VISIBILITY/WIPER
Was passing a 18 wheeler truck that was on my right and behind a second 18 wheeler truck. Light rain but a lot of water spraying up from the road. Probably 1/8 inches of water streaming on front windshield. Wiper speed was set to III, but the wipers would not turn on. Finally issued a voice command and the wipers finally started. TACC, autosteer and fsd we’re not on. Autosteer was the mode that was enabled. A week prior I parked the vehicle in the shade headed in to a stucco building and the wipers would not turn off. No rain whatsoever. The camera system thought the bumbs on the wall were rain drops. But a window so full of water you can’t see is not water to be removed.

Traveling on an interstate highway when the car suddenly decelerated. Cruise control was on at the time. sudden deceleration with cars following made it quite dangerous. We could have been struck from behind. this happened two times on the same trip.

Windshield of my car cracked spontaneously in two different occasions for no apparent reasons. First incident's occurred in June 2023 resulting in replacement of the windshield. Second incident occurred on December 4,2023 & vehicle is scheduled for replacement of the windshield. I thought the first event was due to rock hitting my car while driving on the highway but very likely it’s a spontaneous crack like the second.

ELECTRICAL SYSTEM
I had just parked in a spot 30 feet in front of my clinic building. My foot was not on the brake nor accelerator — the car had stopped and was parked. Tidied my hair and was grabbing my purse placed on the passenger seat, the car seemed to “surge” and suddenly ACCELLERATED QUICKLY straight toward the sliding glass doors toward my medical clinic building! I screamed, dropped everything and stepped as hard as I could on the brake. It stopped suddenly about 5 ft, onto the curb, in front of the doors. No damage (except for the scare!), so no reports. The AAA tow truck driver said he had seen this before in electric cars in his recent 3 months ( 4 out of 5 was Tesla; the other a Volkswagen), and that the black box can’t detect the surge because it’s electrical? Didn’t report it because no damage, no proof. Not looking for compensation, am not hurt (yet), but please look into this. I could’ve mowed down precious babies and children, mothers, doctors in my hospital pediatric clinic. I have heard 2 more incidents from a coworker and a family member (model 3 and S), so although my experts not recent, I cannot with clear conscience not report this. Please, please research this, lives are in your hand. Even if it is just one.

Since purchasing my 2023 Tesla model Y in May, I have intermittently experienced highway phantom braking when either in self-driving mode or just using the cruise control. I would estimate it happens once a month. Just recently it happened 2 days in a row, at night and during the day. The experience is very scary because when the brakes slam on at 70 mph, I'm afraid I will get rear-ended. I have a dashcam video of one of the occurrences this week, it's an MP4 video file.

Date: 12/1/2023 Time: Approximately 5:20 AM Location: Intersection of [XXX] and [XXX] Vehicle 1: Type: Tesla Model Y Mode: Full Self-Driving (FSD) Direction: Eastbound on XXX Avenue Speed: Approximately 30 MPH Vehicle 2: Type: Sedan (Make and Model Unknown) Direction: Southbound on XXXX Avenue, turning right Incident Description: While operating a Tesla Model Y in Full Self-Driving mode, I was traveling eastbound on XXX Avenue in Summit at around 30 MPH. The incident occurred at the intersection of XXX Avenue and XXX Avenue. At this intersection, the traffic light for XXX Avenue was flashing yellow, indicating caution, while the light for XXX Avenue was flashing red, signaling a stop requirement for vehicles on that road. As I approached the intersection, a sedan traveling southbound on XXX Avenue initiated a right turn onto XXX Avenue. Despite the flashing red light for the sedan's direction, the Tesla's FSD system, which was controlling the vehicle at the time, appeared to misinterpret the right-of-way. The Tesla continued at the same speed without reducing, shifting lanes, or showing any indication of yielding to the turning sedan. Recognizing the impending risk of collision, the sedan driver executed a sharp right turn, narrowly avoiding an accident but coming close to the curb in the process. Realizing the Tesla's FSD system was not responding appropriately to the traffic situation, I quickly disengaged the FSD mode and manually took control of the vehicle to avert a potential collision. Remarks: This incident highlights a concern regarding the current capabilities of the Full Self-Driving system in complex traffic scenarios, particularly at intersections with flashing traffic signals. It underscores the necessity for continuous driver attention and readiness to assume control, even when FSD is engaged. STEERING
Steering system malfunction and failure: I have a brand new Tesla Model Y for less than 3 months. Two months after I have the vehicle, the steering rack had an internal fault. The steering rack was replaced at the Tesla authorized repair shop. Eighteen days after that repair. I was involved in a car accident. The reason was because my car had veered to the right on its own while driving on a city street. I then turned the steering wheel to the left, but the car did not turn to the left. My car then hit another vehicle that was parked on the right side of the street. The front of my vehicle was totally damaged on the right side. The entire left side of the other parked vehicle was also totally damaged. I was NOT on cruise control. I was NOT on self- driving mode.

VISIBILITY/WIPER, LANE DEPARTURE: ASSIST, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
These are observations over the past couple of months that I have owned the car. They have been consistently observed and have occurred at speed between 35-70 mph. There are four main issues I’m having with my tesla. 1) I cannot use cruise control because it requires automatic windshield wipers. The automatic windshield wipers cannot sense when it’s raining and just turns on randomly when it’s not raining. 2). The car breaks on its own randomly on both the highway and city streets. I have almost been rear ended several times because of this. 3) the car thought another car was coming in my lane on interstate 95 and violently switched lanes without warning causing me to oversteer and almost wreck. I had my family in the car with me and it was terrifying. 4). The two times I used full self driving the car actually accelerated to make it through a yellow light. I know these are all beta features of the car but in my opinion they are all dangerous to use on the roads and should be disabled until they are ready for prime time

VISIBILITY/WIPER
Auto-wiper settings does not work. They are required to be turned on, while on Autopilot/cruise control, but they actively wipe the windshield, even when no rain (clear blue skies). When there is rain - they do not turn on frequently enough. It's a danger in the first scenario, because it's a huge distraction to the driver (wipers intermittently turning on when not raining). In the second scenario, it's very unsafe to not turn on when it's raining. Fussing with them in both scenarios is a huge distraction and very frustrating.

My car randomly brakes while using autopilot/cruise control on the highway. It will brake and move the car toward one side of the line. This happens sometimes every 20 minutes on a roadtrip. On one occasion, it threw the car to the far left side of the line and then reacted to the on the left. It threw the car back to the right side of the lane, near the car on the right side. There were cars on either side of me and neither were coming into my lane.

False and phantom forward collision warning took place on Nov 25 2023, at 5:42 pm on Alder Woods Dr. The road conditions are clear visibility with no fog, rain, or snow/ice. Legally parked car on the side of the road is mistaken by a moving car in the middle of the road.

I was driving south bound on hwy90 at about 78mph using navigate on autopilot. Without warning the car swerved into a left turn lane and then slammed on the brakes when the turn lane came to an end. This caused a number of personal belongings to be damaged. Thus happened as I was on my way home from a service center visit that stated the autopilot systems were all functioning correctly. Tesla was notified immediately a d has not responded to my complaints.

Following false and phantom forward collision warning took place on Nov 15, 2023 twice on I-66W at Fairfax, VA during the afternoon with clear road conditions without any rain, or snow on slowed down traffic: 1. 2:38 pm on I-66W 2. 2:42 pm on I-66W (same trip as above) I filed bug reports with the car, and rescheduled a service appointment with Tesla's Service Center, with no resolution from the car manufacturer.

VEHICLE SPEED CONTROL
I experienced a sudden unintended acceleration while parking the car in my home garage, something I have done thousands of times with no difficulty. I had this brand new car for a 12 days before it smashed through my garage wall. My garage is a fairly tight fit, so as I always do, I partially entered the garage and stopped to make sure that the side mirrors were going to clear the garage entrance. Then, I lifted my foot off the brake in order to start moving forwards again, at which time the car suddenly accelerated like crazy through the remaining 12 ft of space, smashing the cabinetry through the rear wall of the garage. The car, my garage, and laundry room were severely damaged. I complained to Tesla about this vehicle problem, but they just say there are no problems with the car, and it must be the driver’s fault. They are implying that I floored the accelerator, which is nonsense. I have been driving for more than 40 years, and I do not floor the accelerator while parking in my home garage. This type of sudden unintended acceleration is a danger to public safety, and this matter has to be thoroughly investigated, and a solution has to be found to fix the problem.
