I own a Toyota Sienna and got a notification from Carfax that there was a recall on the car. I looked up the recall on Toyota's website and it confirmed there was a recall. I called my dealer--Jim Coleman Bethesda--and asked them if it was safe to drive, explaining that we have a [XXX] and [XXX] old and use both rows of seats. They told me it was "perfectly safe to drive," including in those two rows. Toyota has, however, separately stated that drivers should not use the second row of seats. Our dealer has now informed us that we can have a loaner, so this isn't an issue for us. But I'm alarmed that the dealership gave this inaccurate information despite my repeatedly pressing them that I was concerned that I had seen different things online. The dealership is who customers will call if they have safety questions and they should not be telling owners that it is safe to drive where Toyota has issued different guidance. The styrofoam on the back headrest broke off on the side. I noticed this one day while taking the cover off to clean. The car seat has only experienced normal use and has not been in an accident. Luckily I noticed this before the child was ever at risk. I called the manufacturer and they could only offer a rebate for a new car seat but they recommended I no longer use it. I believe this to be a design flaw with the way the styrofoam is inserted and glued on. Also please note it is graco extend to fit 3 in 1

Concern 1: My 2022 Toyota Sienna intermittently experiences an unexpected forward surge while braking. When I press the brake pedal to slow or stop the vehicle, I sometimes feel a sudden forward pushing or acceleration sensation before the vehicle slows down. The behavior is unexpected and concerning, particularly when approaching another vehicle, intersection, or obstacle. I am concerned that this condition could increase the risk of a collision Concern 2 – Clicking/clunking from driver's side There is also a repetitive clicking or clunking noise from the driver's side of the vehicle during stop-and-go operation. The noise can be heard when the vehicle stops and then begins moving again. I reported this condition to Nalley Toyota in Roswell and requested that it be inspected as a potential warranty issue. I was advised that I would need to pay a diagnostic fee.

The seat belt latch stopped working. It appears the locking mechanism failed and it is stuck in the locked state which prevents the seat belt from latching. This is a critical failure in vehicle safety. It had been inspected by Toyota and they do not know what happened and are not willing to fix it unless we pay for it. This issue should be a recall.

- #11771295October 15, 2026EXTERIOR LIGHTING
This vehicle has the dash cluster light up as soon as the key is turned on. It does not have automatic headlights which turn on when it is dark outside. Due to the nature of the dash cluster lighting up and the daytime driving lights being automatic to turn on, it is very easy to drive off down streets with street lights completely unaware that there are no taillights or clearance lights on. The dash is lit up, so that is not an indication. The driving lights are on, so you can see enough to drive. I am aware of this now, but in the last week I have observed 3 vehicles driving around with no lights on in the dark because of the same reason. These modern cars with the automatically lighted dash clusters are causing people to lose all ability to assess whether the vehicles lights are on. It is not a question of automatic headlights/taillights, its a question of designing the interior of the vehicle to give the operator a hint that he hasn't turned on a specific safety feature. This agency has implemented so many other mandatory things related to safety (automatic tire pressure monitoring, automatic braking, backup cameras, etc.) and you forget the most basic things. You burden the cost of the cars with automatic systems to protect the people on the road but you forget how they can breed complacency in the drivers of the vehicles. You don't want distracted driving, but it is fine to have to search a touch screen on the vehicle to find the most basic controls for HVAC? If you are going to mandate safety, it should be safety that not only keeps the occupants safe but also doesn't lead them to do even more unsafe things, whether directly or indirectly. Dash clusters should ONLY light up when the headlights/taillights are on as this gives you the necessary indication that the vehicle's lights are actually on. Its called FUNCTIONAL SAFETY!

My 2023 Toyota Sienna LE has experienced a recurring Supplemental Restraint System (SRS) airbag warning that appears to be triggered by normal movement of the second-row seats. The vehicle has been brought to Cobb County Toyota in Georgia multiple times for this issue. Previous repairs included replacement of the wiring harness. Following that replacement, the warning disappeared for a period of time. However, the SRS warning recently returned after I moved the second-row seats forward during normal vehicle use. I am concerned that the seat wiring, electrical connections, or related components may be vulnerable to strain or disconnection during normal seat adjustment. Despite previous repairs, the underlying issue appears unresolved. This is particularly concerning because an SRS malfunction may compromise the proper operation of the vehicle's airbags or other supplemental restraint components during a collision. I believe this warrants investigation as a potential safety defect involving the second-row seating and supplemental restraint system. I am requesting that NHTSA review the recurring nature of this problem, particularly its apparent relationship to routine second-row seat movement.

The contact owns a 2023 Toyota Sienna Hybrid. The contact stated that when attempting to apply the brakes while driving at 30 MPH or at lower speeds, the vehicle suddenly lunged forward. The failure occurred approximately 7 times, with the most recent occurrence causing the vehicle to lunge into an intersection, where oncoming traffic managed to avoid a collision. On that same day, the failure occurred 3 to 4 times. The vehicle was taken to the dealer, where the failure was diagnosed, and the contact was advised that a software update titled "Braking Comfort" was available to improve brake performance; however, the software update was not performed. The failure mileage was approximately 35,400.

"The factory backup camera on my brand-new 2026 Toyota Sienna has a critically low resolution, resulting in a deeply blurry and pixelated image. The video feed is also heavily letterboxed into a tiny box in the middle of the large 12.3-inch screen. This poor quality and tiny layout severely restrict visibility, making it dangerous to spot pedestrians or small kids when backing out of driveways. Toyota Corporate confirmed this is a universal design choice, meaning it is a model-wide defect creating an active safety hazard." Very dangerous, it needs to be updated.

I purchased my 2021 Toyota Sienna Hybrid new in March 2021. For several years I have had an intermittent braking problem: when I apply the brakes, especially while slowing down or driving downhill, the vehicle sometimes lurches or feels like it suddenly accelerates forward instead of slowing. I must press the brake pedal again to stop. This happens without warning and is frightening, especially with my children in the car. I first reported this to a Toyota dealer on 10/10/2023 at about 41,192 miles, and again on 7/11/2024 at about 55,637 miles. On the second visit the dealer reprogrammed the Brake ECU under TSB T-SB-0047-24. The technician could not reproduce my exact concern but said the bulletin described a similar condition. The problem has returned despite the update. Online owner forums show other Sienna owners with very similar braking complaints; some paid for repairs themselves, others sold their vehicles over safety concerns. I have seen claims that too few complaints have been filed to trigger a recall, though I cannot verify that. What concerns me is that Toyota issued a TSB for a similar condition, yet the problem persists. I have owned this vehicle since new and reported the problem multiple times. I have dealer service records from both 2023 and 2024. I ask NHTSA to investigate whether this is a widespread safety defect and whether a recall is needed before someone is seriously hurt.

This is a supplemental correction to my NHTSA complaint #XXX (2021 Toyota Sienna Hybrid, VIN [XXX]). The description of the brake defect itself is unchanged; I am correcting the service-history details. First dealer visit: the correct date is XXX, at approximately 41,192 miles, Repair Order #XXX, at Checkered Flag Toyota of [XXX]. I reported that when braking downhill, the vehicle seemed to speed up rather than slow down, as if something were pushing it forward. The service record states "No Work Performed – Customer Requested Return of Vehicle." No diagnosis or corrective action for the braking concern is documented. Second dealer visit: the visit spanned XXX, at approximately 55,637 miles, Repair Order #XXX, at the same dealership. I reported the vehicle lurching forward when attempting to stop and engine RPM increasing when the vehicle should have been slowing down. The technician's notes indicate the technician felt a small lurch when braking and noted an RPM abnormality — the concern was partially reproduced, not entirely unreproducible as my original complaint stated. The dealer reprogrammed the Brake ECU under Toyota TSB T-SB-0047-24. The problem returned a few months later and has continued intermittently since. I request that this correction be attached to complaint #11770036. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am reporting an ongoing braking-safety problem with my 2022 Toyota Sienna XSE Hybrid. During low-speed braking, at the transition from regenerative to conventional hydraulic/friction braking, the expected deceleration can change abruptly. The vehicle can briefly surge forward even though I continue braking. This reduces the gap to the vehicle ahead, requiring significantly harder and faster brake-pedal input to avoid a collision. This affects the predictability of stopping distance. I purchased the vehicle on June 17, 2024 as a Toyota Certified Used Vehicle. On July 8, 2024, during a service visit — 21 days and 577 miles after purchase — I personally reported to Toyota a noticeable change/jerk in braking force at low speed during the transition between regenerative and conventional braking, and Toyota officially documented my complaint. T-SB-0047-24 already existed and provides for reprogramming the brake actuator control module. The update was not performed then and still has not been performed. In 2026, I again returned to Toyota with the same complaint. Seeger Toyota did not perform a road test because the technician considered the mechanical brakes unsafe. The dealer recommended rotor replacement because of corrosion/pitting, but the final RO contains no separate rotor-thickness measurements, runout measurements, quantitative pitting/corrosion data, or specific Toyota limits/criteria. After my written escalation, the Service Manager agreed to perform T-SB-0047-24 at no charge, but the update remains unperformed and the problem recurs regularly. I ask NHTSA/ODI to evaluate this case as a potential safety defect related to the regenerative-to-hydraulic brake transition. Because of the 2,000-character limit, the full description and chronology are attached as a separate PDF. Key evidence is being uploaded with this complaint; additional service records, correspondence, and photographs/measurements are preserved and can be provided upon request.

1) 2015 Toyota Sienna sliding door is faulty and can cause the door to open at anytime. They need to replace the junction block, rear sliding door wire harness and wire as part of existing recall G04. It was completed for driver side in 2024 and recall was marked complete however in 2026 the passenger side is encountering same issue. This puts any family member or friends life at risk with the potential of 2nd row sliding door to open at anytime without fix. It has been reproduced buy Toyota is saying the recall is marked complete for my vin when it wasn’t for both doors.

The factory-installed backup camera on my brand-new 2026 Toyota Sienna XSE has a critically low resolution, resulting in a deeply blurry, fuzzy, and pixelated image. Furthermore, the video feed is heavily letterboxed into a small rectangle in the middle of the large 12.3-inch screen. This poor image quality and tiny layout severely restrict rearward visibility, making it dangerous to spot pedestrians, obstacles, or small children when backing out of driveways. A local Toyota dealership inspected the system and confirmed it is operating 'as designed' by the factory, meaning this is a universal design defect on the vehicle model that creates a significant safety hazard.
