- #11578372March 19, 2024STEERING, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
On 12/09/23, my 2024 Mazda CX-90 PHEV, purchased on 11/11/23, just got out of the driveway to make a left turn around 500 feet from the driveway when there was resistance in turning the steering wheel left, causing vehicle to swerve to the right and hitting the fire hydrant on the side of the road located just around the corner. Both airbags deployed causing extensive damage to the passenger front side of the car. The automatic emergency braking did not engage to avoid collision with the fire hydrant. One of the passengers suffered avascular necrosis of the hip as a result of the accident. The vehicle was towed to a collision center, then towed to the Mazda dealership, but according to the dealership, no inspection report was available in their system as of 03/19/24. The insurance company declared the vehicle a total loss. There were no warning lamps, messages, or other symptoms of the problem prior to the failure.

At 3200 miles the vehicle had a major powertrain malfunction causing the vehicle to throw multiple error codes - Hybrid System Malfunction; Charging System Malfunction; Electrical Leakage Detected. Was instructed to pull over to a safe area immediately. Was unclear how long car would continue to be drivable. Once car stopped it became completely immobile. Repeated restart attempts would not let vehicle start ICE engine or shift into gear. Had to be towed via dollies to nearest dealer for diagnosis and repair. Dealer found a failed PTC heater unit as source of problem and replaced it (Mazda part- KMV6-61-FHX). Vehicle is once again drivable, but the dealer has no idea what would cause the unit to fail and how to avoid future problems. Dealer has heard about this with many vehicles and repaired several similarly. We are fortunate to have found a safe place to pull off the road before powertrain shutdown, but if not we would have been stuck on the road with no ability to move the vehicle and occupants to safety. I hope NHTSA can investigate whether a recall of that part or the software that controls it is needed. NHTSA should also look into the failure mode of the vehicle to see if a different approach is safer and more appropriate.

Started hearing a rattle in the mid console of the car that gradually got worse. When i checked under the car, there was a bolt missing and one hanging that was suppose to be holding the propeller shaft. When i took it to the dealer, they tried to lie and say that they were only loose which is what you will see on the paperwork but when i confronted them that i saw one was missing and the other bent, they said yes, they don’t know what happened but they fixed it.

There has been two dangerous failures both occurring in an intersection. First instance was in December of 2023 whey the fuel pump failed while turning in an intersection. The second incident was a hybrid battery malfunction, rendering my vehicle unusable in the middle of an intersection again. This was 03/2023

On [XXX], San Diego, CA. (car is new purchase of 1 month 1 week old, 1250 miles, outside temp of 60 degrees, 3/4 battery charge, full tank of gas) Slowed to a stop at a red light in morning rush hour traffic. Engine sputters and multiple warnings appear on dash, including "Hybrid System Malfunction" "I ActiveSense Malfunction" "check engine" etc etc. CAR DIES IN LANE OF TRAFFIC AND LOCKS IN PARK. Attempted to restart 5-6 times, car would start and die immediately with same warnings. 7th restart and engine starts but had to finish drive in sport due to "Hybrid System Malfunction". Per NHTSA and dealer, all previous recalls had been performed/addressed prior to purchase date of [XXX]. Left car with dealer on [XXX] to address and repair issue. Completely unsafe for vehicle to suddenly die in traffic, with no way to move to the side of the road. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11576973March 12, 2024FORWARD COLLISION AVOIDANCE: WARNINGS
An ultrasonic sensor located in the right front grill area was determined to have failed producing the following message on the dash display: I-Activsense Temporarily Disabled. Ultrasonic Sensor Obscured. Drive Safely. This message initially appeared at approximately the 1500 mile mark of service and was documented when the vehicle was brought in for routine maintenance to Fort Myers Mazda at mile mark 3583 on 3/12/24. It is my position that safety equipment should be reasonably durable and reliable which is clearly not the case in this situation. There is a small pin sized dimple in the sensor lens. The sensor is currently available for inspection. It is further the position of Fort Myers Mazda that this sensor is not covered by the Manufacturer’s warranty due to component damage.

In October 2023 I began to notice that the steering was not working properly. When driving I would notice that the steering would stick and require more force to turn. It would click while turning. I brought it to the tully mazda (nashua NH) who told me that others were experiencing the same but that there was no fix for this yet. On 12/27/23 I again notified tully of this issue and there was no fix for it. On 2/9/2024 I discussed again with tully who advised me that there was a recall to be announced shortly. They did not tell me that there was a safety issue driving the vehicle. On [XXX] while making a left turn (there are two side-by-side turn lanes and I was in the inside/left) i completed my turn and while straightening out the wheel the vehicle overcorrected hitting the vehicle to my immediate right. I have approx $5000 in damage but there were no bodily injuries. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11575074March 2, 2024VEHICLE SPEED CONTROL, ENGINE
Car has 5000 miles on it. Numerous issues with car. Unsafe steering. Dealer not able to repair steering until Mazda provides parts for recall. Now on the highway with a medium depressed foot pedal the car speeds up on its own. Happened 2 times so far. Brakes had to be applied to slow car down. It was very brief because I immediately stepped on the brake, but it was definitely an uncommanded acceleration. Both times I did NOT have cruise control on. Definitely not safe.

On [XXX], I was driving on [XXX] near Pasadena at normal speed of traffic when suddenly an error message “Hybrid System Malfunction” appeared on the dash board along with check engine light, and the car began to stall slowing down to nearly full stop almost immediately. When I pressed gas pedal, it would not accelerate. Because I was in the far right lane, I had just about enough time to pull over onto the shoulder before the car stopped completely. The car was in EV mode (using all electric power) when this incident occurred with 20 miles of electric range still left. I was lucky that no cars were right behind me as the traffic was moving about 60 mph or my car could have been rear-ended when it came to such an abrupt stop. I put the car in park and turned off the engine, then turned it back on after a couple of minutes. The dashboard was flashing “Hybrid System Malfunction” and “check engine” light was on but I was able to drive it because it had switched to gas power mode. I immediately drove the car to New Century Mazda in Alhambra where I purchased it on [XXX]. The Mazda app on my phone captured two Vehicle Issues Detected at the same time: Hybrid System Malfunction: There is a possibility of restricted power output” and “i-ACTIVSENSE malfunction.” The Hybrid System Malfunction was the most serious and frightening of a series of malfunctions that my car experienced since it was purchased two months ago. On [XXX] (4 days after purchase) the car displayed “Approaching Vehicle Audible System (AVIS) Malfunction,” on [XXX], a “Brake!”light came on when we started the car after it had been parked, which disabled the steering wheel and the gear shift preventing us from driving (problem resolved by turning the car on and off several times). On [XXX], another vehicle issue was detected: “Air bag system and seatbelt pre-tensioner system malfunction.” Car was fully inspected by Mazda dealer in mid-January. The vehicle's parking "gear" would not engage. Further, the emergency brake would not engage. Puts all in proximity to the vehicle at risk due to the vehicle rolling into or onto them and damaging personal property. Dealership service department could not recreate the the parking "gear" issue, but were able to recreate the emergency brake not engaging. Vehicle has been inspected by Macdonald Mazda West representatives.

The contact owns a 2024 Mazda CX-90. The contact stated while driving the same day the vehicle was purchased, the message "Telematics Malfunction" was displayed. The vehicle was taken to the local dealer, where the message was cleared; however, the failure persisted. Additionally, the contact became aware that the VIN was included in NHTSA Campaign Number: 23V718000 (Back Over Prevention, Forward Collision Avoidance, Lane Departure). The contact stated that while driving, the images of the front-view, side-view, and rear-view cameras failed to appear on the center display intermittently. The vehicle was taken to the dealer, where it was diagnosed, and the dealer determined that the view monitor control module software needed to be updated; however, the part to do the recall repair was not yet available. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the failure. The failure mileage was approximately 8,500. Parts distribution disconnect.

The major issue that happened to our newly purchased CX 90 last February 3 with more than 500 miles on it started around 5:30 pm of February 8, while my wife is driving this vehicle with my daughter, all of a sudden the car stopped at the middle of the road with warning signs popping up in the dash screen like I-active sense malfunction, hybrid battery malfunction, engine malfunction light on, and on top of these, critical vehicle malfunction. The car won't start and disabled. Also, the vehicle cannot switch to neutral gear to push/pull the vehicle to the side that made the vehicle blocking the roadway until the tow truck came in. My wife and daughter's life was put in danger when this vehicle suddenly stopped in the middle of the road with cars passing by on both ways. I also found out based on the NHTSA vehicle safety, Mazda recall website, carfax and Kelly blue book, that this particular vehicle with its VIN has a SAFETY RECALL due to its steering issue that may increase risk for crash. This was not disclosed to us when we are on the process purchasing this vehicle and they did state inspection and passed it that made we doubt how did it happened.. The vehicle is now with the dealership repair shop and the last update I receive was the technician is still checking on it.

On 02/07/2024, malfunction prompts displayed on my dash relating to I-stop functionality, hybrid functionality, engine. Took the vehicle the next day to be diagnosed, the appointment was set as an inspection with oil change, filter change, and the prompts on my dash. I was advised that the issue to the prompts stemmed from the hybrid battery. I was told that contact was made to the manufacturer and that the battery would have to be replaced. The battery is on backlog and it would be about a month to arrive. I called in after leaving with additional questions as to the safety of the vehicle with the hybrid battery problem. The man on the phone said it is safe to drive, what would be affected would be the mileage I would not get because the hybrid portion was not functioning. He mentioned that a separate case was created with another 2024 cx-9 car owner with the same hybrid battery issue. It’s now two known cases with the hybrid battery issue. I called customer service and was provided with case number 00174049. The resolution to that call to corporate customer service is that I would receive a call back from the local dealership about a loaner car with about an hour. I became aware of a fourth recall, that the corporate agent referred to as a campaign, pertaining to the steering wheel. I asked my dealership advisor if that can be looked in addition to the other issues. She stated that she could not because it has not been launched. The corporate agent advised me that notification would normally happen 2-3 weeks. I looked up the steering wheel recall on the dep of transportation website. The recall has been published since the 18 of Jan 2024 and have yet to receive notification. I only found out today when I took the vehicle in for inspection.

While driving CX90 PHEV vehicle, a "HAZARD" error displayed on the dashboard and the vehicle stopped driving/accelerating completely. Displayed error reported a Hybrid System Malfunction and instructed driver to switch from hybrid to normal setting. Vehicle would not accelerate despite switch to normal setting and power remaining. Driver powered off vehicle and error remained on display. Driver again powered off vehicle after short wait and error was no longer displayed. Vehicle could then be driven in normal setting. Dealership and manufacturer have been notified with a request for immediate assistance. Vehicle is new and has only been driven for approx. 6 months. No advance warning signs appeared. Internet search reflects a Mazda owners' forum page [XXX] suggests that error has been known for several months with no resolution or recall. A vehicle that shuts down while driving in traffic is a hazard to us all. Responses are requested from both Mazda and NHTSA.
