The contact owned a 2016 Toyota Sienna. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 16V858000 (Structure); several years ago. The contact stated that the dealer only serviced the passenger’s side sliding door, even though the recall indicated that both doors were included. The contact stated that the driver’s side rear sliding door had not yet failed completely but was malfunctioning. While pressing the button, the power door closed but failed to latch or seal shut. The contact stated that closing the door manually worked properly. The dealer was notified of the issue but declined to assist and indicated that the repair had expired in July 2026. The failure mileage was 196,000.

My name is [XXX]. I am an automotive Technician and I have completed hybrid vehicle system training. I have a certification of completion for the hybrid training.I work for XXX [XXX] maintaining and servicing a fleet of vehicles. I am contacting NHTSA regarding a safety concern with a 2024 Toyota Sienna Hybrid that I inspected. The vehicle has an intermittent braking concern. The driver reported that while braking, the vehicle sometimes seemed to move or jump forward instead of responding normally to the brake pedal. On one occasion, the driver reported that the brakes did not respond as expected and the vehicle nearly proceeded through a red light. I inspected and road tested the vehicle, but I was not able to duplicate the problem at that time. I also performed a full system scan and found DTC P162B87 in the Hybrid Control system. I contacted a Toyota dealership regarding the concern and also asked about Toyota T-SB-0047-24, which appears to relate to a transition between regenerative and hydraulic braking on certain Sienna Hybrids. The dealership told me that the bulletin does not apply to this vehicle’s VIN. Because this is an intermittent braking concern that could potentially affect vehicle safety, I would like to report it to NHTSA and find out whether there are other similar complaints or any additional information available regarding this issue. The foam under the headrest was discovered to be cracked when the cover was removed while performing the the repair on the recall for the babies eating the foam (can't find the recall paperwork now). The instructions provided with the remedy for the recall said if the foam was cracked to contact evenflo's hotline immediately. I contacted evenflo on July 28th via phone and was emailed a request for more information. I provided more information and video of the damage on XXX. No reply was received acknowledging the receipt of the requested information so I emailed on XXX asking to confirm receipt. I emailed again on XXX, and Evenflo finally responded on XXX with the following reply: "Thank you for reaching out to Evenflo regarding your Revolve360 car seat. I'm sorry to hear your headrest foam has become broken/detached. If the foam is detached, tape is allowed and no glue, and the seat pad holds the foam in place, the seat is still safe to use. If the foam is broken in larger pieces, you have all the pieces, they fit together like a puzzle, tape allowed and no glue, and the seat pad holds the foam in place, the seat is still safe to use. Evenflo tests all car seats to energy levels approximately twice that of the federal crash test standard. Additionally, our engineers developed the Evenflo Side Impact test protocol, which simulates the energy in severe 5-star government side impact tests conducted for automobiles. Now we are leading the industry by incorporating a dynamic rollover test. Rest assured, as long as the foam is in place, broken or not, it is still doing its job of absorbing crash forces." I no longer feel safe using the carseat with my [XXX] and wanted to reach out to the NHTSA to verify that cracked headrest foam is safe to use. The front passenger door lock actuator does not work properly causing the door to stay locked or unlocked despite attempts to change the door lock through the internal power locking system or remote key. This presents a safety issue in that it requires a manual lock/unlocking of the door should an emergency situation arise and expeditious ingress or egress from the vehicle be required. The door actuator is presently working intermittently and appears likely to fail permanently in the near future.

The rear power liftgate hinges on my 2021 Toyota Sienna failed structurally under normal, everyday operation. The power liftgate motor and struts appear to exert uneven or excessive pressure on the rear hatch assembly, causing the metal hinges to crack. Before the failure, we noticed a loud popping/grinding sound when opening the hatch, which quickly progressed to the liftgate binding and misaligning. This is a severe safety hazard because the structural failure prevents the heavy rear hatch from closing or latching securely, creating a risk that it could fly open while driving, dropping cargo onto the roadway or exposing passengers. It also creates a risk of the heavy door falling unexpectedly on someone loading the vehicle. The vehicle had to be taken to a dealership body shop for structural repairs due to the cracked metal, costing me over $1,500 out of pocket. Toyota has not issued a recall or good faith reimbursment for this known design defect.

The contact owns a 2025 Toyota Sienna Hybrid equipped with Firestone Tires, Tire Line: FT 140, Tire Size: 235/65/R17, DOT Number: 1WT87FT01. The contact stated that there was a screw in the tire. The contact stated that while driving 70 MPH there was a sound of an object in the tire. The message that the air pressure was low was immediately displayed. The tire pressure indicated 25 PSI. The driver was able to take the off-ramp but was unable to locate the spare tire. A good Samaritan attempted to assist. The driver was able to locate the Fix-A-Flat and a compressor, but the tire failed to hold air. The contact stated that the hole in passenger’s side rear tire was too big to be sealed. The contact was unaware that the vehicle did not have a spare tire. The contact was on the side of the road for two hours. The tire was changed and plugged by her husband after two hours of being stranded. The contact had enough fuel to run the air conditioner. The tires were previously rotated. The contact stated that the situation could have been worse, and not having a spare tire puts the drivers in a dangerous situation. The failure mileage was approximately 11,474.

The electronic driver's seat motor and track assembly failed completely on my 2021 Toyota Sienna (VIN: [XXX]) at less than 18,000 miles. The seat is permanently seized in a fixed position. This creates an immediate safety and crash-protection hazard, as drivers of different heights cannot safely adjust their distance from the steering wheel airbag or properly reach the pedals. Toyota corporate is refusing to repair this premature manufacturing defect on a known problematic seating system, leaving a severe safety risk unresolved. My liftgate has been making loud noises upon closing. The dealership checked it and diagnosed that the trunk door is mis-aligned, leaving a gap so that the door WOULD NOT SECURELY CLOSE. I was referred to the body shop to have the shocks and hinges changed. Further discussions mentioned this was a common problem with the Toyota RAV-4 where the hydraulics fail causing a bad alignment therefore affecting the hinges. Although my vehicle is a Toyota SIENNA, I had asked if this was caused by mishandling on my part (ie, slamming/ forcing the door or stuffing the cargo) but the technician mentioned that there was nothing I could have done to prevent it nor done to have caused the hydraulics to fail or go bad. There were no lamps or alerts that warned me of the problem other than the "scraping noise" upon closing that was getting louder ever since. The problem had been assessed and confirmed by the body shop.

The fuel filler door assembly on this 2017 Toyota Sienna failed structurally due to a defective hinge mechanism. This defect directly compromises a critical vehicle safety system. The Sienna is engineered with a physical safety interlock cable that links the fuel door to the driver-side sliding passenger door. This system is designed to prevent the heavy power-sliding door from operating while the fuel cap is off, protecting occupants from injury and preventing the door from violently striking a gas station fuel pump nozzle during refueling. Because the fuel door assembly failed, the safety interlock mechanism was compromised, presenting an immediate risk of fuel pump damage, fuel spillage, or passenger injury from the sliding door. Toyota corporate previously issued a Customer Satisfaction Program (CSP) to address this specific structural defect. However, my local franchised Toyota dealership refused to preform comprehensive repair, claiming they "only cover the pin". This dealer non-compliance forced me to seek immediate out-of-pocket repairs at an adjoining facility to restore the vehicle's structural safety interlock system. Toyota Corporate is refusing to remediate this safety-related failure. I have made several attempts to resolve this directly with Toyota Corporate (6 or more), without success. This included several phone calls, and a letter written to the Toyota Brand Engagement Center, which went unanswered.
