The contact stated that while driving approximately 5 MPH and making a right turn onto the driveway of the home, the vehicle suddenly experienced unintended acceleration and drove forward into the garage door. The contact indicated that during the incident the accelerator pedal was not being pressed. No injuries were reported. The garage door and home were damaged. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer was notified of the failure. The local dealer was not yet contacted. The failure mileage was 160. The contact stated that the vehicle was a total loss and not repairable.

While driving from Austin Texas to San Deigo Cal. Had on at least a dozen occasions sharp braking while vehicle was in cruise control. Most of these sharp braking incidents occurred in very light traffic, sometimes no traffic on the interstate hwy. our speed would drop as much as 20 mph in an instant.

What component or system failed or malfunctioned, and is it available for inspection upon request? experienced phantom braking, especially during dry weather with sun, where there are the mirage of water shimmering across the road. this happen too often when mirage are present. How was your safety or the safety of others put at risk? It get to the point I would rather disable its safety feature and drive manually. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? None. Note the driver is deaf, so they are also poor in visible warning compared to audio warning.

Frequent, random, phantom braking using cruise control. Informed Tesla with times +/- one minute. Sometimes the braking was very hard and could cause a rear-end collision (at highway speeds). Always occurred with no warning and highly variable conditions (light, or hills, or overpasses, and then apparently nothing). I called Tesla and technician said overpasses were a problem. Vehicle and components have not been checked.

I have experience phantom braking on my Model Y many times when I used FSD(full self drive) on the freeway. The car just randomly slows down / decelerates abruptly even if there are no obstacles or other cars in front. This results in a serious situation when the vehicle behind may ram into you causing a fatal accident. I have always reported this issue using voice control REPORT BUG and got acknowledgement saying THANKS FOR THE FEEDBACK. But I have never heard from Tesla that they have fixed or are fixing it. This is repeated happening every time I use FSD on freeway even as of last week (week of Aug 21, 2022)/

RE: Problems with cruise control and random braking I purchased the above model Y performance in June 2022 and we just took our first road trip the last two weeks of August 2022. We started in Sacrament, California and drove to Twin Falls, Idaho, Salt Lake City and return to Sacramento, CA. I started using the auto pilot steering and cruise control on the freeway (HWY 80 eastbound). At freeway speeds the car would suddenly and randomly apply the brakes, sometimes a very hard brake, which was scary and unsafe. I expected this normal feature to work properly on this road trip. As I traveled from Reno, Nevada to Twin Falls, Idaho, this random braking happened over 50 times. I tried several work arounds by disabling the auto steer, the emergency braking and all the automatic options, however, this random braking issue continued. There was no pattern and often it would occur with no other traffic on the road and perfectly clear road conditions. These so called safety features became a safety hazzard. I turned off the cruise control for long stretches because the random braking caused driver stress and fatigue. When I got tired of pressing the accelerator pedal, I would engage the cruise control and my manual work around was to have my foot hover over the accelerator pedal to override the braking manually. This was stressful since I never know when the hard braking would occur. This problem makes the car unsafe and very difficult to drive on a road trip over freeways. It also significantly impairs the value of this very expensive car since a standard feature (cruise control) does not function properly. It is scary to drive with random sudden hard braking. I bought this car for my wife and now she won’t drive it on a freeway due to this serious malfunction. I would like a response as to how you will remedy this problem

The contact took the vehicle to the local dealer and was diagnosed with needing to clean the side sensors that was thought to have caused the failure. The dealer cleaned the sensors. The manufacturer had not been informed of the failure. The failure mileage was approximately 13,000.

During daylight hours when the autopilot is engaged at normal highway speeds - the Tesla brakes engage and quickly slow down the car. It occurs on long straight roads with no traffic. This appears to be a "phantom braking" issue that is causing my car to suddenly brake for no reason. There are never any cars around when this occurs-- just long clear roads. This can be very dangerous. Last known occurrences were on Aug 13 at 8:30a and 10:29a of this year.

Tesla Autopilot (Adaptive Cruise Control) rarely recognizes parked vehicles or obstructions beside the driving lane (right or left side) based on the display console driving environment display. Bicyclist, pedestrians, and other large roadside obstructions are also rarely recognized. Might explain the recent Tesla collisions with stopped emergency vehicles while in Autopilot mode. Don’t know if Teslas recent elimination of radar sensing made this problem worst or not. This failure to recognize nearby obstructions happens daily.

I had unexpected and sudden braking while using Tesla autopilot while in TAC mode or adaptive cruise control. Not Autopilot that steers and keeps you in the lane and not Full self driving, my car does not have FSD. I had 5 or 6 incidents while heading West on I 10 towards Yuma on I 8. There was no rhyme or obvious reason for the braking. No one in front of me or behind me. The amount of braking over the separate incidents was different each time. On the way back to Tucson it happened another 5 or 6 times.

I own a 2021Tesla Model Y. I took possession of it on August 7, 2021, in Santa Barbara, Ca. Ever since, I have had numerous instances of abrupt automatic emergency braking (phantom braking) episodes while using cruise control or autopilot. On January 26, 2022, I had 47 episodes, in just a couple of hours. This is typical. Several of the phantom braking episodes occurred with vehicles tailgating me. This is a grave safety issue and requires immediate attention. My Tesla Model Y has been to the Tesla service center over 4 times regarding this problem without resolution. They say phantom braking is a software issue and only the engineers can fix it. I was assured soon after delivery that the software would be updated quickly, and it would fix the problem. The issue still exists. I am very disappointed with the vehicle and fear for my safety. Please require Tesla to remedy this very serious safety issue immediately.

I have an ongoing problem with the dashcam recording, which for me is a safety issue. The dashcam constantly records from several cameras what is happening around the vehicle. The dashcam icon will show a red “x,” indicating that the dashcam is not working. Probably 80% of the time I get in the car, the red x is there. The solution involves opening the glove box and pulling the drive, waiting for up to 20 seconds, then replacing it. If that doesn’t work, the usb drive needs to be formatted and all previous clips are lost, but probably 95% of the time the reformat fails. It was recommended by Tesla service that I purchase another usb drive ($45?), but that did not solve the problem. I’ve had my car into Tesla service for this issue or reported it through the app multiple times. For months, I’ve been told it will be addressed with a firmware update, but it has not happened. Other Tesla owners are also having this issue, but it was addressed with a firmware update and the problem was fixed. The owners who have the Full Self Driving Beta have NOT had a firmware update to address the issue. We are the owners who paid $10-15,000 extra for the full self driving feature. I have not been able to get any information from Tesla about an expected timeframe for this update. I ask, but I’m always told it will be a “future update.” I have asked if anyone is working on this issue, and they won’t answer that question either. I would like to know the status. I’ve been very patient, but since there is aggression in the areas where I drive against EV drivers, and Teslas in particular, I need this feature to be working. I have been harassed, had large pickup drivers tailgate or try to run me off the road, and I’ve been coal rolled. I’ve already used data from the dashcam to report aggressive drivers to the police.

In addition to the car only achieving 65% of its actual miles range claimed the cars operating system will freeze while driving (once every 3 days on average), forcing the driver to reboot the car while driving (hold both scroll wheel buttons on steering wheel). This takes away the ability to read the speedometer, to use indicators and basically ALL of the functions of the car. The rear view camera display also fails about 10% of the time which I have recorded. The car will also SLAM on the brakes while on the highway, driving in the rain on autopilot to allow a merging DHL van into my lane instead of switching to the left lane as advised by DOT/DMV and I recorded this on dashcam yesterday coming back from NASA. It is a daily issue that passengers in my Uber cannot buckle their seatbelts because the buckle is too low into the seat for them to easily find, in some cases passengers get disgruntled and I am forced to park and wait until they can buckle themselves sometimes taking over 5 minutes. The driver assistance features do not always work and also alert me and my passengers when it is unnecessary making it seem more like a gamble than a safety feature; sometimes it works, sometimes it doesn't, sometimes when it shouldn't. The false claims and inconsistencies are unlike any other vehicle I've ever driven. Tesla is a CRIMINAL enterprise acting purely in self interest and preservation to maximize profits and MUST be held accountable and to the standards of all other vehicles in the USA driving on our roads. I am APPAULED at the way Tesla has silenced me (blocked emails and blocked my ability to schedule service) and accused me of trying to cheat on my wife with the non-existent female Tesla employees on my BBB complaint which the BBB closed out claiming they solved it when Tesla didn't address a single complaint, just attacked my character publicly also known as slander. Also 250kw superchargers only charge at 50kw after preconditioning without other cars chargingFU

Autopilot was on during a light rain event where I was traveling less than the posted speed limit on the I95 northbound Highway. The autopilot appeared to force the car to abruptly jolt the steering wheel/tires, causing a slip/ hydroplane of the front end which forced the car into a left/sideways slide across the highway. The car slid sideways for about 100 feet or so before going straight into the concrete divider along the Highway (front driver side initial impact, then rear driver side 2nd impact). Control could not be regained with the vehicle. Steering did not work (tried to correct the path), it felt as if brakes did not slow the vehicle as the car approached the divider. Autopilot was fully engaged and lane keeping was active… with at least one hand on the steering wheel and moderate level of attention to the road. I must say I had likely became over-reliant on the autopilot. No warning was provided to me as, prior to the incident. A similar hydroplane problem can also be seen on a YouTube video posted online for the same model car. I experienced the same outcome as the other vehicle except I hit a divider after crossing lanes. I am however very pleased with how well the car handled the impact and how well the airbags went off, although also unfortunate- I do feel like the car may have been partially responsible for the accident itself.I have experienced a few prior occurrences where the car will jerk the wheel when in autopilot to keep or find a lane, even with hands on the wheel and full attention facing forward.

The contact stated while his wife was driving approximately 65 MPH, the TPMS warning illuminated and the contact was advised to pull off the road. The contact stated that the rear passenger’s side tire had disintegrated and the entire tread had separated from the side wall. The contact had the vehicle towed to a tire dealer and had the rear passenger’s side tire replaced; however, the failure reoccurred without warning light. The vehicle was not diagnosed and the tire was not inspected. The manufacturer was not notified of the failure. The approximate failure mileage was 13,926.
