The adaptive cruise control malfunctioned while engaged on the highway/interstate. On a recent trip of approx. 1,000 miles, the vehicle experienced "phantom braking" (aggressive emergency braking for no reason) on three occasions while the adaptive cruise control was engaged on the highway/interstate at approx. 65-75 mph, with no obstruction/vehicle in front and the weather was clear sunny skies. No warnings or messages were prompt by the vehicle. This abrupt phantom breaking could have caused serious injury from a rear-end collision or cause vehicles behind to take unnecessary unsafe emergency maneuvers/braking.

There were no warning lights illuminated. The vehicle was not diagnosed or repaired. Neither the dealer nor the manufacturer was notified of the failure. The failure mileage was approximately 9,000.

The contact stated that while reversing out of a parking space, the audible alarm and collision avoidance system did not activate as the vehicle drove closer to a nearby pole. The vehicle struck the pole causing minor damages to the rear pumper. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 4,000.

This issue is pretty scary and I'm still shocked after a couple of hrs. Luckily no damage was caused but I couldn't stop thinking what will happen if I were on the highway. I was waiting for the red light, fully stopped, and waiting to get onto the highway. However, after 2-3 seconds, the car started to move. I was surprised, shocked and trying to stop the car in the meantime. However, the brake stops working and It seems to be very tight and I couldn't step on it to make it stop. Then I was able to turn right into a gas station(very lucky, no car on the right lane) and then I was able to switch to "P" to fully stop the car. I then called the Tesla road assistance and my car was towed and they're checking the issue now. Wanted to log this since this could be very very very DANGEROUS!

When using Tesla's Cruise-Control, Traffic Aware Cruise Control or Enhanced Auto-Pilot the vehicle severely brakes without any reason. This happens every time the vehicle is taken on the highway. It is sometimes reproducible because it happens every single time at specific geographic locations. Most often it is not reproducible. On the highways it has put me in danger more than once. The vehicle would brake while going at 75mph and slow down rapidly all the way down to 45mph. Other vehicles following at safe distances have to veer away quickly or slam on their brakes to avoid an accident. Everyone inside the car experience a whiplash from their seat belts. I opened a service ticket with Tesla. I was told that they ran a remote diagnostics and found no hardware issue in the car. The ticket was closed by Tesla after they pointed out that Enhanced Auto Pilot is a Beta version. I gave them very specific examples of how to reproduce this issue. No on came to drive the car before closing the ticket.

My car met a major accident and the impact was so severe that it was total loss. The car was badly damaged from drive side and it skid at least 100 meters and it turn around And I am aware that Tesla has side airbags also but none of airbag were deployed I tried to contact Tesla support and report this issue but no one from Tesla gave proper Response . If you want I can share the pics of my car and claim details

Yesterday morning I opened the car and put my dogs in the back seat. I closed the door. I then tried to open the driver’s door but the car became unresponsive. I tried to release the charging cable connected to my home charger. The car did not respond via bluetooth or mobile or access card key. The car was completely dead with my dogs inside. I live in Palm Springs. It was 90 degrees and sunny outside. I called Tesla roadside. Towing was dispatched. Eventually the passenger doors opened and I got the dogs out. My car was towed away. Dead. Nobody at Tesla seems to see how dangerous this situation was. It could have been a baby locked inside the car. Tesla has no culture or attitude of taking this seriously or seeing this as a potentially life threatening issue. It’s shocking to me how everyone I spoke with at Tesla saw this as just another glitch or customer trouble ticket. The severity of this issue was completely lost on every Tesla employee I encountered. Tesla must be held accountable. According to the tow truck driver this happens frequently. People are often stranded in extreme heat due to this 12v battery failure. This issue MUST be investigated and Tesla should be forced to respond.

2022 Tesla Model Y front aero shield bolts become loose and the aero shield becomes loose and becomes a safety hazard for other drivers. Tesla will not warranty part nor will claim responsibility for poor manufacturing & assembly. All Tesla models have this issue and it needs to be fixed.

Have experienced 2 major “phantom braking” incidents almost causing 2 accidents. There has also been several other incidents but not as severe as the 2 almost causing accident. Called Tesla and they said it is a known problem, service person said he has had his car for 5 years and still having issues. Evidently he said it was software related. He reported there is nothing they can do as solution has not been found.

The contact turned off the Autonomous Self Driving and Forward Collision Avoidance: Adaptive Cruise Control however, the vehicle experienced phantom braking 30 minutes later while driving. The contact continued driving to her destination. The vehicle was not diagnosed or repaired. A dealer was not contacted. The manufacturer had not been informed of the failure. The failure mileage was 398.

Cruise control settings are such that the driver engages and sets the cruise control speed based on the current speed of the vehicle. That speed setting should then never change unless the driver changes it. The vehicle unexpectedly and spontaneously resets the speed setting on its own when passing by certain specific landmarks e.g. specific exit ramps on the interstate and where an intersecting road crosses over the interstate on a bridge. The change in the speed setting can be quite large based on the speed limit of the interstate which then causes the vehicle to dramatically slow to meet the new speed setting. For example the speed setting was set at 70 MPH. The vehicle passed by a specific exit ramp on an interstate and the speed setting unexpectedly and spontaneously reset to 55 MPH. The vehicle then goes into a dramatic deceleration to slow down to the new speed setting creating a high risk for a rear collision from a following car.

Unintended Acceleration. I was driving back from a place in brand new Tesla, drove around 11-12 miles and was fine. We went to ice cream place and I stopped my vehicle in parking spot. Car was completely stopped at that time. To fix parking I put steering in reverse (don’t remember putting my foot on acceleration at all) and car went back at fast speed. I was completely taken aback and before I could realize and put brake, it hit red pole. Thankfully all family members were safe and no car or any one was hit while car was reversing. All happened within just few secs. Car was towed away to home and then to body shop, police did come to check on scene but there was no police report as it didn’t involve any other Car/person and no damage was caused to property. Car is damaged in rear but motor and battery is all fine. It’s in body shop but maybe totaled based on claim. Drove thru multiple states in spring and Tesla autopilot would come to abrupt sudden slow down without warning. Bad enough that I would disable the system and drive manually. This problem occurs regardless of one lane or two lane roads. With or without cars in front or behind me. Sunny or cloudy skies. Hot or cold. Straight or windy roads. With autopilot or even adaptive cruise control. I have no faith in the system. At one point it even changed lanes without warning almost hitting another car.

I was driving on the highway and the frunk came off breaking the front shield. The frunk was still showing as closed on the computer, but the screws came off releasing the frunk. Luckily I was able to react appropriately and move the car to the shoulder of the highway but the incident could’ve caused death. Visibility was zero as the front shield was obstructed by the open frunk

1.What component or system failed or malfunctioned, and is it available for inspection upon request? After closing the passenger side door, The 12v battery that controls the Tesla cabin died without any prior warning or car notification that it needed to be replaced, as a result the car instantly powered down , disabling the a/c, windows and door locks and we were unable to open the doors to access the car. 2. How was your safety or the safety of others put at risk? At the time of the event no, just our personal belongings were trapped inside the car but as a parent of a small child I usually put my child in her car seat first and close the door then get in my seat, if this had occurred with my child being in the car , my only option would be to smash in the window to get her out as the car has no back up system to open the car doors if the 12v battery dies. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes , the tesla had to be towed to the service center we’re they had to run system test and replace the 12v battery. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes by Tesla , they ran test and determined that the 12v battery was the issue. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warnings or notifications were given prior , teslas are suppose to receive a notification to alert the owner the battery needs to be replaced as their cars have a lot of 12v battery drain.

While using cruise control on the highway, the car suddenly decelerated twice. Each time the cruise control was set around 78mph, and it decelerated to approximately 70mph. No warnings were issued before, during, or after the decelerations— the car continued as if nothing had happened.
