What component or system failed, and is it available for inspection? The roof/water sealing system failed, allowing water to enter the vehicle interior. It is available for inspection upon request. How was safety put at risk? Water intrusion through the roof caused visible mold growth, interior deterioration, and a severe foul odor. The mold and contamination inside the vehicle is causing constant illness to the occupant, creating a serious ongoing health and safety hazard. Has the problem been reproduced or confirmed by a dealer or service center? The vehicle was brought to a service center for inspection, however they refused to inspect it. The problem has not been formally confirmed. Has the vehicle been inspected by the manufacturer, police, insurance, or others? The vehicle was taken in for inspection but the dealer/service center declined to inspect it. No manufacturer, police, or insurance inspection has taken place. Were there warning lamps, messages, or symptoms prior to failure? When did they first appear? No warning lamps or messages appeared. The symptoms that developed include: Water entering through the roof Visible mold growth Interior deterioration Severe foul odor described as "rotten rat" smell Occupant experiencing constant illness due to exposure

The panoramic moonroof on my 2022 Toyota RAV4 spontaneously shattered and exploded outward at approximately 2:09 PM on June 21, 2026 while my wife was driving and exiting the freeway. There was a loud bang, followed by glass shards ejecting primarily outward onto the roof with a pattern showing the glass failed/bent from the inside out. The outer glass panel broke into multiple large sections with a clean perimeter failure and no visible external impact point or road debris damage. This created an immediate safety hazard: loud distraction while driving at highway speeds, flying glass, and sudden exposure through the roof opening. No warning lights, messages, noises, or prior symptoms appeared. The moonroof had been used normally with no issues. The component (panoramic moonroof glass assembly) is available for inspection. I have a dealer appointment scheduled for tomorrow, June 22, 2026 at 9:45 AM for inspection. The vehicle has not yet been inspected by the dealer, manufacturer, insurance, or police. This matches numerous similar reports of spontaneous panoramic sunroof failures on RAV4 models.

The front driver-side door lock and opener (actuator) stopped working using the key fob and needs to be manually locked from the inside. When I try to access the door after manually locking driver side and using my key fob for the remaining 3 locks, the unit won't automatically open. This causes a safety risk considering I need to access the driver's side back or front passenger side to open the door. According to what I've read this is a known issue.

I purchased a 2024 Toyota RAV4 in March 2024 with the expectation that its advertised safety and driver-assistance systems would function as designed. The vehicle has experienced a failure of the forward camera/ADAS system, resulting in the loss of multiple Toyota Safety Sense features. The following systems are currently unavailable or malfunctioning: • Dynamic Radar Cruise Control / Adaptive Cruise Control • Pre-Collision System • Lane Tracing Assist / Lane Keeping Functions • Road Sign Assist (speed limit recognition) • Related driver-assistance and safety warning functions When cruise control is activated, the vehicle displays a warning that cruise control is unavailable. The dashboard reports multiple safety-system malfunctions. As a result, I have lost essentially all advanced driver-assistance features that were included with the vehicle. Toyota’s dealer diagnosed the failure and Toyota approved the repair under a goodwill case. However, I was informed that the required replacement component is on national backorder with an estimated wait of approximately two months and no guaranteed delivery date. Despite acknowledging the defect and approving repair, Toyota has not provided a comparable loaner vehicle or alternative transportation. I am being asked to continue operating the vehicle for an extended period without the safety technologies that were a significant factor in my purchase decision. I am concerned about the loss of collision-avoidance, lane-assistance, and adaptive-cruise functions, the indefinite repair delay, and the lack of transportation assistance while awaiting parts. I am requesting investigation, assistance, and consideration of repurchase or other appropriate remedies.

As i was driving to work my sunroof shattered out of nowhere, there was no provocation. There was a big pop and then there were noises coming from the roof. When i checked I saw the sunroof had shattered into tine pieces. Toyota does not want to take responsability into this incident.

UNKNOWN - On XXX, at just 7,741 miles, my 2025 Toyota RAV4 XLE Premium AWD (VIN: [XXX]) suffered a catastrophic and life-threatening complete mechanical and hydraulic transmission failure. The vehicle completely lost forward propulsion when placed in Drive mode, rendering it entirely immobile. When placed in Reverse gear, the transmission violently over-pressurized, causing the vehicle to suddenly surge and jump uncontrollably. Simultaneously, the dashboard displayed 6 critical safety system failures: 1. Check Engine — electronic control malfunction 2. Vehicle Stability Control System (VSC/TRAC) malfunction 3. Secondary Collision Brake System malfunction 4. Pre-Collision System malfunction 5. Parking Brake System malfunction 6. Advanced Ultrasonic Detecting and Ranging System malfunction Ourisman Toyota Chantilly (RO# XXX) has confirmed complete transmission failure. Fault codes found: P28167F — Pressure Control Solenoid H Actuator Stuck Off, P08BA7F — Pressure Control Solenoid L Actuator Stuck Off. Estimated repair cost: $11,347.42. While this failure fortunately occurred on a local road, had it occurred while merging onto I-66 or I-95 at highway speeds, it would have inevitably caused a catastrophic high-speed collision. This represents an extreme manufacturing defect and life-threatening safety hazard. Toyota Corporate Cases opened: #XXX (original case) and #XXX (full replacement request). I have owned this vehicle close to three years and these were the all season tires that came with the car.I live in New England where I run snow tires on the car from October to April every year that I have owned the car. The car has 30K miles on it ,but the tires that came with the car only have about 10K or 7/8 tire tread left. The tires have become very noisy and are starting to cup. I did take the car to the dealer and had the front end checked out and nothing is worn or out of alignment. Tires were rotated at 5K intervals .I contacted Dunlop and was told there was not much they would do since the tires were made when Dunlop was owned by Goodyear. Dunlop is now owned by Falken. I was offers 30% off a new set of tires (Dunlop) as a good will gesture. A new set of tires will run about 1K with tax and mounting/balance. I should have to spend that kind of money on tires that are not worn, just poorly made. Falken admitted that they have a lot of complaints about this tire. Tire size is 225/60R18/ 2023 Toyota RAV4 XSE

The contact then noticed that the moonroof glass panel had exploded without impact, causing shattered glass to fall inside the cabin. A police report was filed. The dealer was notified of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 12,838.

Passenger front door actuator\lock mechanism does not lock/unlock the door. There have been numerous lawsuits and yet NO RECALL for this very dangerous known problem. I purchased this car used in December 2025 with 10k miles on it. After 15k mileage, the rear tires were bald and I had to buy new tires. I then took it back to the dealer and had to pay to get an allignment, as I was concerned that is what was causing the tires to go bald so fast. That was less than 2 months ago and now I'm in getting the 20k service appointment and the rear allignment is now off way more than when I paid to get it checked. Apparently the rear wheels cannot be adjusted? The right rear is way off and this is for sure causing uneven wear on my tires. It is for sure an issue and should not be, in that the car is new. It is obvious it is a fault in design and for sure can cause major safety issues, let alone burn my money up in needing new tires every less than a year.

The contact owns a 2021 Toyota RAV4. The contact was traveling at approximately 50 MPH directly behind a van in the same lane of traffic. The van suddenly made an emergency avoidance maneuver to bypass a stationary vehicle completely stopped in the lane. Despite the attempt, the van rear-ended the stopped vehicle. The contact immediately initiated an evasive maneuver to avoid the sudden collision occurring ahead. However, due to proximity and immediate lane obstruction, the contact was unable to avoid an impact and struck the vehicle. The severe front-end impact resulted in catastrophic damage to the contact's vehicle before it came to a complete stop. The airbags failed to deploy, and no warning lights were illuminated. The vehicle was towed to a repair shop. A police report was filed. The driver and her pet were the only occupants of the vehicle, and the contact was the only injured passenger. The contact sustained moderate chest pain, but no medical assistance was received. A police report was filed. The dealer was not notified of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 62,000.

2 incidents of Sudden Unintended Acceleration. No warning lights etc in either. Car damaged but available for inspection. Now encountering difficulties initating Toyota investigation. 1st incident (4/18/25; mileage 967) occurred after a hard left turn. After turn onto uphill street, I took my foot from accelerator to slow and maintain distance from a car backing down hill ahead. Instead, car self-accelerated and shot uphill, over speed bump with increasing speed. Did not respond to braking until it was close to hitting a parked car. Stopped with a slam; probably brake assistance engaged. No damage or injury, but I was shaken. Took car immediately to Toyota dealer to inspect. Was told RoB confirmed an event but not its nature or cause; identified nothing wrong. They hypothesized cruise control might have been triggered accidentally (no evidence, not plausible). Claimed they submitted report to Toyota national center. After 2nd incident, Toyota national said they had no record of 1st event. Their standard safety inspection protocols were not followed. Returned car to me without repair or follow up. 2nd incident ensued. 2nd self-acceleration event (2026) also occurred after hard left turn to make U turn at low speed, braking while completing turn. When I took my foot off brake, before I could press it on accelerator, car shot diagonally across street. Jumped curb and traveled at considerable speed 100' up landscaped slope. Destroyed 6 large shrubs and 3 trees 9' tall. Did not stop despite constant driver braking. Forward collision prevention system did not engage to prevent uprooting trees. Car stopped only after I turned steering wheel sharply to left to avoid hitting a full-size tree trunk. Then car responded to braking, acceleration ended, car stopped next to tree without impact. Terrifying life-threatening event but only damaged landscape and car. Police came, assessed incident, determined not a police matter, did not file police report.

I was driving across a mountain pass in Washington State at an elevation of approximately 4,000 ft. With no warning I hear a very loud noise, sounding like a gunshot. I was in pretty heavy traffic and pulled over as soon as it was safe. A walk around the car revealed no problems, but looking at the top of the car showed that the glass over the sunroof had imploded. I'm not a fan of sunroofs and I may have never opened it. My favorite AI search engine Gemini had plenty of information. I experienced what is known as a spontaneous sunroof implosion/explosion. Fortunately, neither my passengers nor I was injured. Toyota is not accepting responsibility for these incidents.

The brakes have been overly sensitive... but then, without warning, braking power is completely lost. This has happened on 2 separate occasions... the pedal goes all the way to the floor and provides ZERO braking power. We can no longer trust this car on the road. Toyota sent someone to inspect the vehicle and they claim that they have found no problems or issues and that they have not been able to reproduce the problem. There were no warning lamps or message; there were no prior symptoms, other than the brakes were overly sensitive. These symptoms occurred immediately after purchasing the car. Toyota refuses to purchased the car back from us, and now we are stuck with a very dangerous vehicle. Interestingly, I have found others talking about EXACTLY the same symptoms on other Toyota Rav4 vehicles.

My car has shut off for a second time without warning. This time was in a busy chic [XXX] drive through line in [XXX] . The car read “low power steering” and the battery and engine light came on. These have both been checked before in addition to the alternator and they cannot find an issue. I am having it taken back to the dealership tomorrow for further investigation. Given I purchased this vehicle with a little over 19k miles and the car now only has 22k miles on it. It should not have any significant issues such as cutting off while driving on the interstate or while sitting in a busy drive through lane waiting for food. Now I am wondering if the previous owner traded it in for this issue. I will say, both incidences it has been raining and the first time I could have been hit by oncoming traffic due to low visibility and I am wondering if heavy rain has something to do with this. The car has not been through any significant puddles or left out in the rain. It sits in the garage when not in use and has only been used as a work commute vehicle. Something is wrong. was driving on 4 lane hwy and it was raining and my wipers stopped working mid-way on windshield, I had to find a safe place to pull off road and inspect the problem i tried turning car off and back on and wipers would still not work. I drove another 3 miles or so to the Toyota dealership luckily with no accident which made me feel very unsafe without my wipers. They told me i would have to leave car for them to take panel off to find the cause of failure and provided a loaner after i refused an Uber.
