Tire/wheel was losing air, upon investigation I discovered a cracked rim, which is my second in 8 months. Upon looking online, I see that ford is attributing this issue to aggressive stiff-wall offroad tires. I have the fx4 offload package with the factory wheels and BF GOODRICH KO2 all-terrain tires, which are the most common all-terrain tire in the US. I am using the same size tires as came from the factory. The change in tire is the only modification I have done from factory. I have followed the factory rotation guide. I have not done any offroading other than maintained gravel roads with this pickup, and I am not an aggressive driver. I do not believe that that is the cause here.

At approximately 146,451 miles, the transmission molded leadframe on my 2012 Ford F-150 failed while I was driving approximately 60 mph on a highway. Without warning, the transmission harshly downshifted, the truck lost power, the speedometer stopped working, the wrench warning light came on, the seat belt warning light illuminated, and the transmission entered fail-safe (limp) mode. I could not maintain highway speed. This created a serious safety hazard. The unexpected downshift and sudden loss of power in moving traffic could have caused a crash. I was fortunate that no vehicle was directly behind me when the failure occurred. After a diagnostic code appeared, I identified the problem with my own code reader, and a Ford dealership confirmed the failure was the transmission molded leadframe. Ford declined to cover the repair because its Customer Satisfaction Program had expired. They acknowledged that it was a defective part, though. This leaves me paying to replace a part they know is defective and is highly severe and extremely dangerous, as it presents an immediate risk of a high-speed crash. I believe this is a serious safety defect because it can occur without warning and cause an unexpected downshift, sudden loss of power, loss of the speedometer, and fail-safe transmission operation while the vehicle is moving. I don't have the part, but I can ask that they save it. The vehicle has only been inspected by 2 dealerships. One diagnosed the issue. Again, no warning lights that the part was failing until it actually failed. That led to the first dealership misdiagnosing the issue. I believe this is a serious defect and can cause crashes, injury, or fatalities.

Ford issued 4 codes for campaign #26S28 (PCM recall). Numerous owners are being denied warranty/safety repairs due to “other codes” appearing after computer update. Same limp mode, same sudden downshifting and will cost us (and many others) thousands in repairs because Ford put out faulty repairs. We will be paying 1800 for 2 codes NOT included but for a part that is integrated with the lead frame. MANY others are documenting similar circumstances online. My 16 yr old child will be driving this vehicle. I have filed numerous complaints under my case number with Ford should their “cheapest fix” cause additional issues.

- #11753533July 29, 2026SERVICE BRAKES
What happened: On [early July 2026], while towing a cargo trailer at low speed, the electric parking brake engaged unexpectedly. The brake continued to engage repeatedly throughout the duration of travel. Eventually, the parking brake circuit lost power entirely, which is when the repeated engagement stopped. What component failed? The rear axle wiring harness (part of recall 23S35). The harness had chafed against the rear axle housing, causing an electrical short that resulted in repeated unintended parking brake activation. The short eventually compromised the entire circuit, causing loss of power to the brake. How was safety put at risk? A towing vehicle with an unexpectedly engaged parking brake creates immediate risk of loss of control, especially while actively towing cargo. The repeated activations while operating the vehicle could have caused the trailer to jackknife or the truck to slide. This malfunction occurred despite a prior recall repair performed in Quebec under the same recall number (23S35). Confirmation: Both North Star Ford Duluth and Benna Ford Duluth confirmed the diagnosis. Ford Canada opened a claim acknowledging the failed recall repair.

Passenger side LED Headlight assembly daytime running LED's, the vertical led has gone out in my 2021 Ford f150, while not a major problem, LED daytime running lights increase visibility of other drivers to help in safety. From research, it seems that many of these headlights, the led daytime running lights in the 2021 year model are failing.

Fuel Pump Failed. Ford has stopped sale on the Fuel Pump because of recall 25S75. I need a fuel pump to operate my vehicle. Ford has offered no solution. It has been 10+ months since my fuel pump failed. Why is there no urgency? What am I supposed to do? Ford doesn't care, you at NHTSA don't care.

To the Office of Defects Investigation:I am writing to formally report a critical data reporting error on NHTSA.gov involving a 2017 Ford F-150 (VIN: [XXX]).This vehicle is covered under NHTSA Campaign 26V370000 (Ford Recall 26S28) for the transmission downshift defect. I hold the physical manufacturer recall notification letter sent by Ford Motor Company confirming my truck's inclusion. The vehicle experienced the exact unexpected downshifting failure described in the campaign, resulting in a collision in [XXX]. However, when searching my VIN on both the Ford portal and the official NHTSA.gov VIN lookup tool, the database now erroneously reflects "0 Open Recalls."I request that the Office of Defects Investigation audit Ford's compliance reporting for this specific VIN. The manufacturer's database is generating false completion data, masking an active safety risk, and misrepresenting historical defect data to the public. I am having a hard time uploading my documents I will send via email. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Vehicle suffered a violent, un-commanded highway-speed downshift due to NHTSA Campaign 26V237000 / Ford Recall 26S28. The tachometer was redlined during the incident. The mechanical overspeed immediately damaged the engine's cam phasers. Eide Ford in Mandan, ND, applied the recall software patch after the structural failure already occurred, cleared the diagnostic trouble codes without providing a copy to the owner, and is refusing to replace the lead frame or cover the resulting valve train damage, demanding $7,200 out-of-pocket.

I have a problem with my truck with the Molded Lead Frame. It is shifting erratically and not shifting below 3rd gear. I has an open recall for it, 26N04, but Ford won't perform the service because they are not getting the correct code from the computer system. So I am now forced to drive this truck in this unsafe condition until I get the correct codes.

Our 2017 Ford F150 Raptor is having issues with hard shifting. It does a clunking and jerking noise while changing into certain gears, 3rd and 4th. It sounds like the transmission is about to fall out. Ford has had to do a software diagnostic on it. It will work for a while then will hard shift again. It has been doing this on and off again since 2021. This issue is not remedy by a software update for it. We (my husband and family with us) have been driving on the interstate picking up speed to change lanes and it shift so hard it scares myself and my adult children. I am scared to drive it on the interstate because one day I am afraid it is going to do it so bad and cause us to have an accident! This is dangerous. It will even do it now at low speed in the city! I want to know why the F150's and the F150 Raptors have not been recalled for this! Even our Ford service advisor mention we may want to make a complaint to NHTSA.

Vehicle Safety Hazard & Recall Remedy Denial Recall: 26S28 / NHTSA Campaign 26V237 Summary of Issue: I am experiencing active, unsafe vehicle symptoms directly tied to Recall 26S28, including intermittent transmission signal loss, warning lights, and abrupt downshifts while driving. I initially contacted Ford in April/May 2026 immediately following the recall notice, but was informed no remedy or repair parts were available at that time, forcing me to operate a vehicle with a known driveability and safety hazard for three months. Now that the vehicle is at the dealership, Ford is refusing to perform the physical lead frame replacement under the recall/field action solely because the vehicle has not latched a permanent Diagnostic Trouble Code (DTC), despite the technician confirming physical hardware failure. Safety Impact & Systemic Failure: 1. Defect Root Cause: The lead frame degradation is the primary root physical defect of Recall 26S28, not secondary damage. 2. Unreasonable Safety Risk: Forcing drivers to operate vehicles experiencing active downshifting and loss of drive power until a specific DTC latches poses an immediate highway crash hazard. 3. Inadequate Remedy Program: Ford’s requirement for a specific stored DTC creates an unreasonable barrier to recall execution, denying necessary hardware repairs to vehicles experiencing the exact safety failure described in the recall. Ford must be held accountable for providing a functional physical remedy for vehicles exhibiting active recall symptoms, regardless of whether a transient electrical fault registered a permanent DTC code.

I. Vehicle Purchase and Maintenance History In September 2020, I purchased this 2020 Ford F-150 new. II. Chronology of Defect and Repair Attempts Around 33,000 miles, the vehicle began exhibiting severe excessive oil consumption, an audible engine noise, and visible blue/gray smoke puffs emitting from the exhaust system. The following timeline outlines my exhaustive efforts to have Ford remedy this under warranty: • March 14, 2025 (36,370 miles): First formally reported the noise and exhaust smoke to Service Advisor Shelby Ray. The dealership initiated an official Ford Oil Consumption Test. I was instructed to drive 1,000-mile increments and return for unrecorded level checks. • May 21, 2025 (38,200 miles): Second formal report. The dealership performed Technical Service Bulletin (TSB) 192365 CP:6006, an established protocol recognizing inherent oil consumption flaws in this specific 5.0L engine. • August 7, 2025 – September 11, 2025 (41,119 miles): Third formal report. The vehicle was left at Tony Serra Ford for over a month (RO # 6034434) to perform engine repairs mandated by Ford tech lines. A loaner vehicle was provided. • December 9, 2025 (44,144 miles): Fourth formal report. The defect was noted as unresolved and still present between oil changes. The engine was found to be 2 quarts low on oil upon arrival. A new tech line case was opened. • May 7, 2026 – Present (48,086 miles): Fifth formal report. The engine noise and exhaust smoke. Truck has been at dealer for 3 months with no resolution.

The vehicle is currently at Kayser Automotive Group located at 2303 West Beltline Hwy Madison, WI 53713, their direct line is [XXX], since the diagnoses of the corrosion of the DCN. SYSTEMS & COMPONENTS THAT FAILED: A faulty or clogged engine bay drain plug failed to divert water, causing significant water accumulation that routed into the interior cabin. This water intrusion directly soaked interior carpeting and exposed the Data Communication Network (DCN) connector and Body Control Module (BCM) wiring harness to moisture, resulting in severe pin corrosion and electrical communication faults. The vehicle and all affected components are fully available for inspection upon request. SAFETY RISK: The DCN connector and Body Control Module control critical vehicle operations, exterior lighting, instrument clusters, and safety/driver-assistance systems. Water corrosion on these communication buses causes unpredictable, uncommanded electrical failures and module communication dropouts while operating the vehicle at speed, creating a severe crash hazard to occupants and surrounding traffic. DEALER CONFIRMATION & INSPECTIONS: The issue was physically inspected and confirmed by an authorized Ford dealership service department, Kayser Automotive Group located at 2303 West Beltline Hwy Madison, WI 53713. Dealership technicians verified the water accumulation path, identified the faulty/clogged drain pathway, and documented physical corrosion on the DCN wiring connector. A Technical Assistance Center (TAC) case was requested to escalate the defect to Ford Corporate. WARNING LAMPS & PRIOR SYMPTOMS: Initial symptoms appeared as heavily saturated interior floor carpets and standing water pooling near the cowl/dash area. Shortly thereafter, intermittent electrical glitches, warning lamps on the instrument cluster, and network communication Diagnostic Trouble Codes (DTCs) appeared as the corrosion worsened. I am submitting this complaint regarding Ford Recall 26S28 (NHTSA Recall 26V237). My truck exhibited the dangerous unintended downshift condition described in this recall for several weeks before the repair remedy became available. A few weeks weeks before April 27, 2026, the truck began unexpectedly downshifting from approximately 5th or 6th gear into a much lower gear while driving. Each event caused the truck to enter limp mode, illuminate the wrench light, display multiple warning lights, and shake violently. I had to pull off the road, shut the truck off, wait several minutes, and restart it before it would drive normally again. I immediately reported these incidents to Mike White Ford and was repeatedly told to monitor the vehicle because no recall remedy was available. I reported the issue again during an oil change on April 27, 2026. Driving home from that appointment (9 miles), the truck experienced two more unintended downshifts. We immediately called the dealership and were again told to continue monitoring it. The condition progressively worsened over the next several weeks. On May 31, 2026, the truck experienced six unintended downshifts in one day. After the final event, it became difficult to restart and the check engine light illuminated. By my June 8 appointment, the truck was stuck in permanent limp mode and had to be towed to the dealership. The service manager stated it was "unsafe to drive" and provided a loaner vehicle. The truck later stored DTC P0219 (Engine Overspeed) and P2112 (Throttle Actuator Control System-Stuck Closed). I estimate the unintended downshift occurred approximately 75 times before the truck became inoperable. I opened Ford case CXH08666656S0W4Q2 on July 21, 2026. I respectfully request that NHTSA review whether prolonged operation with this known recall condition, while owners awaited a repair remedy, may contribute to additional vehicle failures and increased safety risks.
