Turn signal indicator buttons stick and/or activate the opposite turn indicator of the button pressed. Issue is persistent and I have had two other steering wheels replaced and all three have the same problem. Tesla technician told me that it is an issue with the steering wheel and there was no fix. Have a video of the problem but I am currently unable to upload.

The vehicle’s brakes did not work, automatic emergency brakes and collision warning both did not work. I pressed on the brake for about 5 seconds but the brakes did not work, resulting in a collision with the car in front. Everything is available for inspection if needed. Both my safety and the driver of the vehicle in front of me was at risk. The problem has not been confirmed yet. The vehicle is currently at a shop, waiting inspection. There were no warning lamps, messages, or symptoms.

During a major car accident the airbags for my Tesla, Model 3, 2023 did not deploy airbags although other impacted cars had full airbags deployed. This posed a risk to my safety as a driver due to not having the proper equipment deployed during a major accident. This was noticed by the emergency personnel on the scene who stated that based on my impact the airbags should have been activated from my collision. According to the safety records associated with my car to date there are no safety reports regarding the airbags for my car. There was no warning of my airbags not being able to be fully functioning at the time of impact. This incident happened on November 1st and is in the process of being investigated by the Harris County sheriff's department.

I love my vehicle and its technology. However after installation of the latest (FSD V14) update, I observed issues I believe pose a risk to the safe operation of the vehicle. I have made dozens of reports for 30+ days to Tesla for these issues. 1. Driving Profiles and Speed The update features multiple driving profiles (e.g., "Sloth", "Chill", "Standard", and Mad Max). Only the "Sloth" profile drives at the “speed limit” (what their outdated map thinks speed limit is). All other profiles drive over the posted speed limit (sometimes significantly). The ability to manually adjust the speed limit setting has been removed or disabled in these profiles. Once the vehicle is in a profile, the system stays in its own selected speed. This behavior prevents the driver from easily controlling the maximum speed when using the system, undermining driver-intent. 2. Incorrect or Outdated Speed Limit Data The vehicle frequently displays incorrect speed limits: for example, school zones, reduced speed zones, temporary speed changes, work zones, and even a busy high way I drive everyday that’s been changed for 3 months now, are often not recognized or updated in the system. On several occasions over the past months, the system continued to use an outdated or incorrect posted speed, despite the road clearly being a lower limit. Because the system uses the erroneous speed data as the basis for its automated driving decision (and without manual override of speed in FSD), this situation increases risk-especially in areas where lower speed limits are enforced for safety (pedestrian zones, near schools). 3. Safety Risk Description The lack of manual speed adjustment under autonomous mode means the driver has less control over vehicle speed when using FSD, reducing the driver's ability to mitigate risk in a dynamic environment. I’m asking for Tesla to either make sure they have constant real time speed limit data, or allow again for manual speed adjusts when in FSD.

Yesterday I had another near miss collision and like all the others there was zero reaction from the car, an accident would happen long before the car would react. How many deaths are you going to allow to happen before you take proper action? It’s obvious someone there is being bought because the dangers can be easily proven over and over. My previous car would have braked long before I would have had to steer away from the oncoming vehicle, my Tesla has given me zero protection from front end collisions like the pictures provided.

I own a Tesla 2022 Model 3 Long Range purchased in 2022 New and have been experiencing persistent issues with the HVAC units causing a persistent and prevalent smell of mold/must/vinegar that is causing myself and my passengers headaches and allergies. Tesla has failed to permanently repair and this is a widespread design issue with Tesla vehicles documented thoroughly online including Tesla’s own website, where condensation in the evaporator housing leads to mold and bacteria growth and odor in the cabin as a result. Despite multiple service appointments, Tesla has only provided temporary assistance by installing new filters and has recently offering to use a foam spray to clean the inside. I’ve already used over 15 filters in 3 years and have used numerous canisters of the foam cleaning spray and the problems have not subsided. They’ve refused to cover this under warranty even though this defect was reported initially in 2022 and the vehicle is still under warranty. The odor recurs every few weeks and presents air quality and health concerns as evidenced in our increased allergies and need to seek medical attention as a result and headaches from the mold build up. I’ve filed related complaints with the California BAR and Attorney General who have advised I file this report with your agency and as a result I am now following their instructions.

While driving, the side mirrors closed and the doors locked by themselves. Then, the front trunk opened, the rear trunk opened, and when I opened the door, the headlights turned on and the front trunk opened again. When I opened the door another time, the side mirrors didn’t unfold, and the headlights started blinking. At high speeds (over 50 mph), I hear a ticking or metal-clanking sound, which is really scary. The car seems to be acting on its own, following some strange “manual” or behavior pattern. I feel unsafe driving it.

The contact owns a 2025 Tesla Model 3. The contact stated that while coming to a stop at a traffic light, the vehicle’s autopilot system became inoperable, and the vehicle unexpectedly accelerated up to approximately 13 MPH without any warning lights illuminated. The contact attempted to regain control of the steering; however, due to the failure, the vehicle collided with another vehicle, resulting in severe damage to the driver’s side rear quarter panel, wheel, and tire. The contact stated that the air bags did not deploy during the incident. No injuries were sustained, and no medical attention was required. A police report was not filed. The contact expressed concern that the vehicle may present a potential safety hazard, as the failure had recurred on multiple occasions when attempting to drive the vehicle. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 12,300.

On my 2025 Tesla Model 3 Highland, the right-turn signal button on the steering wheel fails to activate approximately 25–30% of the time. The issue is random and unpredictable. This has occurred while changing lanes on the freeway and when turning into shopping centers or side streets, leaving drivers behind me unaware of my intent and creating a safety hazard. The problem is worse when lane assist is active, as the vehicle resists the maneuver when the signal doesn’t engage, interpreting it as lane departure. The problem started happening at around 3,000 miles on the odometer and continues intermittently. I have not had Tesla service confirm the issue yet, but other owners have reported that replacement of the steering wheel does not resolve it, suggesting a systemic or software-related defect. No warning lamps or messages appear.

* Front passenger safety restraint system issue - sensors under passenger seat intermittently failing for months even after taking it in for service. * Safety of passenger is at risk as airbags might not fire due to occupant classification system failing. * Service center said nothing looks wrong even though car alerts multiple times per drive. Many reports online of Alert RCM2_a636 . * has been inspected by service center * alert from car beeps every 30 seconds to 1 minute intermittently, has been occurring for months

After upgrading to FSD (sull self driving supervised) v14, The speed limit function HAS BEEN REMOVED. THERE IS NO WAY TO SET A MAX SPEED FOR CRUISE CONTROL. THE CAR SPEEDS UNSAFELY AT ALL TIMES WHILE IN FSD. THERE IS NO WAY TO CONTROL HOW FAST THE CAR CAN GO. It just "thinks" what speed is best. It constantly speeds 7-10 over in every setting above "SLOTH". SLOTH setting still DOES NOT have a speed limit setting. THIS IS VERY DANGEROUS. THERE IS NO EXCUSE FOR NOT HAVING A MAX SPEED SETTING.

A recent update has caused the vehicle to no longer acknowledge an important speed sign. This was confirmed by another Tesla, it is due to an update. This is unsafe and is capable of allowing the vehicle to travel through town at 75 mph where pedestrians cross the highway. If the camera gets blinded at the wrong time then it will NOT see people or vehicles as it plows into them. I made Tesla service aware and tried to submit a bug report but Tesla app simply says they are aware that the camera may not always read the signs. However, this was never an issue until a recent update. When are you going to make them liable?

I drove into my neighbor’s driveway to drop her off. As I slowed down and applied the brakes to stop, the car failed to respond. It continued forward, running over the flower and plant pots at the edge of the driveway, crossing the backyard, and finally colliding with the trees and bushes at the rear of the property. All airbags deployed, and the vehicle was damaged beyond repair.

On October 17, 2025, while Full Self-Driving (Supervised) was engaged, the vehicle made an abrupt, uncommanded steering input to the right, causing a collision with a tunnel wall. This crash appears to be the result of a critical vehicle hardware failure. In the 24 hours prior to the crash, the vehicle logged multiple critical steering errors, including EPAS3P_a086_dasMia, which indicates the Electric Power Assisted Steering system was "Missing in Action" and not communicating. Furthermore, the vehicle has logged daily, recurring hardware faults for the Steering Column Control Module (SCCM_a016_hardwareError) since it was purchased. The uncommanded steering action created an extremely dangerous safety condition resulting in a collision. All stability and traction control faults appeared at the exact moment of impact, not before, indicating the steering system failure was the root cause. The problem has been confirmed by the vehicle's own internal diagnostic logs. The vehicle is available for inspection.

On August 7th, 2025 at around 8 AM in the morning, my vehicles mobile phone car key was in-operable and I was unable to get in the car which led me to trouble shoot how to fix this issue with the car. Upon researching I was directed by Tesla to directly complete a soft reset of the vehicle holding down both buttons on the vehicle once I was inside, and pushing on the brake pedal. Upon completion, my vehicle booted up and I realized that all camera systems were down, my navigation systems were down, my safety features were down, my auto pilot, lane keeping assist and auto brake were down, sensors were down and I was no longer able to pre heat my battery to charge my vehicle due to issues with the navigation. I then took the vehicle to Tesla Service Center Irvine where a service advisor quoted me 2900 for a new ECU and Bolts and told me that I needed to replace my whole unit and that I was to cover the cost of the repair even though my vehicle is only 2 years of age and this failure is due to the quality control and software of the vehicle. I have been driving around a vehicle with absolutely no safety features whatsoever such as no cameras, sensors, auto braking, or full self driving due to this ECU failure and I fear for my safety and well being while driving this car.

teslas have blind spot monitor cameras that activate when the turn signal is activated as a turn left or right. there seems to be a noticeable delay on older cars with intel processors where the cameras take longer to display on the screen in split second manuvers and lane changes that can lead to accidents
