Driving on the highway at 70 MPH. Car stated "Stop Safely. Vehicle Will Shut Off." Check engine light came on. Dash light of a turtle with an exclamation point displayed. Pulled over and turned car off. Was able to turn vehicle back on and get back on the road. However, issue would recur every 5-7 miles. Eventually stopped at a hotel for the night and left car at a dealership. Car is currently being looked at. No contact from the dealership yet.

On our daily commute the car came to a stop at a light and then the car then turned off and would not start again. We tried everything but the engine would turnover. After 3 hours, the car then started and we drove it to dealership and they detected no faults or codes.

1215 miles . The adaptive cruise control system was acting sketchy. It failed to maintain proper distance and speed between the vehicle in front. We took it into the dealer who said that the sensor had failed on this brand new vehicle and they replaced it on 2/12/2025. After driving the vehicle when we got it back. There does not seem to be much different. The van tends to accelerate extremely quickly only to get behind the vehicle in front sometimes not keeping proper distance with the way it's set, as well as waiting to the last possible moment maybe 2 or 3 car lengths to break which is extremely scary. Sometimes the warning comes up that says break break . Even with the system said it maximum a car cutting in the system is extremely slow to react. This has led to numerous close calls of rear end collision. We took it back to the dealer on 2/24/2025 1802 miles, who said they couldn't find anything wrong.

The contact's sister-in-law owns a 2021 Chrysler Pacifica. While the contact was stopped at a mailbox to retrieve mail, the contact opened the front, driver-side door. When the contact opened the door, the vehicle's parking brake engaged independently. As the contact attempted to drive forward, the vehicle's rear wheels had locked upon the depression of the accelerator pedal. Due to the failure, the vehicle was stopped in the middle of an intersection as the parking brake failed to disengage. The parking brake warning light appeared on the instrument panel. The contact made multiple attempts to restart the vehicle but to no avail. The contact then stated that a "Service Parking Brake" warning message appeared on the instrument. All occupants exited the vehicle and directed traffic to avoid hitting the vehicle. The vehicle was then towed to a dealer where they managed to restart the vehicle without incident. The manufacturer had yet to be notified of the failure. The vehicle was not repaired. The failure mileage was unknown.

Both middle bench seat head rests will not stay upright, making it unsafe for any passenger to ride in thise seats. Happened after folding the seat forward for passengers to get into the back seat. Infotainment system does not work and touch screen will not work. Unable to connect amazon fire and unable to even play DVDs on the screens.

Subject: Urgent Safety Concern – Chrysler Pacifica Hybrid (VIN: [XXX] ) Dear NHTSA, I am writing to report a serious safety issue with my 2022 Chrysler Pacifica Hybrid (VIN: [XXX] ). The vehicle experiences sudden power loss due to the following error codes: P079A-00 (transmission issue), P0A43-00 (hybrid battery issue), and P0A7D-00 (battery voltage issue). These problems have nearly caused accidents, including: On the highway: Power loss while overtaking, leaving me in a dangerous situation. In city traffic: Repeated power loss, almost causing collisions. The issues are well-documented among Pacifica Hybrid owners in the USA. NHTSA has received numerous complaints, and there are active class-action lawsuits due to unresolved problems. My vehicle was involved in a minor collision in the USA, classified as "clear title" (cosmetic damage only, no impact on the drivetrain or battery). Repairs were done professionally, and there is no connection to the current issues. I have contacted Chrysler and the authorized service center in Berlin, but received no response. This issue poses a direct safety risk, and I urge NHTSA to investigate and ensure a permanent solution. Attached: Error codes and incident descriptions. The electricity is depleted but the gas tank is half full. After turning the car off i can run it for a little while and then the same warning comes back.

While driving my children home from an evening activity, there was a strong odor of gas inside the cabin. I had the vehicle towed to the dealership the next morning for diagnostics and repairs. The dealership 's technician was able to reproduce the issue and upon further inspection discovered a small leak in the fuel line. It was determined that the fuel line had been rubbing against the coil pack, which created a small hole. The fuel was leaking onto the coil pack. This puts both the driver and the passengers at risk because the coil pack is in close proximity to the spark plugs. When there is a spark and gasoline, fires and/or explosions tend to be the result. The technician stated that there had been no tampering with the components and that the gap between the fuel line and the coil pack is very small. I believe that the gap is too small because normal operation of a vehicle should never cause a hole to be rubbed into a fuel line due to it's proximity to the coil pack. Prior to the smell of fuel, there were no other warnings. I have included pictures of the faulty part.

The owner received notification of NHTSA Campaign Number: 24V436000 (Back Over Prevention); however, the part to do the recall repair was not yet available. The contact stated while reversing, the rearview camera image was blurry on some occasions, and was completely black on other occasions. The contact stated that the black image remained displayed while driving the vehicle. Additionally, the infotainment system made an abnormal sound and displayed a glitchy screen that prevented the contact from using the functions of the infotainment system. The local dealer was contacted. The vehicle was not diagnosed or repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure and confirmed that the part was not yet available. The failure mileage was approximately 43,000. Parts distribution disconnect.

The oil cap on oil filter housing had a part come off and plug the oil channel, resulting in engine failure. The car is currently at a Chrysler dealer with noone accepting responsibility.

Dear Sir, I am submitting this formal complaint regarding severe misconduct, fraud, and reckless endangerment committed by Hyundai Escondido, Hyundai Motor America, and Ally Financial in connection with the sale and financing of a dangerously defective 2021 Chrysler Pacifica Hybrid Touring AWD (VIN: [XXX] ). 1. Background of the Case On [XXX] I purchased the above-referenced vehicle from Hyundai Escondido under the representation that it was a Hyundai Certified Pre-Owned (CPO) vehicle that had passed rigorous manufacturer standards. The dealership provided me with a Carfax report showing no prior accidents or major repairs and falsely assured me that the car was in excellent condition. However, immediately after purchase, I discovered that the vehicle had severe hidden defects, including: •Extensive flood damage, which was concealed at the time of sale and significantly compromised the car’s electrical and safety systems. •Prior frame and structural damage from an accident that was never disclosed. •Critical electrical malfunctions causing the vehicle to swerve uncontrollably on its own, without any driver input. •Severe safety risks, which nearly resulted in an accident with another vehicle carrying a baby on board. •Substantial mechanical and structural issues requiring over $20,000 in repairs, making the car unsafe and unusable. Despite reporting these issues on the first day of ownership, Hyundai Escondido refused to acknowledge or rectify the matter. 2. Hyundai Escondido’s Role in Fraud and Misrepresentation •Knowingly sold a flood-damaged and structurally compromised vehicle without disclosure. •Misrepresented the vehicle as a Certified Pre owned. 5-6 times, our car has gone into "turtle mode", where a warning comes on telling you to pull over ASAP, and that your maximum speed will be limited. We've been in and out of Gwinnett Chrysler in Stone Mountain, GA, and they've never solved the problem. Since then, it has happened twice more and we made it to an Auto Zone type store each time, and they read the error code. Both times, the code was POA43. One said "Power Train System Drive Motor 'A' Position Sensor Circuit Intermittent" and the other said "Primary DTC - MotorReSolver Open/ Short". On forums many people report this issue, and most have gotten new transmissions to resolve it. This started on [XXX] AFTER I had Gwinnett Chrysler fix 4 recalls and do routine maintenance. The contact stated while driving approximately 25 MPH while approaching a vehicle that had stopped suddenly for a traffic signal, the Forward Collision-Avoidance brake failed to engage. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the front collision system was working. The contact stated that the failure of the Forward Collision Avoidance braking system occurred again, and the vehicle was returned to the same dealer. The contact stated that the dealer had set up a soft barrier collision test and had driven the vehicle up to the barrier at 10 MPH and the Forward Collision Avoidance braking system failed to engage. The contact had taken the vehicle to two other dealers who stated that the front collision system was in operating condition. The vehicle was not repaired.
