I was driving the speed limit (75) when I heard an explosion. I immediately pulled over to see if I had a flat tire or if someone had shot my car. I could see no damage and nothing was showing up on my dashboard so I continued to drive but heard a whistling sound. I checked all my windows and then my sunroof. Somehow the sunroof had exploded. There were no cars close to me and no overpasses or trees in the vicinity. I have had the car for almost 3 months and have never opened the sunroof.

- #11711581January 17, 2026FUEL/PROPULSION SYSTEM, ENGINE, VEHICLE SPEED CONTROL
I have had multiple instances where the vehicle has failed to accelerate when making a left hand turn. Each time this has occurred, my car has rolled into the intersection from a stop at a small decline, but the gas seems to do nothing. My car silently rolls forward and then I have no power or gas and am trapped with no acceleration. I’m worried this is going to get me into an accident and seriously injured. It does not happen super often, but enough that I know there is a problem. Dealer has not been able to see any codes, and we have not been able to recreate the issue in front of the service advisors.

- #11711231January 15, 2026ENGINE, UNKNOWN OR OTHER, BACK OVER PREVENTION: WARNINGS
On [XXX], at approximately [XXX], in Greenwich CT, while I was very slowly driving into a parking space, the engine suddenly and unexpectedly surged,. The vehicle overcame the brakes and raced forward and collided with a stone wall, about 3 ft away. This was a new (since May 2025) vehicle with only approx 3,500 miles on it. I had not had any previous problems with it and there were no warning lights on the instrument panel. At the time, I was proceeding at a very low rate of speed, 1 - 3 mph. I drive only with my right foot which was on the brake, not on the gas. The vehicle accelerated with NO action on my part and collided with a stone wall approx 3 ft away and bounced back several feet. Fortunately I was close to the wall so the vehicle did not have a long distance to accelerate and I was wearing my seat belt which probably protected me from injury. No other vehicle was involved. I was the sole occupant of the vehicle. I was wearing my seat-belt, the OEM rubber mats were securely in place. The airbags did not deploy and I was not injured. I had the car towed to the Toyota dealership by AAA because I was afraid it might accelerate into traffic at a red light or stop sign if I drove it before the problem was repaired. I have reported this incident to Toyota Motor Corporation USA, through their Brand Engagement Center (as instructed by the dealership) and they are sending out an engineer to the dealership to inspect the vehicle. I do not know which part malfunctioned or failed. The vehicle is at a Toyota dealership and presumably could be inspected (unless they replace some parts). - #11711075January 15, 2026SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
My wife was pulling her car into a parking area consisting of a few small storefronts. When pulling in, the brakes completely failed as did the forward collision warning system and the automatic emergency braking system. As she continued to press the brake pedal it just went to the floor of the car. The end result was that the car struck one of the storefronts causing damage to the building as well as her car. Fortunately the store was closed at the time and no one was injured, nor was she. There were not any warning lamps or messages prior to the brakes failing. The brakes worked fine in the approx. 15 minute drive prior to the accident. I realize that drivers sometimes press the accelerator in error instead of the brakes and I can assure you that this was not the case (we wife is not elderly and is a very safe driver). We spoke to the auto body shop who will be repairing the car and they told us that the brakes currently seem to work. A online search revealed that this problem as been reported / discussed by others in the past. We expect to have the car repaired and returned to us in the next week but are very concerned with driving it as another accident could be fatal.

- #11709145January 6, 2026VISIBILITY/WIPER, EXTERIOR LIGHTING, FUEL/PROPULSION SYSTEM
1)I purchased my car during daylight hours. A few days later I drove my car in the evening and could not see street signs or the road 1/3 of the way up through my windshield. There is a definitive line where the windshield turns black. It is extremely dangerous. The Toyota engagement center told me to take my car to the dealership where it was purchased. I also took photos of what I could not see while driving. The dealership said there is nothing they can do and this is normal. The engagement center called back after they received the photos and told me their head technician could not see the street signs looking at my photos either. I was told to take the car back again to the dealership and once again told " there is nothing that can be done." They told me to put on my automatic brights. This feature does not help. The dealership also told me I could just put on my brights manually. This blinds other drivers on the road and then they put on their brights driving towards me, which decreases my ability to see while driving. the Toyota engagement center dropped the ball after 3 months of trying to report how dangerous this situation is. I am now reaching out to your department because of serious safety issues. I did not buy a car to be placed in a dangerous situation where I can't see to drive at night up. It is very dangerous driving up and down hills. 2)I also want to report - most of the time my gas milage ranges between 18 - 22 mph. This is way below the stats that were given to me when the car was purchased. 3)The phone table does not charge my phone and is a complete fail. 4)The visors are too short and hardly serve a purpose.

I am submitting this complaint to document an unresolved vehicle safety concern and loss of confidence in safe operation. My vehicle experienced serious safety-related malfunctions and warnings, and despite service visits, I was not provided written confirmation that the vehicle was safe to operate. After repairs were reportedly completed, I repeatedly requested written safety verification, which was not provided. Due to the unresolved safety concerns and lack of written safety assurance, I did not feel it was safe to resume driving the vehicle. A loaner vehicle was provided while the safety issues were being addressed. The loaner was later recovered before my safety concerns were resolved and without advance notice, leaving me without a vehicle that I could confirm was safe to operate. I am reporting this because I continue to lack confidence in the safety of the vehicle and want this documented as a safety concern. This complaint is not about customer service, but about unresolved safety issues and the absence of written confirmation that the vehicle is safe to drive.

- #11708110December 31, 2025STEERING, SUSPENSION, WHEELS
2024 Toyota RAV4 XLE AWD, VIN [XXX]. Steering defect appeared at approximately 12,000 miles (April 2025). Vehicle pulls significantly to the right, requiring constant steering correction. Driver must continuously hold left side of steering wheel to maintain straight path. If steering wheel released, vehicle immediately drifts right. Pull is constant and especially noticeable at highway speeds. THREE REPAIR ATTEMPTS FAILED: June 2025 (14,715 miles): Two wheel alignments performed same day - both failed to correct pull. November 2025 (16,091 miles): Replaced blown passenger rear shock absorber - pull persisted. December 2025 (16,819 miles): Performed 10+ wheel alignments. Toyota Corporate Technical Assistance filed case #53530239 and instructed ABS/SAS reset - failed. Technicians documented "rear suspension binding" but did not mechanically repair. SUSPECTED CAUSE: Strut mount bearing binding/failure or suspension binding. Dealership replaced shock but not strut mounts (industry best practice is replacing both together when shock fails at low mileage). Documented binding issue was not mechanically repaired - only software reset attempted. CURRENT STATUS: Vehicle unsafe to drive. 16,819 miles in two years. New rattling noise developed in right front dashboard after December repairs. This is a manufacturing defect appearing in first year at low mileage, not wear and tear. SAFETY CONCERN: Driver must maintain constant corrective pressure on steering wheel to prevent rightward drift. Risk of vehicle drifting into adjacent lane or off road if driver momentarily distracted. Defect creates dangerous driving condition, particularly at highway speeds. My wife was going at about 10 - 15 mph coming up about 2 car lengths to a small curve in a residential area when all of a sudden she heard a pop or snap and the Rav4 went to a super high speed by itself. The acceleration could not be stopped even though she had her feet on the brakes. She had both hands on the steering wheel to avoid hitting a house and ended up going through a wall in the yard. There were no warning sounds and no signals indicating a problem. it appeared that the display was blank. There was no pre-collision warning or braking except as my wife was pushing on the brakes. She also hit the right side into side wall. No air bags deployed even though Windshield and all door glass on sides shattered. My wife the driver was okay and not hurt, and the grandchild in the car seat behind my wife was fine also. Neither had injuries. There were checked by emt. The police looked but all would not try to get in except to remove my granddaughter. My wife was removed also because she could not get out the door was jammed against shed and rock, and handles broken off This happened after the recall work was done by toyota dealer on 12/04/2025. There were no problems prior to the recall. I've listed the recall below. 25TD01 Rear Traction Motor Cable Corrosion - Multiple Model & Model Year Toyota Hybrid (AWD) Vehicles NHTSA Recall ID AWAITING# Recall Date Dealer Reference ID 25TD01 Description The subject vehicles are equipped with a rear motor unit and an HV Floor Under Wire that provide high voltage to operate the motor.
