POWER TRAIN, ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE: WARNINGS
VEHICLE: 2019 Honda Pilot EX VIN: [XXX] CURRENT MILEAGE: 77,000 miles COMPLAINT DESCRIPTION: I am filing a critical safety complaint regarding a catastrophic transmission failure that actively disables my vehicle's advanced driver-assist and automated safety systems while driving, creating a severe crash hazard. At approximately 36,085 miles, the vehicle experienced a severe torque converter judder. The dealership performed an 11-quart transmission fluid service. Immediately following this service, the vehicle began suffering from unpredictable, cascading dashboard warning lights that completely disable the vehicle's automated safety braking, forward-facing camera suites, and vehicle stability systems. The dealership repeatedly misdiagnosed this powertrain fault as a camera/sensor issue, charging me out-of-pocket for a front multipurpose camera replacement that did not resolve the issue. At 77,000 miles, the transmission suffered a complete failure. A diagnostic tear-down confirmed a stuck solenoid valve and destructive metal shards circulating throughout the transmission and fluid lines. The primary safety hazard is that the mechanical failure of the powertrain and torque converter actively sends erratic data to the vehicle's central computer, instantly shutting down the automatic emergency braking system, lane-keep assist, and forward collision warnings while the vehicle is in motion on public roads. This forces the driver to operate without critical automated safety features unexpectedly. Honda has issued an extended warranty for transmission judder on certain 2019 Pilots ([XXX]), but claims my specific VIN is excluded despite experiencing the exact same catastrophic mechanical and electronic safety failure. ENGINE
My vehicle continues to stall when coming to a stop which is a huge safety concern. Please reference service bulletin 23-010 which is exactly my problem as well. Local Honda dealership and Honda directly confirm this is my problem. However since my VIN range wasnt reported to you they will not cover the repair cost. Can you please investigate and include my VIN on the bulletin or create a new investigation.

Multiple dashboard warnings appeared simultaneously and deactivating Adaptive Cruise Control (ACC), Lane Keeping Assist (LKAS), Collision Mitigation Braking System, Road Departure Mitigation, Auto High Beam, Blind Spot Information, and Brake System warning. While on cruise control vehicle tends to slam brakes with no reason and nothing in front - major safety issue for driver.

SEAT BELTS
Seatbelt alarm sporadically goes off on second row passenger seat, causing a great distraction for driver, assuming a child has unbuckled seat belt, when in truth, they have remained securely buckled. Has been inspected by Honda service and we have a quote for right at $400 for seat buckle replacement. This began occurring in aug of 2026 and continues

LANE DEPARTURE: WARNING, FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
My 2022 Honda Pilot SE recently alerted me to multiple system failures in the Honda Sensing features including Lane Keep Assist, Forward Collision Warning and Emergency Braking, Adaptive Cruise Control. These errors were due to a failing front facing camera mounted by the rearview mirror. There is currently a class action lawsuit for this specific issue with more information here: [XXX] I believe that these Honda Sensing features are heavily marketed as part of the safety systems that Honda has installed and need to be covered by an extended warranty as the camera failure causes multiple systems to be cancelled or inoperable. The vehicle was last driven at 12:51 PM in the day; however, at approximately 9:00 PM, after the vehicle had sat for over eight hours, the vehicle suddenly caught on fire. The local fire department responded to the scene and extinguish the fire. The vehicle was inspected and stated that the fire had originated from the battery or an electrical part inside the engine compartment. The manufacturer was notified of the failure. The failure mileage was 180,000.

The contact's company owns a 2016 Honda Pilot. The contact stated that vehicle failed to start after several attempts with the "Emission System Failure", "Trailer Stability Assist", and "Electric Power Steering messages were displayed. Due to the failure, the vehicle was towed to an independent mechanic where a diagnostic test was performed and the mechanic retrieved DTC: P0087. The mechanic associated the failure with NHTSA Campaign Number: 23V858000 (Fuel System, Gasoline) and referred the contact to the dealer for assistance. The dealer was contacted and informed the contact that the VIN was not included in the recall. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 180,000.

On October 7, 2026, at approximately 131,425 miles, my 2020 Honda Pilot Elite suddenly displayed multiple Honda Sensing safety-system failures simultaneously without prior warning. The vehicle displayed Adaptive Cruise Control Problem, Lane Keeping Assist Problem, Collision Mitigation System Problem, Road Departure Mitigation System Problem, and Auto High-Beam Problem. I shut the vehicle completely off and restarted it, but the warnings returned. These failures appear to disable multiple advanced driver-assistance and collision-avoidance functions at the same time. I have found reports from other 2020 Honda Pilot owners describing similar simultaneous Honda Sensing failures, including reported failures of the multipurpose forward camera. I am having the vehicle diagnosed and will retain the diagnostic trouble codes and repair information. I am reporting this because the simultaneous loss of multiple collision-avoidance and driver-assistance safety systems appears to be a potential safety-related defect.

STEERING
Steering wheel is stiff and constantly clicking when I turn the wheel. My dealership said there have been several complaints but no recall and they could not do anything to fix the issue at this time. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
When driving in normal conditions on a 2 lane road and passing oncoming vehicles the ‘auto emergency brake’ will flash on screen and briefly slow/attempt to apply brakes. Very intermittent, no consistency with other conditions such as weather, traffic flow or driving conditions. Concern - safe driving conditions are interrupted causing driver distraction and higher risk of accident.

The vehicle is not starting after coming to a complete stop with the auto start stop feature at a red light or stop sign. After releasing the brake pedal, the engine does not start up again, which leads me to having to turn off the car and turn it back on manually, which could take up to a minute. The Honda dealer has already updated the vehicle replaced the starter and adjusted the valves two months ago and the issue is still persisting.

FUEL/PROPULSION SYSTEM
We have a 2016 Honda pilot. The front fuel rail assembly which included the fuel rail pressure sensor & the high pressure fuel pump had to be replaced in our vehicle and it is currently going to the mechanic shop for the low pressure pump to now be replaced as well. There have been multiple complaints regarding the fuel pump on 2016 Honda pilot and while there was a recall for the pressure one, our VIN wasn’t not on that list. This issue started somewhere between 165-170k miles. This causes sputtering when starting and stalling.
