When driving at night, there is a shadow in the headlight, that shines onto the road and makes driving at night difficult. This shadow looks like it comes from a tall screw or bolt in both headlights. The lights work correctly, but the shadow causes the driver to have a dark "spot" in the beam of light, which then makes it hard to see clearly while driving in dusk, or darkening circumstances. I took the vehicle in to the Toyota Dealer where I bought it and had them investigate this. They said they couldn't repair it, as it was part of the construction of the headlight itself. The Toyota repair people did see the shadow when the lights were turned on to face a wall in a darkened garage bay.

I was originally notified that there would be an option to fix this by the third quarter of 2024. However, that was not the case. On December 26, 2024, I contacted the service center at Sam Leman Toyota in Bloomington, Illinois by phone, and they confirmed that there is still no remedy for repair.

I am writing to formally file a complaint regarding the ongoing issues related to safety recall 23TA15 for my 2021 Toyota Corolla LE. The recall, issued in December 2023, involves a defect in the Occupant Classification System (OCS) sensors in the front passenger seat, which may have been improperly manufactured. Unfortunately, Toyota has failed to address this recall in a timely manner, significantly impacting my ability to use my vehicle safely and as intended. My local dealership has confirmed that my car is affected by this recall and that the OCS sensor is not functioning. However, despite the recall being issued nearly a year ago, the dealership has informed me that they are unable to obtain the necessary parts to repair the issue, and they do not expect to receive them until at least the end of December 2024—if not later. As a result, I have been unable to use my car's passenger seat in a safe and fully functional manner, effectively rendering the car unsuitable for my needs. This prolonged delay in resolving a critical safety issue is unacceptable, and I am concerned for my safety and the safety of others until this issue is resolved. I kindly request that the NHTSA investigate Toyota’s handling of this recall and take appropriate action to ensure that affected vehicle owners, including myself, are provided with timely repairs and necessary parts. I am also requesting clarification on any steps I can take to expedite the repair process. Thank you for your attention to this matter. I look forward to your prompt response.

We were driving on [XXX] near Norcross, GA about 200 ft from exit [XXX] at a constant speed of around 70 mph, when suddenly the vehicle displayed 'system malfunction' and started veering on its own and the brakes wouldn't work. It kept veering to the right and crashed into a vehicle 2 lanes away and rebounded back to the original lane and came to a halt with a message displayed 'Shifting to P'. The malfunction and loss of control, along with the brakes not functioning those few seconds/minutes put our lives and others' on the road at risk. The vehicle is available for inspection. The vehicle was towed on the day of the accident and then taken into a Toyota dealership and is currently there. The vehicle has not yet been repaired. Toyota sent a third-party inspector to gather data from the vehicle but that turned out to be merely the crash data up to 5 seconds prior by which time the malfunction had already occurred and the vehicle speed was down from 70mph to 31mph. The contact's daughter owns a 2025 Toyota Corolla. The contact stated while her daughter was driving at 60 MPH and switching lanes, the driver noticed a stopped truck on the road. The contact stated that the driver depressed the brake pedal to stop the vehicle, but the vehicle made an abnormal squealing sound, lurched forward, and failed to respond. The driver depressed the brake pedal with both feet, and the vehicle decelerated but failed to stop as needed, causing the front passenger's side of the vehicle to drive under the rear of the unoccupied truck and to crash into the truck. The air bags failed to deploy. The contact's daughter was able to exit the vehicle. The check engine warning light and other unknown warning lights were illuminated after the crash. The vehicle was reversed out of the rear of the truck and was driven to the driveway of the residence, where the engine failed. The contact stated that the vehicle had failed to start and was undrivable. The contact's daughter sustained whiplash, body soreness, lower back pain, a seat belt mark and bruises under the chest. The contact's daughter was later taken to the ER, where she underwent CT scans and X-Rays, but no further injuries were found. A police report was filed. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 500.

The safety of any passenger in the front seat is potentially affected as the defect can either cause the front passenger airbag to fail to deploy in an accident or cause it to deploy when it shouldn't, either way causing harm to the passenger in the front. This probem was widely reported in the press in 2023 and 2024. One article I read said that Toyota claimed it would start issuing the recalls for repair in Feb 2024 but that still had not happened by the time I bought my 2021 Corolla in April 2024. I bought it as a used car from a local dealer who shared the info about the recall which was on the CARFAX report too. I am reluctant to have family or friends in the front passenger seat because of the risk of injury in the case of an accident of accidental deployment of the airbag. This reduces the value of my car significantly, as it is required by law to be safe for my passengers as well as myself. Very disappointing behavior from a company that used to be reliable.

The engine, brake collision system, fuel pump all went out. Car was rough idling for about two weeks before the engine completely failed. Toyota has lemoned it.

[XXX] In reading the above article, Toyota recalled Corolla Cross Hybrid vehicles with many complaints and situations mirroring our accident. Before the accident, the vehicle displayed no warning signs. Despite the driver pressing the brakes and the assisted braking feature seemingly turning on (e.g., signal lights on the computer system), neither seemed to work in this situation. The damage was far too severe for the assisted braking feature to have activated, along with the driver pressing the brakes. After viewing the matter with those involved in the accident, many of us concurred that we believe either the braking system or the computer system failed. In the aftermath of the accident, the police, manufacturer, or third party hadn't reviewed the vehicle's status. On top of that, our insurance refused to investigate, urging us to discuss the matter with Toyota.
