The contact stated that while driving forward into a parking space and depressing the brake pedal, the vehicle unintendedly accelerated at a high rate of speed, almost causing the vehicle to crash into a building and other parked vehicles before the failure was self-corrected. The vehicle was towed to the local dealer, who was unable to determine the cause of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 27,000.

The contact's daughter owns a 2024 Toyota Rav4. The contact stated that while his daughter was driving at approximately 35 MPH, she had crossed an intersection when she was crashed into by another vehicle making a U-turn from a cross street. The other vehicle's front end crashed into the driver's side tire and front door. The contact stated that the force of the crash had fractured the left front axle and pushed the tire into the engine compartment. The contact stated that the frame was bent and the driver's door was difficult to open. The contact stated that the side air bags had not deployed and that the seat belts on the driver's side and the passenger side could not be unbuckled. The contact stated that his daughter had sustained injuries to her left shoulder, left arm, and leg as well as head and neck injuries. The contact stated that his wife had sustained injuries to her right ribs, right arm and shoulder, and lower back injuries. The contact stated that his wife and daughter were taken by ambulance to the hospital. The contact stated that the police were on scene and wrote a report, but he did not have the report number. The contact stated that both vehicles were towed away from the scene of the crash. The contact stated that their vehicle was repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 10,000.

The vehicle's dashboard and 9.5" screen go blank while driving down the road. It is random at times and you have no idea how fast you are traveling, viewing screen, etc.. It just goes completely black. The Certified Toyota dealer says that there is a recall on the 12.5" screens blacking out randomly but not on the 9.5" screens so the vehicle does not qualify for the recall and Toyota is saying they don't have a fix for the vehicle. Have tried to contact higher level Toyota contacts (claims) and they have not been of any help. With the screen going completely black, an individual has no idea how fast they are driving, what service indicator lights might be on, no idea how much gas there is in the car. The entire dash and associated screen which also houses the back up camera are completely useless. It happens at random points in time while driving. This is a safety issue as one does not know speed of vehicle or any warning lights. Dash and screen are completely blank.

On 12/24/25 I was pulling into a parking space, took foot off accelerator to brake and vehicle suddenly accelerated and engine revved. I kept pushing the brake, but the vehicle kept lurching forward and slammed into car parked in front of me before finally stopping. My vehicle was going so hard forward that it pushed the car I hit 2 to 3 feet out of it's parking space and even punctured the radiator. It was not raining, the floor mat was not in the way, I did not have cruise control engaged. On 12/10/25 I took my car to Toyota dealership because I had a slow leak in one tire, which had to be replaced. At that time, the service department also updated parking assist software (could that be the problem?). In answer to above questions: * brakes and accelerator malfunctioned and it is available for inspection upon request. * no one was in the car I hit. I was not injured. * the problem has not been reproduced to my knowledge or confirmed. * the vehicle has been inspected by a third-party that Toyota engaged and also Erie Insurance. A police report is available. * there was no warning lamps, messages or other symptoms of the problem prior to the failure on 12/24/25.

The contact stated the front passenger’s side Occupant Classification System (OCS) sensor failed to detect an occupant seated in the passenger’s seat and was deactivated on the instrument panel. The dealer was contacted and informed of the failure; however, the vehicle had not been taken to be diagnosed or repaired. The contact researched and found information on NHTSA Campaign Number: 22V519000 (Air Bags) and associated the failure with the recall. The manufacturer was informed of the failure, and the contact was informed that the VIN was not included in the recall. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was 57,500

This complaint is being filed as a follow-up to a previously submitted NHTSA complaint regarding door lock failures on my 2021 Toyota RAV4 Hybrid. Since the original complaint, a second door has now failed. In addition to the front passenger door, the rear passenger-side door will no longer lock or unlock electronically using the key fob or interior power lock button and must be locked manually. When attempting to lock the vehicle, the vehicle emits a warning beep indicating a door is unsecured, even though some doors appear locked. Upon inspection, the rear passenger door was found unlocked. This issue appears to be progressive, affecting multiple doors. Doors may appear locked when they are not, creating a security concern.

I am reporting a safety defect involving the front passenger door lock actuator on my 2021 Toyota RAV4 XLE Hybrid. The front passenger door will not lock or unlock using the key fob or the interior power door lock button. The door must now be locked manually. This defeats the vehicle’s automatic locking system and creates a safety and security concern. This condition prevents all doors from being secured properly when driving or when parked. A passenger may believe the door is locked when it is not, increasing the risk of unauthorized entry, theft, or passenger safety issues. The failure occurred during normal use and without warning. The vehicle has been properly maintained. I have learned that multiple Toyota RAV4 owners have reported similar door lock actuator failures, indicating a potential widespread defect. This issue occurred shortly after the factory warranty expired. Due to the safety implications, I believe this issue warrants investigation and possible recall or manufacturer repair program. “This failure occurred without warning and with no dashboard alerts.”

Driving up a slope of about 20-30 degrees gradient of a straight lane going into a private parking lot. Braked and decided to make a 3-point turn to get back out. There was no traffic in the lane. Put gear on reverse while on the slope with the vehicle pointing in the up direction of the slope. Vehicle suddenly surge backwards, accelerating. Vehicle sped down the slope and crossed a street. Could not stop. Vehicle was stopped only after placing gear into park from reverse, and after jumping curb across the street. Fortunately, there were no cross traffic in either direction on the street, otherwise vehicle could have been T-boned. Tire wall on the rear passenger side busted. AAA was called to replace tire with spare. There were no warnings on the dashboard or any symptoms anywhere else, although the vehicle had all the safety features.. The next morning, vehicle was taken to Toyota of Carlsbad. Left there for diagnosis and tires change. Vehicle is left with Toyota of Carlsbad until this issue is answered/solved. Told by service manager of Toyota of Carlsbad to call Toyota USA at [XXX] to report incident by customer, and request Toyota USA to send expert from Toyota to diagnose issue. I attempted to remote start my 2022 Toyota RAV4 Hybrid from inside my home using the Toyota mobile app. I received a confirmation notification stating that the vehicle had started and would automatically shut off in 10 minutes. Approximately six minutes later, I exited my home and approached the vehicle, which was not running. After unlocking and entering the vehicle, the interior temperature confirmed that the vehicle had not been running. I attempted to start the vehicle by pressing the brake pedal and pushing the start button. No dashboard lights illuminated, no warning messages appeared, and the vehicle was completely unresponsive. I attempted this process two additional times with the same result. After exiting the vehicle and closing the driver’s door, I attempted to lock the vehicle using the touch sensor on the driver-side door handle. The vehicle would not lock. I attempted this a second time and it still would not lock, indicating a complete loss of electrical power. This failure occurred in cold weather conditions and involved the hybrid battery and associated electrical system. The loss of power created a safety risk because the vehicle could not be secured and could have left me stranded in cold weather. Later that day, I contacted my insurance company for roadside assistance. A tow truck arrived and jump-started the vehicle using a portable jump box, which allowed the vehicle to start and the battery to recharge. A Toyota service appointment is scheduled to inspect the vehicle. The issue has not yet been confirmed by a dealer. The vehicle has not been inspected by the manufacturer or other representatives. Only the roadside assistance tow truck driver responded. The vehicle and battery are available for inspection upon request.

I am aware of a recall for the screens turning off randomly while driving, especially in reverse while using the back up camera. My VIN was not included in the recall, though this has been happening to me. It happened for the first time when I was in reverse yesterday trying to use the back up camera, it typically happens while I’m driving forward on the road. I am including the date of the incident that happened while in reverse, but please know this has been happening since about a month after I purchased the car completely new.
