While moving at a very slow speed parking the vehicle in my driveway, I was slowly turning to the left. The vehicle accelerated suddenly and quickly. This cost the front end to strike the deck and steps to the deck of my home. It also damaged the lower front end of the vehicle.

- #11687045September 13, 2025FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
At low speeds, the cruise control engages, causing the vehicle to accelerate. Stepping on the brake disengages the cruise control as it normally would. I have had this leased vehicle since March 2015. I have reported the issue 4 times to the dealer. Each time they interrogate the computer and tell me there are no errors. I have researched the internet, and there are numerous complaints about this potentially life-threatening issue. "Point of interest." This is the same dealer who insisted my BMW 7 Series had nothing wrong when it was burning a quart of oil every ten days. Eventually, the engine was no longer manufactured. I'm taking the car back next week; they are taking the issue to BMW.

On November 16, 2023, I was driving the vehicle in the far left lane of the freeway when, without any warning, the vehicle lost propulsion and would not accelerate. It also lost power assisted steering AND power assisted braking. I had to decelerate with no power brakes while attempting to safely cross 3-4 lanes to the nearest freeway exit with no power steering, while maintaining enough momentum to not get stuck in the middle of the freeway. After it lost propulsion, the several warnings flashed on the screen noting that the vehicle stabilization system was disabled, "vehicle may roll", the tire pressure monitoring was disabled, and the restraint system monitoring was disabled. I managed to steer the vehicle safely to the bottom off the offramp but had to bring the vehicle to a full stop. As a result, I was stuck blocking traffic at the bottom of the offramp. I attached a few photos showing what was happening but have additional video documenting the loss of power steering and power brakes but they are too large to attach here. The screen stated the roadside assistance feature was unavailable so I used the emergency button on the interior roof, which connected me to BMW's call center. The representative requested that the fire department assist with moving my vehicle out of traffic. We had to towed to the dealership (see attached invoice). We were told it only needed a software update which was then done. We still kept getting the same high voltage warnings, leading to 3-4 additional repair invoices. Finally BMW offered another software update in May 2025. We still receive high voltage warnings, drivetrain warnings, and chassis warnings and are afraid it will shut down again on the freeway. Software updates seem to be a bandaid. The loss of power braking and steering while doing 70 mph was a terrifying experience that could easily lead to serious injury and death.

- #11673410July 14, 2025AIR BAGS, FORWARD COLLISION AVOIDANCE: WARNINGS, POWER TRAIN
Yesterday on 7/13, my family and I were fully stopped and parked in a parking spot (car was in park, to the best of my knowledge). The front passenger had just exited the vehicle and was opening the back door where my kids were just beginning to get out of their seats. At that moment, something occurred that caused the car to speed over the parking curb, across a pedestrian sidewalk and struck a brick and glass building storefront. What's odd is, assuming the car was in park, which i believe it was, the car accelerated to max speed and the airbags did not deploy and the hazard lights never turned on after the accident. In addition, the car has safety features designed to detect objects and avoid collisions and should switch the car to park when the car is stopped and doors are opened. I am happy and motivated to make the car available for inspection immediately. I really want to understand what happened and why for my own benefit (and peace of mind) but mostly because I don't want this to happen to any one else in the car or pedestrians in the area. Had my car been parked just a little further down the block, families were eating lunch at an outdoor restaurant which could have ended in serious injury or worse. My car is currently in the tow yard that the police department picked. My insurance is asking me where to tow and I want to make sure it ends up somewhere that can fully diagnose what happened based on the car's internal computer (if possible). The car has been seen by my local police department and fire department that came on site. But, no formal inspection has occurred yet. But will likely be done this week. No warning messages were shown on the car prior to the accident and potential malfunction.

BMW has issued an update to their software. There is a setting on the car where you press auto H and if you press the break, it will stop as if it’s parked now. This software update has a bug and nkw this feature turns off even though you have it set, so you think you’re parking and you press the brake and lift your foot which should put it into park and the car keeps moving and you hit other cars or people or anything. It is extremely dangerous.

On [XXX] I went to start the car and received warnings about Manual Braking being engaged and to contact roadside service along with other notifications and the car wouldn't power on. After numerous attempts the car powered up and alerted me Drive Carefully Fault. I began to drive slowly through a parking lot and it took over 200 lbs of pressure to bring the car to a complete slow stop. I pulled over and turned off the car. I tried rebooting the car and did so assuming it was a bug in the system. When the car powered back up the flaw had vanished and I carefully drove home. A short time later when I turned the car on I followed the instructions and called roadside service and described my situation. To my surprise, roadside service had never heard of the problem before. I then contacted Rusnak BMW service center and talked them through what was happening. Once again I was surprised to learn that they too had never heard of the problem. I brought the car into Rusnak BMW on May 30th. Over the course of the next week and a half I was never given any concrete news about why the car had acted that way. In fact I was told that they had to elevate the situation to NEW JERSEY where the higher ups would review the situation. No concrete information was provided other than they drove the car for 11 miles and felt comfortable having me pickup the car. Admittedly, it was frustrating to be called to pickup my car not knowing why there was a problem with the braking system and being given no assurances it wouldn't happen again. As I went to leave the Rusnak dealership my car wouldn't start and the process began all over as it had on [XXX]. When I got the car to start the brakes were not reacting and I drove the car 100 feet to the front door of BMW front door and with over 200lbs brought the car to a stop. Rusnak BMW is once again working on a problem they seemingly dont know how to fix. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11661270May 15, 2025FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
My 2024 BMW iX xDrive50 intermittently fails to deploy the rearview windows upon startup. Restarting does not help; they open only after driving above ~25 mph. This typically occurred once or twice a month based on my recollection. I purchased the vehicle in late May 2024, so I had it for only five months before the accident. More concerningly, the vehicle sometimes shifts from Drive (D) to Brake (B) mode without input. I noticed this behavior several times before a crash on 10/27/24, and again after the crash. On 10/27/24, while in B mode, I slowed at an intersection with ~30 feet of space ahead. I was practically at a stop and began to inch forward when the car suddenly accelerated and I lost control, rear-ending the car in front. No automatic braking or warning systems activated. BMW North America inspected the vehicle on 12/20/24 and later concluded that no defect was present after a 6-mile test drive. After their review, my dealership also conducted its own inspection but was unable to replicate the issues I reported. However, neither review addressed—or could replicate—the intermittent switching between D and B modes, which remains unresolved and unpredictable. BMW also could not explain or resolve the issue of the rearview windows failing to deploy. I am concerned that both issues may reflect a broader software or user interface flaw. I am also concerned that BMW’s investigation focused primarily on establishing that I pressed the accelerator pedal before the crash, as a way to default blame for the accident to me, rather than investigating whether a fault in the vehicle’s electronic input or feedback systems could have contributed to the incident. Given the safety implications and the limited scope of BMW's investigation, I request a deeper independent review.

I was driving on Single Pedal Drive on a freeway with the 2024 BMW IX, there was a slow down ahead of me. I removed my foot from the pedal and waited for car to slow down, but car didn't slow down enough. Then I slammed the brakes hard, but the brake didn't engage. In fact, brakes were pushed back against my foot. I was slamming the brakes as hard as I could, but i couldn't feel the brakes letting me me bottom out . This was a very traumatic experience. I have been able to reproduce it once again, but not always. But BMW dealership doesn't want to accept any problem. Thankfully, I had slowed down, otherwise, this is a very dangerous problem. Please investigate this. This has caused a big cost burden for me and also quite a traumatic experience.

Summary of Incident – March 28, 2025 On March 28, 2025, at approximately 8:17a.m., I left my residence and proceeded to my vehicle, which was parked inside my garage. I opened the tailgate of the vehicle, placed my laptop in the trunk compartment, and securely closed the tailgate. Upon entering the vehicle, I observed that the dashboard screen displayed no safety warnings or indications that the tailgate was open or ajar. My key fob remained in my possession, located in my pants' pocket, throughout this time. I then drove approximately one mile from my residence. While executing a right turn at a traffic light, the dashboard screen suddenly displayed a message indicating that the tailgate was open. Looking through my left side mirror, I observed that my carry-on sized luggage had fallen out of the vehicle and onto the road. I promptly and safely pulled over, exited the vehicle, and inspected the trunk, where I found the tailgate halfway open without any apparent cause, I pressed the button on the tailgate to close it. I retrieved my luggage from the roadway, noting that it had sustained significant damage as a result of being ejected from the vehicle and impacting the road surface. Fortunately, there were no other vehicles in the vicinity at the time of the incident, and no injuries occurred. Subsequently, I reviewed footage from my garage security camera, which clearly showed that the tailgate was fully closed prior to my departure from the residence. Furthermore, at no point during my initial departure did the vehicle’s dashboard screen provide any warning that the tailgate was open or improperly secured.

UNINTENDED ACCELERATION The car accelerated unintentionally at 25 mph just before a red light at an intersection. It simultaneously started a bonging alarm, and showed a large red triangle on the dash telling us to pull over and to not drive the car thank God we were able to do before the intersection as we would have crossed a busy intersection at 30mph while we had a red light!!

On [XXX], while driving, the brakes on my vehicle unexpectedly seized, causing all four wheels to catch fire. This incident resulted in significant damage, including the burning of the brake lines. The vehicle was promptly towed to Wide World BMW in Spring Valley, NY, for repairs, as it is covered under the bumper-to-bumper warranty. Following the incident, I was informed that a team from BMW North America would inspect the vehicle due to the severity of the malfunction. I have since been advised that BMW North America is denying liability, claiming that third-party work may have been performed on the brake system. However, both Wide World BMW and I categorically refute this assertion. All maintenance on my vehicle has been exclusively performed by authorized BMW service centers, and there is no evidence to suggest that any third-party work was done. The damage being attributed to third-party work is, in reality, a direct result of the fire caused by the brake system malfunction. Wide World BMW has reviewed the case and agrees that BMW North America should assume responsibility for addressing this issue. - #11639666January 30, 2025VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
This incident it similar to NHTSA Campaign Number: 23V409000. On December 7, 2024, a 2024 BMW iX was traveling on a cul-de-sac street at low speed, preparing to come to a stop. The driver was turning the steering wheel gently when, without warning, the vehicle suddenly accelerated rapidly. The unexpected surge in speed left the driver unable to react in time to stop the vehicle. As a result, the BMW iX rammed into the side of a parked Ford truck. The impact was strong enough to push the Ford truck forward, causing it to collide with another nearby vehicle. No injuries were immediately visible, but the BMW and both other vehicles sustained noticeable damage. The manufacturer has inspected the car but still have not heard back. The incident occurred in a residential area with no hazardous driving conditions. The sudden, uncontrolled acceleration was highly unusual and led directly to the multi-vehicle collision.

- #11639433January 29, 2025FORWARD COLLISION AVOIDANCE: WARNINGS
I am reporting a serious safety failure in my 2024 BMW iX related to the Forward Collision Warning (FCW) and Automatic Emergency Braking (AEB) systems. Recently, my vehicle failed to detect a slowing car ahead, and the emergency braking system did not engage, resulting in a rear-end collision. This malfunction has caused financial loss due to repair costs and raises severe safety concerns, especially since my infant child was in the vehicle at the time. A functional AEB system should have prevented this accident. Given recent BMW recalls for braking system issues, I am deeply concerned that this failure is part of a broader defect. I urge NHTSA to investigate potential system defects in the 2024 BMW iX to prevent further accidents and protect lives. Please advise on the next steps for escalating this safety concern.

The brake pedal is way to close to the gas pedal, when I apply the brakes I often hit the gas pedal causing the car to jerk forward. I have reported this problem to BMW and to my local dealer. I took the car to the dealer and was told the car is fine. I have been driving for 40 years and have never experienced this problem nor have I ever reported a problem. However I am experiencing this problem on a daily basis. Other cars are at risk when both pedals are pressed at the same time. The car has been inspected at the dealership who found no problems.

While parking in a rather standard strip mall parking lot, it was discovered the car was too close to the car on the right. The car was backed up a few feet and braked to a stop. When the gear shifter was moved to drive, the car took off and struck the car in front at 13 mph, causing about $75,000 of damage to our BMW iX M60. BMW did a precrash report with data but no written summary or opinion. It was quite clear from the phone call on the crash that BMW would argue the cause was driver error.

Tire size on BMW IX 2024 model 275/40 r22 sudden lost of pressure psi to 0. I saw a one inch rubber separation just below rim on the car. The tire might have hit a curb at extremely low speed going onto parking garage. no marking of side impact that would explain the opening separation just below rim All warning system after failure functioned correctly. The tire is suspect for this vehicle as the possible problem being on a heavy EV type car.
