STEERING:RACK AND PINION
The contact owns a 2024 Mazda CX-90. The contact stated that the steering rack coil was previously replaced under a recall. The contact stated that while driving 40 MPH off the highway, the contact suddenly lost steering and directional control, and the vehicle independently veered out of the lane. The contact was able to regain control after several attempts. The contact initially believed that the vehicle had hydroplaned; however, the failure reoccurred a month later. The contact stated that while driving 35 MPH on another occasion, the driver completely lost steering and directional control. The vehicle independently veered to the left and moved uncontrollably while the contact attempted to turn right. The steering wheel felt firm and was stuck before overcorrecting, causing the rear passenger’s side rim to strike a curb on the right. The vehicle was towed to the dealer, where it was diagnosed and determined that the steering rack had failed and needed to be replaced. The vehicle was repaired. The manufacturer was notified of the failure by phone. The failure mileage was approximately 39,900.

EXTERIOR LIGHTING
Sometime around Dec 2025 I noticed the headlights would flicker from time to time producing inconsistent and unsatisfactory illumination in front the vehicle and at times causing a blinding effect to oncoming drivers. The vehicle was taken to the Mazda dealership where they loaded some software in an attempted fix and stated that if that didn’t fix the problem, they would have to order the headlight switch and I would need to reschedule the vehicle back for the replacement. It took a few more months before I noticed the problem wasn’t fix. At that time I was getting ready to take the vehicle back to a Mazda dealership for some unrelated work, so I added that to the list of things to be addressed at the dealership during that visit. Before my scheduled appointment, I was able to determine that the problem only occurred while it was raining. At the dealership they mentioned that this problem had now been reported by others and was determined to be the result of water running off the front of the hood onto and down the front of the headlights. The fix was to install some “eyelids” to channel the water away from the headlights but I needed to reschedule a visit due to parts not being on hand at this time. I rescheduled the visit and had the suggested fix complied with, only to find out during the next rain fall that it only helped a little with the flickering but that the problem still existed. I mention this to the dealership to which they responded that they have done all that Mazda recommended and that there was nothing else they could do. Several months later I took the vehicle back to the dealership for another unrelated issue and to have the headlight problem reinvestigated for a possible permanent fix to which I was informed that there was no new info on this issue and that Mazda had done all that they could do at this point.

STEERING
I am experiencing the intermittent sticky steering defect on the highway. It begins as a binding sensation during micro-corrections at high speeds, and then the steering remains notched and heavy for the rest of the drive at lower speeds. It doesn’t happen 100% of the time. I need to be driving for a little while on the highway before it starts. I took it back to dealership but they couldn’t get it to do the same thing but they did add grease to a part of the steering. It seemed ok for a few weeks, but it is now back again. I am going to call the dealership back because they advised to let them know if it does start happening again.

FUEL/PROPULSION SYSTEM
I stopped for gas and when I removed the gas cap air came of out of the filler pipe like it was under high pressure. I could not fuel the vehicle because of the force of the air coming out of the tank blew gas out and shut off the pump. I started and shut off the car numerous times without the gas cap on and with it on which did nothing to affect the issue. It finally stopped after turning off all fans, radio and all accessories in the car and starting it without gas cap. The symptom lasted for more than 25 minutes. When I took my vehicle in for service they told me it was due to temperature change. I have not experienced anything like this with any other vehicle.

At high speeds, when turning around a bend the steering wheel gets sticky and will get stuck and then suddenly move again. This can happen 2-3 times as the wheel is turned around a bend.

The steering gets stuck on making a sweeping left turn on the interstate. It will hold that position until the driver exerts pressure to turn the steering wheel to the right. As it releases, the car swerves to the right a bit. The dealer was asked to check for a malfunction but claimed they can not duplicate it. The is a dangerous problem at 65 or 70 mph. Thank you!

STEERING
The steering seems to be malfunctioning and is hesitant or sticking when operating the vehicle. The steering wheel seems to hesitate or stick only during left hand turns, even slight ones. If you do not manually pull the steering wheel back after turning, it will stay in the position and not return as it should. This issue posses a severe safety risk to my family and others if the steering component does not perform properly. An accident could easily occur as a result of this issue. The issue has not been confirmed by an authorized dealer yet. I am taking the vehicle to a Mazda dealership in the next week or so. No warning lamps or messages occur prior to or during the issue. This issue was first noticed 9/21/26.

STEERING
The steering wheel last week suddenly started intermittently locking up mid-turn. It will catch or get "stuck" in whatever position you held your turn in, and will not go back to the 12 o'clock position without forceful interventions to bring it back. Even knowing it was happening and being prepared for it, it locked up so badly driving 60mph around a curve on the highway that it almost made me fly off the road! This is extremely dangerous and unacceptable. I can no longer let me family ride in this car and I don't know what to do.

The vehicle utilizes I-Stop, which comes on when stopping and while traveling down the interstate when the computer isn’t registering throttle input as a way to increase fuel economy. At least a dozen times now, the vehicle has failed to register throttle input, which isn't allowing the vehicle to restart when traveling down the road. The tachometer will remain at zero even with the pedal all the way to the floor. This has even happened while traveling down [XXX] in Northern Virginia placing myself and my family at risk of a collision. We have taken the vehicle in for service and mazda has been unable to replicate the issue or successfully diagnose the problem. VEHICLE SPEED CONTROL, ELECTRICAL SYSTEM
While traveling approximately 70–80 mph on a heavily traveled interstate with significant commercial truck traffic, the vehicle suddenly stopped responding normally to accelerator input. I pressed the accelerator pedal fully multiple times, but the vehicle would not accelerate. I was traveling with my mother and young child and was able to safely move the vehicle to the shoulder. After shutting the vehicle off and restarting it, a high-voltage battery temperature warning briefly appeared on the instrument display for several seconds and then disappeared. The vehicle was subsequently taken to an authorized Mazda dealer for inspection. Mazda and the dealer performed diagnostic testing, system updates, and extensive road testing, but advised that they were unable to duplicate the original incident and that no related diagnostic trouble codes were stored. We have not been provided with a specific root cause for the loss of accelerator response. No crash or injury occurred. I am reporting this incident because an unexpected inability to accelerate while traveling at highway speed could create a serious safety risk if the condition occurs again.

SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: WARNINGS, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
My 2025 Mazda CX-90 PHEV has experienced multiple incidents of sudden, uncommanded hard braking when there was no visible obstacle or danger in front of the vehicle. The most serious incident occurred on September 4, 2026, during rush hour traffic. The vehicle beeped, a warning that said "BRAKE!!" appeared on the dash, and abruptly braked without driver input, creating a significant rear-end collision risk. My dog was thrown from the back seat as a result of the braking. Another unexpected braking incident occurred the following morning when I was backing out of my driveway. Similar unexpected braking behavior has occurred earlier in my ownership of the vehicle as well. I brought the vehicle to a Mazda dealership for diagnosis on September 10, 2026 (their earliest available appointment). They kept and tested the vehicle, but were unable to reproduce the condition or identify a cause. No repair has been made that explains or corrects the unexpected braking. On October 2, the dealership informed me that the vehicle was ready to be picked up and that they considered nothing to be wrong with it. The service manager, Rocky, told my husband that a "critter or shadow" could have caused the braking. I did not observe any squirrel, pedestrian, vehicle, object, or other hazard in front of or behind the vehicle during the incidents. I remain concerned that the vehicle may unexpectedly apply the brakes again, particularly in traffic and/or with my young son in the car. I do not feel comfortable driving it with my family while the cause is unidentified and no repair has been attempted. Mazda has been notified of the issue and I have requested a repurchase/buyback of the vehicle.

The contact's wife owns a 2024 Mazda CX-90. The contact stated that while turning the steering wheel to the left, the steering wheel became stuck momentarily and there was resistance while attempting to return the steering wheel to center. There was an abnormal clicking sound coming from the steering wheel. The contact stated that additional force was needed to return the steering wheel to center. The vehicle was taken to the dealer where the contact was informed that the vehicle was previously repaired under NHTSA Campaign Number: 24V022000 (Steering). A diagnostic test was performed on the vehicle, and it was determined that the steering gear was inoperable and needed to be replaced. The vehicle was not repaired. The failure mileage was 3,000.

Steering wheel feels “sticky”, I rented this car from Flexcar. Now I’m without a car, because on the 5th day of driving this car the steering wheel got stuck and almost caused an accident. They are inspecting the car now, but they told me the “recall was already repaired” however: I’m had the EXACT same problem as the recall. This recall with the steering wheel is NOT fixed. This car is NOT safe to drive.

Component: Instrument cluster/dash electrical system, with linked wiper activation. 2026 Mazda CX-90, VIN [XXX]. Vehicle available for inspection. Safety risk: While driving, warning icons flood on (amber, sometimes a large red brake/emergency-stop icon) for 2-5 seconds, with an unrequested single wiper pass. Speedometer, gear indicator, and MPG stay active. On this occurrence, the malfunction happened twice in immediate succession — as soon as the reset resolved, it recurred within about one second, which has not happened before. This occurred with 3 passengers in the vehicle during a road trip. The icon flood obscures whether a real emergency warning is present, at speeds ~45 mph. Also occurs up to 65mph highway speeds. Reproduced by dealer: No — dealer (Southern States Mazda) held vehicle 3+ weeks, drove it only ~6 miles, closed as "not replicated." Dealer staff said another local CX-90 owner has the identical issue and is filing a lemon law claim. Inspected by others: 3 dealer visits (16 business days out of service). Reviewed by Mazda corporate case manager (Case #[XXX], opened April 2026) via logs, photos, video. No police/insurance inspection — no accident occurred. Warning signs prior to failure: None — malfunction is sudden, no advance indicator. First occurred [XXX] at ~1,700 miles. Separately, 3 times since [XXX]: on startup, steering wheel warning lights up, i-ACTIVSENSE READY flashes, vehicle won't shift out of Park despite brake pedal depressed, and the AC fails to engage every time this occurs. 49+ confirmed occurrences as of 9/25/2026. Incident log (mileage, speed, pass #) and photo attached. Video of both malfunction types available. This is 1 of 49+ confirmed recurrences since 3/4/2026 (Mazda Case #[XXX], opened 4/2026, unresolved). Dealer held vehicle 3+ weeks, drove only ~6 miles, closed as "not replicated." My boyfriend and I rented the car from Hertz Car Rental in [XXX] on Labor Day weekend. Within moments of driving the car off the lot, the safety camera system stopped working and the lights came on saying they weren’t working. The touch screen also started glitching and intermittently working but we didn’t think too much about it. We started driving across the state to [XXX] and made it about 4 hours away from [XXX] when the car started making a noise like a marble in a tin can, lost power, started shaking, and could not make it up hills at a safe speed. The check engine light came on. We pulled over and popped the hood to find a lot of rat feces scattered all over the engine bay, some of it looked fresh and some was turning white telling us it was there for a while. We called the Hertz rental roadside assistance who could not help us as they rented out all of their cars for the weekend and our only options were to tow us back to [XXX] without a replacement car or try to continue driving it the 1 hour to our destination. We continued to drive the car as we did not have anywhere else to stay and were in the middle of nowhere. We drove the car slowly to our destination but it did keep making the noise and shaking although the check engine light went out. When I got out of the car, I noticed a raw gas oder which worried me. I called my dad who works on cars and he told me not to drive it any farther as it could be a serious safety risk as the rats likely have been chewing wires and hoses and there could be a fire risk. We were also breathing the rat feces in our whole trip unknowingly. We called for the car to be towed and paid to rent another from another company. Since then, we have not gotten any money back and they continue to charge us daily for the car saying we must return it, meaning they have no record of where there car is even though we watched it be towed away and talked to the driver who said he was taking it to the Grand Junction Airport. Car purchased 6/12, drove home and noticed steering wheel sticking when turning to left. Had not noticed during test drive (as all turns were Right handed). Called Dealer and took car in for spring adjustment/repair on 6/24/2026. Car was ok for short time and then steering again stuck when turning to left. Called Dealership again and they took vehicle from 7/17-7/23. Steering was fine for a few weeks and noticed steering sticking again dropped off Monday 9/14/2026. Awaiting call from Dealership as to status on 9/18/2026
