Erratic shifting, sudden down shifts while at highway speed, and transmission suddenly stuck in 3 or 5th gear, ford currently has customer satisfaction program 26n04 covering certain 2015-2017 ford f150 trucks but specifies only certain codes to flag the need to fix the problem by changing the molded lead frame which is the defective part. Unfortunately my truck is not tripping those codes but it is showing codes p0715,p0717,p0720 that all call for the molded lead frame to be changed. The problem is codes, p0720,0722, where covered in previous recall 24S37, for f150 tucks made in 2014 with the exact same transmission and molded lead frame. So my problem is why in the previous recall where these codes acceptable and all of the sudden now there not

My F-150 previously experienced an unintended tailgate opening while the vehicle was being driven. I brought the vehicle to Sugar Loaf Ford on May 14, 2025, specifically because of this problem. According to my repair documentation from that visit, Sugar Loaf Ford performed extensive work addressing the unintended tailgate-opening condition, including: •Installed a relay jumper harness •Installed a bypass harness •Installed a body-length harness •Replaced the tailgate switch •Reprogrammed the Body Control Module (BCM) •Changed BCM parameters Despite this extensive repair, the vehicle has now experienced another unintended tailgate opening while being driven. The recent incident resulted in the loss of property from the truck. Most significantly, a Remington Model 552 Speedmaster .22-caliber rifle with a 3x9 powered scope and its hard case were lost. Firearm identification: •Make: Remington •Model: 552 Speedmaster •Caliber: .22 •Serial Number: 1443879 Upon parking the vehicle at my home, I discovered the tailgate was open and that the rifle and its case were missing. I immediately backtracked my route in an attempt to locate the firearm but was unable to find it. I then called my son, who lives very close by, and together we backtracked the route a second time to no avail. Following this second unsuccessful search, I immediately reported the firearm loss to local law enforcement and filed a police report. I sent a certified/return receipt letter explaining all of this to FORD MOTOR CO. Customer Relationship Center on 9-24-2026 (Receipt was acknowledged), and hand delivered a letter, on 9-24-2026, addressed to Mike Harrison, Parts and Service Director at Sugar Loaf Ford Inc. To date, neither Ford Motor Co. or Mike Harrison has contacted me to resolve this issue. NOTE: Winona County Sheriff (about 4-5 days afterwards) did find the gun and it's case and returned it to me. But I do NOT trust the tailgate to hold securely hold my property.

My vehicle is experiencing the exact severe safety risk detailed in NHTSA Recall 26V237 (Ford Campaign 26S28) - dropping gears and risking a crash. The dealer technician confirmed the lead frame has physically failed but is denying the remedy due to a technicality over ancillary speed sensor codes embedded in that very part.

Vehicle had mechanical failure in the transmission at 70 mph. The PCM shifted my transmission in to 1st gear and locked up my rear tires at 70pm, which is covered under NHTSA 26V237000. Ford has an open recall: 26S28 for a software update, which is currently paused. They also have 26N04 to replace to failing lead frame within the transmission. Ford is refusing to repair the vehicle under 26N04 (hardware) while 26S28 (software) is paused, which are two separate issues. A software pause should NOT invalidate a physical component failure that is explicitly covered under an active safety mandate.

Our 2016 F-150 experienced a dangerous transmission sensor failure (Ford Service read code P0722). Ford's 26S28 safety update was performed prior to this, but the physical lead frame has now failed. Ford Corporate (Case #CXH-09620415) is refusing to replace the defective safety component under companion program 26N04 because the vehicle threw code P0722 instead of the four codes arbitrarily listed on their bulletin, leaving a known mechanical defect active on the road. In this case we pulled out from a driveway onto a road and the truck stayed stuck in 1st gear or was in limp mode and the check engine (wrench icon) lit. Had this been a busy intersection we could have been hit by another vehicle. With this part failing, which is known to cause other similar problems, the truck can shift erratically or get stuck in one gear or downshift or suddenly go into limp mode from any speed. Any problems associated with the defective lead frame should be fixed not just those that throw certain codes.

The contact owns a 2021 Ford F-150. The contact stated that while driving at an undisclosed speed, the check engine warning light illuminated, and a alert was received through the mobile application. The vehicle was taken to a dealer, where a diagnostic inspection revealed that the knock sensor failed and required replacement. The dealer informed the contact that water intrusion on the intake manifold caused the knock sensor failure. The vehicle was scheduled for repair at the dealer. The contact attributed the failure to the factory omission of the engine cover and the cowl design directly above the engine compartment, which allowed water to collect on the manifold. The manufacturer was notified via telephone, and a case was filed. The failure mileage was approximately 64,900.

I own a 2014 Ford F-150 equipped with the 6R80 six-speed automatic transmission (VIN: [XXX]), currently at approximately 133,000 miles. On XXX, while driving at ~70 mph on [XXX], my truck experienced an abrupt, unintended downshift — the precise failure mode described in Recall 24S37. The software patch prevented it from making it worse and a orange wrench came across the dashboard, but still it required some evasive maneuvering. I proceeded directly to the dealer to investigate; however, they were booked for the day. Not understanding the full impact I left to come back in the morning. The vehicle eventually became unsafe to operate, would not start, and had to be towed to Gullo Ford. The dealer then pulled diagnostic trouble codes, confirmed the transmission lead frame had failed, and replaced the lead frame at my expense of $1,977.04 (2127.04 with a coupon of 10% off) (invoice and repairs completed enclosed). The truck randomly hard downshifts and the gauges stop working and it wont shift back out of third gear. I can stop the truck and shut it off and when I turn it back on it still will be in third gear. If I turn it to manual sometimes I can get it to go down to second but it is a hard shift and then will hard shift back into third and stay. A wrench light comes on and sometimes I will get a check engine light that reads when scanned "output shaft speed sensor circuit." It has been doing this sporadically for over a year since my recall was fixed.

I put my truck in reverse to get out of a driveway and it kept going and accelerating...the brakes did not work. I ended up goign through a fence at the end of the street and into a shed and my truck finally stopped. I have significant damage to my truck as well as to the homeowners property...the Police classified it as equipment failure and I did not receive a ticket.

I own a 2022 Ford F-150 FX4 with the 5.0L engine and 10R80 automatic transmission. The vehicle experiences repeated severe and unexpected transmission malfunctions, including extremely harsh upshifts and downshifts between multiple gears. The most serious safety concern occurs when the transmission unexpectedly locks or abruptly engages a lower gear while the vehicle is in motion. During these events, the rear drive wheels can momentarily lock or skid, causing loss of traction and affecting vehicle control. This condition has occurred while slowing for intersections, making turns, and during normal driving conditions. On multiple occasions, the transmission's sudden downshift or lockup was severe enough that the rear tires chirped, skidded, or lost traction while the vehicle was moving. This created an immediate concern for loss of control and increased crash risk. The transmission frequently slams into lower gears when decelerating and approaching stop signs or traffic lights. The condition is unpredictable and creates a significant safety hazard because it can cause the vehicle to lurch, skid, or lose stability in traffic. The vehicle has been inspected and repaired previously by a Ford dealership; however, the condition persists. I am concerned this defect could lead to a crash, especially on wet roads, during turns, or in situations requiring controlled braking and steering. I am requesting that NHTSA investigate this issue and determine whether similar complaints involving the Ford 10R80 transmission indicate a broader safety-related defect.

I responded to recall 26V237 by bringing my F150 into our local Ford dealership, Lenoir City Ford, in Lenoir City, Tennessee, on [XXX]. At the time the truck was dropped off for the recall fix, it was running and operating normally, without any issues. I picked up my truck on [XXX] after the dealership applied the reprogramming fix. As soon as I pulled out of their parking lot onto the main road, the truck started acting up: erratic shifting, delayed shifting, spontaneous downshifting, lack of response from acceleration, and loud rattling noises. I immediately returned to the dealership and had the service technician ride in the vehicle with me. She asked me to turn around immediately and had the service manager ride in the vehicle. He also immediately told me to turn around and return to their parking lot, saying the vehicle was unsafe to drive. The only way to got the vehicle back to the parking lot was to use the manual shift mode and shift with the paddles. As of the date of this filing, the truck remains in the possession of Lenoir City Ford and is undriveable. The Ford dealership has completed all troubleshooting requirements with Ford Motor Company. It has been determined that the transmission needs to be replaced. Ford Motor Company is refusing to make the necessary repairs to the vehicle that their software fix created. The dealership continues to try to resolve this through their channels, and I keep reaching out to Ford Motor Company to resolve it. I have a case file started with Ford, [XXX]. Since this started, Ford Motor Company issued an internal memo to all dealerships instructing them to stop performing the recall software fix, which is under investigation. See attached. The vehicle is still in Lenoir City Ford's possession. On October 14th, the dealership will have been in possession of it for one month. I am reporting a safety-related transmission problem I experienced with my 2022 Ford F-150 equipped with the 10R80 automatic transmission. While driving normally, the transmission suddenly slipped in 2nd and 3rd gear. The RPM went up, but truck did barely move. This occurred while going uphill in Ctrl. Florida when turning into a side road, creating a potentially dangerous situation because the truck did not respond as expected when I needed predictable acceleration or propulsion. I believe this incident may be related to the known 10R80 CDF clutch-cylinder/sleeve problem. Ford technical information identifies axial movement of the CDF sleeve that can cause hydraulic-circuit leaks and harsh or delayed engagement and shifting. This condition can result in slipping, RPM flares, delayed engagement, unexpected gear changes, harsh downshifts, lurching, or loss of propulsion. This is a safety concern, not merely a transmission quality or reliability issue. Unexpected loss of propulsion or unpredictable shifting while merging, accelerating into traffic, crossing an intersection, turning, or traveling at highway speed can create a collision hazard. Because I personally experienced this unexpected transmission event, I request that NHTSA investigate the 10R80 CDF clutch-cylinder/sleeve condition in Ford F-150s. Please review Ford warranty claims, dealer repair records, transmission replacements, consumer complaints, and field-failure data to determine whether this is a safety defect requiring a recall. I believe owners should not have to wait for a potentially dangerous transmission failure before this known condition is addressed.

The vehicle is covered by Safety Recall 26S28 (NHTSA 26V237) for a transmission range sensor signal fault causing unintended downshift. Before the recall, the truck was slipping, shifting improperly, and downshifting unexpectedly. Dealer (Wayne Akers Ford, Lake Worth, FL) solicited the recall by text on [XXX] and performed the PCM reprogramming on [XXX] (RO [XXX]) at 201,991 miles. The repair order shows no DTC scan before or after the reflash, no road test, and identical mileage in and out. The same symptoms returned the day of pickup. Owner returned promptly and was told the dealer was too busy to accept the truck. The condition worsened to a sudden unintended downshift and deceleration while driving in traffic, after which the owner parked the truck for safety. Owner reported the worsening condition by text on or before [XXX] and brought the truck in on [XXX] where he was told a diagnostic fee would be required to look at the recall condition. The dealer now says the codes are “similar but different” from the recall codes, that the condition is outside coverage, and quotes about $6,500, with no written diagnosis, code list, estimate, written denial, or SSSC submission under Program 26N04. No crash, fire, or injury. Vehicle remains unrepaired at the dealer. The owner reports this because the 26V237 remedy performed did not correct the unintended downshift condition, and the dealer’s process may have cleared the codes that would have qualified the truck for no-cost leadframe replacement. Vehicle: 2026 Ford F-150, White 1. Component or system malfunction: The steering system appeared to malfunction while I was driving, causing serious concerns about my ability to safely control the vehicle. The vehicle is available for inspection upon request, subject to its location and availability. 2. Safety risk: I experienced a near-accident while driving because of the steering problem. I believe this malfunction presents a significant risk of losing control of the vehicle, potentially causing serious injuries to myself and other motorists. 3. Dealer confirmation: The vehicle was taken to the Ford dealership for diagnosis and repairs and remained there for approximately three weeks. I received repair history records, but I have not received sufficient documentation confirming that the steering problem was permanently resolved. 4. Manufacturer or other inspections: The vehicle has been examined by the Ford dealership's service department. Whether Ford Motor Company independently inspected the vehicle is UNKNOWN. No police or insurance inspection has been confirmed. 5. Warning lights or symptoms: The steering malfunction became apparent while I was driving. Whether warning lights or dashboard messages appeared is UNKNOWN. Incident summary: I leased this 2026 Ford F-150 as a new vehicle, although it already had approximately 1,009 miles at delivery. Shortly afterward, I experienced a serious steering-related malfunction that nearly caused an accident. Despite the dealership's involvement and repair attempts, I remain concerned about the vehicle's safety and do not feel safe driving it. I respectfully request that NHTSA investigate this potential steering safety defect.

1.The component that is failing & causing a safety hazard is the vehicle transmission. 2.The transmission shifts violently at highway speeds & causes partial loss of control by the driver & potential additional damage to the powertrain; dashboard wrench light (powertrain warning light) goes on when this happens. 3.The problem has been confirmed by Ford Motor Vehicle Customer Satisfaction Program 26N04 (safety recall notice) the vehicle owner received in a written letter from Ford in July 2026. 4. The Ford Dealership at Liccardi Ford, 1615 Route 22 West, Watchung, NJ 07066 908-561-7500, arranged for a mobile repair on 07-21-26 (Recall Repair 26S28). The repair was made but the condition was not corrected. 5. The vehicle was brought into Liccardi Ford dealership on 09-13-26 for further service to correct the powertrain fault consistent with the Safety Recall Notice (customer Satisfaction Program 26N04). 6. The Dealership did nothing to correct the problem and in part, stated on their work order, "PERFORMED DIAGNOSIS RETRIEVED CODE PO219 ENGINE OVERSPEED CONDITION, PO61A INTERNAL CONTROL MODULE TORQUE PERFORMANCE, PO722 OUTPUT SHAFT SPEED SENSOR - NO SIGNAL - NOT RELATED TO 26N04 - NEEDS FURTHER DIAGNOSIS. 7. The vehicle owner was told by the Ford Dealership that they would not correct the safety recall problem (evident by the powertrain failing in a manner consistent with the recall notice and the powertrain warning light) without further diagnosis at a cost to the vehicle owner of about $220/hour for diagnosis plus any costs for service work. 8. This failure by Liccardi Ford to correct the hazardous powertrain safety problem consistent with Customer Satisfaction Program 26N04 regardless of what codes were found (or not found) without invoicing the vehicle owner is an unconscionable bait-and switch dereliction of Ford Motor 9. I humbly ask the NHTSA to require Ford to make the repair without cost to the vehicle owner, [XXX] [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5

My daytime running light has burned out. I also see a lot of other f150 around town with the same exact problem. I also don’t want to get stopped from the police for having a bulb burned out. I have researched this problem and you can’t fix it unless you buy a whole brand new headlight assembly for $1500.
