- #11625952November 18, 2024LANE DEPARTURE: BLIND SPOT DETECTION
Blind spot sensors blocked error messages. This will happen at least a few times a year. Usually on long freeway drives, using cruise control, at night, and when cooler out (less than 60f). Restarting the vehicle makes the error codes go away and the system functions normally afterwards. Seems like either a software glitch or the system detects a guard rail for a while and gets confused and throws a Error message. This shouldn't be happening on a newer vehicle.

The side mirror defrosters are not operable. This is an extreme safety hazard to be able to see my surroundings while driving. This function should operate when the rear defroster is activated and is not working. I cleared the side mirror prior to leaving my house, during driving the mirror frosted back over causing me to not see other vehicles. The alert when a car is next to me would not function due to ice build up. If a crash were to happen due to the safety features not properly working, this could cause issues. This seems it should come standard on all Mazda vehicles considering there safety satisfaction. This is the third Mazda I've owned and I've never had a complaint until now.

I have a 2021 Mazda CX-5 touring with just a little under 26,000 miles. A couple of weeks ago, I started hearing an indescribable, quiet but noticeable sound on the passenger side of the vehicle that started at 15 mph. While attempting to get an appointment at two different booked up Mazda dealerships I noticed the sound getting worse and louder. I was able to get the vehicle into a local shop (not a dealership) close to my home. The technician there stated that my rear differential was leaking and needed to be replaced. He advised me not to drive the car because he said that I could lose total control of the vehicle. I was surprised to hear this as I was planning on picking up my son from school but also because my car has low miles on it. Luckily the repair was covered under my powertrain warranty but I felt the need to share with others. To me, this is a safety issue that Mazda owners should be aware of.

The vehicle is low mileage (11,500 miles) and is burning an excessive amount of oil (up to a quart every 1,000 miles) The dealership is aware of this and just keeps saying to add oil.

- #11622304October 28, 2024VISIBILITY:REARVIEW MIRRORS/DEVICES:EXTERIOR
The outside mirrors failed to rotate open, and you could not see behind the vehicle. The mirrors rotate inward when the doors are locked and are supposed to open fully when the doors are unlocked. These outside mirrors can freeze when rotated in during freezing temperatures. When the vehicle is unlocked, the mirrors if frozen, can be kept from rotating out to the locked position. This condition causes the mirror motor assembly to fail and that prevents the mirrors from rotating to the outboard and locked position. Latter year models have an added option to turn the rotating feature off but no retrofit option was added to earlier model yesrs.

Just got a oil change. Told my battery is almost dead. Upon looking up information I found a letter for recall for 2021 Mazda Cx5 for Dealer Rework 41 telematics communication unit concern. I called the Dealership and they are refusing to fix the problem. Looks like its just a software update is needed. My car only has 24,666 miles and according to the letter should be fixed. [XXX] I bought the car used in April and did a search for recalls prior to buying the car. There is non on Mazda website or vin search. 2021 Mazda CX-5. On 8/6/24, I turned right into a parking space between 2 parked vehicles at the DMV parking lot. I turned right and eased into the parking space facing a large Ford F 150 white pickup truck. I was approximately 1-1.5 feet from the front of the white truck. I had my foot on the brake, suddenly the vehicle accelerated by itself and I heard the car revving at a very high rpm and the car took off. My sister was a passenger and she heard it as well. None of the collision or brake notification avoidance features came on the screen or were implemented as a safety measure or accident prevention. I had my foot on the brake and I had no control over the vehicle. The car lurched forward hitting the right side near the headlight of the car on my left and the pickup that was in front of me. My car hit the white pickup so hard that it pushed it out of the parking space in front of me and perpendicular to the parked cars in the row. We both were injured, my sister in ICU and had to have emergency cervical surgery for fractures impinging her cervical spinal cord. The Mazda was totaled. It is impossible for a Mazda to push the Ford F 150 pickup truck, that was facing me , out of the parking space and perpendicular to the row of parked cars with just 12- 14 inches between the pickup truck and my vehicle unless it was revved up to a very high rpm. The police report has some inaccuracies, it states I was turning left into the space but was actually turning right and says I possibly passed out but that is not true. I was in shock when the police questioned me. I have also relived every second of this accident in slow motion many times since. I am a 5 foot women and would have had to floor the gas peddle and even then would not be able to push a large pickup truck out of the space in front of me, and continue out of the parking space landing sideways in a 12-14 inch space between the front of the 2 vehicles.

- #11610548August 23, 2024VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Will driving on the interstate, using the adaptive cruise control feature, vehicles were slowing down to a stop ahead and my car accelerated instead of slowing down. I then pressed the brake and simultaneously, the car breaks locked down (automatic braking). Unfortunately I wasn’t able to avoid an accident as I rear ended the vehicle in front of me. My vehicle has been turned over to the insurance company to assess damage. I’m sure it can be inspected by nhtsa. Mine and others safety were at risk due to the malfunction of the cruise control accelerating instead of stopping. It caused an accident. Luckily no one was extremely injured. I have neck and back pain from the collision that will hopefully heal in a few days. Time will tell. But this could have been much worse had I not reacted as quickly as I did. This could cause a fatal accident. The care isn’t drivable so it has not been reproduced by anyone. I have notified the insurance company about the issue and It is my hope that they will inspect this. There were no warning lights or messages displayed prior to the incident.

- #11609317August 19, 2024STEERING: STEERING WHEEL/HANDLE BAR
Was driving up [XXX] when simultaneously my dash board lit up with errors and malfunctions, my interior lights started flickering and my steering wheel and brakes locked. Unable to control the vehicle I gripped the wheel for dear life praying as it veered left. The 2023 CX 5 with only 18K struck the guard rail and went black on the inside and out. Only then was I able to counter the ricochet of the impact by redirecting the vehicle back on the grass to avoid getting smacked by 2 tractor trailers. SMH, I really almost died and now have to fight with my insurance to prove to them this is a real issue with these vehicle malfunctioning. I hope this helps someone and possibly save a life(s) - toyota_would_never, Greensboro, US INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

when you are driving in rainy weather on a highway the side view mirrors and windows fog up and you can't see. Putting on the defroster doesn't help. You have to drive with the windows open. On most cars if you open the windows the water would be evaporated from the rubber on the windows. The rubber is not thick enough on the front windows to do this. Bought it to a dealer and they said the same thing as far as the front windows are.

I bought this car brand new with 51 miles on it on 7/24 and have taken it to the dealership mechanics twice for the same problem. Within 3 seconds of “letting go” of the steering wheel, the car veers and pulls directly into the right berm. The car requires continuous redirection of the steering wheel to get it to drive straight on. The dealership where I purchased this has not reported this defect to the manufacturer despite me requesting it. The car was inspected prior to me purchasing it, and upon me noticing that it veers off the road, I took it back to the dealership garage (on 7/31) and the mechanics reported that they had rebalanced and realigned the tires. The mechanics also told me they had realigned the front and rear axles and said that they had probably been affected by being strapped down tightly on the delivery truck (?). I picked up the car the same day (7/31) after they said they fixed it, and within 10 miles of leaving the dealership, I noticed that the car continued to pull me off the road to the right. I turned around and returned to the mechanics’ and requested they report this to the manufacturer. The car has remained at the dealership mechanic’s since 7/31. No warning lights or messages have ever shown on the car. I consider this a major safety hazard as the car is constantly and consistently veering either off the road into the berm, or into the right lane if attempting to pass.

For the third time in 3 years we have had to had the timing cover and gasket replaced due to major oil leakage. This appears to be a very common issue with this car, and even after getting it “fixed” the first time it is leaking again.

- #11605788August 1, 2024SERVICE BRAKES
Something malfunctioned with the brake system of the car. The car was in park and off. I exited the car, reached back into the car for an item, and shut the door. I then walked up the driveway and to the front door. At that point, the car began to roll down the driveway. Someone coming down the road in front of the driveway hit it on the front passenger side. The car then continued to roll across the street and part way up the driveway across the street, coming back across the street and then ran into the same car that hit it. I was then able to open the car door and get inside. It was still off and in park. I depressed the brake pedal, started the car, put it into drive, and moved it to the side of the street. After the incident, I tried multiple times to replicate the incident with my car. I tried leaving it in reverse, even though I know it was off and parked. I also tried leaving it in neutral and turning it off, even though I know it was off and in park. Each time, the car began to roll immediately. There was no way that I could have done what I did that day, which was to step out, reach back in to get a bag, shut the door, and then walk away. The car was definitely off and parked when it happened. The vehicle is currently at a Mazda authorized repair shop, and I am awaiting the final word on whether or not they can detect what exactly caused the brakes to malfunction. There were no warning lamps or messages prior to the issue; however, I did notice that something seemed to be going on with the screen of the car prior to the incident. though the clock was set to the correct time, the screens auto-sensor for day and night mode was not functioning correctly. (When it got dark, the screen was still staying lit up brightly. I confirmed that it was on "auto" mode, and it should have dimmed since it was after 9:30pm and dark out, but it didn't. I had to manually switch it to "night" each time it was dark.)

- #11604254July 24, 2024SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
The most recent incident: Driving 35 MPH on a 4 lane divided boulevard, vehicle 5-6 car lengths in front of me indicates intention to turn right. No traffic in lane to my left should I need to change lanes. I continue to coast. At approximately 2-3 car lengths, the vehicle in front has nearly completed the right turn - there is no risk of collision - but my car BRAKES HARD. This is the 4th time my car has activated emegency braking in a situation it is not warranted. I have been dirving for 36 years with no accident. I do not need nor want this technology as it is more prone to causing accidents than preventing them. Additionally, the car throttle response is slow to recover in these situations, so while I mashed the acclerator to the floor to avoid being rear-ended - the car did not accelerate for 1-2 seconds (RPM went up, but the engine did not engage for several seconds). In situations where AEB might have been marginally helpful, detecting cross traffic running red lights, the car has NEVER detected this - and thankfully I'm an experienced driver so AEB was not needed. This technology, IMO, is not safe and will lead to over-reliance causing more accidents in the future.

When the vehicle light position is on Off at night the daytime running lights on the front of the vehicle come on which appear to be the same as the normal headlights, but the back lights are completely out. The brake lights work, but the back tail lights and licence plate light are out. The car's light switch is on the turn signal arm and can be moved inadvertently when using the turn signal. At this point you assume you have all around vehicle lights because the front headlights are on which are really the Day TIme Running Lights (DTRL). The DTRL are as bright as the normal headlights at night. This gives you the false impression that all of the outside vehicle lights are on. When this happens you are driving with no back vehicle lights which is a major saftey issue.
