I have never been in an accident in my 31 years of driving. However, I am worried that this van is going to cause me to be hit. Several times I have pulled out to make a left or right turn and my gas pedal does NOTHING. I can push it clear to the floor and NOTHING!!! Cars end up slamming on their brakes and honking at me but there's nothing I can do. This is EXTREMELY scary and a HUGE HUGE HUGE manufacturing flaw. There needs to be a recall for this to be fixed before people are injured.

I am reporting a potential safety issue involving the passenger-side power sliding door on a 2022 Toyota Sienna. On XXX my [XXX] child was outside the vehicle in a wagon while the passenger-side sliding door was closing. He reached toward the vehicle, and his fingers became trapped in the sliding door, closest to the front passenger door. The power door did not stop or reverse after contacting his fingers. It continued closing, clicked shut, and fully latched with his fingers caught in the door. We immediately opened the door and removed his hand. Thankfully, he was not seriously injured, but the incident had the potential to cause a severe hand or finger injury. I am concerned that the obstruction-detection or pinch-protection system did not detect an infant’s fingers in this location. I am submitting this complaint so the incident is documented and can be compared with any similar reports involving Toyota Sienna power sliding doors. Vehicle safety SOS button is not functioning properly. When the SOS button is depressed in case of emergency, the vehicle notifies user at the emergency system is un operational. Toyota includes this for 10 years from the date purchase and my vehicle is a 2021 making this feature active till 2031. By having the SOS button nonfunctional occupants of the vehicle, which may rely on this feature may sustain injury or death by this feature not operating correctly in the event of an accident.

Seat height not adjusting is ludicrous and a hazard! My husband is 6'4", my sons, 6'1", they seat height is not safe and braking/turns cause them to hit their heads. This could be life threatening in a real accident. There should be a lower profile seat that could be installed by Toyota. This is not safe!

I am submitting this complaint to request that the National Highway Traffic Safety Administration (NHTSA) document and evaluate what may be a broader defect affecting certain Toyota Sienna vehicles and determine whether it presents a potential safety-related defect trend. My 2015 Toyota Sienna was diagnosed by an authorized Toyota dealership with an external rear-bank head gasket leak. A head gasket is a critical engine component, and failure can result in coolant loss, engine overheating, reduced engine performance, stalling, or complete vehicle disablement. These conditions may create a safety risk, particularly if they occur while driving in traffic or at highway speeds. An authorized Toyota dealership confirmed the diagnosis and submitted a goodwill assistance request to Toyota. The dealership noted my history as a repeat Toyota customer and recommended that assistance be provided. Toyota denied the goodwill request based on the vehicle's age and mileage. I subsequently appealed through Toyota's customer relations process, and the denial was upheld. I understand that NHTSA does not resolve individual warranty or customer-assistance disputes. My purpose in submitting this complaint is to request that NHTSA determine whether similar reports exist involving premature head gasket failures in this model year or related vehicles and, if warranted, evaluate whether a broader defect trend exists that could present a safety concern. Potential safety consequences include engine overheating, loss of power, stalling, and unexpected vehicle disablement. The vehicle has been maintained in accordance with manufacturer recommendations and has not experienced any overheating event that would reasonably explain a premature head gasket failure. Thank you for your consideration and for documenting this complaint as part of NHTSA's review of potential vehicle safety issues. Respectfully, Vehicle Owner

- #11750824July 16, 2026TIRES, TIRES:TREAD/BELT
I have requested urgent warranty evaluation regarding two Kumho Solus TA51a tires installed on my Toyota Sienna (Model: Kumho Solus TA51a, Size: 235/60R17 102H, DOT: 1824) Both front tires are exhibiting abnormal tread chunking and visible surface shredding. The tires are relatively new (purchased about 1-1.5 years ago) and have not been heavily used (only riding this car occassionally, about 5000-7000 miles per year). Our 2 rear tires are Kumho tires of different model, and although older, they are performing fine and do not experience these issues so it seems to be an issue on this particular Solus TA 51a model Kumho tires. Because this is our family vehicle and we transport our children in it regularly, we are very concerned about the safety of continuing to drive on these tires. I also returned to the tire shop that sold and installed the tires. After inspecting them, the owner advised me to contact Kumho for a warranty evaluation, informing me that he received similar complaints from other customers (especially Toyota Sienna owners) using this tire model. While I no longer have the original purchase receipt, the dealer can verify that the tires were purchased from and installed by his shop. I reached out to Kumho Tires USA by completing their online contact form 2 wks ago, which informed a response within 48 hours, but I have not received any response. I called and left a voice message in their warranty department, and in their general customer service line earlier this week, but still not response. I even completed another contact form and also reached out by email, but still no response. Then I remembered my mom recently sharing a concern about her tires, and just found out that she had same 'chunking' issues on her tires, which happened to be the same Kumho Solus TA51a tires from the same tire shop. Her car is Toyota Camry, not a Sienna, so this is happening on this specific tire model across all types of cars they are installed to. Please help!

- #11750825July 16, 2026EXTERIOR LIGHTING
Rear Tail Lights Failed... Brake and Signals work. This was repaired through the dealership. 4 connectors and housing replaced invoice shows part 909890-10942. I am writing to express my concern regarding repeated tail light failures on my black Toyota Sienna that appear to be caused by wiring connection issues. This defect creates a serious safety hazard for my family, other motorists, and pedestrians, particularly in rural areas where there is little to no street lighting. I live in the country, where many roads are unlit and visibility at night is already limited. Because my vehicle is black, it blends into the darkness when the tail lights fail. Drivers approaching from behind may not realize my vehicle is on the road until it is too late to stop safely, significantly increasing the risk of rear-end collisions. This issue is especially concerning because I frequently transport children in my vehicle. Their safety depends on other drivers being able to clearly see my vehicle at night. A tail light failure caused by a wiring defect could place them at unnecessary risk of serious injury or death if another vehicle is unable to see us in time. The defect also puts other drivers at risk. Motorists traveling at highway speeds on dark rural roads rely on tail lights to judge the presence, distance, and speed of vehicles ahead. Without functioning tail lights, they may have little or no warning that another vehicle is in their lane, particularly around curves, hills, or during poor weather conditions. This appears to be more than a routine bulb replacement issue. If the root cause is an unreliable wiring connection, the lights may work intermittently or fail unexpectedly, making it difficult for owners to recognize the problem before driving. An intermittent electrical failure is especially dangerous because drivers may believe their lights are operating normally when they are not. Started a case with Toyota 260715001670

Moving about 55 miles per hour on right lane of I-95 the Jeep in front of me braked abruptly. I braked hard but as my Sienna was quickly slowing down to stop, the brake pad felt spongy when it was slower and as I pressed even harder, it seemed to have stopped decelerating at that slower speed and that was when my Sienna slammed into the back of the Jeep. Happened today on July 15, 2026 at around 4:24 EST. I will also file this same report to Toyota. My wife and I are at the hospital getting checked, but my Sienna looked undrivable.

- #11750202July 14, 2026POWER TRAIN
Vehicle: 2018 Toyota Sienna XLE AWD, VIN [XXX], approximately 112,000 miles. Component: Power Train / Automatic Transmission (UA80F 8-speed). The vehicle has developed a pronounced whining noise from the transmission that rises with road speed, accompanied by harsh/rough shifting. A Toyota dealer inspected the vehicle and diagnosed a failing transmission requiring replacement. The symptoms and diagnostic trouble codes match those described in Toyota TSB T-SB-0160-18 ("Transaxle Whine Noise, Harsh Shift, MIL ON, or Reduced Power") for the UA80 8-speed transmission. This is the same failure mode Toyota acknowledged in Customer Support Program ZJC, which attributes the defect to a washer/locking tab that was not sufficiently bent during assembly, allowing an internal nut to loosen and causing progressive internal damage. However, this vehicle's transaxle serial number falls outside the range covered by ZJC, so Toyota has not provided coverage despite the identical defect and symptoms. This represents a safety concern. Based on documented failures of this transmission across the UA80 platform, the condition can progress to loss of power, limp mode, or sudden loss of forward propulsion while driving — including at highway speed and without further warning — creating a risk of a crash or of being stranded in traffic. The vehicle has been maintained through authorized Toyota service. I am requesting that NHTSA investigate whether the UA80/UA80F transmission defect warrants a broader safety defect determination and recall, given the volume of similar complaints across affected model years INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

While driving on the freeway with my small children in the backseat a loud pop sounded and glass from the moon roof came shattering down upon us. The glass hit my face, arms, hair, and fell into my seat and all around the interior of the car. Though glass fell along my face my vision was not obstructed and I was able to safely exit the freeway without accident. The glass, though did make its way towards the backseats, did not hit my children. There was no warning or issues with the car or window.

The automatic side doors of this Toyota sienna le have malfunctioned twice between expensive repairs. When the hardware comes off the guide rail, it hits the window behind the door and shatters in covering the interior and occupants with glass. I had the window repaired after I had the door hardware replaced after its first failure in spring of 2024. The door gave no indication it was failed until I noticed the window bulging and nearly broke again. The Toyota dealer didn’t seem very interested in covering the cost of the repairs I made in 2024 and can’t get me in until mid August which leaves me without transportation for a month.

We purchased a used 2025 Toyota Sienna a few weeks ago. The other day, my husband put his phone and wallet into the center console compartment and closed the lid. When we went to retrieve those items, the lid only opened partially. We were finally able to remove the wallet but the phone had slid down into the compartment where the door to that console opening slides into when you open and close it. The lid would not open all of the way and blocked our access to that space when you opened it. The only way to remove the phone was to take it to our local dealership where they spent hours removing seats and pieces of the console to remove the phone from the inaccessible area and then put everything back together. This cost us roughly four hours of work time and $311.99 out of our own pocket. Apparently this is a known issue with this vehicle and is a design flaw where items of the right size (including iPhones) can slide into that compartment while the lid is closed and then are inaccessible.

- #11750094July 13, 2026SERVICE BRAKES
FAILURE: Brake control system. Vehicle intermittently surges forward while braking at low speed, most often through a curve — the brakes momentarily release and the vehicle lurches before braking resumes. Appears to occur during the transition between regenerative and friction braking. Vehicle available for inspection. SAFETY RISK: Unpredictable momentary loss of braking at low speed, in parking lots, residential streets and curves where pedestrians and other vehicles are close. Driver cannot anticipate it. WARNING LAMPS: None. No lamp, no message, no stored trouble code. Present since early in ownership; purchased new September 2021. CONFIRMED BY DEALER: No. Reported to a Toyota dealer in 2022 under warranty. Dealer test drove, could not reproduce the intermittent condition, took no action. Also reported in writing to Toyota in August 2022; no substantive response. MANUFACTURER REMEDY: In May 2024 Toyota issued TSB T-SB-0047-24, a brake control module software update for this exact low-speed braking condition on 2021-2022 Sienna Hybrids. Warranty had lapsed, so it was applied at owner expense in late June 2026 at ~156,000 miles. The defect persists after Toyota's own remedy. The surge still occurs, including within the past week at ~157,000 miles. REQUEST: A TSB is not a recall — owners are not notified, and the software remedy did not eliminate the defect on this vehicle. Request that ODI evaluate whether this condition warrants a defect investigation and recall.

- #11750096July 13, 2026LANE DEPARTURE: BLIND SPOT DETECTION, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING, ELECTRICAL SYSTEM
FAILURE: Blind spot monitor module failed due to a corroded internal electrical connector. The failure disabled both the Blind Spot Monitor (BSM) and Rear Cross-Traffic Alert (RCTA). Both remain inoperative. The failed component has not been replaced and is available for inspection. SAFETY RISK: Two active safety systems are disabled. The vehicle no longer warns of vehicles in the blind spot during lane changes, and no longer warns of cross traffic when backing out of parking spaces or driveways. WARNING LAMPS: Yes. Multiple warning lamps illuminated simultaneously in February 2026, including BSM and RCTA malfunction messages. No prior symptoms or gradual degradation. CONFIRMED BY DEALER: Yes. A Toyota dealer diagnosed the corroded connector as the cause. Repair quoted at approximately $2,500-$3,000. Repair not performed. INSPECTED BY MANUFACTURER: Diagnosed by a Toyota dealer. Goodwill assistance was requested from Toyota and declined, on the stated grounds that the vehicle had not been regularly serviced at a Toyota dealer. NOT A WEAR ITEM: A sealed electrical connector that corrodes to failure is a component defect. No step in the manufacturer's published maintenance schedule inspects, services, or protects this connector — no owner action could have prevented it, and service history has no bearing on a connector that no service procedure touches. Vehicle purchased new September 2021; failure occurred at approximately [XXX] of age. REQUEST: Request that ODI evaluate whether the BSM sensor and connector design used on this vehicle is inadequately sealed against moisture intrusion, resulting in premature failure of two active safety systems.
