The contact owns a 2025 Lexus RX350. The contact stated that while driving at an undisclosed speed, the vehicle suddenly stopped and the "BRAKE" warning light was illuminated with the accelerator pedal depressed. The vehicle was taken to the dealer, where it was diagnosed as not having a malfunction in the collision avoidance system. The dealer stated that the collision avoidance camera had captured a white van, which prompted the system to activate; however, the contact stated that there was no van near the vehicle. The contact requested further details of the footage captured. The failure mileage was 48,000.

While driving my 2026 Lexus RX350h, the vehicle suddenly lost propulsion and would not respond to the accelerator. After coming to a stop, pressing the gas pedal did nothing. I had to shift to Park, press the brake, and press the Start button to get the vehicle moving again. A short time later, while making a right turn, the vehicle lost power again while still moving, forcing me to repeat the Park–Brake–Start sequence in the middle of the turn. This is a serious safety issue because the hybrid system appears to shut down unexpectedly, causing loss of propulsion in traffic. The vehicle was towed to the dealer, who stated the battery was “good,” but no cause was identified. This needs investigation because the vehicle can unexpectedly lose power and require a restart while driving.

On October 16, 2026, the panoramic sunroof glass on my 2024 Lexus RX 500h shattered without any impact. The vehicle had about 78,000 miles. I was driving at roughly 70 mph on a highway when the glass burst with a loud bang. The glass blew upward and outward, not inward, and there was no road debris, no overhead object and no sign of anything striking the roof. No impact point or chip is visible on the remaining glass. No one was injured. The vehicle was out of the 4-year/50,000-mile basic warranty. The Lexus dealer inspected it and is replacing the sunroof assembly. I am filing because the failure happened with no external cause and matches other reports of sunroof glass shattering on this model.

My 2024 Lexus RX 500h experiences an intermittent failure to respond when I press the accelerator pedal. During these episodes, pressing the pedal does not produce the expected acceleration, making the vehicle’s response unpredictable. Lexus service bulletin L-SB-0026-25 was performed on my vehicle to address the issue. However, the accelerator nonresponse continues after that repair. The issue remains unresolved. I am concerned that an unexpected inability to accelerate could leave my vehicle exposed to approaching traffic when entering a roadway or crossing an intersection. This is a safety concern because I cannot reliably count on the vehicle to accelerate when needed. I request that NHTSA review this recurring loss of accelerator response, including cases where the condition persists after the software update. The underlying cause of my vehicle’s continued problem has not been established.

Dangerous Vehicle Failure on Highway XXX —XXX On the morning of XXX, at approximately 9:00 a.m., I drove the vehicle from [XXX] to the [XXX]. It was raining during Hurricane ISAIAS, and traffic stopped because of an accident approximately 2,000 meters ahead. After waiting in traffic and rain for approximately 15 minutes, my vehicle suddenly displayed multiple warning messages, including hybrid-system and shift-system warnings. Within seconds, the vehicle became inoperable. My vehicle was left stranded in the middle of Highway 141 as traffic began moving again. Vehicles behind and beside me honked, and I was frightened because I could not restart the car or move it out of the traffic lane. Also, the vehicle's Lexus SOS function is not working that I try. I called the dealership's service department, but after being transferred to roadside assistance, I couldn't get help. I subsequently contacted [XXX] for help. An officer arrived on site and helped move the vehicle to the roadside in the storm. I continued trying to restart the vehicle. After approximately 10 minutes of repeated attempts, it finally started and became drivable again. I was advised by the officer that I could drive home or take it to a repair facility. Given the seriousness of the incident and the vehicle's history, I drove directly back to the Lexus dealership. This incident was extremely frightening. In the storm, a vehicle that suddenly loses its ability to operate in moving highway traffic can put the driver, passengers, and other road users at serious risk. Below are the pictures showing the car error message while the car stopped to run: Image1: Hybrid system stopped, Start Power Low. Image0 & Image2: Sift system malfunction, Apply Parking Brake, Secure when Parking, See Owner's Manual. History: 1.XXX, back to dealer for shit trouble 2. XXX, shift problem at dealer after accident repair in Lexus center. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

We had a known oil line ruptured the oil cooler, there was a serviceable and sit out to the dealers to only repair if it was leaking and they extended it to 10 years $150,000 miles our vehicle had a rupture at 170,000 miles we were never told about the recall or we would have it done earlier we now have sparkles in our oil this is the part that it doesn't leak it ruptures I'm sure all out could place a fire and it certainly is can cause catastrophic damage to the engine 1500 to repair

What component or system failed or malfunctioned, and is it available for inspection upon request? Response: Door Actuators How was your safety or the safety of others put at risk? Response: Response: Doors can’t be opened from outside as doors are permanently locked, plus doors must be manually manipulated from the inside to open versus pressing a button. I am concerned as this affects the child lock and can trap the occupants in the case of an accident. Has the problem been reproduced or confirmed by a dealer or independent service center? Response: Response: Yes, confirmed by the dealer. This issue has been occurring multiple times since 2018 and is currently an issue. I have documentation of this fact. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Response:Yes. Inspected and confirmed this is an issue by the dealer. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Only "warning alerts"are multiple "beeps" when trying to open the door from the outside.

- #11767853September 29, 2026STRUCTURE:BODY, FORWARD COLLISION AVOIDANCE: WARNINGS, BACK OVER PREVENTION: WARNINGS
The factory-installed front spindle grille and bumper assembly on this brand-new 2026 Lexus RX 350 suffered severe thermal material failure, resulting in extensive warping, bubbling, and physical melting under normal, predictable outdoor ambient daylight parking conditions. This thermal deformation poses an immediate safety hazard because the deformed exterior structure directly houses the vehicle’s critical front-facing Advanced Driver Assistance Systems (ADAS). This includes the pre-collision safety radar sensors, pedestrian detection cameras, and proximity sensors located right behind the front Lexus emblem and grille assembly. The physical distortion of the plastic mounting housing can cause sensor misalignment, resulting in unexpected automatic emergency braking, failure of the pre-collision warning systems, or sudden blind-spot radar failure while operating the vehicle at highway speeds. Lexus Corporate (Case #260917000986) officially inspected the vehicle and explicitly denied all warranty and goodwill safety repairs, formally stating the vehicle's body components are melting due to a "solar event" concentration. Furthermore, the authorized Lexus dealership service management explicitly stated that this is a widespread, "common issue" that takes only 5 minutes of outdoor sunlight reflection to cause catastrophic component melting. The vehicle remains in its exact original factory configuration with under 2,000 miles. It is a severe safety defect to manufacture a modern premium passenger vehicle with safety sensor housings that melt and lose structural integrity under normal outdoor sunlight exposure. The manufacturer is actively refusing to correct this material hazard, leaving the safety radar systems compromised.

- #11767292September 27, 2026SERVICE BRAKES, ELECTRICAL SYSTEM
Electronic Parking Brake (EPB), 12V Battery/Electrical System. My vehicle's 12V battery failed, with the dealership measuring voltage below 10V during the event. This caused multiple simultaneous module malfunctions (confirmed by the dealer as caused by the battery failure), including a complete inability to move the vehicle after jump-starting on three separate attempts — the engine ran, but the vehicle would not drive despite a functioning alternator. The vehicle's steering wheel also locked, preventing the car from being pushed to a safe location. Approximately 3 months after this event, a routine service visit revealed the rear brake pads had worn from 8mm (measured at time of purchase) to 2mm (failed), while the front pads wore from 10mm to 7.8mm (normal wear) over the same period and mileage. The rear pads wore roughly 3 times faster than the front, which is inconsistent with normal vehicle operation, since front brakes handle proportionally more braking load and are fitted with larger rotors for this reason on this and virtually all vehicles. The rear brake caliper is the only caliper equipped with the EPB motor/actuator; the front calipers are purely hydraulic. The selling dealer's pre-purchase report also noted noise from the rear brakes at time of purchase, which is consistent with the EPB's position calibration having been disrupted at some point prior to my ownership (e.g., from a prior battery event or service) and never properly relearned/recalibrated afterward — producing a low-level drag from the outset. My own battery failure event (voltage measured below 10V) would then have compounded this already-uncalibrated system, worsening the drag and producing the rapid 6mm of wear observed in the following months. This points to one underlying EPB calibration fault, aggravated by repeated voltage instability over time, rather than an unrelated mechanical issue — see attached documents for more information.

While driving on the highway, the right rear fender trim piece detached from the vehicle. It could be seen flying off in the mirror. It’s likely that the trim piece wasn’t properly secured during the vehicle’s manufacturing process. It could have hurt someone driving behind me.

This is original Goodyear Eagle Touring tire on 2025 Lexus RX 350. It M+S 235/50R21 101V. There were no low tire pressure alerts before the blowout. I was entering a ramp of a freeway (XXX in [XXX]) and suddenly heard a loud noise. Unfortunately due to ongoing construction there is no shoulder or curb in the stretch of the road. Also it was dark; about 4:30AM. As a result I had to drive some a couple of miles before I could exit and part safely. By that time tire was shredded and wheel damaged.
