NHTSA Safety Complaint (Loss of Emergency Connectivity) Subject: 2025 Chevrolet Equinox EV — OnStar/telematics failure disables Emergency Services & Automatic Crash Response; part on national backorder VIN: [XXX] Make/Model/Year: Chevrolet Equinox EV, 2025, RS trim Mileage at first occurrence: 4000 Date problem first occurred: [XXX] Frequency: Continuous (services remain unavailable) Injuries/Crashes: None to date Component(s) affected: Electrical system — telematics/communication (OnStar); emergency calling/SOS; crash notification (ACR). Description of the safety defect: The vehicle’s OnStar/Connected Services display “Services Unavailable.” The dealer diagnosed a failed telematics module. Replacement is on national backorder with no estimated timeframe. While in this condition, Emergency Services and Automatic Crash Response are disabled—the SOS button does not connect, and the vehicle will not automatically contact OnStar after a crash. Parts availability (scope/delay): My dealership and multiple GM/Chevrolet and OnStar owner forums indicate 3+ month waits for the telematics module with no ETA, leaving vehicles without emergency connectivity for extended periods. What has been done so far: Dealer confirmed module failure and opened a parts order; advised no ETA due to national backorder. Manufacturer has not provided an interim fix or loaner vehicle. Issue remains unresolved as of Aug 11, 2025. Why this is a safety concern: Loss of emergency calling and crash notification defeats safety features marketed as critical for post-crash response. Owners cannot rely on the vehicle to summon help after a collision or medical emergency. Requested NHTSA action : Track this failure and parts unavailability, assess scope, and determine whether a safety investigation or recall is warranted. There is a very dangerous glare-reflection of the dashboard on the windshield whenever it is sunny. It is difficult seeing pedestrians, bicyclists and oncoming cars. We have had this car for a few days.

Discovered water underneath passenger footwell carpet, causing a musty and mildewy smell. Many reports of similar discoveries online prompted the initial search. Safety concern is mold growth and rust. Dealer was unable to replicate issue after two attempts (ran through car wash, hose over windshield, ran A/C for extended period, checked AC condensate drainage). Dealer dried up water and carpet and told owner to monitor. Owner pulled up carpet to do own investigation after the second failed attempt by the dealer. Confirmed dry before starting investigation on warm sunny day (no rain). Owner found leak in passenger footwell underneath black access plate that only appeared with A/C usage AND during/after driving, not while car was stationary. Owner brought vehicle back to dealer. Dealer removed black access plate and confirmed leak was coming from a compromised body seam weld. Dealer repaired the failed body seam and no water intrusion has been seen by owner since repair was completed. Owner still has concerns about underlying cause of where water was coming from - theory is that condensation is forming ON the the A/C lines and is pooling somewhere in the body then sloshing around while the car is moving, causing further potential for water related issues, including rust.

•Initial incident: July 2, 2025 •Recurring occurrences: August 8, 2025 Vehicle Issue Description: Several months ago, the vehicle’s check engine light came on. A diagnostic scan was performed at O’Reilly Auto Parts, but no error codes were detected. The vehicle was taken to an independent repair shop for further inspection. The shop found no mechanical issues and confirmed the vehicle was operating normally at that time. On August 6, 2025, the “Service Brake Assist” warning appeared briefly on the dashboard but turned off shortly afterward. The parking brake warning light also came on that same day, then went off. These warning lights reappeared intermittently on August 8, 2025. No repairs have been made as there are no active recalls related to these issues. However, it is noted that there are consumer complaints reporting similar problems. This raises concerns about potential intermittent or undetected safety system faults, particularly involving the brake assist system and parking brake functionality. Checked brakes again no issues determined per Bridgestone and Mavis.

On two occasions as I have pulled in to park this vehicle, while pressing brake the car lurched forward. The first time no damage happened however the second time damage occurred. While pressing the brake as hard as I could would not stop the lurching car. None of the avoidance warnings helped either.

At certain times of the day and depending on the lighting, the dashboard of my vehicle, has a very significant reflection that interferes with my ability to see through the windshield clearly. This puts myself and those in my path at risk of injury or death. I have contacted the person who sold me the car but he has not responded. I have read numerous online accounts of the same concern. GM is aware of the issue and chooses to do nothing.

We where driving to do some errands I noticed the engine light came on. Then the engine cut off. Took it to a garrage near by they did a scan said it is my fuel pump. I was told to check to see if there was a recall on this. I checked there is a recall on this problem. I called the number it said to call. Gave them my Vin. Number said my Vin number was not on the list. 2022 Chevy equinox LT. Why isn't it on the list it has the problem described on recall. Michael bethea dillon sc

What component or system failed or malfunctioned, and is it available for inspection upon request? The air conditioning (AC) condensate drain system is either blocked, misrouted, or improperly sealed, causing AC condensation to pool and leak into the front passenger-side footwell through the factory floor sealant. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? The continuous leak has caused significant mold growth on the interior insulation behind the passenger footwell, creating a potential health hazard for occupants, especially those with respiratory conditions or allergies. Prolonged exposure to mold inside a sealed cabin environment poses a serious risk to occupant health. Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was brought to a dealership for this issue, but the problem persists. I later reproduced the issue myself by running the AC for 30 seconds, after which water visibly seeped through the floor seam. Mold growth was also confirmed behind the interior trim. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The dealership previously inspected the vehicle but failed to resolve the issue. No other parties (e.g., manufacturer representatives, police, or insurance) have inspected the vehicle at this time. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lights or messages. The only symptoms were damp carpet in the front passenger footwell and a persistent musty odor, which appeared prior to discovering the mold and leak source.

I was driving just got off the freeway and went to hit the brakes and the brake pedal was hard as a rock I had to use force to slow down to safely get over from the left lane to the right lane to get into a parking lot was a pretty scary experience, called AAA had it towed to the dealer they said the vacuum pump failed which I suspected because of the other complaints regarding the same thing, there was no recalls on my car dealer fixed it under warranty and they said there was no damage to the camshaft. The dealer was amazing and would reccamend them, I also had to pay 100 deductible. As others have stated theres a recall on the vacuum pumps but certain Vin #s are not included. Gm needs to included all Vin #s for the equinox in the recall whats it gonna take somebody to get killed!

I purchased my 2025 chevy equinox in January of 25. In March I noticed some distortion on the passenger side of my window while making a right turn. I have had the windshield replaced twice by chevy/gm. Both replacements have the same issue. The dealership pulled up a new 25 or 26 equinox to find it has the same issue. The local chevy/gm rep claims, even without seeing my vehicle, that this is with tolerances/spec. This is a safety issue as it distorts my vision.

Vehicle Information: 2019 Chevrolet Equinox VIN: [XXX] Mileage: [Insert your current mileage] Engine: 1.5L Turbo ⸻ Complaint Summary: Sudden and complete loss of brake assist while driving. Brake pedal became extremely hard to press, making it difficult to stop the vehicle safely. A warning message appeared on the dashboard stating “Brake Assist”. This happened without warning and made braking unpredictable and dangerous. If I had been driving downhill or in the mountains, I could have easily lost control or crashed. I was lucky to be on a relatively flat road when it happened. I inspected the vacuum hose and brake fluid level — both were normal. After researching, I discovered this is a known issue in 2019 Equinox models with the 1.5L turbo engine, related to vacuum pump failure or loss of vacuum supply to the brake booster. There are service bulletins and limited coverage programs (N192268490 and N182202780), but my VIN does not show an open recall. I believe this is a serious safety defect that could affect many vehicles not yet covered by the recall or service campaigns. GM needs to expand the recall and properly warn drivers before someone gets killed. Please investigate this problem and hold GM accountable for fixing this safety-critical issue. Faulty windshield. Replaced with two additional windshields (1GM, 1 Aftermarket) which were both defective. All Chevrolet and Cadillac vehicles on the lot had defective (Warped) windshields. These windshields distort images, cause dizziness and motion sickness. The Dealer purchased the vehicle back.

Issue Description: While driving during daylight hours, especially when the sun is positioned at a certain angle, I have observed an intense reflection from the dashboard onto the inside of the windshield. This reflection is bright and widespread, directly obstructing my field of vision and making it difficult to see the road ahead clearly. The glare is particularly noticeable on clear or sunny days and significantly increases the risk of an accident. This visibility issue appears to be related to the dashboard material and design, which may not adequately diffuse or absorb sunlight. I believe this condition may affect other drivers as well, potentially leading to widespread safety concerns. I respectfully urge the NHTSA to investigate this matter and consider whether this condition warrants further examination or manufacturer guidance to reduce the risk associated with glare-induced visibility loss. Please let me know if any further information or documentation (such as photographs or video recordings) would be helpful in reviewing this issue. Thank you for your attention to this matter and for your continued efforts in promoting road safety. Now, it won’t even start, and the battery was only at 73%. I can’t help but imagine my car stopping in the middle of the road. I would have died. My high-voltage battery isn’t even being recognized by the system. This is a highly safety hazard. If this happens on the highway, I would have been killed. Something is wrong with a less than 3 month old car.

I was driving on the highway and when I exited and tried to stop at a red light the brakes stopped working. No warning, the brake assist engaged so I barely came to a stop in time before hitting the car in front of me. I can barely use the brake pedal and my check engine light came on with it. Its very hard to press the pedal like its got something stuck under the pedal but there was nothing. The brake assist stays on and basically is the only way the car will stop and still doesn't completely stop.

My breaks stopped working while I was driving, they became hard to press and unable to function. The vacuum pump attached to the engine failed. The vacuum has broken pieces that are now inside of the engine. No warning indicator lights appeared before the incident. As far as I'm aware other similar models are having the same issue but my vin was not included in the recall.
