On XXX at approximately 12:56 PM Pacific on [XXX], my 2024 Tesla Model 3 VIN [XXX] was operating in Full Self-Driving (Supervised). The system detected oncoming traffic, stopped, then reversed to yield and backed into a parked 2015 Ford Transit van that had an occupant inside. I am the registered owner. [XXX] was the driver. TeslaCam saved events at 12:56:33 and 12:56:43. There was little visible damage, no injuries logged, and no police report. Tesla's consumer privacy form does not accept a crash narrative. I requested a Vehicle Data Report / copy of my data on XXX. USAA/[XXX] claim XXX later assigned 100 percent fault to the Tesla driver for backing, treating this as a conventional reverse collision rather than an FSD commanded maneuver. I am reporting this as a safety defect in FSD (Supervised) because the vehicle reversed into a parked occupied vehicle while the feature was engaged. - #11759922August 27, 2026SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, VEHICLE SPEED CONTROL
On XXX at around 3:30 PM EST, I was driving my 2023 Tesla Model 3 on [XXX] about 10 miles outside of [XXX]. I had the adaptive cruise control set at 75 mph. I was driving in the right lane of the two available lanes of travel in that direction. I noticed an unattended car on the right shoulder about a 1/4 mile up the road and continued my drive as normal. The car was at least 5' to 6' off the highway. As i got about 30 yards from the car, my car suddenly slammed on the brakes automatically, which forced me to react in a corrective manner. Thankfully there was nobody behind me or the severity of the braking would have likely caused a rear end collision. To my understanding, this phantom braking is a known issue with Teslas, so I wanted to document my experience to add to the record. The Tesla service center in San Antonio Texas installed new tires and a new wheel on my vehicle and one of the tires was installed inside out based on the manufacturer’s specifications. I’ve been driving this vehicle for about 4 months /5k miles with the risk of this tire malfunctioning and causing an accident.

To Whom It May Concern, I am reporting what I believe is a foreseeable child-safety risk involving Tesla’s Toybox feature. Tesla allows parents to PIN-restrict Arcade, Theater, and the web browser, but Toybox remains accessible. Recently, I briefly went inside my home to use the bathroom, and my child slipped outside and climbed into my parked Tesla specifically to use the touchscreen and Toybox. Fortunately, I discovered him quickly. In hot weather, however, a child remaining unnoticed inside a parked vehicle could face serious injury or death, particularly if climate control were disabled, changed, or failed. Tesla already has the software infrastructure to restrict entertainment features. Adding Toybox to Parental Controls appears to be a straightforward safety improvement. I believe this omission creates an unnecessary and foreseeable risk to children and warrants review as a potential vehicle safety concern. On 8/24/2026 at approximately 12:02pm, my 2023 Model 3 was parked in my assigned space in a covered apartment carport, with a concrete support pillar approximately one to two feet from the passenger side. I entered a destination and activated FSD (Supervised) from Park, a Tesla-designed feature. Vehicle telemetry recorded in the dashcam clips shows FSD shifted into Drive, activated the right turn signal, turned the steering wheel to near full right lock, and accelerated to 3 mph, contacting the pillar with the passenger mirror and front door approximately 1.5 seconds after first movement. No accelerator or brake input was recorded from me before the collision. I applied the brake within one second of the impact beginning, and FSD disengaged. The pillar was within view of the vehicle's side cameras throughout. No warning was given. Damage to the passenger door; no injuries. Dashcam video with telemetry overlay and photos available.

Muffled or significantly quieter horn during rain. Users on Reddit generally agree that the sound typically returns to normal once the area dries out. Why This Happens: Speaker Design: Newer models (such as recent Tesla Model 3 and Tesla Model Y versions) utilize electronic pedestrian warning speakers or integrated units for the horn function rather than traditional enclosed metal horns. I have an issue with my car where every time I wash it or drive it even in light rain, the horn is extremely muffled. So muffled that I was sounding it driving down my road with people out walking, and no one heard it. I took it to the service center and they basically said “we confirmed your complaint, it’ll work when it dries out”. My car now has two notations on file from two visits to service center. This is a very wide spread problem - hundreds of comments online. Google: Tesla horn rain.

My 2022 Tesla Model 3 has experienced recurring intermittent loss/reduction of power steering assistance. The vehicle displays “Steering assist reduced – Steering may require increased effort.” The problem has occurred multiple times over a period of months and may disappear after the vehicle is power cycled or the alert clears. Tesla previously evaluated the vehicle for this condition. On July 8, 2026, Tesla documented the steering-assist-reduced complaint and recommended replacement of the RWD steering rack/steering gear, with an estimated repair cost of approximately $3,018. On August 20, 2026, the warning occurred again. Tesla Service Mode recorded fault EPAS3S_a005_sent2Mia, described as “Torque Sensor 2 signal not received,” with the potential impact listed as “Steering assist reduced.” The vehicle simultaneously generated UI_a020_SteeringReduced. The warning subsequently cleared again, but this is a recurring intermittent failure involving the vehicle’s electronic power-assisted steering system. I am concerned that the steering assistance could become reduced or unavailable while driving. Tesla has already determined that replacement of the steering rack is required, but the repair is customer-pay. I am reporting this because this appears to be a recurring safety-related steering defect and I am concerned that other vehicles may experience the same failure.

On June 16, 2026, while operating the "Autopilot" self-driving system, my vehicle autonomously activated the right turn signal to enter a parking space. Subsequently, the vehicle experienced an unexpected acceleration. Despite my immediate application of the brakes, the vehicle collided with a storefront window, resulting in property damage. My insurance provider covered all associated damages, including vehicle repairs. Immediately following the incident, I informed Tesla and requested access to the vehicle's electronic records, which were made available at a Tesla service center. Tesla is currently rescheduling my vehicle, acknowledging the severity of the self-driving system's malfunction. Concurrently, Tesla continues to charge me for the self-driving capability, despite having advised me to refrain from using it until all necessary vehicle tests have been completed. What further actions should I undertake, and how can the National Highway Traffic Safety Administration (NHTSA) provide assistance in this matter? Thank you.

Tesla’s replacement of physical steering column turn signal stalks with capacitive touch buttons on the steering wheel face creates an active safety hazard. First, the capacitive sensors frequently become entirely unresponsive. If a driver rests a finger near the buttons, the system triggers a timeout/lockout, rendering the signals non-functional for several seconds when pressed. They also fail to register inputs under normal pressure or when wearing gloves. Second, because the buttons rotate dynamically with the steering wheel, it is structurally impossible to signal by muscle memory during sharp or sequential maneuvers, like navigating roundabouts. To signal while the wheel is turning or when buttons fail to respond, the driver must repeatedly look away from the road to visually target the wheel. This defective layout causes delayed signaling, severe driver distraction, and elevates the risk of a collision.

Date: XXX, Time: 5.09.2026 State: [XXX] Location: [XXX] We were on Full Self Service Driving Mode A [XXX] came and hit my car in the passenger side behind the front wheel and front buffer. We were on left/fast moving lane and two semi-trucks were in the right lanes slight ahead of us (one had overweight sign). Something came out of the direction of trucks and hit our car with a big sound (moving object). When we stopped and checked the car later, it was evident that the damage was a sizable, it would have been something solid or heavy. Our concern is the Tesla was on Full Self Driving Mode, and it did not generate any alert, recording or more importantly, it did not break, change the driving direction nor try to avoid being hit by the object. This diminishes the reliability of Tesla FSD and raises serious safety concerns. We are in the process of filling insurance claim. I have been driving Tesla using FSD from [XXX] to [XXX] and passing [XXX] (XXX around 5 pm). We took an exit for rest room break near Gas Station. As soon as Car took exit using FSD, the car completely stopped suddenly in the middle of the road without any reason and warning. There was not any rain, visibility issue or driver distraction. There was not any alert or recording. As far as I remember, there was not a vehicle on the front 100ft. It was on a small bridge. My family was in the car (with [XXX]). This was a scary moment (had it happened on the highway or a vehicle behind crash) INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My 2021 Tesla Model 3 with approximately 70,000 miles developed front suspension problems. It began with squeaking, creaking and groaning, similar to a rusty playground swing, especially while turning or when the suspension moved. The noise was worse in hot weather and progressively became extremely loud and constant during normal driving. I also began hearing intermittent knocking/"bonking" or clunking. I could even hear the suspension when the vehicle compressed as I sat in the driver's seat. There were no warning lights or messages. Tesla inspected the vehicle and diagnosed both front lower lateral links and both front lower compliance links as needing replacement, along with an alignment. Tesla estimated approximately $1,960. CPR EV Repair replaced the diagnosed lower suspension components. After removal, the bushings showed significant visible deterioration, including cracked, torn and separated rubber around the inner sleeves. CPR EV also advised that the front upper control arms were worn and would need replacement. I was concerned for my safety because failure or separation of a deteriorated front suspension component while driving could affect steering or vehicle stability and potentially cause loss of control. Although I did not experience a complete separation or crash, the rapidly worsening and eventually constant symptoms made me concerned about continuing to drive. I have Tesla's estimate, CPR EV documentation and photographs of the deteriorated components after removal.

- #11757060August 14, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The vehicle is experiencing severe, unpredictable software and hardware regressions related to the Full Self-Driving (Supervised) system (v14.3.7, Software build 2026.21.5) that create an immediate, active traffic hazard and a high risk of rear-end collisions. Primary Safety Issues: 1. TRAFFIC STALLS UPON DISENGAGEMENT: When Full Self-Driving (FSD) is engaged, auxiliary sensors malfunction, causing the headlights to cycle on/off and windshield wipers to run at full speed in clear, dry, midday daylight. When attempting to manually disengage FSD to correct these malfunctioning systems, the vehicle fails to smoothly transition control. Instead, it triggers critical red flashing alert lights, sounds severe warning chimes, and initiates an immediate, aggressive brake application—stopping the vehicle dead in active, live traffic. 2. PHANTOM BRAKING: The vehicle routinely experiences sudden, severe, and unprovoked phantom braking events while driving at speed, without any forward obstacles or environmental hazards present. 3. STEERING INSTABILITY: When navigating parking lot entrances, the vehicle exhibits extreme "indecisive behavior," resulting in violent, rapid shaking of the steering wheel before completing the maneuver. Tesla customer service was formally notified of these dangerous system behaviors on August 14, 2026. A formal remote engineering log review has been requested from the manufacturer, as these sudden braking actions in active traffic pose an immediate threat to public safety.

After disengaging Tesla Full Self-Driving (FSD) and taking manual control of the vehicle, a feedback popup appears on the center touchscreen asking the driver to select a reason for disengaging FSD. The popup does not automatically disappear and remains on the screen unless the driver interacts with it. I believe this creates an unnecessary driver-distraction risk. The moment immediately following an FSD disengagement is precisely when the driver has resumed responsibility for manually controlling the vehicle and should be focused entirely on the road and surrounding traffic. Presenting a nonessential survey at that moment encourages the driver to look at and interact with the touchscreen. There should be an option to disable this feedback request, dismiss it without touchscreen interaction, or have it automatically disappear after a short period. I am reporting this because I believe requiring or encouraging touchscreen interaction immediately after transitioning from FSD to manual driving is a safety concern and should be reviewed.

I am the owner of a 2022 Tesla Model 3. The front control arms on this vehicle are defective as manufactured. From the factory, these components have demonstrated premature failure, abnormal noise, loosening, and/or separation risk that is not consistent with normal wear on a low-mileage vehicle of this age. Symptoms experienced include [describe your exact symptoms clearly and factually — examples: progressive clicking, clunking, popping, or creaking from the front suspension/wheel area; especially noticeable during low-speed turns, reverse, or over bumps; any feeling of looseness, pull, or change in handling; any actual detachment or near-detachment]. These symptoms began at approximately 75000 miles on July 27th 2026. This is not isolated wear. Front control-arm and related suspension-link failures (including lower lateral links that can detach from the subframe) have been the subject of multiple owner complaints, prior Tesla recalls (including NHTSA campaign 21V-835 for improperly torqued lateral-link fasteners on certain Model 3/Y vehicles and the later 23V-235 expansion), technical service bulletins addressing water intrusion and noise in upper control arms, and a current NHTSA preliminary evaluation (opened 2026) into front lower lateral-link detachment on a large population of Model 3 and Model Y vehicles. Owners report that failures can occur with little or no prior dashboard warning, only possible audible cues, and can render the vehicle undrivable or cause sudden changes in alignment and directional control. A sudden separation or failure of a front control arm/lateral link while the vehicle is in motion creates an unreasonable risk of loss of vehicle control, crash, or injury to the driver, passengers, and other road users. Because the condition appears to originate from factory assembly, design, or component durability issues rather than owner abuse or ordinary high-mileage wear, and because similar failures are widely reported across Tesla owners.

When disengaging Full Self Driving (FSD), a feedback notification pops up on the bottom left corner of the infotainment screen that fully blocks the media control panel and does not disappear until the driver provides feedback to Tesla for why FSD was disengaged. By forcing the driver to answer the feedback question in order to access the media controls, Tesla causes a distraction to the driver, who is forced to take their eyes off the road and select a feedback response to why they chose to disengage FSD, increasing the chance of an accident. This is an intentional but dangerous design decision.
