I am leasing a 2024 Tesla Model 3. The vehicle has a steering column defect that was previously acknowledged and temporarily repaired by Tesla Service. Within days, the issue returned. Tesla has now informed me that the steering column has a known design flaw, that replacement parts are still in development, and that any repair attempted now would fail. Despite this, I am being asked to continue driving the vehicle until at least January. This defect affects the steering system, a critical safety component, and raises serious concerns regarding vehicle control and driver safety. Tesla has not offered a loaner vehicle or alternative transportation while awaiting a permanent fix. I believe this vehicle is unsafe to operate and am reporting this as a safety-related defect in the steering system.

I purchased this used Tesla Model 3 (VIN: [XXX] ) from a licensed dealer on July 15th, 2025. The vehicle is still under the original manufacturer warranty. Since ownership, I have experienced a persistent metal-impact noise coming from the suspension and steering area, especially when going over speed bumps, uneven road surfaces, or low-speed turns. The noise sounds like loose metal parts making contact. Tesla service has attempted repairs multiple times, including a recent visit that lasted several weeks. Although some minor issues were addressed, the abnormal metal impact noise continues and has never been fully resolved. The repeated unresolved noise raises concerns about structural integrity and potential failure within components related to steering, wheels, or suspension. While the vehicle remains operable, the issue affects driving confidence, comfort, and concentration. Over time, the repeated metal noise has caused stress and anxiety because I am unsure whether a part will loosen or fail while driving. Since the problem remains after multiple attempts to repair, I am reporting this as a potential safety-related defect. Thank you. Central computer (motherboard), cameras (front/rear/side), autopilot/auto steer system. Error code: APS_w132. Description of Problem: On or around [XXX], Tesla pushed a mandatory over-the-air software update to my vehicle. Immediately after, multiple safety-critical systems failed: all cameras (including backup) became intermittent or non-functional, auto steer/autopilot stopped working, and the car emitted constant distracting beeping alerts due to a critical motherboard failure. This made driving hazardous—impaired visibility for parking/reversing, loss of collision avoidance features, and constant audio distractions that could lead to accidents. I had to drive in this unsafe condition for weeks while waiting for service. Tesla attempted a remote fix but it failed. I scheduled service on November 19 for December 3 (19-day delay). Diagnostics confirmed the update induced the hardware failure, a known issue reported by other 2021 Model 3 owners on forums like Reddit (e.g., software bricking motherboards post-update). Despite this, Tesla refused warranty/goodwill coverage, quoting $2,576.83 for repairs. Service advisors Josh and Andy were unprofessional: Josh was rude/condescending; Andy was aggressive, sarcastic, and retaliatory. After I mentioned filing complaints with the AZ Attorney General and News Channel 3, Andy threatened $100/day storage fees with an impossible deadline (called at 4 PM Friday, close at 5 PM, demanded pickup by morning, then noon). He closed my ticket without authorization, delaying fixes and forcing me to contact another location—prolonging my exposure to the unsafe vehicle. This defect poses serious safety risks: sudden loss of cameras/autopilot could cause crashes. Tesla's awareness (from prior reports) and poor handling (intimidation delaying repairs) exacerbates the danger. No crash/injury yet, but potential is high. Regen braking sporadically not working when going down mountain or steep roads. Sometimes they work and sometimes they won’t. This is after extensive driving on warm days while battery is between 70-80%. Support could not help me and they don’t have any technicians available. I’ve sent them videos and time stamps and they are ignoring the issue and unable to help due to lack of knowledgable representatives This is a serious safety concern

This car uses auto-dimming rear view and side mirrors. However, the rear view mirror is only able to be dimmed automatically, or be disabled in the settings. The issue is that the dimming is not based on a light sensor at all, it is merely timed to dim around 30 minutes after sunset and return to normal 30 minutes before sunrise. I have confirmed this timing on many drives and it is repeatable. The safety concern is that there is no way to manually dim the rear view mirror, which is a big issue around sunrise and sunset when cars headlights turn on before the mirrors dim. This is especially an issue with larger trucks behind a sedan like my Model 3. The only option I have to fight the blinding glare from headlights is to manually move my mirror so that I can no longer see out of the rear window, which makes lane changes much more dangerous.

I am submitting this complaint to report a safety issue with the turn-signal buttons on my steering wheel of the 2024–2025 Tesla Model 3 Highland. The turn-signal buttons are located on the steering wheel surface instead of a traditional stalk. When the steering wheel is turned at any angle, the buttons are difficult to locate by feel, often requiring the driver to look down to activate them and take his eyes off the road. Trying to signal during turns, lane changes, or roundabouts is not intuitive, and I have nearly had several collisions. This design presents a potentially dangerous safety hazard and could increase the risk of collisions. Tesla should be required to recall and retrofit these vehicles with turn signal stalks on the steering wheel column.

NHTSA Complaint Narrative (Copy & Paste) CRITICAL SAFETY DEFECT: SALE OF VEHICLE WITH DISABLED AIRBAGS Incident: 10/24/2025 | Dealer: Braman Cadillac (Miami, FL) | VIN: [XXX] Braman Cadillac sold me this 2024 Tesla Model 3 representing it as "clean/no accidents." In reality, they released a vehicle with a disabled airbag system resulting from a crash that occurred in their inventory. 1. DISABLED RESTRAINT SYSTEM (RCM): Tesla Service Mode diagnostics confirm the Restraint Control Module (RCM) is in "Post Replacement" state, displaying the prompt: "Post Replacement Tasks - Routine requires Service Mode Plus authentication." This proves the airbag computer was replaced following a crash/electrical trauma but NEVER calibrated. Consequently, the SRS is disabled and will not deploy. The vehicle explicitly warns: "The Safety Restraint System may not perform as designed." The dealer released a non-roadworthy vehicle. 2. EVIDENCE OF DEALER KNOWLEDGE: Internal logs show critical crash fault codes (VCFRONT_a059 & RCM2_a003) logged on Oct 15, 2025. Dealer records confirm the car was in their exclusive inventory on this date (listed since Sept 23). These faults were active 9 days before sale. The dealer ignored active safety warnings to facilitate the transaction. 3. CONCEALED DAMAGE: Inspection by Classic Collision confirmed hidden structural damage requiring $9,676.69 in repairs, consistent with the uncalibrated module. CONCLUSION: Braman Cadillac knowingly sold a vehicle with a lethal safety defect. I demand an investigation into their release of vehicles with uncalibrated safety modules. Using the navigation system in self driving mode the turn signal indicators start 500 feet from the intended turn, regardless of speed, and not influenced by cross streets or parking lot driveways thay are present before the intended turn. This takes place regardless of upcoming cross streets or driveways, but based on distance only, giving drivers coming out of these cross streets and driveways an indication that i intend to turn sooner. This has caused several "close calls". Reference: "The California DMV driver handbook similarly instructs drivers to start signaling about 100 feet before a turn so that other road users have time to see and react to your intention. On freeways, the DMV further recommends signaling for at least five seconds before changing lanes, which is a related but separate guidance from the 100‑foot rule for turns." I consider this a serious safety issue.

I drove car for about 15 minutes and pulled into a parking space. There was no bumping or other incident. I left the vehicle for about 10 minutes and returned. The screen on my return showed 2 warnings, 1. Steering assist disabled, and 2. Collision avoidance system currently inoperable. Dealing with warning number 1: On starting to back out of parking space, i discovered there was no assist for steering, and a high degree of force was needed to turn the steering wheel and guide the car. This required considerable force. I tried a "system reboot" twice to no avail (this is done by holding 2 steering wheel buttons for about 10 seconds). I called Tesla for service and they towed to car to their service center. I do not know what steps were taken to diagnose or repair the problem. Warning number 2 disappeared from the screen when I started trying to back up..

The car, with no known or evident driver, stopped the lane awareness, cruise control, etc. All of those just stopped working and per Tesla that's expected which is surprising since it's all part of the safety systems. The car's visual cues stopped identifying lanes and as a byproduct of that, none of the dependent features work.

On September 25, 2025, the trunk opened on its own overnight due to what appears to be a software malfunction, resulting in the theft of $500 in personal belongings. The Tesla app sent only one notification while I was sleeping, with no alerts for over eight hours, failing to provide proper security warnings. Tesla customer support was unable to resolve the issue and suggested multiple drivers might have access, even though only one friend has access to the car and it was not her doing. Vehicle data from Tesla privacy support showed two hours missing during the trunk event, and Tesla could not explain why, only speculating a software glitch. This incident demonstrates a critical software and security failure that compromises vehicle safety and personal property. Tesla has failed to provide an explanation, solution, or preventive fix. Submitted Under 49 CFR §552.3 Request for Defect Investigation & Safety Recall Tesla Model 3 / Model Y: Interior High-Voltage Heater Fire Hazard, Missing Firewall, and Entrapment Due to Electronic Door Failures. My 2022 Tesla Model 3 experienced an interior flash fire caused by a design defect. Tesla placed the high-voltage PTC cabin heater and HV wiring inside the dashboard with no firewall. After a crash, I saw a straight “strip flame” shoot out of the HVAC dash vent, exactly like a gas heater. The fire began INSIDE the dash/HVAC ducting within seconds of impact, not from the battery. This shows an arc-flash failure of the HV heater or wiring inside the cabin. The flame entered the cabin instantly because there is no firewall separating HV components from occupants. During the fire, all electronic door releases failed. Tesla hides the manual front releases and provides NO rear mechanical releases. I was trapped inside a burning cabin and suffered major injuries trying to escape by kicking the rear passenger window before bystander helped me brake out the rear passenger window to get me out. Most people cannot escape this design. This is a dangerous combination of defects: (1) HV heater and wiring located inside the cabin with no firewall; (2) HVAC ducting becomes a flame pathway; (3) Electronic door releases fail during fire/crash; (4) No labeled manual rear-door exits; (5) No backup 12-V actuator power to open doors. Tesla must: relocate the HV heater outside the cabin (frunk), add a real firewall, add fire-retardant HVAC materials, add labeled manual releases on all doors, and include a 12-V backup to door actuators. Please open a defect investigation. This design nearly killed me and will kill others.

Component/system that failed: Tesla Full Self-Driving (Supervised) software version 14.x (2025.38.x and later). The issue is software-only and remains installed on any vehicle updated to v14. Description of safety defect: FSD v14 deliberately removes the driver’s ability to set or adjust maximum speed offset using the right scroll wheel (feature present in all prior versions including my current v13.2.9). As a result, the system routinely exceeds posted speed limits by 5–20 mph with no way for the supervised driver to correct it short of fully disengaging FSD. The vehicle ignores speed-limit signs, relies on inaccurate map data, and follows surrounding traffic speed instead of the legal limit, even in school zones, construction zones, residential areas, and adverse weather. This significantly increases risk of collision, injury, and traffic citations. Reproduced/confirmed: Widely reproduced by thousands of owners on Tesla forums, Reddit, and X immediately after v14 rollout in late 2025. I have chosen to remain on v13.2.9 to avoid this exact defect. Inspected by manufacturer or others: No in-person inspection required; Tesla has full remote log access. Multiple owners have submitted in-car bug reports. Warning lamps/messages: None. The speed-offset control is simply grayed out and non-functional by design in v14. Vehicle available for inspection upon request.

Safety restraint warning in car, unsure of actual issue but warning keeps popping up. Presumed that the seatbelts or airbags would not properly work in event of a crash. Dealer/company says it's a known issue, with other cars needing a fix too, however they aren't able to specify exactly the issue or issue a timeframe on repair. Issue first started appearing in Late August of this year, and has become more frequent as of recent. Potentially this is a critical safety issue regarding airbag/seatbelt's potentially failing to work as designed for all similiar models.

To Whom It May Concern, I am submitting a formal safety complaint regarding a potentially dangerous defect in my 2024 Tesla Model 3. Unlike traditional vehicles that use a dedicated horn assembly, this model relies on the external speaker system as its horn. While this may function under normal conditions, during rain or heavy moisture the speaker becomes saturated with water, causing the horn’s output to become extremely quiet and ineffective. This is a serious safety concern. The horn is a primary means of warning surrounding drivers and pedestrians of danger, especially in poor weather conditions, where visibility is already compromised. Ironically, during heavy rain when a horn is most needed it becomes significantly quieter and nearly unusable. I have personally experienced this issue multiple times, and it poses a clear risk to myself and other road users. I believe this design presents a safety defect because: The horn does not function at full volume when wet. The speaker-based horn assembly is exposed to moisture and not adequately protected. Drivers may be unable to alert surrounding traffic during hazardous weather conditions. No warning is provided to the driver that the horn is impaired. I request that the NHTSA investigate whether the use of a speaker-based horn system without adequate waterproofing meets federal motor vehicle safety standards, and whether Tesla should issue a service bulletin, recall, or design modification to restore safe horn functionality regardless of weather conditions. All safety and driver-assistance systems on my 2022 Tesla Model 3 failed due to an internal short in the vehicle’s computer, as confirmed in writing by Tesla’s technician. All cameras (rear, side, front) are non-functional, navigation does not work, and all ADAS features (Autopilot, lane-keeping, collision warning, blind-spot monitoring, emergency braking, etc.) are disabled. The vehicle has no visibility when reversing and no active safety protections while driving. Tesla documented the cause as an internal computer failure, not related to damage or misuse. This creates a dangerous condition on public roads since the car loses all safety systems. Tesla quoted ~$2,900 for a new computer even though the failure involves critical safety components. This may indicate a broader safety defect.
