I entered then attempted to start my Lexus on November 11, 2025. The car would not start - the dashboard lights fleshed briefly, then the car was completely dead. There was not a light on anywhere. The doors would not open and the windows would not roll down. I was trapped. fortunately I had my cell phone so I was able to call my wife who was inside of our house. She could not open the door with the manual key, She called AAA. The tech hooked up some power device under the hood and the car started and seems OK. This was frightening. I was trapped in the car and running out of air. I could have suffocated.

The contact owns a 2024 Lexus RX350H. The contact stated while attempting to start the vehicle, the vehicle failed to start as intended. The contact stated that the doors failed to unlatch, and the key fob failed to function as intended. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. Additionally, the failure had occurred while the contact and her husband were occupying the vehicle. The contact stated that luckily the weather was not cold, or a baby was not in the vehicle during the failure. AAA was contacted and provided a jumpstart to the vehicle. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 16,342.

My vehicle, a 2017 Lexus RX 350 with approximately 111,872 miles, experienced a near-total failure of the infotainment/navigation screen approximately five months ago (around June 2025). The system would only display the introductory safety message, backup camera feed, a single radio station, and basic map locator functionality—rendering the navigation, full audio controls, climate integration, and touch interface inoperable. This issue severely impacted the vehicle's usability and safety features, forcing me to seek immediate professional service.

On the evening of October 29, 2025, my 2025 Lexus RX 450h+ suffered two complete electrical failures within a few hours. The vehicle lost all power and became totally unresponsive — no lights, sounds, or electrical function. During the first failure, both of my young children (ages [XXX] and [XXX] were trapped inside because the doors would not unlock or open manually from the inside or outside. The failure occurred at night, and I was stranded for nearly five hours. I repeatedly tried to reach Lexus Roadside Assistance, but the calls disconnected multiple times before I reached an agent. A tow truck was dispatched with a 90-minute ETA, but before it arrived, the vehicle suddenly restarted on its own. We finally got home close to 1:00 a.m. I am a single parent and this was a terrifying, unsafe situation that could have resulted in serious harm if my phone had died or if outside help had not stopped to assist us. The vehicle was brand new (leased April 2025) and has approximately 5,000 miles. It was taken to Serramonte Lexus for inspection, and Lexus Corporate is reviewing my request for replacement or buyback. This is an extremely dangerous defect affecting the 12-volt electrical system and electronic locking mechanisms. The vehicle doors should never be inoperable during power loss, especially when children are inside. I am filing this report so NHTSA is aware of this incident and can investigate whether similar failures are occurring in other 2025 Lexus RX 450h+ vehicles. While driving, the instrument panel went blank. I believe it is not safe to drive with no instrumentation or knowledge of your vehicle or speed. The dealer confirmed it needed a software update and performed this yesterday. We were not advised of this needed update, no recall or no communication from the manufacturer. We should be advised when something this critical is needed.

The electronic screen on dashboard that displays map, HVAC, etc. turns pink then black and disappears for several minutes. One then has no control over the various functions displayed on the screen. I've done a hard reset by disconnecting the battery for several minutes...to no avail. Thank you.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I was driving the vehicle up a road I drive every day, when suddenly the instrument cluster showed a picture of a red pedestrian icon, and slammed on the breaks. It threw me forward, but quickly proceeded on like normal. It detected a pedestrian that was not in front of the vehicle. This was only 20 days after I got the car. It proceeded to do this three more times, with similar severity, but at different speeds and different locations. The fourth instance, I was driving home and the vehicle had the same pedestrian detection of an imaginary person, this time breaking me from 40 mph to 12 mph with a car behind me. Thank the lord I was not hit. I made a service appointment asap, and it was there for 8 days. I had it happen to me 5 times in 3300 miles and 2.5 months, and they drove it 12 miles and said they couldn’t duplicate it. Cooperate told me to send it up to another store which I did. It has now been there for 16 days, and they can’t find an issue either. I am not comfortable driving the car because it could have a breaking incident at any time, and is putting me in danger. Cooperate has told me to file a lemon case or quote “wait for it to happen again and try and document it in better detail.” I have given them locations, time and date estimates and weather estimates for every instance, and I’m still getting put back in a car that someone at Hendrick Lexus North considered a “liability.” I am beyond disappointed in Lexus for going against their word, is making me the guinea pig in a effort to try to replicate the issue. This is my third Lexus, and my image of the brand is ruined.

While we were driving, our sunroof just exploded! There was no-one around us. There was a loud bang, glass was flying and the wind was so loud we couldn't hear each other. If my wife would have been driving, we probably would have wrecked because she screamed and through her arms in the air. We exited off the road, inspected our vehicle and took pictures. The only damage we could see was the front section of our sunroof was shattered. We drove to the nearest dealership where they took pictures and ordered a new sunroof. I might add they couldn't guarantee the replacement would be under warranty, which I find shocking. My wife and I are on vacation and we had to leave our car in another state along the way and rent a car.

Heard clicking noise coming from steering wheel. Car shimmied when breaking off of freeway. Newish car, only 20,611 miles. Took into dealership before anything went awry. Dealership said never heard of the steering problem before. Turned out it was a tie rod going bad and repair was covered by warranty. Break issue was also covered by warranty.

Odometer Fraud. The contact purchased a 2010 Lexus RX450H. The contact discovered that there was a mileage discrepancy after the purchase of the vehicle. The vehicle was a private sale. At the time of the purchase, the vehicle mileage was 118,000. It was later discovered that the mileage was 183,000.

After adding gasoline to the tank, gasoline begins dripping from the left rear of the vehicle to the rear of the left rear wheel well. This happens even if the tank is only filled to 2/3 of a tank. I have stopped filling up since when the tank is filled up a LARGE amount of gasoline leaks out onto the ground. FIRE HAZARD while parked or driving. I have seen HUNDREDS of reports of this problem reported online.
