2015 Ford F-150 Platinum, 6-speed automatic. Ford recall 26S28 (NHTSA campaign 26V-237, PCM software update for unintended transmission downshift) was performed at All Star Ford, [XXX]. BEFORE the recall service, this truck ran smoothly, quietly, and flawlessly. AFTER the PCM update, the transmission developed severe jerking and hard downshifting that never existed before the recall work. The dealership has held my vehicle since XXX. The mandatory post-update reset drive was skipped after the first update; the recall has since been re-performed twice and the malfunction persists - it was directly caused by the recall calibration. On XXX, the general manager stated the vehicle was "safe to drive" yet admitted he could not guarantee no further operational or safety issues would occur, and confirmed Ford engineers currently have no fix. I was also told Ford halted work because this recall update damaged my vehicle and thousands of others. I have been without my vehicle since May 2026. The recall remedy has rendered my vehicle unsafe to drive. I am filing so NHTSA is aware the 26S28 remedy is producing new transmission malfunctions. I have experienced ongoing issues with transmission. Two separate dealerships have done partial "rebuild" to no avail. The vehicle has lost power several times which include XXX at approximately 3:30 pm. while pulling onto a busy highway in heavy traffic. As soon as I got home, I parked the truck in the carport where it remains as of this writing. There have never been any warning lights, symbols, etc. on dash when these and other multiple problems with this vehicle happened. Multiple technicians have duplicated all of the issues that I have reported to the dealerships. I have been in touch with Ford Motor Company several times and have an open case with nothing from their representatives except to say how sorry they are. Second dealership has stated both verbally today, XXX, and in writing on XXX that he personally oversaw a complete teardown of this transmission on XXX when C-clutch was replaced see service order#XXX although parts list doesn't support his statement. Please refer to service order #XXX which is a documented failure of the E-clutch in my vehicle. The service manager at [XXX] O[XXX] states that this document is "speculation" on the part of the technician. I am in possession of his email stating such. I am at this time requesting any investigator be assigned to my case/claim # XXX, which I was given by [XXX] (last name unknown) Respectfully yours [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11769484October 7, 2026EXTERIOR LIGHTING
2023 Ford F-150 Lightning with 42,220 mi. The rear light bar has formed a crack which will allow moisture intrusion (photo attached). This is the same condition named in recall 25SB6 (NHTSA 25V731) and predecessor 23S30 (NHTSA 23V418): water droplets in the lightbar outer lens, which can cause inoperative or flickering reverse lamps. On 10/06/2026 I took the truck to Elmhurst Ford. The dealer did not point to any outside forces that caused the crack, and acknowledged the recall stating they would attempt to have it covered under warranty or extended service plan purchased through Ford as my VIN was not listed/included. The dealer later said the part number was not covered and wanted to charge a diagnostic fee for searching a part number in a database. The service manager did relent on charging the diagnostic fee. I contacted the Ford BEV customer service and they confirmed it was not covered under any warranty or extended service contract and my VIN was not part of recall VIN population even though the defect is same model year truck (2022-2023), same trim (Lariat), same part number as recalled VINs. The dealer confirmed I have the earliest part number, not superseded. SAFETY RISK: A truck backing up without working reverse lamps is more likely to strike a pedestrian or a child below the driver's sightline, since others are not alerted the vehicle is reversing. Reverse lamps are federally required. This failure can result in serious injury or death. REQUEST: Please document the recurring light bar cracks allowing moisture into the reverse-lamp housing, even on VINs excluded in the initial recall. This issue is more widespread than what VINs are included in the initial recall. Owners cannot be expected to pay several thousands of dollars to replace 1 defective light on a truck, when the issue is already acknowledged with a recall.

The automatic transmission has experienced repeated unexpected downshifts and other abnormal gear-selection behavior. Before the recall repair, the vehicle experienced multiple unexpected downshifts, including a shift from 4th to 2nd gear. Safety Recall 26S28 was performed on July 20, 2026, consisting of a PCM software update. The problem continued after the recall repair. Approximately one week later, while operating in 3rd gear, the transmission momentarily shifted into 1st gear and then returned to 4th. On August 31, while operating in 5th gear, it again momentarily shifted directly into 1st and then returned to 4th. The vehicle was returned to an authorized Ford dealer in September for evaluation. The dealer road-tested the vehicle three times and scanned the PCM but could not duplicate the condition and found no diagnostic trouble codes. The dealer contacted Ford regarding the condition, but no additional repair was authorized because no qualifying codes were present. The problem has continued. I have manually limited the selectable transmission range to 4th gear in an effort to reduce the possibility of another unexpected downshift. On multiple occasions, without any driver input, the selected maximum transmission range has changed from 4th back to the full 1-6 range. On October 3, 2026, the vehicle experienced a hard shift into 4th and then shifted back and forth between 5th and 6th. During this event, the transmission range display went blank and subsequently returned. On October 4, I again manually selected 4th as the maximum gear. During a drive of less than three miles, the selected range independently returned to the full 1-6 range multiple times. There have been no warning lights associated with these events. The condition is intermittent and was not reproduced during the dealer inspection. The vehicle remains available for inspection. I am concerned about the risk of loss of vehicle control from an unexpected downshift. The condition has continued afte

- #11769644October 7, 2026STEERING
I am writing to express my serious concern about a power steering failure in my 2015 F-150. Within the last few weeks, it began losing electronic power steering assist intermittently. The loss of assist happens suddenly, with no warning light or message on the dash. When it happens, the steering becomes extremely heavy and difficult to turn, and it can happen at any speed. The vehicle is very hard to steer without power assist, and becomes a serious risk to avoid an obstacle, hold my lane in a curve, or make a necessary maneuver in traffic. A sudden loss of steering control like this can easily lead to a serious accident, with the risk of injury or death to me, my passengers, and others on the road. Because of this I have already had a close call. The power steering went out while I was making a right turn at a traffic light on a major road. The truck drifted wide into the oncoming lanes, and I had to brake hard to avoid a collision. I pulled over and turned the truck off. When I started it again, the power steering seemed to work, and I was able to drive to the nearby Ford dealership to ask about the problem. Because I was about 30 miles from home, they advised that the best thing I could do was to get the truck back to my local dealer. When I brought my vehicle to my local dealer, the service department confirmed that recalls and bulletins exist. However, they told me that my truck's VIN does not fall within the affected range. This is the pattern I have seen every time: the power steering returns after the truck is restarted, but then goes out again at random, with not warning, and with no way for me to predict when. It is clearly not normal for a power steering system to fail on a vehicle with only 60,000 miles. A steering system is a critical safety component and should be expected to last well beyond this mileage. A failure this early points to a defect, not ordinary wear and tear,

I am submitting this complaint because my 2013 Ford F-150 3.5L EcoBoost developed a serious and potentially life-threatening engine defect that placed me and my family in immediate danger. While traveling on a major interstate/highway, the engine warning light and wrench light illuminated, and the truck subsequently lost power and stalled while I was driving. This occurred in active highway traffic, creating an extremely dangerous situation. My family and I were almost killed because the vehicle unexpectedly lost propulsion while we were traveling at highway speed and surrounded by other vehicles. The defect is especially hazardous because the failure can occur without providing a safe opportunity to move the vehicle out of traffic. An unexpected engine shutdown on a major interstate can leave the vehicle disabled in an active travel lane or in an unsafe location, exposing the occupants to a high risk of being struck by another vehicle. The loss of engine power also compromises the driver's ability to accelerate away from dangerous traffic situations. The condition is consistent with the **timing-component degradation and engine performance/stalling concerns described in Ford's technical-service information, including TSB 18-2305**, concerning the 3.5L EcoBoost engine. The timing components can deteriorate to the point that the engine experiences serious drivability problems and may stall. In my case, this was not merely a warning light or minor mechanical issue—the vehicle actually **stalled while being driven on a major interstate with my family inside**. This defect represents an immediate **hazardous safety condition** because an engine that unexpectedly stalls at highway speed can cause a driver to lose the ability to maintain speed, merge, maneuver, or safely remove the vehicle from traffic. The danger is substantially increased when children or family members are passengers. Ford is well aware of this defect. I am asking NHTSA to investigate

I am writing to formally report a severe and dangerous safety defect regarding the 10R80 10-speed automatic transmission in my 2020 Ford F-150. While driving, the vehicle routinely experiences a complete "neutral-out" or severe shift flare when attempting to transition out of 1st gear. Instead of engaging 2nd or 3rd gear, the transmission completely drops out of gear into a false neutral. During this failure, the vehicle experiences an immediate and total loss of forward propulsion for several seconds while the engine revs freely. After a dangerous delay, the transmission violently slams back into gear. This is an acute safety hazard. Losing all forward acceleration unexpectedly—specifically when pulling out from a complete stop, navigating intersections, or merging into fast-moving traffic—leaves the vehicle stranded and completely defenseless against oncoming traffic. It creates an immediate risk of a severe rear-end or broadside collision, as trailing drivers do not expect a vehicle to suddenly lose all power. Ford is fully aware of this mechanical defect, as documented in their internal Technical Service Bulletin TSB 25-2126 (and its predecessors). The bulletin notes that an internal CDF clutch drum bushing physically slips out of place, blocking hydraulic fluid passages and starving the transmission of pressure. Because this is a known factory engineering defect that causes an unpredictable loss of propulsion in active traffic, it should be treated as a critical safety hazard rather than a standard wear-and-tear component. I urge the NHTSA to investigate this systemic 10R80 CDF drum failure to protect consumers from catastrophic accidents.

The automatic braking was activated when we were backing out of a driveway onto the road, there was nothing next to us in the driveway, there was a car that came over a hill but not close we definetely had time to get backed out. The brake engaged and stopped the truck, my husband put the truck in drive to pull forward and the truck accelerated quickly forward and would not stop the tires were squealing and locked up, the truck was still going forward, my husband tried putting the truck in park to stop and finally had to really shove it in park but it was not easy to get it to go in park. There was a house and several cars in front of us and the truck was not stopping for anything in front of us, we did not hit anything but only because my husband was able to get it into park at the last second. We came within feet of our truck going through a car and through the house where there were several people inside. The problem has not been reproduced or confirmed by the dealer. If also has not been inspected because the dealer told us that no reports came up in the computer and that there is nothing that they can do about it. There were no warning messages or alerts when this was happening. This happened on September 25th while we were in Omaha Nebraska.

Hello, so while driving I am noticing a loud, hard thunk between gears. It is really noticeable when pulling our camper. When pulling our camper it is so loud and so hard I that if I have coffee in my cup holder, it would spill. We have contacted Ford about this and they are denying there is a transmission issue with their 10 speed transmissions. I feel there should be a recall on these transmissions because there are thousands of F150 owners having the same issue and replacing a transmission the way prices are for everything, who can afford to pay thousands of dollars for a new transmission and have it replaced.

I have noticed that my turn signal lights work sometimes, and sometimes they give me the rapid-fire warning light indicating that one of them is out (front or back). The outage may or may not last the entire duration of when the turn signal is deployed - sometimes it fixes itself, and other times it remains out. Sometimes, it even fixes itself and goes back out in the same signal instance. Additionally, I have noticed that one or both of the headlights will sometimes fail. I have learned to turn on the fog lights in order to ensure some sort of light remains working to illuminate my path forward. I've not yet experienced the fog lights going out as they seem to be unaffected by the intermittent outages. If it wasn't for this, I would have stopped driving at night altogether. I have already begun limiting my time on the road after dark, and attempting to handle anything that requires transportation earlier in the day. Please feel free to reach out with any additional questions, as I'd like to ensure this gets resolved for myself and any others ASAP!

During a panic stop the brakes failed and the pedal went to the floor resulting in a rear end collision. The vehicle is currently with the mechanic but he believes it a failed brake booster. When I checked for related problems it seems the same brake booster (EL3Z-2140-E) in my 2014 F150 V8 5.0L was used in the same year 3.5L but yet the recalls (16S24 and 20S31) didn't include my vehicle

While driving my 2015 Ford F-150, the vehicle experiences a severe and hazardous transmission defect. The transmission unexpectedly drops out of gear while in motion, resulting in an immediate loss of power and acceleration. Simultaneously, the 'D' (Drive) gear indicator light on the instrument cluster goes dark, and both the wrench warning light and check engine light illuminate, forcing the vehicle into an unsafe limp mode. This vehicle is subject to active safety Recall 26S28 / NHTSA 26V237 regarding unintended transmission downshifts and Transmission Range Sensor signal loss. I contacted an authorized Ford dealership to have this federally mandated safety recall performed. However, the service department explicitly refused to execute the recall repair, stating that the recall procedure does not solve the problem and makes it worse. The dealership is refusing to perform the required safety service, leaving my vehicle unrepaired and creating a dangerous ongoing safety hazard on public roads

Sudden downshifts too 1st gear and 3rd gear from highway speeds. Also goes into limp mode. The codes needed to replace the molded lead frame are present, was at the dealer yesterday and Ford will not let them make the needed repairs under warranty. Ford said I must wait an undetermined amount of time for them too try and find a fix for this before anything else can be done. Besides this being the only vehicle I have at the time too drive, it is totally UNSAFE to drive. Ford needs to come up with a fix now or replace the molded lead frame now so repairs will be complete and I will have a safe vehicle to drive. This problem is happening almost everytime I drive. It started on 9/26/26 and continues today.
