The contact owned a 2024 Mazda CX-90. The contact stated that while driving at various speeds, a clunking noise would emit from underneath the engine compartment. The vehicle was taken to a dealer where it was diagnosed that the control arm had failed. The vehicle had been repaired. The contact stated 3 days later while driving approximately 35 -45 MPH, the brake pedal was depressed but failed to respond. In addition, the vehicle was attempted to be veered to the left or right and it failed to drive in either direction. As a result, the vehicle crashed into the rear-ended of the vehicle in front. The air bags deployed. The vehicle was towed and deemed totaled. The contact stated that the other party's vehicle was damaged and towed away. A police report was filed. The contact sustained a left leg, right arm, shoulder, wrist, and neck injuries that required medical attention via hospital. The contact's wife was seated in the passenger seat at the time of the crash and sustained injuries to the right shoulder, wrist, and both the left and right foot's toes that required medical attention via hospital. The contact was unsure of any injuries to the occupants of the other vehicle. A dealer was contacted. The vehicle was not diagnosed or repaired. The contact received notification of NHTSA Campaign Number: 24V349000 (Forward Collision Avoidance) however, the part to do the recall repair was unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The manufacturer had been made aware of the failure and opened a case. The failure mileage was 23,209. VIN tool confirms parts not available.

I was pulling into a parking spot. When I do this, my foot is always on the break. I'm an experienced driver (30+ years) and have never had an incident when I felt the car accelerate without my initiation. When I was maybe 5-7 ft from the car in front of me, the car suddenly accelerated (like it was possessed). I immediately slammed on the break and turned the wheel. Thankfully, and amazingly, I avoided hitting the car both in front of me and immediately on my left (the direction I swerved the wheel). No warning lamps, actions, etc were taken by the car itself (other than willing me to smash into the car ahead of me). This incident occurred in a school parking lot. Thankfully, there was no harm to anyone but my confidence in Mazda. But, if I had chosen to pick my children up through the pick-up line, I may have easily hit the car in front of me or a child! Obviously, this can be a tragic situation. Prior to this incident, I have felt that the car regularly does not operate smoothly at slow speeds, especially when accelerating from stop. This is my 3rd Mazda, and I have never felt this lack of control before. I have also noticed that the i-stop works erratically, including when I am moving at 10-30 mph. The light goes on and off when I am slowing down at a light, etc. Plus, when I am stopped at a light, the i-stop will go on for about 20 seconds, then suddenly turn off and the engine will rev up. I shard the stoplight information with the dealer the week after I took possession of the car and was told by the sales manager that this was just normal and a weird part of the electronics of the car. Following this incident, I called the dealer. The service department and manager said that they weren't aware of this issue but took my concern seriously. I then did an online search and found a conversation string on [XXX] with others sharing the same problem. I was driving 65 mph and the hybrid system malfunction warning came up. Car dropped to 25mph - pressing the gas pedal would not increase the speed. Continued to coast with no acceleration until it came to a stop and shut off

The car’s driver side mirror shakes (glass in mirror) and causing distraction while driving, which seem to be the widespread case. Other 2 CX-90 driven in dealership had the similar issues. The shaking is more prominent at highway speed and while going over bumps.

4 weeks ago my wife was driving on a busy highway and lost propulsion. Hybrid System malfunction message. She was able to pull over before causing an accident and put the car in gas only mode. She never shut the car off and drove on to her destination. I met her and picked up the car and took it to the dealer. Car is new and has 5000 miles on it. Car has been at dealer for 4 weeks with no answers and several unanswered phone calls to customer service at Mazda Corporate. Been told that is is a software problem with no fix and no time frame of when there will be a fix. I do have a case number with Mazda. Today after threatening calls to invoke "Lemon Law" in my state Mazda has suddenly said that the car has a faulty inverter and that the part is available and will be replaced. My feeling is that they are just throwing parts at a problem. This car is unsafe and NHSTA should put pressure on Mazda to either resolve issue or buy back cars. The majority of the complaints listed here are the same. Please take action!!

My car senses a vehicle in a lane to the left or right of me as a vehicle in my lane and slams on the breaks. There is a recall on this with Mazda, but not for my manufacturing date yet. It's very dangerous. Occurs all of the time

When cruise control is active but slower than set speed vehicle will full throttle acceleration to get to set speed creating a situation where loss of control of the vehicle partially occurs. If this happened in wet or winter conditions loss of vehicle control is very possible.

Car jas adaptive headlight and it malfunctions every other But when it happen low beam and high beam depth go down atlest 20%. This car has very bright light for low beam and since low beam projection tilt down and stuck there while you r driving this make situation so dangerous in night. Light low beam depth(till where it illuminates the road) is so bad it makes you blind and wont be able to judge where the road solid line going to turn. I almost ran into road divider Mazda dealer saying they don’t have solution and mazda has to provide it. I had this in writing when last went for servicing

- #11584033April 19, 2024SERVICE BRAKES, ENGINE
On April 3rd, around 5:20 PM, while driving with my daughter on a suburban street, my Mazda hybrid experienced sudden brake failure as I approached a stop sign. Attempts to engage the brakes failed, but they unexpectedly resumed, allowing the car to stop safely. This incident triggered multiple dashboard warnings including "Engine Malfunction," "Hybrid System Malfunction," and "i-Activesense System Malfunction." The hybrid battery’s range indicator also fluctuated erratically between 1 and 4 miles. The primary malfunction involved the vehicle’s braking system, with concurrent issues in the engine and hybrid systems as indicated by the alerts. The vehicle and all related components, along with the computer fault codes, are available for inspection and further testing at the Mazda dealership. The brake failure significantly endangered the safety of both myself and my daughter, as we nearly entered an active intersection uncontrollably, which could have led to a severe collision, posing a grave risk to both occupants and pedestrians. Despite extensive diagnostics and approximately 33 miles of test driving under various conditions, the dealership was unable to replicate the brake failure, raising serious concerns about the braking system’s reliability and predictability. The dealership's technicians, in cooperation with Mazda corporate engineers, have inspected the vehicle and analyzed the diagnostic data and fault codes. As of April 19th, the car remains with the dealership for further analysis. No additional inspections by police, insurance, or independent service centers have been reported. The inability to reproduce the brake failure and the spontaneous nature of the dashboard warnings underscore the necessity for continuous investigation by Mazda. Without a clear resolution, my trust in the vehicle’s safety is deeply shaken, especially for driving in high-speed or heavily trafficked areas.

- #11583951April 18, 2024POWER TRAIN, UNKNOWN OR OTHER, ELECTRICAL SYSTEM
Driving up snowy pass on I-90 approaching the top of Snoqualmie Pass. Vehicle stalled and warning lights flashed critical warning and loss of power. The vehicle stopped in the middle of the highway and would not start or go into drive. After a few minutes, because of speeding and adverse conditions, another driver collided with my vehicle from behind. Another following vehicle lost control and ran off the road. Nobody was injured. Mazda has not provided any information to what happened with the vehicle. We added about 12k miles in the 5 months of driving it, so we had lots of testing with it, and never ran into this serious of an issue with this or any other vehicle.

- #11583119April 15, 2024FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
When using adaptive cruise control on multiple lane highways/freeways the vehicle detects other cars/trucks that are in other lanes as being in your lane. (Shows on the dash display as in your lane). This fools the vehicle into believing it needs to make an emergency brake as a vehicle is in your lane and a crash is imminent when no such maneuver is necessary. This could cause a rear end crash or loss of control of the vehicle when it brakes unexpectedly with no real world cause. This has been an issue since vehicle is owned (July 2023) it is also widespread with this specific model [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

While connected to a level 2 charger, overheating caused both the charger and charging port of the vehicle to melt together such that it was very difficult to disconnect, and both were then unusable – needing replacement. (photos attached) We feel that there should be some safeguard in the vehicle to prevent this situation as it is a fire hazard. There were no warning lights, sounds, nor signals from the vehicle to warn of the problem. We were present when this occurred, so we could disconnect before a potential fire may have occurred. When we called for service, we had to wait two weeks for an appointment, and then were told that the car was not safe to drive until repaired. This was not considered a warranty item even though the car had fewer than 2800 miles and only owned by us for fewer than 7 months. We were charged $5000 for the charging port and cable assembly and $800 labor. (Copies of service receipts attached.) We contacted Mazda regarding our concern and were given Case #00182286. We received a reply by phone telling us that Mazda bears no responsibility for this (they feel ii was the fault of the charger) and that it is an insurance matter. In fact, auto insurance does NOT cover this. Our concern is that it very well may be a safety issue as people usually charge in their garages or near other cars and buildings in public. In either situation, a fire could be a serious hazard for many people.
