Sudden deceleration and braking while at interstate speeds and crossing over a bridge. I was using cruise control only, overcast day, dry pavement with moderate traffic. Cruise control strongly applied the brakes from interstate speeds while crossing over an overpass bridge. There was no danger or traffic ahead of me that should have triggered the emergency braking. This needs to be investigated, it will get someone killed.

Hello, me again! So I downloaded the newest software update and in less than 24 hours and less than 1 hour of using 'Full Self Driving' and 'Auto Pilot' before the car tried to kill me and other people as usual. Same road I've been reporting for months 429 Eastbound and Westbound (Sanford FL) the car will confuse 429 with SR46 (1,000ft north) and when it gets to any of the overpasses with round-a-bouts underneath the bridge the car starts to stop on the highway (0mpg in a 70mph zone). This is absolutely terrifying; what's even more terrifying is that Tesla is claiming the cars FSD works on 'Artificial Intelligence' but that is a LIE because the car and Tesla have not learned from my many reports and have made zero changes. The only good change they made was my suggestion to Tesla in one of my many complaints about this lemon car they ripped me off on is allowing me to send them a 10 second voice clip describing what went wrong in the car (which once again was my idea) when actually using it sometimes my heartrate is still so high from cars whipping past me 70mph while I'm going 0mph that its hard to even articulate in words what happened so quickly; assuming Tesla even reviews the report and makes changes which they haven't for about 6 months of me reporting it. Car still tries to drive me off road over a transformer at home that is clearly visible on Tesla gps map which could turn the car into a bomb (so I was told by an Uber passenger who works on transformers and was leaving an OSHA meeting about a transformer blowing up on him and his coworkers). 9/10 times it's right but I know its not truly 'Intelligent' because why would it ever revert to a path I've canceled so many times? This is the second instance of this issue happening, first at 1800 miles ODO, and second at 4690. Both instances, when driving, the vehicle suddenly and violently shuddered to a stop. The first instance happened as I pulled out from a stop sign to take an unprotected left onto a street. In the middle of the intersection, the car shuddered to a stop violently and a lot of warnings and alerts came up on screen. Cars moving both directions had to suddenly stop to not hit my car dead in the intersection. The gear stalk inputs to put the car in drive or reverse and allow function of the vehicle did not work. I pushed the park button on the right stalk and then moved it into drive, which it allowed. I was then able to limp the car out of the intersection (with power vastly reduced and speed reduced) and await a tow. The second instance I had pulled into a parking lot at around the same speed (~15 mph) in which the car again shuddered violently to a stop with no ability to override. Traffic behind me had to brake as to not hit my vehicle which stopped in the middle of the lot. Again the car was inoperable until this time locking and unlocking the car from outside of it and reentering to limp it to a parking spot for tow. Safety of myself and others were put severely at risk suddenly and without warning from the car stopping itself. If this were to happen in other, less fortunate scenarios, risk to human lives is great. Problem has been confirmed by Tesla service center, but not reproduced while in their possession. The vehicle was inspected by Tesla service after the first incident and was explained to have found no faults with any hardware, but found a corrupt line of code in the firmware (no update or firmware change was noted to be available). Warning messages and alerts populated at time of incident and no sooner (no warning is given prior to issue). Alerts include power reduced, rear motor disabled, vehicle shutting down, automatic emergency braking disabled and more.

Tesla model y 2023 phantom braked on the intestate. The emergency braking system and adaptive cruise are not correct. I almost got hit because my tesla auto pilot braked on the interstate for no reason.

This is my second NHTSA report for this car, there was no reply to my first report. Since the manufacturer, without warning or owner consent, disabled radar systems for automated cruise control, the car is now exhibiting a new dangerous behavior when the camera detects the presence of emergency vehicle flashing lights and suddenly, and dangerously applies an emergency brake to slow the car, without taking into consideration the location of the emergency flashing lights relative to my car, or the presence of other cars around me that might be affected by the sudden, un-announced braking of my car. In many cases, the police/emergency lights are on the opposite side and direction of the freeway, 8+ lanes away from my car, and I am under no legal obligation to brake, nor would it be safe to do so. There is no warning by the car, only the sudden braking action and I have almost been hit from behind by other cars who were not expecting my car to brake while at freeway speed. This event has occurred at least 10 times in the past year regardless of time of day, weather or traffic conditions. I would like the previous complaint I filed about sudden, unexpected braking, as well as this new complaint reviewed by the NHTSA, as this is evidence of a critical, significant safety hazard by the car which the manufacturer has deliberately created.

I have owned this car for about a year with ~10,000 miles of driving. Four times in the last year, during high way driving, I had sudden deceleration using adaptive cruise control of at least 20 mph at highway speeds. It seems to me that in each case I was to the left of a semitrailer truck and it appears to me that the imaging software believed the truck to my right was entering my lane. This is speculation as to cause but the circumstances are common. I regard the adaptive cruise control feature of Tesla’s Autipilot to be unsafe without extraordinary vigilance - which is not the purpose of cruise control. So I no longer use it. Tesla asserts there is no problem. This occurs during multiple versions of Tesla software.

Took 1st long trip with the Tesla. I had "phantom braking" happen 5 times while on the trip. Once on the way to the destination. Then, it happened 4 times on the return trip home in a span of 2 hours. This was in the first autopilot mode (Traffic-Aware Cruise Control), not the Autosteer version. Each time it happened on an open road in a rural area at a speed of around 70mph. This puts my vehicle and others at risk where someone that is close behind me would be surprised with a sudden decrease in speed and cause an incident. It also is nerve-wracking to the driver and passengers of the vehicle. This problem is known within the Tesla community and was reported to Tesla. The vehicle was "inspected" by looking at incidents in the cars software log. The Customer Service agent could not confirm any incidents and asked that I file a "bug report" through the car immediately after it happens again. There are no symptoms or warning of the problem - it happens at random.

We recently traveled about 300 miles on a roadtrip in this Tesla and while using the adaptive cruise control, the car would randomly slam on the brakes for no apparent reason. There was nothing in the road or any hazards at all. Also, it was not just a one time thing. It happened multiple times before I decided to not use the cruise control because it to dangerous.

ON MARCH 18, 2023 WE WERE DRIVING ON HIGHWAY 80 FROM Winnemucca NV TO FERNLEY NV. At 3pm and about 3:10pm the car suddenly decelerated from ~82mph to ~50mph. The road ahead was clear of obsticles. Luckly there were no cars following close behind us, if there had been the unnecessary and unexpected deceleration could have been dangerous.

This car is unsafe. 70K+ purchase price with the inability to use basic cruise control, must-less the 10K FSD purchase that is USELESS. The phantom braking is constant while trying to use the basic cruise control, auto-pilot/FSD. Driving this car makes me a nervous wreck and I am beginning to HATE it and regret my purchase. I am seriously concerned for the safety of myself and other Tesla owners based on the current radar-less autonomous features. NHTSA should get involved and force Tesla to address the phantom braking issues before more people are involved in an accident that could be avoided.

Bought a brand new Tesla model Y with 11 miles on it. 1 mile down the road it tells me to “pull over immediately” “reduced power” I was going 45 mph downhill and thankfully not a lot of cars around. I pulled over and it said reduced power, exit and enter vehicle to restore operation. I did and it worked fine. I drove home with no problems for 25 miles. An hour later I was headed somewhere else and stopped at a light, when it was green I accelerated then it did the same thing but I was on an entrance ramp to a bridge and 5 cars were behind me. The car then was in reduced power mode and told me to do the same thing again, exit then re-enter. It was busy traffic and took me 5 minutes to get out when there were no cars going past me. It reduced power and couldn’t excelerate and caused a dangerous situation. This happened 3 more times while I was on the phone with Tesla support before I had to wait 2.5 hours for a tow with 47 miles on it.

My Tesla model Y reports that cameras are blocked or blinded nearly every trip. This happens day or night, and seems to be related to condensation in front of the camera. There is no other condensation in the vehicle; and there is nothing I can do to remedy this. I have brought it to the service center multiple times. On the last visit, the manager informed me that this is a known/common issue and that there is no current repair available. When this occurs, it either disables autopilot which can be alarming/distracting, or stops it from being enabled. Assumedly multiple safety features are disabled while the vehicle is in this state. I can provide additional documentation if required. The image supplied is from the last trip to the repair center.

While driving the vehicle on a 2 lane highway in sunny conditions, the vehicle suddenly braked while using cruise control. There was no traffic on the road ahead, no oncoming or traffic in my lane. Car slammed on the brakes slowing from 55 to 30 in about 2 seconds. If there was a vehicle behind me this would have caused a serious accident. This is NOT an isolated incident on my vehicle. The exact scenario has been reported a multitude of times by other people. This will kill someone and needs a recall. This is the standard equipment cruise control provided on all Tesla models. This isn't self driving or any other advanced feature. This is a critical issue that needs to be resolved.

This isn't another case of the Auto Pilot being engaged and the driver not paying attention. I was driving, in the rain, on a normal road surface, with both hands on the wheel. The car attempted to steer away and out of my lane when there wasn't an obstacle or any other hazard near me. My hand went from the 10 and 2 o'clock positions to the 11 and 3 o'clock position before it straightened back out. Because of this abrupt movement that the car made, the rear end of the car lost traction and spun around on me before the car came to a stop. Fortunately, no one else was hit during this accident. By looking at the Dashcam video, the car does show brakes were applied before spinning out of control but this wasn't by my input. I feel that the car made a descision for me that I had no control of. The car was not on Auto Pilot but yet still steered for me. Accident report indicates no issues with road surface, no speeding, and no citations.

Car randomly slams on the brakes while cruise control is active. Happen most frequently mid-day while the sun is out. Most frequent when coming over and overpass or small hill on the interstate. ~5 time in a day and of travel. Brake are apply and vehicle slows from 80 to sometime as slow as 60 in a couple seconds. No visible hazards or vehicle on the road at the time. Very scary and dangerous. Contacted Tesla about it and they said a software update would fix it, still happening since I purchase last June and reported in July.

I got the delivery of the car VIN [XXX] on March 10 7 PM and got the car out and drive to home from Kennesaw tesla delivery center. On my way home the car stopped right after crossing the light signal at the intersection of Medlock bridge rd. and parson rd. And it does not move, I had to turn on the emergency lights to save myself from accident and the cop had come after some time. We were lucky to save ourselves without an accident as the car stopped all of the sudden and there was no car behind. I think this is a problem with the car and needs to be inspected and nobody should be in this situation in the future. I want to raise this as a safety concern please for Model Y in the very first hour after the delivery of the car.
