The contact owns a 2020 Ford F-150. The contact stated that while driving at various speeds, there was a delay in the transmission shifting, and the vehicle hesitated while accelerating with the accelerator pedal depressed, and the vehicle jolted. The engine revved to 7,000-8,000 RPM. The vehicle was taken to an independent mechanic, who determined that the transmission needed to be rebuilt or replaced. The contact associated the failure with Technical Service Bulletin: 25-2301 (Harsh/Delayed Engagement and/or Harsh/Delayed Shift) and Technical Service Bulletin 25-2475(Harsh/Delayed Engagement and/or Harsh/Delayed Shift). The failure mileage was approximately 88,000.

Our 2025 Ford Lightning has experienced multiple instances of low-speed unintended acceleration. On May 26, 2026, while coasting into a parking spot on a slight incline, the vehicle suddenly accelerated and caused a collision. The defect has occurred approximately six times over 7,300 miles of operation (approximately once every 1,200 miles). Professional collision repair was completed July 7, 2026. Defect recurred after repair. Ford Factory Service Engineer inspection on August 10, 2026 conducted a seven-mile test drive and reported "concern not duplicated". The vehicle's EDR (Event Data Recorder) failed to reliably document the collision event, recording a timestamp 1 hour 19 minutes after the actual collision occurred (NHTSA requires ±3 millisecond timing accuracy). The vehicle is unsafe and cannot be reliably driven. The defect occurs in a pattern that cannot be detected by brief test drives and poses an ongoing risk to occupants and the public.

The rear tail lights do not function. The vehicle cannot be driven at night because it has no rear illumination. It puts both the operator and other vehicles at risk of collision. I took the vehicle to a Ford dealer for repair. They reported the tail lights failed due to corrosion of the wiring harness pin connectors due to water intrusion. The wiring harness would have to be replaced at a cost of $4,375.32. I was unaware of the taillight failure until the power tailgate became inoperable. The tailgate and taillights are operated by the same wiring harness. There are no warning lamps or messages on the vehicle indicating the lights are not working. I believe this issue may be related to recall 23S30 and the harness may have been improperly connected during the initial failed recall repair.

- #11765735September 21, 2026SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
While I was driving, I was pressing my brakes and the vehicle would not stop at a stop sign. Then with the brakes pressed all the way down, it was a very elongated stop so this rolled me through the stop sign. Thankfully no cars were coming. It felt as if I was trying to stop on ice. Supposedly this was fixed with an OTA update when the initial recall came out last year.

The contact owns a 2018 Ford F-150. While approaching a light and entering a loop while driving 55 MPH, the transmission unexpectedly shifted from 7th gear to 1st gear. The vehicle stalled, was shifted into park, and restarted. There was vibration in the floor at the time of the purchase. The vehicle was taken to the local dealer, where the torque converter was repaired under warranty. The vehicle was taken to the local dealer again, where it was diagnosed and the failure was associated with the valve body. There were no codes present. The vehicle stalled on Interstate I-20 while driving 60 MPH, causing an unsafe situation. The wrench symbol was displayed after the failure. The vehicle was taken to a transmission center for a scheduled repair. The contact associated the failure with NHTSA Campaign Number: 25V164000 (Power Train). The manufacturer was contacted on two occasions, and the contact was provided a case number. The failure mileage was approximately 108,000.

The contact owns a 2014 Ford F-150. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 24V444000 (Power Train); however, a failure occurred after the recall repair. The contact stated that while driving 40 MPH, the transmission unexpectedly downshifted into first gear. The transmission warning light was illuminated. The contact stopped, turned off, and restarted the vehicle. The vehicle resumed normal operation. The vehicle was taken to O'Reilly Auto Parts, and DTC: P0722 (Output Speed Sensor Circuit No Signal) was retrieved. The vehicle was then taken to the dealer, and the molded leadframe was replaced. The manufacturer was notified of the failure by phone. The failure mileage was approximately 145,500.

Can phasers failed. Ford will not accept any responsibility for a known dangerous issue.Ford refuses to help even though they knew about this issue. They did not issue a recall, instead offering a Customer Satisfaction Program so they could dodge responsibility for future repairs. This is a dangerous defect as it can cause an engine to seize up on the highway. I’m stuck with a repair bill in the thousands of dollars and Ford will not even assist with the cost.

XXX: I scheduled an oil change online and selected the open transmission safety recall. [XXX] Ford executed Recall 26S28 (PCM Programming) under verification code RGLKT7MASIPIJ. The truck had absolutely no shifting issues prior to this flash. XXX: Within two weeks of the software update, the truck began experiencing the exact erratic shifting and violent gear-dropping safety hazards described in the recall. I returned the truck to the dealer. XXX: After days of complete silence, I had to initiate contact. First by phone, then by physically walking into the service desk on XXX to get answers because the dealer ignored my vehicle. Mid-XXX: After the truck sat on their lot for three weeks, they finally scanned it and retrieved code P2701. The dealer stated the recall wouldn't cover it and immediately quoted me $8,000 to rebuild or $10,000 to replace the transmission. They claimed they would submit the case to Ford Corporate. XXX: Having heard nothing, I returned to the dealership in person. I was told they still had no response from corporate. The Paperwork Contradiction: The technician's notes on Page 3 explicitly state: "No update so that one recall cannot be performed, must been updated at some point." This proves the technician failed to look at their own shop's service history from XXX (Page 2), where they signed off on completing the update. Furthermore, they condemned the entire transmission based strictly on a digital code scan; they never dropped the pan or performed any physical diagnostic work. - #11765868September 21, 2026FUEL SYSTEM, GASOLINE
The contact owns a 2021 Ford F-150. The contact stated that the vehicle lost electrical power, and the contact initially believed that the failure was due to a dead battery. The battery was replaced and the vehicle was hesitating while accelerating. The contact received notification of NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline); however, the part to do the recall repair was not yet available. The local dealer was contacted and stated that the contact had to pay for a diagnostic test to verify if the failure was associated with the recall. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue.

I'm calling to report what I believe may be a safety issue involving the panoramic sunroof on my 2026 Ford F-150 Raptor. I was driving at approximately 9:45 in the morning when the front panoramic sunroof glass suddenly and violently shattered. There was an extremely loud explosion-type sound, and essentially the entire front glass panel disintegrated. I did not see any rock, debris, or other object strike the sunroof before this happened. There was no warning beforehand. The glass broke violently enough that pieces went across the exterior roof and caused additional scratches and damage. Fortunately, I had the interior sunshade closed. My concern is that if the shade had been open, shattered glass could have entered the passenger compartment, and the sudden explosive sound itself could potentially cause a driver to react or lose control. The truck is a 2026 F-150 Raptor with only approximately 6,000 miles. My Ford dealer has referred me to my insurance company, and insurance is handling the immediate repair, but I want this incident formally reported because I believe NHTSA should be aware of the possibility of a sudden panoramic-glass failure. I have photographs taken immediately after it happened showing the completely shattered panel and the resulting damage, and I would be happy to provide them

Truck suddenly shifted from gear 8 to gear 2 then 4 reducing my speed suddenly causing other vehicles to almost hit me at around 40 mph. I drove home with the transmission erratically shifting up and down Even though I kept my speed under 30 mph. That was about 10 days ago and I made an attempt to drive it a few days ago after researching and in the same result happened. It seems I have been victimized by the CDF drum crawling out of its space leading to an imbalance of transmission fluid pressure. About 2 miles after the initial problem. A wrench symbol appeared on the instrument cluster with a message under it that says see manual. Just as every time I've tried to find something in the manual. After spending several minutes of frustration, I still never found the wrench symbol or what it means.

- #11765619September 20, 2026POWER TRAIN, ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE: WARNINGS
I am writing to formally escalate my ongoing complaint regarding my Ford truck and the repeated problems I have experienced with it over the years. At this point, this is no longer simply a matter of dissatisfaction with a dealership repair. I am asking Ford Motor Company at the corporate level to take responsibility for reviewing the complete history of this vehicle and the repeated unsuccessful attempts to correct the problems I have experienced. Most recently, my truck was brought to **Salem NH Ford on August 25, 2026, because of the ongoing powertrain malfunction and sudden loss-of-power problem**. This was the **fifth time** I have brought the truck in for this same ongoing powertrain problem. The dealership kept my truck for **ten full days**, from August 25 through September 4, 2026. During that time, Ford provided me with a loaner vehicle. I was eventually contacted by the dealership and told that my truck was ready to be picked up. I was also told that they believed they had fixed the powertrain problem. Unfortunately, that was not the case. ### THE POWERTRAIN MALFUNCTION RETURNED ALMOST IMMEDIATELY After picking up my truck on **September 4, 2026**, I began driving it again. Then, on **September 7, 2026 — only three days after picking it up from Ford — the powertrain malfunction happened again**. This was extremely upsetting and frustrating. Ford had my truck for ten days. This was already the **fifth service visit for the same ongoing powertrain problem

I bought this truck used about one year ago. Since then I have not had much time to use it. In the last three months I have had some time to drive it. Three times in those months this issue has occurred. Luckily they all occurred on my property and no damage happened except to the truck. First time it happened an exterior mirror was knocked off, a running board was bent, scratches and dings to the to the paint, and a tailight was damaged. This all happened in reverse due to a gate opening I was going through. The motor all of a sudden had a massive spike in RPMs and a violent lunge and slide of the vehicle. I learned later that it was due to a flawed part in the transmission called a molded lead frame. Transmission is Fords 6R80 6-speed automatic. Last time it happened was on 09/16/2026. It has not been inspected by any entity because apparenty I live too far in the "boonies" for anyone to come this far but the vehicle is available for inspection upon request.

2019 f150 with 115,600 miles equipped with 10r80 transmission recently experienced a 10th gear shift down to 1st gear while doing 65mph down a bridge almost causing a really bad accident. Since then truck has randomly and sporadically shifted out of higher gears into lower gears I cannot afford to repair a factory defect.

The component that failed is the Back Up Camera Screen. It is all blue. It is available for inspection upon request. My safety or the safety of others is at risk because I can't see directly behind my truck without this camera. I could end up hitting a child or small item directly behind my truck. We will be contacting the dealer about this problem. My vehicle has not been inspected by the manufacturer, police, insurance or others at this time. No warning lamps, messages or other symptoms of this problem occured prior to the failure. This problem first occured on Saturday, September 19, 2026 I don't know what the cause of this problem is.

Daytime Running Light in the headlight assembly overheated and stopped working. Thousands of 2021-2023 F150 owners have had the same issue. This is clearly a mass scale manufacturer known defect and Ford already tried sending an update to lower the LED brightness but the issue still happens.

I participated in Safety Recall 26V237 -2016 F-150 Ford Truck transmission issues. Under the recall, Ford completed a transmission programming update to resolve the safety concerns. The campaign DID NOT resolve the transmission issues. The truck is still a hazzard to drive & needs further repairs or transmission replacement. Please do not allow Ford to remain unaccountable for the failure to correct the issue.
