after fueling strong fuel smell in vehicle within 100 miles check fuel cap light came on. next day check engine light came on. made appointment at dealership . dealership performed evap smoke test which found smoke coming from top of fuel cell. installed fuel pump ring and evap hose clip. performed evap test and smoke test -passed cleared codes. got car back next day less than 50 miles later ck engine code and fuel cap and fuel odor back.

The contact stated that while the vehicle was stopped on a curb with another vehicle approximately 8 -10 feet ahead, the brake pedal was depressed to shift into park; however, the vehicle independently accelerated. The contact stated that because of the failure, the contact's vehicle rear-ended the Honda CR-V vehicle that was parked in front of the contact's vehicle. The contact stated that the vehicle's speed was only 3–4 MPH. No air bags deployed. The other vehicle sustained damage to the rear bumper. The contact's vehicle was towed to a local tow yard. The contact's vehicle was condemned as a total loss by the insurance adjuster. No police report was filed. No injuries were sustained. The manufacturer was made aware of the failure. The contact owns a 2023 Toyota Corolla. The contact stated that whenever the vehicle was shifted to park(P), the word (P) was illuminated on the PRNDL, but the word was not displayed on the instrument cluster. The contact stated that while shifted to drive(D) there was no indication of drive on the PRNDL, or the instrument cluster, and the vehicle failed to respond. The contact stated that the failure had occurred while shifted to reverse(R). The contact had been advised by a mechanic on how to reset the parking brake to be able to shift to park(P), drive(D) or reverse(R). The contact had been advised to lift the parking brake lever and hold it for 5 seconds and then depress the lever and hold for 5 seconds. The contact was able to shift to any gear, and the vehicle would move after performing the reset. The contact normally did not engage the parking brake, however, when the failure first occurred, the warning "EPB Stopped Completely" illuminated twice but had not illuminated in weeks. The contact stated that the failure was intermittent but had gradually become a recurring failure. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed and was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 55,000.

I am writing to inform you of some recurring issues with my vehicle. The car has been displaying several (5) alerts, including those related to the parking brake system, drive-start control system, check engine light, vehicle stability control system, and secondary collision brake system. I took the car to a dealer for repairs, and they assured me that the issues were resolved. However, the following day, the problems reappeared. Additionally, when I start the car, it emits an unusual sound and begins to shake. Unfortunately, The dealer had 5 days my car to repair and denied to provide a rental car. I don't understand the reason if my car is new and the problem is manufacturing I would appreciate your assistance in addressing these ongoing concerns.

My engine stalled while driving. My low tire pressure light came on and an hour or so later I was turning left at a light. My left turn signal was on. Suddenly my car stalled and the message that stated Put in park, steering power low showed on screen. Put car in park and restarted and it started right up. Dealer said no codes and could not replicate. Only 18,000 miles on car. Huge safety issue!!!!!

I am filing this complaint regarding a serious defect in my 2021 Toyota Corolla (Manual). At approximately 42,000 miles, the clutch slave cylinder failed. This defect caused hydraulic fluid to leak and contaminate the clutch assembly, resulting in significant damage and loss of proper function. Despite the fact that this issue stems from a defective part, the dealership has refused to cover the repair of the clutch assembly, stating that the clutch is not covered under warranty. Instead, they are requiring me to pay for the repair out-of-pocket, which would not have been necessary if the damage had not been caused by the defective clutch slave cylinder. I believe this is unfair and unsafe, as the failure of a clutch slave cylinder directly impacts the drivability and safety of the vehicle. This is a significant safety issue. A defective clutch slave cylinder can lead to sudden clutch failure, loss of vehicle control, and increased risk of accidents. This is not a wear-and-tear issue but a manufacturing defect that compromises the safety of the vehicle. I respectfully request that NHTSA investigate this matter to determine whether similar failures are occurring in other Toyota Corolla vehicles of this model year. If this is a widespread defect, I urge NHTSA to take appropriate action, including a recall or requiring Toyota to cover repairs.

2022 Toyota Corolla Hybrid Touchscreen/Infotainment Display randomly shut down while driving without warning and no longer functions. This could not be reset by holding the power button or disconnecting the vehicle battery. The backup camera still works, and the radio can be heard when the power button is pressed, but the display remains on a black screen and none of the controls on the steering wheel or headunit work. At night, the lights on the buttons for the multimedia display turn on and off every 10 seconds which is extremely distracting and has the potential to cause an accident. This defective part serves as a safety issue, as this is where critical maintenance and hybrid battery power related errors displayed. In addition there are many critical controls locked behind the screen related to door locks, hybrid battery usage, and other vehicle controls, as well as personal data that may now be at risk and unable to be able to be deleted. Vehicle was inspected by a Lia Toyota of Rockland Dealership Service department and determined to be a faulty display, requiring a compete unit replacement. The quoted cost is $2500, and despite my car having only 35500 miles, this is not covered under any warranty due to being outside the 36 month time window. The multimedia display in this car is a critical component and is just as vital to the car as the engine/transmission. It is a shame that Toyota reliability is a thing of the past.

The contact owns a 2026 Toyota Corolla. The contact stated while driving at an undisclosed speed and attempting to avoid striking a deer the vehicle crashed into a tree. The air bags did not deploy. The contact stated that the SOS system detected the impact to the vehicle and alerted First Responders. The vehicle was towed to the local tow yard and later towed to a Collision Center. The contact stated that the vehicle had not been accessed by the adjuster. The contact stated that there were damages to the front of the vehicle, the windshield was shattered, and damages to the hood of the vehicle and the front passenger's door failed to open. No Police report was filed because the crash occurred on private property. The contact later sustained chest pains, however no medical attention was received. In addition, the contact stated that the chest pain was continuous. The contact was advised by the insurance company to contact the NHTSA Hotline to report the failure. The manufacturer was made aware of the failure. The contact was informed that the vehicle event data recorder (EDR) was retrieved; however, the contact was not provided any additional information regarding the data. The failure mileage was approximately 883.

While attempting to change a flat tire on a 2021 Toyota Corolla Hatchback, two wheel studs sheared off completely, leaving the wheel unsafe to drive. My wife was stranded alone in the mountains with no cell service. She managed to loosen three lug nuts, but the final two were seized. When a passerby assisted, both seized studs snapped. The spare could barely be mounted. The vehicle has been serviced exclusively by Toyota dealers since new. Lancaster Toyota later replaced the broken studs free of charge, yet simultaneously claimed that another dealer, Frontier Toyota, had “switched the tyres.” so Lancaster are not responsible. That claim is false — Frontier’s invoice (Sept 25 2025) clearly shows only an oil change and inspection, no tire or wheel service of any kind. Replacing the studs at no charge while denying responsibility suggests an attempt to quietly resolve what may be a dealer-induced over-torque or defective-stud failure. The broken studs were not returned to us despite multiple requests — effectively withheld by the dealer, which constitutes unauthorized retention of customer property and prevents independent verification of failure cause. Toyota Corporate (case #251103002581) was notified but refused to investigate, referring the issue back to the same dealer. This complaint is being filed so NHTSA can document a potential wheel-stud safety defect or service-related failure and dealer mishandling of critical evidence that could conceal a wider safety problem.
