My vehicle experienced a total, premature radiator failure at 43,500 miles, after the manufacturer's factory warranty expired. The vehicle began leaking engine coolant rapidly. This creates a severe safety hazard, as a sudden loss of coolant leads to immediate engine overheating, potential engine stall, and catastrophic failure while driving at highway speeds, increasing the risk of a crash.A certified technician diagnosed the vehicle and provided video/photographic evidence confirming that the coolant leak is originating from a specific crack on the lower section of the driver's side radiator.This is a known, systemic manufacturing defect based on my research. Numerous other 2024 Mazda CX-90 owners on automotive forums have documented the exact same failure occurring in the exact same location on the lower driver-side radiator. The defect required a complete radiator replacement, forcing me to pay approximately $2,204 (with taxes and fees) out of pocket for a premature component failure that should safely last past 100,000 miles. I have digital photographic/video proof of the cracked unit from the service technician available for review.

I bought my 2024 Mazda CX-90 PHEV 10 months ago with 22000 miles on it and no and now has 29000 miles on it. First thing I had a issue with was a Motor Mount was bad, took 12 weeks to get part was told it was safe to drive. Next, since I have had the vehicle I have been having a issue with the transmission making a noise while both shifting from first to second and also on down shifting. It does not happen all the time but is happening. I took the vehicle in to the dealership to have it looked at and they were able to reproduce the issue. When they conferred with Mazda on the issue they were told that it was a known issue but there is no fix for it at this point and was dertermined safe to drive. I also talked to Mazda myself who also told me that it was a known issue but there is no fix and determined it safe to drive. I love the vehicle but to have this noise keep happening and being told they are working on a fix does not make me feel confident in the vehicle with this especially when my wife also drives it. My biggest question is this is a 2024 Mazda and I am told the 2025 also has the same problem. How long is the answer were working on a fix to long to believe that they actually are since there are now 2026 and soon to be 2027 models coming out. I am being told it is only the PHEV vehicles having this issue which explains the reason the PHEV model I believe is being discontinued. Would love to hear from you what my recourse would be. Recently, while driving our Mazda CX-90 (2025) we noticed a persistent and constant odor of fuel/gasoline vapors arise from the vehicle. We noticed this after driving the vehicle approximately 120 min from Las Vegas, NV to St. George, UT. The smell was intoxicating and nauseating. I took it into 2 local dealerships to investigate and called Mazda USA headquarters and spoke to a supervisor located in Irvine, CA. She explained to me that she gets complaints like this all the time during the summer months and that the larger Cx-90 is known to have the issue of leaking gasoline fumes/smelling of strong gasoline odor. I expressed my concerns for the safety of my family and surrounding environment including the flammable nature as well. The remedy that was provided was similar to the previous stating to park it outside and not fill up the gasoline tank greater than 3/4 full. I expressed that I drive a 2012 Honda Accord and do not have a gasoline leak nor strong odor of this magnitude nor have I ever experienced this before. We live in Las Vegas, NV. I am expected to drive my vehicle less and park it on the street all summer or when temperatures are above 100 deg? She stated, “We live in times where these newer vehicles are not made like the old ones.” I asked, “So is it this model that has the issue?” She replied, “Its the big one right, the CX-90? Yes, the bigger vehicles have this issue.” Is this considered normal to have a valve to release fuel vapors due to high pressures?

Driving down the freeway, the car throws an error code, reduces speed from 75 to 40 in seconds, with enough time to pull off the freeway. The car has an error code stating "Hybrid battery malfunction." Now the car will not start. The Dealer could not get it started and now wants to charge me for a diagnostic. But from my understanding, they should cover a new battery under EPA regulations (40 CFR Part 85). Any help would be greatly appreciated.

I purchased the vehicle used. The issue I have been having is the steering feels sticky, heavy, or briefly resistant when turning. It is very prominent when turning the steering wheel to the left, especially going around corners and rotaries, but even bends in the road. When trying to straighten out after the turn, I have to jerk the steering wheel back to the other direction. This becomes a control issue to keep within my lane. Driving straight the issue is present as the steering doesn’t self‑center smoothly. The issue happens once the car is "warmed up" - if I started cold in the morning, it is not present, but within a few minutes of driving the steering effort changes unexpectedly. The issue has been ongoing since March 2026.

The vehicle previously underwent Mazda Safety Recall 6524A involving the steering system and increased steering effort. After purchasing the vehicle, it developed a serious steering problem. At approximately 50–65 mph, the steering wheel can become stuck or extremely difficult to move when I attempt to steer left or right, as if the steering is physically sticking. This creates a serious safety risk because the driver may be unable to make an immediate steering correction at highway speeds. I brought the vehicle to an authorized Mazda dealership and specifically requested that they determine whether the problem was related to the previous 6524A recall. The dealership documented my complaint as the steering wheel getting “STUCK” at 50/65 mph and referenced Recall 6524A and increased steering effort. The dealership inspected the vehicle and marked the steering concern FAILED. Their written recommendation states that the vehicle “needs steering rack.” The estimated repair cost is $2,236.45, with a total estimate of $2,524.19 including fees and taxes. I escalated this issue to Mazda Corporate and requested coverage because the vehicle previously received a safety recall repair involving the same steering system. Mazda Corporate denied coverage, leaving me responsible for the repair. The financial burden is also a serious concern. I currently cannot afford the approximately $2,500 repair, meaning I am unable to have this documented safety defect corrected. This puts my family in the difficult position of potentially needing to operate a vehicle with a known steering issue and also raises concern that other owners could face the same situation if they cannot afford the repair. I am requesting that NHTSA investigate whether Recall 6524A adequately addressed the underlying defect or whether this represents

On multiple occasions, the steering wheel became stiff or locked while driving or during start-up, multiple dashboard warning lights illuminated, and the vehicle could not be shifted from Park to Drive. This occurred repeatedly since the vehicle was purchased. The dealer inspected the vehicle multiple times; on the first attempts they could not duplicate the issue. On a later visit, the vehicle displayed a lost communication fault with the electric power steering (EPS) system, and the dealer replaced the steering gear/rack assembly and EPS motor per the manufacturer’s technical hotline instructions. Despite this part replacement, the same steering/warning light condition recurred weeks later. At that point the dealer confirmed the defect but stated there is currently no authorized repair or parts replacement available, and that engineering is developing a permanent corrective action. The vehicle remains in my possession and is available for inspection. This defect creates a serious safety risk because the vehicle cannot be steered or shifted normally when the condition occurs, including while driving. No crash or injury has occurred to date.”

I am reporting a recurring safety-related issue with my 2024 Mazda CX-90 Turbo S Premium involving the mild hybrid system. The first failure occurred at approximately 28,000 miles. While driving, the vehicle displayed multiple warning messages, including "Mild Hybrid Battery Malfunction," "i-Stop Malfunction," and "Engine Malfunction." Shortly afterward, the vehicle entered a reduced-power (limp mode) condition. The loss of normal power made it difficult to safely accelerate and keep pace with surrounding traffic, creating a significant safety concern. The dealership diagnosed the vehicle and replaced the mild hybrid battery. My vehicle remained out of service for approximately one month while repairs were completed. At approximately 56,000–58,000 miles, the same problem occurred again. The vehicle once again displayed the "Mild Hybrid Battery Malfunction," "i-Stop Malfunction," and "Engine Malfunction" warning messages and again entered reduced-power mode while driving. The repeated loss of power created another serious safety concern because the vehicle could not accelerate normally when merging into traffic or driving on higher-speed roads. After diagnosing the second failure, the dealership informed me that, in addition to replacing the mild hybrid battery, corrosion had been found in the wiring harness and internal wiring associated with the hybrid battery system. Those components also required replacement. Due to parts availability, my vehicle remained at the dealership for an extended period before repairs could be completed. This vehicle has experienced the same hybrid system failure twice within approximately 58,000 miles, despite the first battery having already been replaced. I am concerned that this is a recurring defect affecting the mild hybrid electrical system. A failure that causes multiple system malfunction warnings and forces the vehicle into reduced-power mode while driving increases the risk of a crash, especially when merging onto highways

ISSUE 1 – UNINTENDED DECELERATION (Power Train): While driving at highway speeds (60–80 mph) with foot off accelerator for approximately 5+ seconds while coasting, the vehicle unexpectedly executes a sharp, single downshift causing sudden unintended deceleration and an RPM spike. No warning lights illuminate. Documented on 11 occasions between May 20, 2026 and July 29, 2026; incidents occurred prior to May 2026 but were undocumented. Occurs most frequently decelerating off highway exit ramps but also during general highway coasting in Houston, TX. A passenger was present during multiple incidents. Trailing vehicles are not anticipating the sudden speed reduction, creating rear-end collision risk. ISSUE 2 – FALSE BLIND SPOT / REAR RADAR ALERTS (ADAS): The Blind Spot Monitoring and Rear Radar Sensor systems generate daily false alerts — both audible and visual — when no vehicles or objects are present. Triggers randomly across varying conditions. False alerts have caused multiple aborted lane changes that were otherwise safe to execute. ISSUE 3 – LANE KEEPING ASSISTANCE (ADAS): The LKA system applies forceful steering corrections during intentional, signal-indicated lane changes, including in areas without lane markings. Occurs at highway speeds during on/off-ramp transitions. Corrections have triggered reactive maneuvers by surrounding drivers in dense traffic, causing near-collision situations. Dealer service appointment scheduled August 6, 2026.

The contact owns a 2024 Mazda CX-90. The contact stated that while her husband was driving 70 MPH with the cruise control activated, her husband attempted to deactivate the cruise control before exiting the highway; however, the cruise control failed to deactivate. The brake pedal was depressed, but the brakes failed to stop the vehicle. The contact's husband exited the highway and drove into the median to avoid crashing with other vehicles. The driver turned the steering wheel, causing the vehicle to flip and then slam into the ground, fracturing both axles. The cruise control then deactivated. The contact stated that only the front driver's side air bags deployed. The driver suffered hip and arm bruises, while the contact suffered a lump on the head, and as bruises on the collarbone and arm. The driver received medical attention at the scene, but the contact declined. A Police report was filed. The vehicle was towed to a tow yard but had not yet been declared a total loss by the insurance company. The contact stated that prior to the failure, the message "Trailer Attached in Tow Mode" was displayed and the vehicle was taken to the dealer, who replaced the wiring harness due to corrosion. The manufacturer was not made aware of the failure. The failure mileage was approximately 15,000.

While driving approximately 50 mph, we heard a sudden, extremely loud bang, followed immediately by the sound of shattering glass. We pulled over and discovered that the panoramic sunroof had shattered and appeared to have exploded outward. There was no collision, no object that we saw strike the vehicle, and no warning before the failure. The glass remained largely in place but was completely fractured. This occurred while my family was in the vehicle, including my young children. The sudden explosion startled the driver and created a potentially dangerous driving situation. I am concerned that a spontaneous sunroof failure at highway speeds could cause a driver to lose control of the vehicle or result in injuries from flying glass. I am reporting this because I believe it represents a serious safety issue that should be investigated to determine whether this is a manufacturing defect or other vehicle safety problem.
