Vehicle: 2019 Honda Pilot VIN: [XXX] My 2019 Honda Pilot is experiencing an "Emissions System Problem" accompanied by Diagnostic Trouble Code (DTC) P219A (Air-Fuel Ratio Imbalance). This defect causes engine hesitation, rough idling, and a severe risk of sudden loss of motive power or engine stalling while driving at highway speeds, presenting a significant safety hazard to myself and other motorists. American Honda is fully aware of this manufacturing defect. They issued Service Bulletins 20-100 and 21-010 extending the warranty to 10 years/150,000 miles for this exact code and symptom on 2017–2019 Honda Pilots due to defective fuel injectors and faulty PCM software. However, Honda has arbitrarily excluded my specific VIN from this extension (case number Case # XXX). Honda Corporate has officially denied assistance, forcing me to drive a vehicle with a known fuel system defect that threatens sudden vehicle failure in traffic. I request that the NHTSA investigate Honda’s arbitrary exclusion of affected VINs for this acknowledged safety defect. SERVICE BRAKES
The contact owns a 2025 Honda Pilot. The contact stated that while driving approximately 30 MPH on a clear day, the braking system activated without driver input and caused the vehicle to come to a sudden stop. There were no other vehicles, pedestrians, or objects nearby to cause the automatic braking system to be activated. The sudden braking was recorded by the Insurance App. Due to the abrupt stop, the contact struck her head on the steering wheel when the vehicle jolted forward. The contact stated that the seat belts failed to respond or properly restrain the contact during the failure. There were no warning lights illuminated. The vehicle resumed normal operation after the failure. The local dealer was contacted, and a service appointment was scheduled. The vehicle was not diagnosed or repaired. The manufacturer was notified and opened a case about the failure. The manufacturer advised that a representative would provide a follow-up call. The contact was additionally provided with the telephone number for Roadside Assistance. The failure mileage was approximately 17,500.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, LANE DEPARTURE: WARNING, FORWARD COLLISION AVOIDANCE: WARNINGS
Warning lights for Adaptive Cruise Control, Lane Keeping Assist, Collision Mitigation, Road Departure, and Auto High Beam all came on simultaneously. From reading several Pilot forums, this seems to be very widespread and dealerships want anywhere between 1500 and 1900 to replace and calibrate the camera system which controls these functions. If it’s widespread, the vehicles should be recalled.

In November 2025, my 2021 Honda Pilot started experiencing stalling while driving, hesitation when accelerating, and sudden power loss while idling. I took the vehicle to my local Homda dealership and each time they dismissed the problem, indicating that they could not find any issues. On 4/24/26, the vehicle experienced sudden engine failure in the middle of moving traffic. There were no warning lights or dashboard messages leading to the issue. The vehicle was towed to the local Honda dealership, where they kept the vehicle for a month and charged over $20,000 for a repair (not replacement). Ten (10) days after picking up the vehicle from the dealership, the engine locked and seized again on 6/9/26 in the middle of traffic and nearly caused a serious accident with 3 other vehicles.

The contact owns a 2015 Honda Pilot. The contact stated that upon pulling into the gas station and parking the vehicle, the vehicle made an abnormally loud sound, and the contact also noticed a grinding sound coming from the underbody of the vehicle. The contact stated that the grinding sound persisted and became louder while reversing out of the parking spot. The vehicle was towed to the residence. The vehicle was then towed to Mavis Tires the following day. The vehicle was inspected and diagnosed with sway bar, suspension, and strut failure. The vehicle was repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 270,000.

What component or system failed or malfunctioned, and is it available for inspection upon request? The engine stalls out and shuts the car down. How was your safety or the safety of others put at risk? While driving down the road, if the engine shuts off, myself, passengers, and other vehicles could cause a fatal crash. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I have brought this in several times to try and fix this issue. The fuel pump has been replaced and the auto idle recall has been take care of. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The check engine light comes on occasionally when it happens; sometimes when it does not. Other details: Several times now my car has randomly shut off especially when driving down the road. I wasn't sure what happened, but I finally figured out a way to replicate it. Basically if you drive it after about an hour of non-stop driving / engine being turned on, the engine overheats causing the car to stall out and eventually turn off. I timed this at 10:15AM to 11:25AM and my suspicions were correct. At 11:25AM, the car stalled and shut off while I was parked in my driveway.

The car makes a popping/static sound weather turned off or on . The infotainment center and screen has shut off while driving and using GPS leaving me to scramble with my phone to get a GPS on and this happens without any warning so I don't have any time to pull over to a safe place . I have taken the vehicle to 2 different mechanic shops and they did not find anything when hooked up to their computer . This is a random problem that happens at different times ,there is no telling when it will happen .when the popping/ static sound starts it is difficult to hear the music and the GPS on the car is not always correct it leads to the wrong places.

Please find the details regarding the sudden, dangerous failure of my vehicle below. The itemized findings are pulled directly from the dealership’s multi-point inspection and diagnostic report. When was the first time (approximate date) you noticed the issue? The issue culminated in an immediate, hazardous crisis on XXX when the vehicle completely shut down while in operation and refused to restart. How often does the issue happen within a time frame (e.g., once a month)? This is a permanent, catastrophic mechanical failure. The engine is completely inoperable and the vehicle is entirely undriveable. What is the name & title of the person(s) assisting you at the dealer? I am being assisted by Victoria Dalessandro at Honda City Long Island in [XXX]. What is the reason/explanation provided by the dealer, as to what caused this issue? Following a formal engine teardown, the dealership confirmed a severe internal failure: a lower engine knock originating from the front connecting rod bearing. This internal component defect completely seized the powertrain, causing the vehicle to shut down and rendering the engine unrepairable. What repairs/services have been completed (if any)? Only the initial teardown and diagnostic inspection have been completed. No repairs will be authorized until American Honda Motor Company, Inc. assumes corporate financial responsibility for this premature powertrain failure. What repairs/services have been suggested? The dealership has stated that a complete engine replacement is required, suggesting a used engine swap ($7,609.92). In conjunction with this catastrophic failure, they have itemized necessary peripheral parts and critical safety wear, including new spark plugs, a replacement drive belt, failed lower control arms ($1,704.28), leaking front struts, degraded stabilizer links, and fluid exchanges. costs estimated for the repair (if any?) Automatic transmission problem. Got onto freeway as I started to pick up speed the car did not go out of first gear. I could hear it. The car would not move forward at all. It felt like it was in nuturel. I was able to pull over side of road. This happened about last week may 2026. This also happened about 6 months ago. I bought this car in 2022 hardly any miles on car. There is a defect in transmission. If I was on busy highway I would of gotten hit for sure. Transmission problem. car not shifting to higher gear when getting onto freeway!

She took the car to the dealer, she asks if any recall for power break buster .They reply NO. She ask how much is to check the problem with the breaks. They reply $ 350 just to tell her the problem . The repair is $1000 dollars or more .She took the car to King Muffler Hollywood were they drove the car to see the complain . They check the car give her an Price for $ 880 for parts and labor . After 3 mounts she whent to the Nation wide honda to see if is any reimbursement . They told her to call Honda Corp.we call Honda Corp says NO . I am filing a safety complaint regarding a 2024 Honda Pilot Touring with approximately 46,000 miles. The exact component or system that failed is currently UNKNOWN. My best assessment is that this may involve the electrical system, powertrain, engine control/software system, charging system, Auto Idle Stop system, or another related vehicle control system. The vehicle is currently available for inspection upon request. The vehicle has had intermittent multiple warning lights appear on the dashboard. On June 7, 2026 around 9:00pm, while the vehicle was in Drive, the vehicle shut off/lost power. This created a serious safety risk because the vehicle was being operated when the shutdown/loss-of-power event occurred. I am concerned this could happen again while driving, including at freeway speeds or while transporting a child. The problem has not yet been reproduced or confirmed by the dealer. The vehicle was taken to a Honda authorized dealership for inspection. The dealership advised that the battery tested fine and that no diagnostic trouble codes were found so far. Diagnostics are still pending, and as of this complaint, no confirmed cause, diagnosis, or repair has been provided. The vehicle has not been inspected by police or insurance representatives. American Honda Motor Co. has been contacted, and a case has been opened regarding this concern. Warning lamps/symptoms did occur before the shutdown/loss-of-power event. The vehicle had intermittent multiple warning lights on the dashboard before/around the time of the incident. The warning lights first appeared on or around June 3, 2026. The shutdown/loss-of-power event occurred on or around June 7, 2026. I am requesting that NHTSA document this safety issue involving a 2024 Honda Pilot Touring shutting off/loss of power while in Drive with no confirmed diagnosis or repair at this time.

SEAT BELTS
The middle right side passenger seat belt alarm is going off and indicating it is not clicked when it is. I have had my [XXX] son and [XXX] daughter (different weights) sit there and the alarm will go off constantly indicating their seat belt is not fastened. I have also had adults who were heavier sit there as well and its doing it constantly. The alarm will not shut off and flashes on the dashboard that it isn't fastened. Is there an issue with others? I have used canned air to blow out the seatbelt thinking there may be something inside the latch. When this first started beeping it would only beep a few times and indicate it wasn't fastened. Now the alarm keeps beeping and you can't even sit in the seat due to the alarm constantly going off and indicates not buckled on the dash board. We had the engine serviced for recall on stop start issue on 12/4/25. On May 15, 2026 the engine failed. The dashboard came on with every warning light and a mechanic said the engine was knocking and had to be replaced. Honda denied any issue and said the timing belt had to be broken. The mechanic confirmed no issue with timing belt and said you can’t start the engine if the timing belt was broken. The mechanic said the engine was knocking and had to be replaced because the car could no longer be driven safely.

ENGINE
There is a loud metallic knocking sound coming from my engine. I have lack of acceleration power, and the check engine light came on. You are more than welcome to come inspect the car. My safety was placed at risk as my engine can blow up, and/or I lose acceleration while driving and cause an accident. The sound is continuous and will be until the Rod Bearing is fixed so it’s easily reproducible. Yes it’s been looked at by a mechanic.
