The vehicle's FSD system malfunctioned and initiated an unintended reverse toward a closing garage door. To prevent a total-loss event (the door crushing the glass roof and trunk), I was forced to perform an emergency manual takeover. Due to the vehicle's position caused by the malfunction, I had to maneuver through a confined space, resulting in the side-scrape. I am filing this as a malfunction claim because the damage was a direct consequence of the car's autonomous failure

HVAC system went out at 7500ish miles. It was repaired under warranty, and a "fix" was administered through software by tesla. My HVAC has gone out again at 84k miles FOR THE SAME REASON. about 77k miles after it was just repaired. Quick check online. this is still a very common issue. No heat creates a significant safety hazard due to lack of the ability to defrost the vehicle or frost building up on cold days.

There is NO mechanism to unlock the rear passenger doors in case of electrical failure. No Emergency mechanical door opening in case of emergency. Passengers will NOT be able to get out of the LOCKED vehicle during an emergency (i.e. crash, battery fire, submerged in water, etc). This is an EXTREMELY IMMINENT DANGER for small kids who are sitting in car seats, since it will take even longer to release them from their straps in order to get out. The small rear windows are useless for escape. THIS FLAW DESIGN HAS ALREADY CAUSED DEATHS. Changes SHOULD BE MADE to enable emergence mechanical opening of the rear doors.

The entire bumper assembly fell off my Tesla Model 3 during the heavy rains on December 24. I didn't see it happen. It was just missing. But it may have happened on the freeway, when we drive through a flooded area. There is a history of this happening with this model when there is a lot of rain or you drive through a puddle. They admitted to this defect in 2020, but claim it was fixed. Apparently not. They are now telling me "we are unable to cover the repair under vehicle warranty because the condition was caused by outside factors," i.e., water. This cannot be normal.

Incident Description (in my own words): This incident occurred in the parking lot of the [XXX] in Bretton Woods, New Hampshire. It was evening after sunset. The parking lot was dark but illuminated by overhead parking lights. My Tesla Model 3 headlights and vehicle lights were fully on. I was reversing slowly out of a tight parking space, traveling approximately 2–3 miles per hour. I was intentionally moving cautiously. My attention at the time was focused on the front right corner of the vehicle, as clearance there was tight. While reversing, the rear of my vehicle struck a parked truck behind me. The truck was relatively high (its headlights were positioned high on the vehicle). Neither I nor my passenger heard any rear collision warning, cross-traffic warning, or parking alert prior to impact. The vehicle also did not apply automatic braking. The contact occurred at very low speed and resulted in only minor cosmetic damage (a small dimple/deformation), but I was surprised that no warning or braking intervention occurred. Based on prior experience driving this vehicle, the collision and parking warning systems are typically configured to warn early, and I had confidence that a warning or automatic braking would activate in this scenario. Following the incident, I conducted a simple test by placing cones in front of the vehicle to see whether forward collision warnings would activate. They did not. While I understand cones may not trigger frontal collision alerts, this further contributed to my concern about object detection reliability in low-speed scenarios. The vehicle’s cameras were clean and unobstructed. I regularly clean the camera lenses. I am filing this complaint because my confidence in the vehicle’s collision warning and automatic braking systems has been shaken. To Whom It May Concern, I am submitting this complaint to report a serious and potentially dangerous safety defect affecting my 2021 Tesla Model 3 Performance. After exposure to heavy rain or wet conditions, the vehicle’s horn completely stops functioning. The failure occurs regardless of the activation method—neither pressing the steering wheel horn nor using the horn via the mobile app produces any sound. The horn remains nonfunctional even though the vehicle is otherwise operable. This issue has occurred after the vehicle becomes wet due to rain and appears to be related to moisture intrusion or an electrical/design flaw. Once the failure occurs, the driver fully loses the ability to use the horn for warnings or emergency signaling. This is a critical safety concern. The horn is a federally required safety device and is essential for alerting pedestrians, cyclists, and other drivers to avoid collisions. Losing horn functionality significantly increases the risk of accidents, especially in urban driving, highway merging, and emergency situations where immediate warning signals are necessary. This issue is not isolated to my vehicle. Numerous Tesla owners have reported the same horn failure after rain or wet conditions on online forums and owner communities, indicating a widespread and systemic defect rather than isolated wear or misuse. Given the safety implications and the volume of similar reports, I respectfully request that NHTSA investigate this issue to determine whether a design or manufacturing defect exists and whether corrective action or a recall is warranted. In addition, the contact stated that there was no evidence of impact on the front windshield. The contact stated that the failure occurred while the vehicle was parked in an underground parking garage. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure, but no assistance was provided. The failure mileage was approximately 4,050.

The blinkers do not respond to the change in blinkers when I initiate the blinker (right or left) by using the stalk. There has been numerous times that I fully pull down or push up the blinker stall and it would not engage the blinker but today I am reporting it because now I almost changed the lane after engaging the blinker but it didn’t happen

Lately I keep getting the following concerning message on the dashboard of almost brand new 2025 Tesla 3 car, even after reset the screen : Steering Assist Reduced Steering may require Increased Effort See the attached picture of this message, I took this photo On Dec 25, 2025 at 12:56 pm with parked car. I am pretty concerned with potential safety issue with the important steering-function while driving on highway and other roads. The car has only 804 miles as of Dec 26, 2025. The new car was picked up on May 31, 2025 with 15 miles. I have the FSD on monthly subscription. I have sent a message to Tesla. It has provided me with a service appointment at its West Bloomfield Township Center in Michigan on Jan 26, 2026 at 10:30 am. At the time of setting the appointment an estimate of about $450 was provided. It is concerning since the new car is under warranty. I am submitting this concerning safety related issue to NHTSA - to please investigate.

While driving, the center display intermittently goes completely black. When this occurs, I lose access to the speedometer and all driver information. The issue has been ongoing and progressively worsening. The vehicle was purchased new. The issue was reported during the warranty period and initially attributed to software updates. A recent service visit confirmed the vehicle computer is failing, and the issue immediately recurred after pickup. Loss of speed and driver information while driving creates a safety concern.

Because the system repeatedly re-enables itself, the driver must divert attention from driving to manage wiper behavior, often multiple times during a single trip. This is especially hazardous when using standard cruise control without Autopilot, where full driver attention is required.

After Tesla replaced my high voltage battery in May of 2024 all of the following started to happen. Prior to the replacement none of these issues were present. While driving under normal conditions, my 2021 Tesla suddenly and violently steered into an oncoming lane or toward the roadside without any driver input. The steering wheel physically turned on its own with force. Full Self-Driving was NOT engaged at the time of the incident. This was not lane assist correction and occurred without warning. Importantly, the vehicle did NOT issue a warning at the time of the steering event. A “Take Control Immediately” message appeared several minutes AFTER the incident, not before or during, indicating a delayed or failed system response. This defect began immediately after a High-Voltage Battery replacement performed by Tesla Service in May 2024 and has occurred multiple times since. The incident presents a serious safety risk, including loss of vehicle control, roadway departure, or head-on collision. In addition to the steering defect, the vehicle began exhibiting multiple electronic and control anomalies immediately after the High-Voltage Battery replacement. These include navigation/map screen failures, windows adjusting themselves while driving, random “cannot shift into drive” warnings, system failure messages, camera malfunctions, and unexpected screen reboots. These issues did not exist prior to the battery replacement and support the presence of an underlying electrical or control system instability related to the steering defect.

This is a new one but not new for me as this vehicle was the first under my own registration/not rental. Had passenger in car and with music down the passenger heard a very clear odd word which I can only assume was its desire, we declined. This is the umpteenth of many odd and offensive remarks that can cause odd distortion to within the immersive sound system and was what I previously attributed to rental vehicle blue tooths or dysfunctional family raising. Its offensive, distracting, and sometime low-level so unhealthy. Im awaiting the response from tesla but it should be documented as ive seen ISAC section on this website, and ive dealt with it since 2018 , and ended up having to put boxing gloves in my trunk for potentially violent followers trying to pair and talk obscene. Im not sure what but its been confirmed and im awaiting the reply from manufacturer.

SUPPLEMENT TO PREVIOUS COMPLAINT (VIN: ...XXX): CRITICAL NEW EVIDENCE OF MULTI-POINT SAFETY FAILURE. On Dec 10, 2025, Tesla Service performed a full diagnostic and repair of the airbag safety system. In addition to the previously reported "damaged wires" in the passenger seat (OCS Module), the technician ALSO had to replace the Steering Column Control Module (SCCM) to resolve the active safety faults. Safety Implication: The vehicle was sold with simultaneous hardware failures in both the passenger occupant sensing system AND the driver's steering column airbag controls. This confirms widespread physical/electrical trauma to the vehicle's Supplemental Restraint System (SRS) that was concealed at the time of sale. The dealer released a vehicle with compromised safety hardware in both the driver and passenger zones. I have attached the official Tesla Invoice confirming these replacements. I drove my car today after it snowed a few inches the night before and while I was driving, I tried to press the horn, but there was no noise coming from the horn. when I went back home I noted that the speaker used for the horn is located near the passenger side wheel well. The speaker was blocked due to snow accumulation. This is dangerous because that means whenever there’s any snow on the road and I try to press the horn in an emergency then I cannot I might get into an accident because of my horn failing.

SUPPLEMENT TO ODI # 11702355 and 11698467: Validated proof of physical defect: Tesla Service diagnosis (Dec 10, 2025) confirms "damaged wires at the OCS module" caused the airbag failure. This finding indicates the vehicle suffered physical trauma that was concealed prior to sale. The presence of damaged internal safety wiring is inconsistent with a factory-condition vehicle and suggests the vehicle was released to the public after substandard repairs to a prior wreck. The airbag system was disabled due to this physical damage.

The steering was hard to turn, and the fully supervised driving mode didn't work. This has happened twice, and both times the issue went away on its own after a few minutes. I performed a soft reset both times, but it did not resolve the issue. The first time, the steering was very heavy, i.e., hard to turn. The second time was less difficult, but in both cases, FSD (Full Self-Driving) did not function. General Guideline questions: 1. The steering system malfunctioned. The car is available for inspection. 2. The car was very difficult to drive. I was able to turn the wheel, but I won't let my wife drive it. 3. No. The nearest service center is 90 miles away, and so far, they want me to bring the car to them. I am still working to resolve this, as I believe this issue requires mobile service. 4. No inspections have been done. 5. Yes. An alert message on the main display stated: "Alerts - Steering & Braking - Steering assist reduced/Steering may require increased effort."
