1st time: I was exiting my driveway and dash presented with no torque and a picture of a turtle came up saying power issues. Running on hybrid engine at this time. Pushed down on gas and no power. I turned van off waited a few minutes, restarted, gas engine came on along with the engine light on dashboard. I was able to return van to the garage. Came in and called the dealership. Took car to dealership next day and they plugged it into the computer to check. They returned the van to me saying they gave it a reset and said I should be OK to go. I was leaving on vacation in a couple of days and gave me the OK to drive. Drove to SC from Ohio and all went well until the third day is when it happened once again...... 2nd time while in SC: Dash board gave warning, stop safely vehicle will shut off soon....engine shut down as I was turning left in traffic. Van was running on the batteries hybrid system with 26% left of charge. I took the van to a dealership to make sure it would be safe for us to drive home. They too plugged it into a computer and information came up about the battery. The tech told me it would be OK to drive home. They did not reset as they told me to take it to my home dealership to have them document the issue. Made it home safely, thank goodness. Dealership had the car for a week. They were told by Chrysler to open a case for the battery issues before they would OK ordering a new battery. They had my car for a week and Chrysler told them to reset and give the van back to me and have me drive the vehicle to see if it would happen again. My dealership shared with them it had happened twice but Chrysler refused to allow them to order a new battery and repeated to give the vehicle back to the owner after a reset and have the owner drive and see if it happens again. My feeling is Chrysler has no regard for my or my families safety. I did have the same experience with another hybrid Chrysler van I rented going 70 MPH on PA turnpike.

I have completed recall 66B/NHTSA 24V-436 on my Chrysler Pacifica. I'm continuing to have issues where the uconnect system locks up and restarts, the rear view camera freezes or turns on when driving forward. Once the system locks up you can't operate certain vehicle controls such as HVAC as they are controlled through the system and the buttons do not respond. I tried contacting Uconnect but their phone system says "error" after multiple steps to get to a representative. I've read many other people having these issues online also. This is a serious safety issue if the controls for your vehicle lock up while driving and the rearview camera doesn't work. I've updated the system per the recall and the issues persist.

SEATS
Headrest does not stay in the upright position in the 2nd row due to manufacturer defect. My child is supposed to sit there and now there is no headrest, thus putting his safety/life at risk. This defect has been frequently reported across multiple forms on the internet of other vehicles with the same make and model experiencing the SAME defect.

Issue Timeline: • [XXX] (944 miles): Check engine light code P1A01 (high voltage electrical system). Vehicle serviced at dealership; issue unresolved. • Mid-February 2025: Check engine light code P0A0A appeared and cleared itself shortly thereafter. • [XXX] (1607 miles): Check engine light code P0A0A reoccurred. Vehicle was serviced at Criswell Auto (Gaithersburg, MD) from March 4–19, 2025. Power Inverter Module (PIM) replaced. No comparable loaner provided, resulting in inconvenience and additional expense. • [XXX] (2419 miles) : P0A0A error code recurred again. A clicking sound was heard and the dealer determined that the front axle nuts that hold the wheel hubs to the axle loosened. Dealer re-torqued to 120 ft. Lbs. and added Locktite, however problem reoccurs. Chrysler issued a Technical Service Bulletin (03-008-24) to torque the axle nuts to 228 ft. lbs. After this was done at the dealer, clicking sound was heard again in just 3 weeks. Several attempts by the dealer to correct the problem at the 120 ft. lbs. spec. and once at the 228 ft. lbs. spec. have not worked following Chrysler's instructions. Risk of hub and wheel coming loose while driving. Dealer has attempted repair/correction six (6) times.

I bought a 2022 Pacifica Hybrid in October 2022. About 2 months ago(Feb 2025), while I was driving on the [XXX] highways, my car suddenly showed "Stop Safely Vehicle Will Shut Off Soon" and a turtle symbol. At this time, I was driving 70 MPH and the car started to slowly stop. Then, I tried to drive into a nearby Starbucks to check it out. I turned off the car. Everything was OK such as oil, coolant, a/c, windshield. Then I restarted the car and the "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol were gone. I thought it was okay, so I continued driving. 5 days ago (XXX), I had a trip to San Antonio. When I was driving close to Austin. My car showed "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol again. At this time, I stopped on [XXX]. I feel very dangerous for me, my wife and my 2 children. I feel very scared and confused when I am driving on [XXX] and the car stops like that. I feel like our lives are in danger while driving. Then "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol appear more frequently. My car has appeared "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol 6 times in 3 days. Yesterday was [XXX]. When I parked in my driveway, "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol also appeared. Every day, I must take my 2 children to school in my 2022 Pacifica Hybrid. Now, I feel very scared when driving. I do not know when "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol will appear again and will cause an accident for my family. We sincerely hope that you will consider and request Chrysler to help us check and fix this as soon as possible. Thank you very much. My 2022 Pacifica Hybrid is experiencing a known transmission issue as identified in TSB 21-042-24 My VIN falls squarely within the build date window. The bulletin instructs dealers to replace the transmission—not rebuild it. I am refusing an inferior repair that contradicts the manufacturer’s own instructions. I am requesting immediate escalation, in writing, confirming that my vehicle will receive a factory-new transmission in accordance with the TSB

When driving on hybrid mode with battery charged up suddenly warning message showing “Transmission Failure” and message saying “Stop Safely Vehicle is going to shut off soon”. Breaks don’t work as normal when this happens and vehicle wouldn’t stop immediately. Feels unsafe to drive. Engine warning lights turn on and stay for a day after this happens. Checked with dealership and they mentioned recall is still being worked upon and not yet available. Tried reaching MOPAR directly but no response yet.

Numerous times over the last eighteen months, our 2023 Chrysler Pacifica PHEV enters 'Turtle Mode' where a red turtle illuminates on the dashboard, and the car loses all motive power. Sometimes this occurs on local streets, but most recently it occurred during interstate driving, requiring evasive maneuvers to exit the driving lanes and get to the shoulder before all forward momentum stopped. So far, turning off the vehicle, opening the driver's side door and re-starting the engine clears the problem temporarily, but yesterday - [XXX] - the vehicle entered 'Turtle Mode' eight times during the four-hour car-ride from northern West Virginia to Northern Virginia. Several visits to Chrysler dealerships have resulted in zero fixes as they have been unable to replicate the conditions. We noticed our van having issues getting stuck in gear and then jerking to get out of the gear. After doing further research it seems to be a common issue in 2021 Pacifica vans. The next issue is electrical. Our van rear door opens if we are too close to it with the keys and now the sliding door will not open. They just fixed a recall regarding the entertainment system and back up camera. The entertainment system is still having issues. When we start the car up the system will reboot itself.

The vehicle randomly went into limp mode. The check engine light came on with error code P0A3F. The vehicle was restarted and drove normally with the check engine light (CEL) still on. We immediately made an appointment with the dealership to get it looked at. As we were driving into the dealership parking lot the CEL cleared itself. There is an existing TSB for this exact scenario and symptoms. The dealership and Chrysler corporate (Stellantis) are refusing to fix this issue because there isn’t an “active code.” The code is in the history. Since then the CEL has come on multiple times but cleared itself too quickly to get it to the dealer in time. We took it a second dealership this time with an active code, but the code cleared itself upon restarting the car after leaving it there. This vehicle is not safe to drive since it randomly goes into limp mode with no power. This poses a very dangerous and unsafe situation that can endanger me, my family, and other drivers around me.

We rented a '23 Pacifica from Enterprise. While driving 70mph on an interstate in Houston, a safety warning popped up "Stop Safely Vehicle Will Shut Off Soon". On the interstate, I wasn't able to keep my speed. Speed dropped quickly and dramatically. Luckily, I was able to get to the side but could have been hit in the process. My 2 daughters were in the car. It was terrifying. According to my research Pacifica did a recall of 67K Hybrids and their solution?? Add software that gives a warning. That was the solution. The headrest on the third-row seat is loose and does not stay in the upright position. In the event of an accident, this could pose a serious risk to passengers, as their head could move forward, and the headrest may fall down, potentially causing neck injury or further harm.

I want to say my first issue started in December 2024 the vehicle began to slow down lights started flashing in the vehicle the steering became difficult to turn I pulled over waited in the vehicle for over an hour with my [xxx] son and grandchildren in the car then the vehicle started I thought it was fine so I'm thinking this car is really smart it fixed it self then January it happened again I called the dealership south Chicago dodge spoke with larry they said bring it in but they were backed up I was like omg I'm not going to have a way to get around 😢 so the next problems I had called hawk jeep chrysler because they'll put me in a loaner or a rental I've done business with them in the past spoke with Ann Marie she said bring it in honey on my way to the dealership the car went crazy on the expressway and stopped on the expressway the state tow truck said you can't stay here it's unsafe mind you my [xxx] son is in the vehicle and it was freezing he towed us off the way I contacted Ann Marie I had the vehicle towed from there to the dealership long story short the vehicle has been in the shop 4 times for the same problem the vehicle has stopped on my with my family in the car numerous times in unsafe places I purchased the vehicle because it met my family needs in an a [xxx] I need a safe reliable running vehicle which has not been the case I've been paying my car note my insurance and this is the 3rd month I haven't had my vehicle mind you each time I've picked the vehicle up I had to bring it back for the same problem I was told they vehicle was ready went to get it Jim the manager at hawk came back said the vehicle was dead they don't know what's wrong with it at that point I had my vehicle towed back to south Chicago dodge where I purchased the vehicle from now I'm upset I'm in an unsafe vehicle that stops on the expressway in the middle of the road I need a reliable safe vehicle I transport my grandbabies and [XXX] son with his wheelchair INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (

The vehicle was purchased on 1/8/2025. On 1/13/2025, the vehicle had to be towed to the dealer because the transmission wouldn't shift to park. Dealer performed a software update to correct the issue. On 1/21/2025, the vehicle had to be towed to a dealer because the transmission wouldn't shift to park. Dealer replaced the gear selector. On 1/27/2025, the vehicle had to be towed to the dealer because it wouldn't shift to park. Dealer stated the battery failed. On 3/13/2025, the vehicle was taken to a dealer because the rear hatch opened on its own while driving and wouldn't close. Transmissions that won't shift to park present a serious roll away hazard that could injure or kill a person.

POWER TRAIN
MAY CAR HAS STARTED REPEATEDLY FLASHED CHECK ENGINE LIGHTS AND WARNING SIGNS THAT THE VEHICLE IS ABOUT TO SHUT OFF SOON AND TO STOP SAFELY WHILE TRAVELING AT HIGHWAY SPEEDS. THE VEHICLE BECOMES UNRESPONSIVE AND I HAVE TO COAST TO THE SHOULDER OF THE HIGHWAY. AFTER TURNING OFF THE VEHICLE I AM ABLE TO RESTART AND CONTINUE DRIVING WITH THE CHECK ENGINE LIGHT ON FOR NEXT 2 TO 3 STARTS. THE DEALER HAS ALREADY ATTEMPTED TO REPAIR THIS LAST YEAR BY REPLACING THE TRANSMISSION, BUT THE PROBLEM STILL REMAINS AND HAS NOT BEEN FIXED. THE DEALER WAS NOT ABLE TO REPLICATE THE ISSUE AS THERE WAS NO ACTIVE CODES WHEN IT WAS BROUGHT INTO THE SHOP. SINCE THAT VISIT IN EARLY MARCH 2025, IT HAS HAPPENED AGAIN. BACK OVER PREVENTION
The contact owns a 2021 Chrysler Pacifica. The contact stated that while reversing, the rearview camera image was either blue, blank, or there was static. The vehicle was taken to the dealer, where the radio software was updated under NHTSA Campaign Number: 24V436000 (Back Over Prevention); however, the failure persisted while reversing. The vehicle was taken back to the dealer, where it was determined that the rearview camera needed to be replaced. The dealer informed the contact that the rearview camera replacement was not covered under the recall. The contact stated that the recall remedy failed to correct the failure. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 15,000.
