FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, STEERING
I started my car and drove out of the parking lot of my apartment complex. I was trying to drive to the grocery store. At that time, the car's auto-pilot and navigation features were not on. After turning onto the main road near my apartment complex, I turned on the car's auto-pilot feature. I did not turn on the navigation feature. But the car suddenly started navigating to my work address anyway. I wanted to keep driving straight on the main road, but the car immediately turned left to go into a side street that I was already passing. The car sped up when it turned, and I did not have enough time to change direction or to break. The car steered into a tree off of the main road. I attached photos to the claim form.

Driving from L.A. to S.F. on I-5 yesterday, we twice experienced phantom braking while using Autopilot. In both cases, there was no car close in front of us. The system just malfunctioned and suddenly braked from maybe 70 to 55 or even 50, which feels very dangerous in case a car behind us might not notice and rear-end us. It was late afternoon and we were driving somewhat into the sun. It feels like it might have something to do with sun angle and the fact that there was no car in front of us. There were also some mirages on the road that it might have mistaken for a car.

While adaptive cruise control, braking occurs unexpectedly. Might be related to hills, seems somewhat repeatable. Scares the snot out of you when you are doing 80 miles per hour on autopilot/cruise control and your car just stars braking.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The "Traffic Aware Cruise Control" and "Auto Steer". Yes, available for inspection. During driving on a long highway trip with these features turned on, the car suddenly braked severely for no reason(no objects or other vehicles were in front of car). This caused a large truck, which was following me to also brake and nearly collided with my car's rear. Repeat experiences like this "phantom braking" occurred. Based on other reports, this is common in other Teslas. No warning alerts in car.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Many times over the past long highway drives while in cruise control, the car will suddenly and violently brake with no obstacles in the way. Thankfully no car has been immediately behind me so far, but I can concerned that this may happen again and I won't be so fortunate. I have a mobile service appointment upcoming but Tesla tells me they need to know the exact date and exact time (to the second) that this occurred which I am unable to tell them. This is frustrating.

While driving on freeway on a clear day with no other cars in front of me the car suddenly hard brakes such that one could feel the anti locking activating. I was lucky there were no cars behind me or it would have been vary dangerous. There was a car on my right but it was holding its lane and there was no visible reason for emergency braking.

When on cruise control, the vehicle frequently brakes hard at highway speeds, jerking our bodies and forcing cars behind us to brake hard as well. Sometimes this happens when we’re beside a semi truck or a car changes lanes far in front of us, but often there’s nothing. There is no warning prior to the braking.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I have a new 2022 Tesla Model Y and in less than 500 miles it has already "phantom emergency braked" on 5 separate occasions. The conditions was freeway driving, no traffic within 200-300 yards, daytime, and the geography was gentle rolling hills with little to no vegetation. On all occasions my speed was set at 75-80 mph. The events ranged from momentary autopilot brake application slowing the vehicle about 10 mph to very aggressive autopilot braking slowing the vehicle 30-40 mph. There are no other warning lamps, messages, or pre or post symptoms. Of course it goes without saying how dangerous this is especially if there were a vehicle following closely behind. Tesla is very aware of this problem. This particular vehicle, being brand new, has not had a service appointment to date but I am in the process of setting one up.

EXTERIOR LIGHTING
What component or system failed or malfunctioned, and is it available for inspection upon request: Auto high beams; car is available for inspection How was your safety or the safety of others put at risk: In Arlington Heights, IL the car started automatically switching the high beams on and off. At first the auto high beams would engage for 7-10 seconds, then disengage for 7-10 seconds. Then the auto high beams engaged for 2-4 seconds and disengage for 2-4 seconds, as if flashing them. This is unsafe as it is distracting to the driver (me), it can obstruct the view of oncoming traffic, and it makes it appear as if I am driving erratically which can result in a traffic stop / unnecessary police intervention. Has the problem been reproduced or confirmed by a dealer or independent service center - no, they will not take the vehicle in to their service center and only provided mobile service appointments during the work day. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others - no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear - the car was recently serviced (October 5, 2022) for a nonfunctioning A/C compressor and heating pump. I am not aware of what other service / software changes+resets were made to the car while out of my possession.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, LANE DEPARTURE: BLIND SPOT DETECTION, BACK OVER PREVENTION: WARNINGS
I started a drive from my house. I entered a destination into the NAV system, it plotted a route. I then proceeded to start my drive. When I tried to backup, the backup cameras did not display any video, I waited over 1 minute and there was still no video. I then proceeded with my drive. The car would not start the cruise control, autopilot or FSD functions. Whenever I tried to engage the systems the car would state that they are unavailable. At this time, I also noticed that the NAV system was not updating the display, it continued to display the same image that showed that I was at home in the driveway. After 15 minutes, I pulled into a parking lot to video the problem. First, I put the car in reverse and recorded that the cameras did not display video for over 30 seconds, I then started to drive and recorded that the navigation display was not updating. The display that usually has the lane markings and displays the other vehicles was also not working, there was no information other than a picture of my car with a totally blank gray area around it. I have contacted Tesla about the problem, but they keep saying that they don't detect any problem with the car. I sent them the video and am waiting to hear back from them.

While driving on the interstate with the cruise control on the car suddenly applied the brakes. There were no other cars around and no objects on the roadway. This happened twice without warning on 10/09/2022. I have not reported this issue to Tesla yet but plan to.

My 2022 Model Y has severe phantom braking problems when cruise control or auto steer is activated. While driving at highway speeds the car slams on the brakes for no reason. This happened several times in 50 miles. I spoke to many other Tesla owners with the same problem. While driving at 70 mph on the interstate the car will suddenly and instantly apply the brakes hard. This is a severe safety issue. I am so scared that this will cause an accident that I have stopped using the cars adaptive cruise control and auto steer functions. I have contacted Tesla like everyone else, but have not gotten a response or fix. Please add this note to your pile of Tesla owners that are very pissed off that nothing is being done to resolve this safety issue. FORWARD COLLISION AVOIDANCE: WARNINGS
Vehicle was driving with auto steer enabled at 65 mph on freeway and braked unexpectedly and unnecessarily 3 times within a few minutes. Each time it occurred, I canceled auto steer and re-engaged. At no time were any obstructions or objects visible in front of the vehicle, and at no time did any warning lights or messages appear, the car simply braked without notice. Safety was at risk as cars behind me were traveling closely and were not expecting my car to brake suddenly, although I responded quickly enough to prevent further braking and accelerated out of danger. I have researched "phantom braking" and it seems this incident is exactly what was described in those reports. The car has not been inspected since the incident. I am hesitant to use auto steer since I do not trust that the cameras are adequately detecting what's in front of the vehicle.

Component Failure A/C Compressor / Heat Pump + Defogger / Defroster Safety Risk Windshield visibility was limited. With climate control limited was extremely hot in vehicle and resulted in physical discomfort / distractions while driving Problem Confirmation Yes, was confirmed. Dealer allegedly replaced "A/C Compressor (Heat Pump)" Component Inspection Unable to personally inspect component Warning Lamps / Messages 0/1/22 ~12PM – driving and received notification that “Cabin climate control system requires service – Cabin heating / cooling limited or unavailable” VCFRONT_a447 error Issue did not correct itself App showed car climate control as “LO”; car would only start with climate control set to “LO” Defogger did not work; windshield visibility limited, had to wipe inside of windshield with towel which created streaks and decreased visibility. Wipers to remove condensation from outside Heated seats and heated steering wheel would automatically be turned on when car was started

Intermittently the ability to interact with my touch screen has stopped. This means I cannot access crucial vehicle controls while moving. After trying to get a service appointment to address, Tesla claims that this should be resolved in a future software update but refuses to give a timeline for the release while I continue to have to drive my vehicle in an unsafe manner.
