SERVICE BRAKES, STEERING, VEHICLE SPEED CONTROL
Vehicle often applies brakes on its own . At first it was only in turns . However now it does it when driving straight on the the freeway or just down the street at 45mph. I have fixed my alignment ,new tires, all of the front suspension and nothing prevents the car from trying to brake for me. it is a safety feature that is not safe at all. a car slamming on your brakes at any speed is dangerous in addition to that it seems to cut off your fuel , leaving you unable to accelerate until its done beeping which can take a long time. the vsc , chenk engine and trac off flash there is no crash to report I have been lucky so far

SERVICE BRAKES
The contact owned a 2024 Toyota Sienna Hybrid. The contact stated while driving at an undisclosed speed, the brake pedal was depressed, however, the vehicle failed to stop as intended. No warning lights were illuminated. The contact stated the vehicle was unstoppable. The contact stated the option was to crash into the three vehicles which were coming from the opposite direction or to crash into the pole to bring the vehicle to a stop and avoid injuring pedestrians. The paramedics and the Police department arrived at the scene of the crash. The contact's wife, who was the occupant of the front passenger's seat, was transported to the local hospital. The contact sustained bruises and an open wound which was continuing to heal. The contact's wife sustained bruises and bumps. The vehicle was towed to a local tow yard. The vehicle was condemned as a total loss. A police report was filed. The manufacturer was made aware of the failure. The manufacturer ordered an investigation of the black box. The investigation was completed, and the contact was provided a copy. The failure mileage was approximately 3,000.

The contact owns a 2021 Toyota Sienna Hybrid. The contact stated that while driving at an undisclosed speed, there was an abnormal rattling sound coming from the second-row seats. The message to "Check Rear Seats" was displayed. No warning lights were illuminated. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 25V086000 (Seats); the VIN was included, and the vehicle was repaired. However, the contact was informed that the remedy was for the third-row seat only. The manufacturer was notified of the failure, a case was opened, and the contact was referred to the NHTSA Hotline to report the failure. The failure mileage was 169,000.

Pressure Control Soleneoid "A" Prformance stuck oof -P0746. Second opionin: Valve bddy Assembly clutch kits inside transmission. Toyota of Dallas refuse to provide service. I had done Tranmission service done by them . Need to get it fix van with new transmission installed and correct any other recall of 2025 to be fix.

Van purchased new October 2024. On two separate occasions, one in late 2024 and next today, [XXX], when pulling into street from a stop, there is no throttle response. You push the accelerator at varying pressure and no response for 2-3 seconds. This is a severe safety issue as it could cause an accident with oncoming traffic. No error lights illuminated on dash. Have seen other complaints on NHTSA website and social media as well. Will take to dealer but reporting a major safety issue. STEERING
While driving typically around 35 mph and either left or right turn the vehicle traction control engages and the van brakes and steering is affected. The roads have always been dry and in each instance the speed of the van was at or below the speed limit - traction was never lost and yet the traction control engaged as if we were in a skid. This occurred at least 12 times while driving to and from a day trip in the mountains (windy road). It has also happened multiple times at around 50 mph while turning left on a nearby flat freeway. An independent service shop has not been able to replicate the problem. The safety of the driver and passengers were at risk when it occurred on the freeway as a driver behind us got close to rear ending the van while it was in its brake mode.

VISIBILITY:REARVIEW MIRRORS/DEVICES:EXTERIOR
Driver side mirror visibly rattles when driving, but even worse when on highway. Safety concern is that visibility is blurred when driving. Vehicle is available for further inspection. Safety is put at risk since visibility becomes blurred when trying to change lanes. Yes concern has been reproduced, however I was told Toyota won't declare a warranty issue because other Sienna's and models of Toyotas have similar issue. Component has not been inspected by Toyota. However research I have done shows that Toyota is not acknowledging as a safety concern. No warnings. I purchased the 2025 Sienna Van February 8, 2025. I noticed almost immediately when I left the dealership, and turned back around for the dealership to inspect.

I have had the traction control go on while driving on the highway even when I turn it off. Sometimes the VSC and TRAC OFF lights go on while driving. The roads are dry. Whenever I take it in to the mechanic, all the lights are off. They pulled a zero calibration code and recalibrated. It still kept happening. Then I was driving around a curve on highway speeds when the traction control light went on and the van suddenly braked on its own and I almost lost control. The tires are new and the roads were dry.

The contact stated that after several attempts, the sliding door failed to close completely. The contact stated that no warning light was illuminated. The contact removed the button under the sliding door to manually close the door. The manufacturer was not notified of the failure. ELECTRICAL SYSTEM
The vehicle has a standard installation of a kick sensor for the rear hatch. I purchased the vehicle used and was able to start the vehicle and drive it for a period of time hoping to charge up the 12V battery. However, upon stopping the vehicle I could not restart it. Even after repeated jumpstart and driving the vehicle for over 45 minutes, the 12 V battery could not restart the car. I read about a similar issue with 2022 Toyota vehicles and so I followed the directions to disable the kick sensor. I then jumpstarted the car, drove it around and I found I could restart the car even after a short drive. I left the vehicle sitting in my driveway for two days and I could still restart the vehicle. I believe the 2023 Toyota sienna has the same issue as the 2022 model and the kick sensor needs to be replaced so it doesn’t drain the 12V battery when the sensor gets dirty. I live in the Midwest and this happened during January when there was snow and slush on the streets.

2024 - Toyota Sienna Woodland Edition - Purchased Oct 3, 2024. When you park the vehicle and put the vehicle in Park, the vehicle thinks that it is park. However, when you come back and start the vehicle, the vehicle again things it is in park, HOWEVER, you are not able to open the power side doors or the rear tailgate using the overhead open button, or using the remote. The vehicle thinks that it is in gear.

I purchased the car with 0 Miles. The tire pressure monitoring system would show an error every so often. Then go off immediately. But would never stay on long enough to get it diagnosed. It’s an issue and has be discussed on many forums as being a problem.

Drivers and passengers front doors make the sound as if they unlock and lock. The manual lock lever will flip as well however the doors will not unlock. This happens randomly both while trying to exit the vehicle and enter. When locked inside you have to crawl over the console to get to a door that functions . This is a major concern if you are in an accident and cannot exit the vehicle. Dealer look at my vehicle but could not duplicate it. I have a video of this when it happens
