While driving, my 2022 Ford F-150 equipped with the 2.7L EcoBoost experienced abnormal engine operation, a check-engine light, and reduced engine power. The unexpected reduction in available engine power created a safety concern because loss of acceleration or propulsion can be dangerous when merging, crossing traffic, passing, or traveling at highway speeds. The vehicle was inspected and diagnosed by an authorized Ford dealership. The PCM stored DTC P0332, Knock/Combustion Vibration Sensor “B” Circuit Low. The dealer performed Ford Pinpoint Test DG. Testing reportedly found no open circuit, short to ground, short to voltage, or intermittent circuit fault. The dealer determined that the knock-sensor assembly required replacement. The repair involves replacement of Ford knock-sensor assembly FT4Z-12A699-E, which contains both knock sensors. Access to the assembly requires extensive disassembly of engine fuel and cooling-system components. Ford’s service procedure includes removal of both direct-injection fuel rails, draining the cooling system, removal of the coolant pump, and removal of the coolant outlet connector.I am concerned about the possibility of coolant leakage or contamination affecting the knock-sensor assembly because of its location in relation to cooling-system components. I do not currently know whether coolant contamination caused this particular failure. I request that NHTSA investigate whether coolant leakage or contamination in this area contributes to premature knock-sensor failures on 2.7L EcoBoost engines. I have also identified another 2022 Ford F-150 with the 2.7L EcoBoost that was diagnosed by a Ford dealership with a similar knock-sensor circuit-low fault, P0327 also quoted replacement of the same FT4Z-12A699-E assembly. I request investigation knock-sensor failures on 2.7L EcoBoost F-150s, including Ford warranty/field data and whether coolant leakage or contamination contributes to failures capable of causing unexpected reduced engine powe

- #11760105August 27, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Adaptive Cruise Control and/or Camera System misreads speed limit signs. 1. System misreads speed limit signs by acquiring information from State Route sign. Example: SR248, Speed Limit 55mph, SR248 marker 20, Reads it as 20mph. Example: SR2004, Speed Limit 45mph, SR2004 marker 60 and 70, reads it as 70mph. 2. System misreads Truck Speed Limit Signs as vehicle Speed Limit. The problem occurs when both Speed Limit and SR signs are in close proximity to each other. Vehicle is available. The system if engaged, has the potential to accelerate or decelerate as much as 35mph in these examples, causing potential loss of control because of rapid acceleration or deceleration. The vehicle has been at the dealer twice and they have not been able to replicate. They may not have been able to replicate because of sign placing in their area. Under these conditions, the Adaptive Cruise Control is a safety hazard. I have it disengaged and is useless.

I was driving and went to press the brakes and the pedal went to the floor. The truck would not stop right away and then the rear tires locked up. Light came on the dash ABS, service traction control and brake system failure, this was a similar issue with the 2021 and 2025 model having issues with the EBB/ECU that was on recall.

- #11760024August 27, 2026FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS, BACK OVER PREVENTION: WARNINGS
While maneuvering the vehicle to park near a building, the truck unexpectedly accelerated forward and crashed into the building. I did not intend for the vehicle to accelerate into the structure. During the incident, I did not receive the expected assistance from the vehicle’s front sensing/collision-warning systems before impact. The vehicle struck the building, and no airbags deployed. Because of the nature of the incident, I am concerned about a possible malfunction involving the accelerator, braking system, electronic controls, driver-assistance systems, front sensors, or associated vehicle software. I am also aware that Ford previously issued Customer Satisfaction Program 24B56 and related Special Service Messages concerning inoperative front parking sensors on certain 2022–2024 F-150 Lightning vehicles

- #11759546August 25, 2026ENGINE, ELECTRICAL SYSTEM, POWER TRAIN
While driving at speed, my vehicle experienced a severe safety hazard: the transmission jerked violently and the engine completely stalled on the road, causing an immediate loss of power. The dealership diagnosed the truck and stated that tree debris had plugged the cowl drains at the base of the windshield. Because the drains plugged, rainwater backed up and overflowed directly into the passenger cabin, soaking the Body Control Module (BCM) and crashing the central computer network. A vehicle's central computer network should not be engineered in a way that normal outdoor tree debris can completely flood an interior cabin module and cause a dangerous drivetrain shutdown while driving.

- #11759473August 25, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Vehicle Information 2021 Ford F-150 XL Supercrew approximately 35,000 miles. While traveling southbound on [XXX] near [XXX], Ok, I was operating my 2021 Ford F-150 XL at approximately 75 mph with the cruise control engaged. As I approached slower traffic ahead, I attempted to disengage the cruise control by pressing the cruise control cancel/off button on the steering wheel. Despite pressing the button multiple times, the cruise control would not disengage. Same thing while tapping the brakes it would not disengage. The vehicle continued to maintain speed, and the cruise control system remained unresponsive to my commands. In response, I applied the brakes to safely reduce speed, I did not perform a panic stop or abrupt braking maneuver. Once I had the vehicle slowed, I moved safely onto the shoulder of the highway, well away from traffic. I then shifted the vehicle into park, shut off the engine, and restarted the truck. After restarting the vehicle, I elected not to use the cruise control system for the remainder of my trip, which lasted approximately eight additional hours to Houston, Texas, I have not used the cruise control since the incident because I am concerned that the condition could recur and create a safety hazard. No collision, property damage, or injuries occurred. However, the apparent failure of the cruise control system to disengage when commanded raises significant safety concerns and warrants investigation. - #11759360August 25, 2026UNKNOWN OR OTHER
Within the last 6 months, the driver's side front door, the driver's side rear passenger door, and the passenger side rear doors actuators stopped working and the doors do not lock. The doors do not lock manually with the key fob or lock automatically when the truck reaches 13 mph. This is a safety issue as passengers could inadvertently open the doors when driving when the doors should be locked. This also prevents the factory installed alarm to work properly. it seemed odd that the 3 actuators seem to stop working around the same time.

As I was stopping at an intersection for a redlight, the brakes locked up and the collision assist warning and another message came up on the screen. There were no cars in front of me at the intersection. This is the first time that this has ever happened and the truck has 32,665 miles. No servicing has been performed on this truck beyond oil changes and tire rotations.

- #11759245August 24, 2026FUEL/PROPULSION SYSTEM
I own a 2021 Ford F-150, VIN [XXX]. NHTSA recall 25V455 / Ford recall 25S75 affects my vehicle because of a potential fuel-pump failure that can cause loss of fuel pressure and an engine stall while driving. The recall was issued XXX, and my vehicle still shows an unrepaired recall with the remedy not yet available. I am concerned about the length of time that has passed without a repair being available for a safety-related defect that could cause the engine to stall while driving. I am requesting that NHTSA review the continued delay in providing a remedy. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My 2021 F-150 has been diagnosed with a failed electric steering gear Code PSCM C1B00 49 2F. I understand there was a recall on some 2021 trucks with similar steering issues, although my VIN is not included. The part is on national backorder and my dealer says there are approximately 150 vehicles ahead of mine. This is a safety-related steering component, part number "ML3Z3504G"

My truck was parked on a slight slope in my driveway. I got in to move the truck to my normal parking space. When I started the truck I noticed the emergency brake was on. I put the truck into drive- the truck accelerated with my foot on the brake. The truck struck the corner of my deck and didn't stop until I hit the corner at my house. I had the brake pushed to the firewall and the truck would kept accelerating. Thankfully the airbags did not deploy. Significant damage was done to my house and my truck.

My vehicle was left rendered unable to steer during use. It was taken to a local shop All Japanse Auto, who found the damage was consistent with Ford Saftey Recall 21S28. Upon speaking to AutoFair Ford of Haverhill service they informed me Ford did not include my truck in the recall despite it matching year and model and they would not be addressing the issue for me. Loss of steering mid use is amazingly careless and should be cause for taking more aggressive action
