The Harness Adjuster Strap (item 5 in attached snip from user manual) loosens while in use. This item was manufactured post-recall 24C002000, using Nuna's new design described here: [XXX] This design change did not prevent the functionality of the strap to fail. There was zero to minimal debris (no food, drink, or water) in the mechanism causing it to fail. The Harness Release button (item 4 in snip from user manual) actuates freely. The adjuster strap, and subsequently the entire harness strap, moves freely, while the carseat is loaded or unloaded. I have a video available showing that the strop in the seat freely moving. We discovered the product failure while loading the child in the carseat and testing it's security by pulling on chest straps. Nuna's response was lackluster and unprofessional. Here is the sequence of events: We immediately reported the issue the first business day available, May 27th, and Nuna responded 24hrs later, informing us they would again respond in 1-2 business days to send us a form and shipping label to send back the defective product. We responded the same day asking them to expedite the process and send us a new product immediately as we had no backup for a carseat and my wife could not leave the home. A little over 48 hrs later they denied our request, and sent us the shipping and RMA form. We are now a full business week past the initial request and now are returning the carseat, with no assurance of when the replacement will be delivered. The contact owns a 2015 Toyota Sienna. While the vehicle was parked, the contact discovered that the fuel lid door hinge had fractured, causing the fuel lid to intermittently fail to open or close as needed. The contact stated that with the fuel lid intermittently inoperative, the rear driver’s side door failed to open when needed. No warning lights were illuminated. The vehicle was taken to an independent mechanic where it was determined that the fuel lid door and hinge needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 117,000.

October of 2024 my van stalled while I was driving. The van was towed to an auto shop. The shop suspected a battery & alternator so replaced it. Later that same week I filled my tank at Delta Sonic and immediately proceeded to the car wash. My van stalled in the car wash. Once I was pushed out I had it towed again. I drove it for a few weeks and filled up the tank at Delta Sonic but this time the van didn’t start at all. Towed the van again. Shop thinks my issue is resolved- did a complete check of everything & returned my van to me. A week or so goes by and I filled my tank late at night and it wouldn’t start after fill up. A tow was not available. Scared and frustrated I called multiple people for help. Two hours pass, before I try to start the van again. It starts. Now I am starting to correlate a fuel fill up with my van not starting after re-fueling. My safety and my grandchild’s safety was jeopardized when my van stalled while driving in an extremely busy roadway. The next time, Multiple people had to push me out of the car wash and the last time I was forced to sit at gas pumps waiting for help. I stop filling my tank to full and take the van to Toyota. They confirmed the fuel pump was the issue and fixed it.

multiple times this has been an issue. I stepped on the break and the car essentially leaps forward typically when going up down or straight. In November 2024 it happened while going up a hill slowly and I bumped into the vehicle ahead of me. My vehicle had no damage, but the car in front of me had to get a new skid plate and my insurance doubled because of it. Since the incident in November 2024, my car has unintentionally accelerated intermittently. finally, April 2025 this happened my car started shaking jolted forward, engine revving, and then my check engine light came on then immediately contacted my local dealership and scheduled an appointment to get my car in. They have had my vehicle since then and have said it is not safe to give it back to me so I am currently driving a loaner vehicle.

My engine caught on fire. We had seconds to get out prior to it engulfing in flames. Fire department had to extinguish. Not sure what caused it. The entire vehicle caught on fire. There was an engine light warning I had the vehicle repaired but the warning light was not cleared

On [XXX], I was involved in a car accident in Saint John, Indiana, while driving my 2017 Toyota Sienna. The vehicle was hit on the passenger side, but none of the airbags deployed. As a result, I sustained serious injuries including cervical disc herniations, nerve compression, and ongoing pain. Since then, I’ve been unable to perform daily tasks, drive, care for my children, or work as I did before. I confirmed with Toyota that my vehicle has no active recalls, including for airbags, yet this failure occurred. There were no warning lights or messages prior to the incident. The airbag system has not yet been inspected or confirmed defective by a service center, but I am willing to make the vehicle available for inspection if needed. I am requesting that NHTSA investigate this failure, as it may represent a serious safety defect that could affect others driving the same model. I can provide supporting documentation, including medical records, police report, and photos of the vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

There are no messages/warnings. The vehicle will surge at very low speeds such as when I am coming to a stop, trying to avoid a person in a parking lot etc. It was SO scary this evening as I accelerated into a very very busy intersection...I pressed on my brakes harder and it was instant - but about 1 second (which is a LOT in this scenario) later it then slowed instead of accelerated. I have owned the vehicle almost a year. I will be bringing in for inspection this week and let the dealer know. I have put on about 11K in miles and would say this has happened a dozen times thus far with this evening being the scariest one yet. I figured it was just something normal for a hybrid but as I researched online - many people have this issue. TOO SCARY for me and dont want to hurt anyone or myself.

- #11659970May 9, 2025SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, STEERING
The Traction Control System commonly malfunctions while making turns on curving road sections. I will lose power through turns as the TCS light blinks on my dashboard and makes a beeping sound. On some occasions the brakes will automatically engage in the tire that the car perceives as experiencing slippage. This problem is continuously worsening and commonly endangers me and my passengers, as the car engages brakes or cuts power to the engine at unexpected and in opportune times.

2013 Toyota sienna van sliding doors. Driver sliding door opened on its own while driving down the road. Passenger side door will not open from the inside. This is a danger to my daughter who sits in the backseat if the door opens while I’m driving down the highway. Or she can’t get out because it will not open

The interior dash lights, when turned up to max brightness, take several seconds to read when making adjustments to the climate controls. These dim controls pause a hazard as it takes your attention away from driving. The dealer told me that they could not make them brighter. The issue is at night and day times.

Transmission failure. The dealer confirmed the issue and was repaired at an independent shop. There are no warning lights and only audible sounds that caused us to have it checked out. This appears to be a more widespread problem as Toyota did issue a TSB for a subset of VINs of this model year.

Purchased a **2025 Sienna XSE** with severe alignment issues since new. After **two failed repair attempts** at Fort Bend Toyota, the steering is now worse, and they damaged my wheel without offering solutions. This is a **safety hazard**—the car drifts dangerously at slow and highway speeds. The dealership dismissed my concerns, forcing me to escalate to **NHTSA** and **Toyota Corporate**. Avoid for serious mechanical issues—safety clearly isn’t their priority.
