Phantom braking with Autopilot (adaptive cruise) engaged. 8 hard braking incidents on 300 mile trip. 3 hard braking incidents on the return. Not all incidents can be explained by shadows, bridges or other vehicles. Braking occurred without warning on the interstate with near maximum brake being applied by the vehicle. It is very dangerous to pass a vehicle, have another car trailing you, and then have then have maximum braking with no warning or reason. The incident was reported to Tesla service, but Tesla provided no immediate solution. They stated that the software would improve over time.

The contact's wife owns a 2022 Tesla Model Y. The contact received notification of NHTSA Campaign Numbers: 22V702000 (Visibility) and 23V085000 (Steering, Electrical System) however, the parts to do the recall repair were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The contact stated while his wife was driving at various speeds with the Full Self-Driving Beta (FSD Beta) activated, the vehicle failed to recognize objects and other vehicles. The vehicle drove into the path of oncoming traffic. The contact's wife corrected the failure by maneuvering the steering wheel to realign the vehicle back into the lane. Additionally, the vehicle was shaking while making turns. The vehicle also experienced phantom braking while driving under an overpass. The failure nearly caused a collision. The dealer was made aware of the failure and confirmed that parts were not yet available. The dealer informed the contact that the vehicle performed as designed. The vehicle was not repaired. The manufacturer was made aware of the failure and confirmed that parts were not yet available. The failure mileage was approximately 1,000. Parts distribution disconnect.

Car was purchased December 30, 2022. The first time I engaged auto pilot, an oncoming semi was in its lane and my car slammed the brakes on while on auto pilot. It happened the next time I used auto pilot with zero perceived “threat”. I was extremely hesitant to use it, henceforth. I tried auto pilot over and over with my foot, poised and ready to take over the throttle, and my hand poised to disengage auto pilot. 12 times it slammed the brakes on-every time I used the feature it randomly braked, there was absolutely nothing around. These incidences were on two lane highways, no oncoming traffic, nothing in the road. Phantom braking. I do not feel safe using this feature on my brand new car because if someone is traveling behind me, it will cause an accident. I took my car in for minor service (for adjustment and stern issue) shortly after purchasing and mentioned this to the Tesla service Center representative. She said to me, “oh yeah, it does that sometimes. You have to be ready to take over.” Even being poised and ready to take over, I fear my reflexes will not be fast enough if there were someone traveling behind me at 60 mph. and it certainly isn’t how cruise control should work. I have a second service appointment, and actually noted the phantom braking (I subsequently learned this is what it’s called after googling, “random, braking with no threat Tesla model Y 2023”) on my service request to ensure that it has been documented because it’s a problem. A dangerous, unacceptable problem that needs to be fixed. My service appointment is 3/28/2023. Thank you.

I was driving my Tesla Model Y on 02-26-2022 with my wife in the passenger seat. We were on our way to pick up our son and his two friends from the movie theater in Lincoln Square North, Bellevue Square, Washington. As we entered the Lincoln Square parking garage approximately around 6 30pm and turned into the narrow passage leading down to the several floors of parking lots at approximately 5 miles an hour, the Tesla on its own, and to my alarm and surprise suddenly picked up tremendous speed and went careening down the parking driveway and slammed to a stop into the concrete boundary of the Pillar on parking level P2. The distance travelled from start to finish was to the best of my very rough guesstimate 60ft to 70 ft. My repeated attempts at slamming the brakes did not stop the car. The airbags deployed. •With the impact the car was completely totaled •The front right wheel broke free and hit the rear fender of a car parked in the Parking Garage in P2. - No one in the parking garage was impacted My wife was in great pain and very shaken. She was helped out of the car by a good Samaritan. Lincoln Square security reached us almost immediately. We called 911 and the fire brigade and police came to the accident site within minutes. The police inspected the vehicle and filed a case report. Post the accident, I took my wife to the ER. She was badly bruised with the impact of the air bags and seat belt restraint. We continue to get treated to manage the pain and the bruising. The experience was a horrific one and my wife has been badly impacted …it will take time for her to begin driving again. We have not been contacted by Tesla regarding this accident as of March 9, 2023.

Getting flat tire or leak in tires since day one. There is a gap between tire and rim edge and whenever i drive over a road bump or railroad tracks rims cut into tire and get flat tire. Been to tesla service center 3 times already each time they charge me for service and tires. Yesterday they changed a tire and this morning that exact tire got flat again. Towed to tesla service center and they wont accept the fault and charged me again another 432$.. tire getting blown daili

Periodically while using cruise control, most often on toll lanes with a different speed limit than the non-toll lanes, the car slows down rapidly and unpredictably. There are no warnings, and the braking is can be quite considerable (10-15 mph). This only happens when using cruise control.

3 days after purchase, headlights would not work, car towed to service center, both front headlight assemblies were replaced. 3 weeks after purchase, fault detected on passenger restraint, service center states passenger seat needs to be replaced to make airbag operational. I live 90 miles from service center, making these repairs quite difficult

Bought 2023 Tesla Model Y on 29-Dec-2022. Noticed that when I am backing up the car, even when there is another car right behind, or any pedestrian crossing there was no warning. At first I thought some settings is turned off. Later, when I was changing lanes in freeway, no lane departure alert. Also no blind spot chime came on even though that option was turned on in settings. Almost I was about to crash into another car which was in my blind spot. I started googling and learnt that Tesla has removed ultrasonic sensors from Oct2022 onwards and they are working on Tesla Vision which is a camera based system. But Tesla vision is not ready as of 08-Mar-2023 when I am writing this. How is the 2023 Model Y manufactured without ultrasonic sensors and no Tesla vision passed basic safety test? I feel threatened every moment I drive the car without these basic safety features like rear object detection while backing up, lane departure alert, curb sensor alert, blind spot warning. I feel like I am driving a 2000 model old car which doesn't have any of these features. How does NHTSA deem Tesla cars manufactured after Oct22 safe on roads? I will be at financial burden and stress and risk of death without these basic features in a 70k+ car. Why is Tesla even allowed to make such low standard cars without any of the basic safety features? The first time it may have been because the freeway led into a curve, and the second time the car was headed into a tunnel.

There are multiple issues: 1. When trying to back up slowly, or move forward slowly, the car goes automatically on park "to prevent roll". I am intentionally trying to slowly back out of parking, and the car keeps stopping 2. When I try to change lanes, in many cases the car gives a solid "beep" which is equivalent to me almost crashing to another car. But there are no other cars! 3. The car keeps giving forward collision warnings when there is no immediate danger to collide with anything, and in most cases there is nothin in front.

The USS Park Assist safety feature was eliminated from the vehicle without providing an alternative method or feature to monitor the front of the parked car in case an object or child was placed in front of the car after it was parked. The original USS Park Assist feature would detect this but the current Vision feature would not if the object or child is closer than 3 feet. It is irresponsible for a car manufacturer to eliminate a safety feature without providing a similar function as a substitute immediately. Just promising that this will come in the future should not be acceptable to the NHTSA. No problems have been recorded to date but this is just an accident waiting to happen. I'm asking the NHTSA to contact Tesla to require an immediate resolution.

After just 250 miles on the car and just after one month of the purchase, my wife along with my 4 years old daughter was driving to get my 7 years old son from the school when it suddenly stopped on the road with displaying errors on the screen as follows, the car battery was sufficiently charged, later from the inspecting what it appeared that the "rear drive unit" of the car got screwed up. Warning and error messages were: "Acceleration and top speed reduced" "Power Reduced" There was a huge school traffic jammed behind the car, luckily other car owners offered help but the car was not moving, it stranded in the middle of the road. We contacted Tesla they towed the car after one hour of the call, they kept the car for a week and told us the whole new "rear drive unit" need to be replaced. After a frustrating experience they offered no replacement but to fix the car. We leased the brand new Model Y on Tuesday January 24th at Tesla, Rockville, MD. On Friday January 27th at approximately 4:30pm we were driving on RT773 Riverside Parkway in the right hand lane and the steering wheel came totally out of the column with wires and electronic exposed. I was able to slow the car down and the car went up on the curb. I kept trying to put the steering wheel back in the column and somehow managed to get it in enough to drive home which was 5 miles away. All of the electronics did not work. Tesla came and towed the car the same evening. There was no working warning. Tesla towed the car and the next day on Saturday said the bolt to the steering column was loose and became dislodged.

The trim that includes the controls for the front passenger seat became detached. I returned to the vehicle to find the seat had fully reclined on its own. This occurred in October 2022. I confirmed with other Model Y owners that they had the same problem occur. Tesla said the clip holding the trim in place popped out and replaced it. A similar repair was done for other owners. The problem reoccurred in March 2023. I confirmed with other owners that the problem reoccurred for them as well. Tesla replaced the trim this time instead of just replacing the clip. There were no notices from the car either time that the seat controls had become detached from the seat. While waiting for the repair to take place, Tesla did not inform me whether or not it was safe for a passenger to sit in the seat. I question if it was safe since the seat reclined on its own, the seatbelt is attached to this detached piece, as are the seat controls.

The contact owns a 2023 Tesla Model Y. The contact stated that while at a stop with the brake pedal depressed, the vehicle unintendedly accelerated. The contact stated that no warning lights were illuminated. The contact stated that she rear-ended the vehicle in front as she turned the steering wheel to the left and landed in a ditch. No air bags deployed. The contact stated that she and her front-seat passenger sustained no injuries however the other vehicle driver was seen by the EMTs on the scene. A police report was filed. The vehicle was towed to an authorized Tesla Center and was awaiting diagnosis. The manufacturer was notified of the failure and a case was opened. The contact stated that she was informed by the manufacturer that a diagnosis team was working on the diagnosis. The approximate failure mileage was 473.
