My truck is shifting erratically going from 5/6th gear down to 1st/2nd. It looses power. My battery light comes on, the wrench comes on, check engine light comes on but no codes come up. I have already put in a new transmission at my cost.

It had been raining hard for a few days. Husband was taking truck down hwy 98 to a drs appointment an hour away. Driving 50 when the steering wheel starting going in and out, the seat started moving forward and back, running boards going in and out, lights flashing on and off and horn blaring and the 2020 F150 sputtered, almost stopped in the middle of the road; to name a few things. Frightened he was able to pull into a gas station, wait a bit then try another time when same happened. He eventually made it to his appointment. Came out and truck doing same. Was towed to Group 1 Ford dealership in in Pensacola. They had it a week and their mechanics could only say it was the BCM, Body Control Module that was wet and malfunctioned. Recommended an $8,000 dollar replacement but as they could not find the problem suggested we wait till we found it. After 10 days at the dealership, they couldn't. I, a 67 year old woman did. The upper windshield was leaking over the A and B pillar and the drops were landing on the BCM. I have pictures. l If they had taken the side panels of dash off by door they would have found it also. Husband could have died

Vehicle Information: 2013 Ford F-150 SuperCrew 3.5L V6 EcoBoost (VIN: [XXX]) While operating the vehicle, experienced a severe and sudden degradation of brake hydraulic pressure. The brake pedal travels abnormally deep and requires the driver to "double-pump" (fully let off the brake pedal and re-press it a second time) to achieve enough line pressure to stop the vehicle. Additionally, while stopped at an intersection, the vehicle began violently jerking and resisting movement because the brake calipers failed to release, holding the vehicle back until a heavy secondary press of the pedal forced the system to recede. Safety Risk & Background: This behavior matches the exact mechanical failure profile of Ford Safety Recall 16S24 and Recall 20S31 (internal failure of the brake master cylinder rear cup seal leaking corrosive fluid directly into the vacuum brake booster). I am the original owner of this vehicle from new. My exact VIN is currently excluded from the official recall database due to an early 2013 manufacture date boundary. However, the vehicle is suffering from the identical catastrophic failure of the identical under-recalled factory braking component, resulting in extreme deceleration failure and unintended brake lockup while moving. This constitutes an immediate crash hazard to my teenage driver that is currently operating this vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11768973October 5, 2026EXTERIOR LIGHTING:HEADLIGHTS:DAYTIME RUNNING LIGHTS:BULBS
The contact owns a 2021 Ford F-150. The contact stated that the LED daytime running lights was flashing quickly before burning out. The contact stated that while driving 30 MPH and then stopped at an intersection, another driver informed the contact that the passenger's side headlight was flashing quickly. The contact towed the vehicle to the residence. The vehicle was taken to the dealer, where the passenger's side headlight assembly was replaced under warranty. The dealer informed the contact that the driver's side headlight might fail as well. The contact stated that the failure also occurred on the driver's side. Upon inspection, the contact became aware of a burning plastic odor coming from the headlights. Upon researching the failure online, the contact learned that the failure resulted from the headlights overheating and melting. The manufacturer was notified of the recent failure but denied coverage because the vehicle had exceeded 100,000 miles. The dealer advised the contact to report the second failure to the NHTSA Hotline. The failure mileage was approximately 99,000.

Ever since the last OTA software update to SYNC 26.2.8.3 on 9/11/2026 my auto high beam functionality has started working erratically. It seems to be much worse below 50 MPH. The high beams will not shut off for approaching cars or it they might start flashing on and off. I have had multiple cars flash their high beams back at me. They will also not shut off consistently while following vehicles. Blinding oncoming traffic certainly has to be considered a safety risk.

I am reporting a catastrophic component failure on my 2024 Ford F-150 equipped with the 9.75-inch heavy-duty 3/4 float rear axle design. Both the driver-side and passenger-side rear axle hub bolts have completely sheared off due to metal fatigue. The bolt heads are missing entirely, and the threaded studs remain snapped off flush inside the wheel hub assemblies

While traveling home on a 2 lane highway 55 on a busy Sunday, the truck started misfiring and jerking, shaking then stalled. Engine light was flashing and we were able to pull off to side as it stalled. After a short time we proceeded on the highway, as we were in a precarious position, about 3 miles and it began misfiring and stalled again. This could have been a tragic situation had there not been a pullout with our travel trailer. We had to have our truck towed to the ford dealer and our RV towed home. While seeking similar cases online we believe it to be a fuel pump driver module and contamination. Our truck VIN is not currently on the national recall list and feel it should be.

- #11769136October 5, 2026UNKNOWN OR OTHER, SERVICE BRAKES, POWER TRAIN
I purchased a used 2023 Ford F-150 PowerBoost on April 3, 2026. Within days, the vehicle began making a hard clunk during initial acceleration and when stopping. It also hesitates before accelerating, especially while transitioning between electric and gasoline operation. At times, pressing the accelerator does not produce an immediate or predictable response. The problem is worse when accelerating uphill. This creates a collision risk when entering traffic, crossing intersections, merging, or driving uphill. The clunk can be heard and felt through the vehicle. There is also a loud, high-pitched whine that follows engine speed and certain gear transitions and can become extremely loud at highway speeds. On April 22, a Ford dealer confirmed the clunk during acceleration and stopping. The dealer found excessive movement in the rear leaf-spring and shackle bushings and replaced them, but the clunk and hesitation continued. On May 11, the dealer confirmed the high-pitched whine, traced it to the Hybrid Drive Unit, consulted Ford’s escalation team, and replaced the unit. The whining, clunking, shifting irregularities, and hesitation remained and have progressively worsened. Another Ford dealer later inspected the vehicle, but its road test was fewer than two miles, remained below approximately 30 mph, and did not include highway driving, uphill acceleration, merging, or entering traffic. The conditions causing the most serious symptoms were therefore not evaluated. Associated warning lights or messages are UNKNOWN. The vehicle is now parked because I no longer consider it safe to drive. It is available for inspection upon request.

On 10/5/2026 the front part of the sunroof on my Lightning burst into pieces and fell onto me and all over my cab while I was driving down the road. I had glass pieces and shards all over me and the front of the truck. I've read online that some other have also had issues with the same component. There was no warning. It wasn't hot, it was around 50degrees. I can't say there wasn't an existing crack or rock fracture, but I didn't find any evidence of rocks in the debris. I also can't say if I got popped with a rock, but since so many others have had the same issue, I felt it needs to be reported. This is very dangerous to happen while you are driving.

Component Affected: Power Train / Electrical System / Transmission Description of Incident: My 2016 Ford F-150 was taken to a licensed Ford dealership to resolve Safety Recall 26S28 / NHTSA Campaign 26V237000 (PCM software update to prevent sudden downshifts). Prior to the recall, the truck drove fine with no issues. Immediately after the software update was performed, the vehicle suffered a catastrophic, uncommanded downshift from 4th gear into 1st gear while traveling at highway speeds. This caused the rear wheels to violently lock up, mimicking a sudden slam on the brakes in active traffic and nearly causing a severe rear-end collision. Diagnostic scans show codes P0720 and P0722, indicating a failure of the Output Speed Sensor, which is integrated into the transmission molded lead frame assembly. The recall software remedy actually caused this life-threatening downshift, and the vehicle remains entirely unsafe to drive. The manufacturer's remedy did not resolve the safety defect. Ford has refused to repair this safety issue and say that it only qualifies for free repair if it throws codes P0705, P0706, P0707.

My truck has the 3.5 ecoBoost twin turbo engine with the known cam phaser problem. Ford dealership replaced cam phasers 4 years ago and told they were putting in new designed cam phasers to prevent this from happening again. Last week, I changed the v v t cylinder because of a p0018 code, and found that the replacement was the old cam phasers prone to break again. Truck is just now out of warranty. And ford is doing nothing to help. Drivability is not the same has hesitation. So far, and I'm sure it's going to get worse. Check engine light just went on last week. This truck is at dealership now. My biggest problem with this situation is Ford. Dealer lied said they put in new cam phasers, which they put in old ones. I would have got an extended warranty if I knew they were the old cam phaser. Now, I will be out 3 to 5 thousand dollars because ford dealership lied to me. I have all paperwork. Service reports everything on the truck. The truck was babied. There's no reason why in 70000 miles catastrophic engine failure twice. Fixing the problem with the same problem. Parts is not a fix. It's a bandade until warranties are over. And let the people deal with it.
