My vehicle will jerk into the left lane of the road when lane keeping assistance is activated. It appears isolated only to situations when I am travelling on roads that were recently repaved and lines haven't been painted yet. However, these roads are equipped with consistent reflective indicators marking the center of the road because these roads remain unpainted for months. There are many of these roads where I live so it is a frequent issue that I experience multiple times everyday. I have never allowed the vehicle to fully execute it's correction in fear it would drive off of the road or into oncoming traffic but it is a very scary event when my car tried to kill me.

I turned on the vehicle and an Engine Maintenance Required light and warning went off on my car. I noticed the warning would only come up once I turned on the A/C so I presumed it was an issue with the Coolant Bypass Valve

Hi, I recently relocated from Vancouver, Canada to Seattle, Washington, and I am in the process of importing my Canadian-purchased 2022 Toyota Corolla into the United States. I have already contacted Toyota Canada, but they informed me that they do not issue compliance letters for vehicles distributed under Canadian regulations. I then contacted Toyota USA, who confirmed that this matter falls under NHTSA’s responsibility and advised me to request the compliance letter directly from you. Therefore, I respectfully request that NHTSA to solve this issue so that I may complete the import process in compliance with U.S. regulations. Please advise me on the next steps to obtain this letter, including any required forms or supporting documentation I should provide. Thank you for your prompt attention to this matter.

Vehicle driving thru a city street with no lights at 7 mph around 10pm when the Lane Keeping Assistance read a trash can on the left and pickup truck on the right on the narrow street pulling it completed to the right side and hitting a parked vehicle. Vehicle presented initial issues with quick light and noise the day before the incident. Damages does not reflect a vehicle driving at 7mph(GPS tracker video available but unable to upload) Vehicle manufactures ran a completed FTS inspection on 08/22/25 at local dealership - Results will not be provided for another 45 - 60 days.

While driving my vehicle Southbound on [XXX] in the Simpsonville/Fountain Inn area of South Carolina, my vehicle suffered some sort of catastrophic failure while driving and became completely engulfed in flames. The vehicle was a total loss. The vehicle had been serviced a little over a month prior to this incident happening. The fire department took a report for the fire. My car was parked in my driveway on am 80 degree afternoon. I went into the car to get something. When I closed the driver door, the entire back window shattered. I am the only driver of the vehicle and there were no issues with the window prior to this- no cracks, no incidents, no rocks on the road.. The only indication there was any issue was condensation on the inside of the back window two days before this happened. I thought this was unusual, but it did not seem like something to call the dealer about. I took the car to IRA Toyota in Saco, Maine. They informed me there was no way to 'prove' a defect, as there was no longer a window to inspect. They suggested I call my insurance and lie about the incident, stating that 'nobody would believe a window just blew out' and they were just trying to help me get the repair covered by my insurance. I know there is currently a class-action suit against Honda for this same issue, and found over 20 shared instances in Toyotas (of various models) from 2021,2022, 2023, and 2024. I understand glass is not included in warranties, and I think this only makes sense for manufacturers. The fact that a window could spontaneously shatter at any moment is certainly a safety issue and should be covered under warranties or some other level of accountability. When my window combusted, all of the glass fell into the back window and back seat. If any kids or animals had been in the back seat (as they almost always are), they would have been covered in glass. Thank you for your attention to this matter.

What component or system failed or malfunctioned, and is it available for inspection upon request? The wireless charging unit/electrical system failed. The unit is still in the vehicle and available for inspection upon request. How was your safety or the safety of others put at risk? While driving with all doors securely closed, the system displays false “door open” error messages and disables the wireless charging function. These false alerts distract the driver and could cause confusion or unsafe reactions. The issue undermines confidence in the reliability of the vehicle’s electrical and safety systems. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The Toyota dealership confirmed the problem and reported replacing the wireless charging unit, but the defect persists. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Yes. The vehicle has been inspected by the Toyota dealership and reviewed by Toyota Customer Care. Toyota denied further assistance (Case #250811002888, letter dated August 21, 2025). Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes. Shortly after purchase, the vehicle displayed false “door open” messages when attempting to use the wireless charger. These warnings cause the system to stop charging and continue to appear despite the dealer’s attempted repair.

There is a persistent high-pitched noise coming from my vehicle, which has now led to significant physical symptoms including migraines, nausea, dizziness, and increased sensitivity to sound, has made driving a painful and unsafe experience. The problem occurs nonstop if the a/c or heat is on. The noise is high-frequency, sharp, and continuous, and appears to originate from within the dashboard area or HVAC components. I went to the dealership multiple times with this complaint and they claim it is normal operation. I did not hear the same issue in the cars they compared mine to, but after Toyota's refusal to address the issue i looked it up and there are other reports [XXX] ). When at the dealership, I am continually faced with dismissive responses from the service departments and technicians, who have failed to address the underlying cause of the sound. This sound creates a serious distraction while driving and has caused physical symptoms including headaches, nausea, ear discomfort, and dizziness. These symptoms impair my ability to concentrate and safely operate the vehicle. On longer drives, the effects intensify, increasing the risk of driver fatigue, disorientation, or even potential loss of control. I believe this presents a significant safety hazard, not only due to the physical impact on the driver, but also because it undermines focus and reaction time, especially in high-traffic or high-speed conditions. This issue should be investigated for potential defects in the HVAC design, acoustic insulation, or related components. Please investigate whether this condition is present in other vehicles of the same make/model and consider whether it warrants a safety recall or manufacturer intervention. Purchased 6/25 a 2025 Toyota Corolla LE with 16,000 miles as a Pre-Certified Gold Platinum. Car has a constant front end, ride side rattle that does not go away. Advised by dealership it is a manufacturer characteristic. Impossible on a new car and from a billion dollar manufacturer. I was advised to turn up my radio in order not to hear it. Dealership and or Toyota itself has not serviced the vehicle after attempts with the explanation that it is road safe. Well if road safe, how do you know when you don't say what the problem is other than a piece of metal clanking and no remedy by Toyota. So how do I know this vehicle is safe to drive?? No one helps. Words mean nothing and to then say raise radio volume therefore causing another road distraction. If the car can't be fixed than what? Is it really road worthy?

My car has 5358 miles. Started car. DRIVE START CONTROL MALFUNCTION LIGHT; CHECK ENGINE LIGHT; SEE DEALER LIGHT all came on. Car would hardly move forward or reverse. Felt like the brakes were on. Heard subtle clicking sounds from engine. Looked this up as I never heard of this. Toyota says you MUST STOP DRIVING ASAP, assuming you "can" drive. You can LOOSE CONTROL of the car. There are many things that can cause the light to come on. Dealer "says" they never heard of this. My car had to be towed. Mechanic found nothing wrong and reset computer modules. I got my car back after 3 days. Dealer says NO GUARANTEE it won't happen again WITHOUT warning. So; anywhere, anytime, my car will just stop driving? HOW IS THIS SAFE? Internet searches show it's NOT UNCOMMON. Toyota makes it hard to file a complaint.

- #11680486August 13, 2025AIR BAGS, SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
There was a recall on my vehicle for the skid and emergency braking and was told it was taken care of Feb 2025 at my service appointment. I was involved in a car accident on Aug 11, 2025, everything that was supposed to be fixed in the recall did not operate during my crash. I reconfirmed with the dealership that they did the recall repair and they again said yes but if they did then the repair completely failed. There was no emergency braking available, the car skid a long distance, no airbags deployed even though all sensors in the front were engaged and collision center confirmed they should have gone off. Vehicle is currently at collision center and is available to be inspected.

Odometer Fraud. The contact purchased a 2021 Toyota Corolla. The contact stated that while attempting to sell the vehicle to a Toyota dealer, the dealer informed the contact that there was a mileage discrepancy. The vehicle was a used car dealer sale. At the time of purchase in October 2024, the vehicle mileage was 60,000, and the current mileage was 74,000. However, the dealer informed the contact that the mileage on the Carfax report was 110,000.

When the sun is overhead (this happens daily in South Florida) the reflection of the ambient light on the instrument panel cover makes it impossible to look at the information on the instrument panel. This essentially makes one drive while being blind. Not a safe concept. It is pathetic that no action has been taken against the company.

I purchased a used 2024 Toyota Corolla from Chapman Chrysler Jeep on 8/9/25 at 31,890 miles. When exiting the freeway 3 miles later, the vehicle suddenly swerved uncontrollably without any steering input from me. This was an extremely dangerous incident that could have caused a serious accident injuring myself and others.

This is my fourth service visit for the same unresolved safety issue on my 2024 Toyota Corolla Hybrid. The problem involves a cracked fuel line and fuel vapor leak in the same area as recall 24TC08 (Fuel Tank Filler Clamp and Vapor Leak Detection). The dealership initially failed to detect this during recall work and later claimed “rodent damage” after two prior visits. I have since used a professional-grade MultiRAE Lite gas detector and documented VOC (volatile organic compounds) levels up to 123 ppm inside the vehicle cabin with the windows up. These readings indicate hazardous fuel vapor and exhaust gas intrusion, which pose serious health and fire risks. I have driven over 200 miles in these conditions), exposing myself to harmful fumes. The dealership has failed to properly repair the issue, and Toyota has not resolved the safety hazard despite repeated service attempts.

My vehicle was backed into while ì was inside of it in a parking lot. Due to my horn failing., I was unable to alert the other driver to see me. The clock spring is what failed the first time at 95k they replaced it. I'm now at 147k and it failed again . Now I'm discovering that if it does not work, there's an electrical circuit failure and that the airbag will not deploy if I get in an accident. This is a serious safety issue and I feel Toyota should be responsible for it for the life of the vehicle . A horn should never go out. Especially if it not working will keep my air bag from functioning properly . 850 out of my pocket is way steep. The replacement part lasted half as long as the original part did when I bought the car that to me seems like a faulty part this should be obvious and they should just fix the problem cuz I might have to just consult an attorney should they choose not to. The attached photo shows what happens when you can't beep your horn at somebody who's about to back into your car !!! my car still has not turned on any kind of warning lamp for the airbag or any kind of malfunction light, no warnings on my radio. I have noticed that my Lane departure warning seems to work intermittently along with the automatic high beams that like to flash my high beams in people's face for some unapparent reason which also seems like a safety issue. If I had to take my car to an inspection station it would not pass an inspection due to the horn not working ! You cannot take your child to the Department of Motor Vehicles and let them take their drive test if your horn does not work ! which happened to be my case !! Very disappointing to say the least especially when you've paid $30,000 for a car.
