TSB T-SB-0010-24 P0303 code 163,000 miles Toyota denied goodwill assistance Cost: $2,549.65 Check engine light came on with vehicle hesitation on acceleration almost caused to stall with reduced power while driving , the dealership diagnosed the issue as TSB T-SB-0010-24 with cylinder #3 misfire requiring exhaust valve lash adjuster to be replaced. Contacted Toyota corporate to request help with goodwill or discount , was denied , was told they couldn't help no reason given .This issue is a well know Toyota RAV4 defect and even though they claim it only happens when is cold this happened to my SUV at 60*F. Please help.

The vehicle was taken to a local dealer, where it was diagnosed with a deteriorated sealant. The vehicle was repaired. In addition, the contact stated that while driving at various speeds during rainy weather conditions, water would intrude into the rear driver-side door and would accumulate inside. The local dealer was contacted; however, the vehicle was not diagnosed or repaired due to costs. The manufacturer was not made aware of the failure. The failure mileage was approximately 40,000.

My daughter reported an incident that occurred on XXX while driving this vehicle. As she drove, it slammed on the brakes (without any input from her) causing her to skid. The vehicle brought itself to a stop and the engine turned off. She reported that there were no vehicles or other object close to the front of her vehicle. The incident did not cause a crash or cause anyone to be injured. It was, however, a very scary situation that almost caused her to be rear-ended by the vehicles that were following her. The car was taken to Cobb County Toyota in Kennesaw, Georgia for an inspection the morning of May 28, 2026. As of this submission, the vehicle is still at the dealership service department for inspection.

Gear shift makes noise when moving it, as in you can actually hear the gears/electrical equipment moving and making noise when going from park to drive. The keyless entry when the fob is near, just doesn't work well from the drivers side... I can stand there trying to allow it to unlock and it just locks again or wont unlock the passenger door even if allowing it extra time to.

Water sloshing around inside the passenger door after rain or car wash. If you pull at the seal a little bit, water pours out. This has happened a few times now in the last year of owning the car. The rear seatbelt sensor also goes off randomly, when no one is sitting in the back seat.

The trim around the sunroof is cracked and causing a leak. I took the vehicle to South Hills Toyota in Cannonsburg PA, for maintenance and state inspection, specifically identifying this issue. The car is less than 2 years old still under warranty Initially I was told it was not a material defect and not covered by warranty, remedy would be replacement of entire sunroof. Discussion with the service manager resulted in the defect being escalated to Toyota. Pictures were taken by the Toyota service team and sent to Toyota for further review.

I apologize if this does not meet the NHTSA bar of a safety hazard. It does reduce the ability for hands free navigation and acts as a distraction for drivers. Toyota Rav4s have very poor GPS tracking which makes the directions in the integrated head unit and guage cluster very distracting because they are consistently incorrect. There are dozens, maybe 100+, reports online of people having similar issues. Toyota dealership has looked at it many times, escalated for troubleshooting, and as of today they say there is no fix. I have many videos to demonstrate the issue is not a fluke. There are no warning signs and the GPS will just start giving poor directions. The "danger" is in the fact that the only workaround now is to engage with a phone or other device while driving. In theory a human can rise above this and be smarter than the car but isn't that true of many safety components like non-functioning wipers(drive slow) or blind spot protectiong(check your mirror). The car is giving bad directions that the user is relying on as a navigational aid.

The multimedia screen which shows the map and direction guidance became to blank occasionally, just white grey since February 2026. I immediately lost the direction. It could be back by restarting the engine at the beginning. The problem got worse, happened more often, some times the screen couldn't be back by restarting engine, but I found that it could be reset by pressing the volume button for long time. I reported to Toyota local dealer - Hendrick Toyota Apex and Toyota Brand Engagement center, the case manager is Freddy. The local dealer run diagnosis twice, couldn't detect any problem. On May 11, the screen totally died, black, no any light. Restarting engine or pressing the volume Botton couldn't get the screen back any more. the audio system is still working, I tried to use the map on my iPhone, the direction map couldn't be showed, only the text direction shows. Because the system let map show on screen, it assumes that the map shows one screen but it is not. I lost car screen and the phone direction at the same time. Plus the backup camera doesn't show on the screen. Therefore, it is underivable in any unfamiliar area, and dangerous to drive in neighborhoods. I reported to local dealer and Toyota Brand Engagement center with videos. May 18, the local dealer run diagnosis again, found nothing wrong. But before I leave, the screen became blank. This morning it happened again. The problem is not fixed. Freddy told me he suggested to local dealer to replace the screen, I am waiting Hendrick Toyota Apex call me for the appointment. COMPONENT: Electronic Throttle Control System (ETCS) / accelerator pedal. Vehicle is available for inspection at Autocentro Toyota, San Juan, PR, where it has been retained since 2/28/2026. INCIDENT: On 2/28/2026 at 7:45 AM, while entering a private driveway at under 5 MPH to park, my 2025 Toyota RAV4 Hybrid SE (VIN [XXX] , 3,164 miles) suddenly and involuntarily accelerated without driver input. I categorically affirm I did NOT press the accelerator. The vehicle struck a metal gate, causing frontal damage. Airbags did not deploy. OEM floor mat was properly secured; no aftermarket parts installed. SAFETY RISK: Sudden unintended acceleration at low-speed parking maneuver. Had a pedestrian, child, or another vehicle been in the path, this would have caused serious injury or death. The defect is unpredictable and the driver cannot anticipate or override it. INSPECTION: Vehicle inspected 3/20/2026 by Engineering Analysis Associates (EAA), a firm contracted and paid by Toyota — NOT independent. EAA downloaded electronic data and concluded the accelerator was pressed 1 second before impact. I dispute this finding: I did not press the pedal. No independent inspection has been allowed. Full EAA report was never provided to me. On 4/23/2026, Toyota Motor North America (Ms. Delve L., Legal Claims Administrator) declined assistance based solely on EAA's data. WARNING SIGNS: UNKNOWN. No warning lamps, messages, or prior symptoms appeared before the incident. The vehicle had functioned normally since purchase on 4/25/2025. ADDITIONAL: Vehicle retained by dealer 73+ days without repair or substitute vehicle. Consumer complaint filed with Puerto Rico DACO (Case XXX). I am submitting this complaint to report a known and widespread safety defect involving the door lock actuators on my Toyota RAV4. My vehicle’s door actuators have failed at approximately 39,000 miles, just outside the 3‑year/36,000‑mile warranty. This failure prevents the doors from locking and unlocking properly, which poses a significant safety hazard while driving, especially in emergency situations. When I brought the issue to Beechmont Toyota, the dealership refused to repair the defect under goodwill or warranty consideration. I spoke with Stacy Meyer, Assistant Service Manager [XXX] ), who stated that the refusal was directed by Rodney Wetenkamp, Service Director. Despite the well‑documented nature of this defect, the dealership declined to assist in any way. This is not an isolated incident. There is an active class action lawsuit in Texas regarding defective Toyota RAV4 door lock actuators, demonstrating that Toyota is aware of the widespread nature of this problem. Toyota advised me to work through the dealership, stating that the dealer should escalate the issue through their internal channels. However, the dealership refused to do so, leaving me without support despite this being a manufacturer‑related defect. Door lock actuator failure is a serious safety issue. When the locks fail: Doors may not lock while driving, exposing occupants to risk Doors may not unlock in an emergency Children and passengers may be unable to exit the vehicle quickly The vehicle becomes vulnerable to theft and unauthorized entry Given the known defect, Toyota’s awareness of the issue, and the dealership’s refusal to escalate or repair the problem, I believe this represents a failure of manufacturer responsibility and a potential violation of safety standards. Upon taking my 2021 Rav4 Hybrid in for the rear traction motor cable corrosion recall (25TD01) on 5/7/2026, I was told there are no parts, no estimated arrival time, and therefore not even a rough estimate for a timeframe of repair. I was told that no one would inspect or evaluate the part in question to indicate if the car was safe to drive. I expressed my extreme concern given the recall includes risk of power failure, even while driving. I received inconsistent, avoidant answers and an absolute denial to remedy the situation in any way. I was told the dealership is "Dolan" and not "Toyota". I again expressed my concerns about safety and the dealerships' denial to, at minimum inspect the recalled part and/or housing of the part. Emily, the Service manager stated that Toyota does not "allow" loaner vehicles for this particular recall. She could not provide any sources for this information. To summarize; the dealer with not provide an inspection of the recalled part and/or housing, or give an estimated timeline for repair given I am #125 on a waitlist. There is a complete lack of regard for mine or my children's safety as the dealer will not provide any assessment of how bad the damage could be. They suggested that I continue to drive the vehicle, acknowledging all of the safety hazards that could occur. I am terrified of the possibility of having an accident with my kids in the car due to this recalled part failing, not having a vehicle to get to and from work, and the potential additional costs that this unaddressed recall issue could cause. Please advise!

“Required Engine Maintenance- Visit Your Dealer” message came on the dash. After doing some research and troubleshooting; it appeared to be the coolant bypass valve that everyone has had issues with. The problem was then confirmed with a local provider who instructed us to contact the dealer to see if they’d cover the repair. Otherwise the repair by them would be $700. Toyota customer service told us that there is no TSB or Recall for this issue. We would need to have the issue diagnosed and then pay out of pocket for the repair. This is an issue that spreads throughout so many vehicles that it needs to be a recall.
