- #11497826December 16, 2022SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
I Purchased a new 2023 Telsa Model Y, picked it up today and on the drive home I experienced a phantom braking event which caused the vehicle to immediately stop and become inoperable. Despite my attempts to restart or reset the car it would not move. I sat in the middle lane of a six lane roadway for 90 minutes waiting for a tow truck to take the vehicle to the nearest Tesla Service Center. There was no warning and the vehicle drove flawlessly prior to this event, although I had only driven about 125 miles. I was in heavy traffic and I responded quickly to stop the vehicle and avoid an accident.

In Traffic Aware Cruise Control (aka Autopilot) car randomly changes the maximum set speed. This has caused a dangerous dramatic slowing of the car when highway driving. Car should not change speed setting without human input. I have video of this occurring.

The car stopped suddenly in the middle of the street. By the grace of G-d, this happened on a quiet street because we decided to take a detour and could have instead been on the freeway! We were with our 3 young children (all in carseats). We could not restart the car after it stopped, and there was no warning it was going to stop. We were not using autopilot.

My Model Y applied the brakes suddenly when it was in Adaptive Cruise Control while I was driving in the interstate at 75 mph. This caused the vehicle to decelerate very abruptly. There was no vehicle in front of me and no obvious debris in the road. This happened several times on a trip from California to Kansas and back. On one occasion , I had just pulled back into the right lane after passing several tractor trailer rigs going 70 mph. I had to tromp on the accelerator to avoid being rear-ended.

On Saturday night between 9:30 to 9:45 PM while driving on 495, Tesla Model Y 2022, Auto pilot was engaged at 70 miles per hour in three lane highway. My car was in middle lane with no vehicle in front in any of lanes. Suddenly without any warning, auto pilot was disengaged and car moved from 70 mph to 30 mph within few seconds shockingly. I was alert to pull accelerator in timely manner and avoided a possible rear-ended accident . It was scary moment with family onboard and luckily cars where behind enough to evade a possible disastrous accident. This must be fixed

While using cruise control the vehicle brakes hard when no obstacle is any where near the car in front or sides. Some times hard enough to engage abs and you can feel the car on the edge of losing traction/control. This has happened multiple times on multiple trips.

When i do a full trip level charge on the Tesla Y at home, I leave the house and start down a hill on the way to work. When I brake, the brakes chatter severely and do not slow the car. I pump the brakes and it happens again, pump the brakes and they start working. This has happened numerous times with a full trip charge. it does not happen with a 70% charge. I feel that this is very dangerous and could easily cause an accident if braking is required. I attempted to contact Tesla, there is no easy way to report this problem. So, this is now in your court. Thanks for looking into this. Thomas Cox

I am writing to file a safety concern with vehicles manufactured by Tesla Motor Company Inc (“TESLA”). The complaint relates to safety failures with the automatic retracting window system. This complaint is different from the NHTSA previous issued recall for pinch points (issued on or around September 19th, 2022). During the vehicle software update process, owners can enter the vehicle and close the door (there is no engineered prevention of opening the doors). However, upon attempting to exit the vehicle the window does not automatically retract. When the owner closes the door after exit the windows are prone to break or shatter (see attached photo) which creates a safety hazard. The specific hazard is with broken glass spread in the cabin of the vehicle. Tesla charges customers to replace the window in spite of the known defect (see attached paid invoice, which documents how the damage occurred). Tesla states "it is a known documented issue and is therefore the customers responsibility". In lieu of correcting the acknowledged known issue or providing a proper engineered control to avoid the safety hazard created by the defect; Tesla has apparently accepted the defect and passed the risk of the defect to customers who are not formally made aware of the problem and are expected to understand the problem based on documentation in third party forums. The issue poses a cutting hazard to the driver and occupants of the vehicle and also a foreseeable egress concern in an emergency.

Car did not stop after I parked and continued to go forward and hit another parked a car Even with my foot on the break. No warning light of any sor. This incident happened at 5:35 PM Pacific time on Monday, September 12. I made an appointment with the service department and it took the car there on September 23, 2022, . They only asked me about the time this happened and they said you’re going to send it to engineers and will get back to you. They did even Care to look at the car or inspect it. Since then, no one ever contacted me. I was lucky that this truck was in front of my car otherwise if there was anyone walking they would have been killed.

Sudden Phantom braking happened while driving north on garden state parkway. I was in the 2 lane express northbound highway, and there was heavy traffic and flashing police lights on the separated 3 lane local northbound highway. I was using adaptive cruise control when Tesla suddenly braked, reducing speed 20-30 mph in a matter of seconds, despite no cars in front of me. I took back control and disengaged cruise control

All safety features - backup, turn signal and blind spot camera detections stopped working on November 22, 2022. Tesla gave me appointment to bring in the car on 11/28/22 then emailed me informing appointment pushed back to 12/8/22 and today emailed informing appointment pushed back again to 12/12/22. The car is not drivable without these safety features as it poses the risk of accident that could harm driver and other on the road. Tesla is not responding at all for providing a replacement vehicle, while this car is still under warranty.

Phantom Breaking. On Saturday -12/3/22, around 3.50 PM CST en route to Arlington on HWY 360, I experienced phantom breaking twice in quick succession. I was driving on autopilot at 70MPH and suddenly the car applied brakes and it was very scary to me and all my family. After the second event, I stopped using the pilot and drove with manual control.

When using Autopilot on interstate, car did phantom braking when approaching new concrete sections of highway (I-4 between Orlando and Daytona Beach) on a primarily black asphalt highway during sunny afternoon conditions. This happened on a couple of occasions during drive.

The wipers "swipe" when passing interstate streetlight when in the "Auto" mode. Additionally, the "Auto" mode is automatically enabled when the adaptive cruise control is set. It is not possible to turn off the wipers while using the cruise control, making it basically useless at night. I have been advised on two occasions by Tesla Service that this is a firmware issue and there is no fix. The complaint was last verified by Tesla Service on Dec. 1, 2022.

- #11495639December 1, 2022STEERING, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, LANE DEPARTURE: ASSIST
I was driving with my wife and two kids at around 8:30pm (it was dark outside already) on November 29, 2022. The street was pretty empty and it had 2 lanes in each direction. I was driving the left lane with the speed at about 40 mph and had the autopilot (the one that keeps lane, not FSD) turned on. When the car was approaching the intersection with the green light on, it suddenly decided to turn left (even though there was a dedicated left turning pocket on the left hand side). It looks like it was confused and after trying to change the direction it pointed the car right to the road sign. Hopefully I had my hands on a wheel and was watching the situation, so I immediately turned the wheel right to get back to our lane. If I wasted a fraction of second we could end up smashing into the road sign at about 40mph.
