise. Told this will end in catastrophic failure and loss of vehicle control but apparently Tesla wants me to drive it to the service department for next service

While driving home from purchasing my new tesla a under dash panel just to the left of the steering column detached an fell on top of the brake pedal. This made it impossible to apply the brakes. Fortunately because of regenerative braking i was able to stop the car and replace the panel. The panel is only held in place with 2 magnets. I find this extremely dangerous and could have easily caused a life ending accident.

While driving with FSD engaged and set to standard profile, FSD accelerated to 76mph in a 55 zone. This is on software version 14.1.4. Unfortunately this new update no longer allows you to set your speed while using FSD, you can only select sloth, chill, standard, hurry, or mad max.

The contact owns a 2025 Tesla Model 3. The contact stated that the vehicle was vibrating while driving at 60 MPH, and the vibration was going to the contact's head. There were no warning lights illuminated. The vehicle was taken to the local Tesla service center, where it was diagnosed with needing the four tires balanced, and the passenger’s side rear tire was replaced. The vehicle was repaired, but the failure recurred. The vehicle was taken to another local Tesla Service Center at 9950 NW 25th St, Doral, FL 33172) on four occasions. The dealer determined that the tires were unbalanced. The vehicle was repaired, but the failure recurred. The vehicle was taken back to the Tesla Service Center, where the suspension torque was adjusted. The tires were rebalanced, but the failure persisted. The vehicle was taken to the Service Center, and it was determined that the failure was related to the front suspension. The vehicle was taken to the same Service Center, where it was diagnosed and determined that the steering column was the cause of the failure. The electric steering column and steering wheel were replaced. The manufacturer was made aware of the failure. The failure mileage was approximately 8,434.

I have been very cautious on charging my Tesla Model 3 and have followed all of the manufacturer's recommendations. The HV battery recently failed at 105,050 miles. South Korea has noted a battery reliability issue for this year car and model, so I am reporting this to you so if a recall is needed, you can track this. I believe my battery is a Panasonic NCA.

The suspension of the car is making sounds, Tesla wants me to pay labor charges for fixing it under warranty. During a head-on collision on [XXX] in Georgia, the electrical system in my 2022 Tesla Model 3 lost power and the electric door releases stopped functioning. The vehicle interior caught fire and I was trapped inside. I was unaware of the location of the hidden mechanical emergency door release because it is not visibly labeled, not explained upon delivery, and not intuitive in an emergency. I was forced to climb to the back seat and break the rear passenger window with my legs to escape while the interior was burning. I suffered a broken hip, broken arm, and later required a full hip replacement. The mechanical emergency door release was concealed and unlabeled, resulting in entrapment during a fire, which is a violation of FMVSS 206 and presents a continuing and life-threatening safety hazard to all drivers and passengers. Tesla has not provided warnings, recall guidance, labeling, or owner education regarding how to exit the vehicle during a power-loss emergency. This defect can result in deaths. I was driving on a straight interstate road between 60-70 mph using Full-Self Driving around 4:15pm and there was no traffic in front, behind, or next to me. Suddenly, the car tried to make a sharp right turn that felt like more than an obstacle avoidant move or lane keeping move. I had to grab the wheel even harder than I was (my hands were fortunately on the wheel) and force it back into the lane. If i did not make this intervention, I would've gone off the road and crashed.

Shortly after purchasing my 2022 Tesla Model 3 RWD in February 2025, I began noticing a persistent musty, mold-like odor coming from the HVAC vents whenever the air conditioning or defrost was on. The odor worsens when driving with the windows down. I reported this issue to Tesla multiple times between May and October 2025. Tesla replaced the cabin filters but refused to perform a full evaporator or HVAC disinfection, stating the odor was “normal.” Despite the filter changes, the smell always returns within a few weeks. Between these visits, I also tried to resolve the issue myself by replacing filters, using odor-eliminating products, and placing moisture absorbers inside the vehicle, but nothing has resolved the problem. This ongoing issue has led to repeated respiratory and eye irritation, including a documented pneumonia case in April 2025. The symptoms clearly worsen after time spent in the car. Tesla has not offered any permanent fix, warranty repair, or replacement. The problem appears to stem from mold or microbial buildup within the HVAC evaporator and duct system, which could affect air quality and occupant health. This raises safety concerns regarding possible HVAC design flaws or moisture retention issues that may affect other Tesla vehicles. I was in process of completing a right hand turn, likely doing around 30 - 35 mph, when the vehicle suddenly and significantly applied the brakes, slowing the vehicle almost instantly down to around 12 mph, from what I had noticed. The road was entirely clear of any other vehicles, hazards, pedestrians or cyclists, with exception of a few parked cars along the curbs. There was absolutely NO reason for this abrupt braking maneuver, and it is at least the second time it has occurred within the past 11 months, both on a right hand turn. Today's incident jerked my head and neck abruptly, leaving me with a headache. It has undermined my confidence in the vehicle.

In early 2025, while reversing the vehicle, the backup camera proximity alert system failed to provide audible warnings of an obstacle. The visual display worked, but the system provided no beeping or audible alerts as I reversed toward a pole in the vehicle's blind spot, even though I was within the range where warnings should have been activated resulting in over $4K in damages. The same alert failure happened on 10/23/25 as I backed out of a parking space. I no longer live in the local area, but reported the safety issue in person during a visit on August 27, 2025. The acting service manager told me to expect a response in 10-14 days. After not hearing back, I followed up via the app on September 28, 2025. When I received a response on October 2, 2025, I was told my request is beyond the timeframe of when this could be inspected (30 days). The vehicle is covered under Tesla's 4-year/50,000-mile New Vehicle Limited Warranty, which covers defects in materials and workmanship. A failed backup camera system is precisely such a defect. California Song-Beverly Act: California's lemon law (Civ. Code § 1790 et seq.) requires manufacturers to repair defects covered by warranty. There is no 30-day reporting requirement under CA law. Federal Safety Standards: The backup camera is required safety equipment under 49 CFR 571.111. A malfunctioning backup camera renders the vehicle non-compliant with federal safety standards. Presumption of Manufacturing Defect: Under California law, defects occurring within 18 months/18,000 miles are presumed to be manufacturing defects (Civ. Code § 1793.2(d)(2)). This accident was well within that window. When I reported this defect to Tesla, they initially refused to: Conduct a safety inspection of the proximity alert/parking sensor system Retrieve diagnostic data or video footage to verify the system failure Test or repair the defective equipment under warranty As of 10/30/25, the local dealer agreed to a vehicle inspection
