It was raining, when I was driving on I95 highway, the vehicle suddenly started to spin by itself, and out of my control. When I tried to hit the brakes, there was no response, then it swung out of the lane and hit on the tree. I have reported to police, and I just filed a claim with insurance company, and I will also report this to Tesla later. However, I think it is really very dangerous, and I hope it can be investigated for safety reason. I have video recordings, I can provide if you need them. Thank you.

After about 2 minutes from leaving a patients house, I was driving at approximately 25mph when my frunk(front trunk) of my Tesla flew open and hit my windshield. There was no alert through the Tesla app or in the car itself. I stopped the car stepped outside and the frunk would not close after when I tried to close by hand, it would stop at the latch. When I closed it from the Tesla app it was able to close properly

Whenever the self driving feature is disengaged, a pop up menu is forced into the user asking for a reason for the disengagement. The user is forced to submit a response for the pop up menu to go away. This menu shifts sections of the screen around and can cause distractions due to the user looking for specific information that is typically there but is not due to the pop up menu.

The steering is not functioning properly. This is sealed system and no where else I can get it. This a safety issue and I haven't seen any car steering failing like this. Even cars as old as 20 year has no such issues. This is huge slap on of $3k+ on consumer. The cars is hardly 5 year old.

The right rear aluminum rim on my 2026 Tesla Model Y (Juniper trim) has a crack in it, causing a slow air leak. I discovered this issue approximately 2 weeks ago when I noticed the tire was repeatedly losing pressure. I have had to refill air in the tire every 2-3 days. The crack is located on the rim itself, at the sidewall area between the rim and the tire, where air bubbles can be observed escaping when submerged or sprayed with soapy water. I have no recollection of hitting any potholes or curbs that would have caused this damage. The rim appears to be defective from the factory. The vehicle component (rim) is available for inspection. No crash, fire, or injuries have occurred at this time, but continued driving poses a safety risk due to the potential for a sudden tire blowout. Photos and a short video documenting the crack and escaping air are available.

* What component or system failed or malfunctioned, and is it available for inspection upon request? FSD. It is available upon request. * How was your safety or the safety of others put at risk? Over the course of the last two months since the last FSD update (currently on v12.6.4), at least 20 times the vehicle has not properly stayed in the lane under certain circumstances listed below. These circumstances are always the same and it is repeatable and dangerous as the vehicle would have crashed into the center median if I had not intervened. Repeatable conditions: * 79 MPH * Low curvature highway at highway speeds when turning to the right only. * FSD version 12.6.4 with software version 2026.14.6 is engaged and in Assertive mode. * Has the problem been reproduced or confirmed by a dealer or independent service center? Tesla wanted $1k in order to reproduce and diagnose the problem, so I declined. * Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Same as above; Tesla wanted $1k in order to reproduce and diagnose the problem, so I declined. * Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? None of the safety systems fired, including lane departure warning or steering correction. FSD would not have veered out of the way of the median.

Using Tesla FSD 14.3.3. It began raining hard, causing standing water to pool on the road. The standing water was making the car lose traction and started sliding within the lane, it did change its speed or driving to account for the rain. FSD was doing over the speed limit and would not slow down slower than the speed limit. Changing the Tesla speed profile from Standard to Sloth slowed it down to exactly the speed limit but no lower. At this point the car could not maintain its lane, I feared for our safety, so I had to disengage FSD to regain control of the car. Tesla have removed all methods of allowing the driver to maintain accurate speed control of the car and it often ignores speed limit signs, which means it is traveling too slow or too fast for the currently posted speed limit.

2021 Tesla Model Y suffered a complete failure of the front lower lateral links, compliance links, and upper control arms at only 29,103 miles. The vehicle has been inspected by the manufacturer. It is currently being repaired at my cost. There was no warning lights or messages. I had noticed a creaking sound upon turning, especially while in reverse. I believe this mirrors the exact mechanical failures seen in NHTSA Campaign 21V835000. However, my VIN number was not listed in this recall.

Backed-out LH Rear Lower Fore Link bolt causing structural elongation damage to the rear subframe mounting hole, requiring full subframe replacement. Vehicle had been serviced exclusively by Tesla. Tesla issued Service Bulletin SB-25-31-004 on April 17, 2026 covering this exact condition. Tesla declined to perform the repair under the bulletin citing out-of-warranty status. A touchscreen alert directing me to service was active at the time of denial.

While driving in FSD the car is ping ponging between the lane. It didn't used to do this but now does it on any road. Highway, city, turns, etc. The oscillation causes me and my passengers to be sick and it looks like a drunk driver on the road.

I was involved in an accident while using Tesla FSD. Driving down the freeway my car hydroplaned and spun out and hit another car. My son who was with me said to me that it seemed the car was going a bit fast for the conditions.

VEHICLE INFORMATION Year: 2024 Make: Tesla Model: Model Y Mileage at time of incident:1,288 Date of incident: XXX Location of incident: [XXX] On XXX, I was operating my 2024 Tesla Model Y in [XXX] when the vehicle experienced a sudden and unexpected malfunction of its Autopilot/autonomous driving system. The vehicle activated in Autopilot mode without my intentional input, causing me to lose control and resulting in a serious crash. Prior to the collision, the vehicle's Autopilot system, engaged without my input and abruptly accelerated. I was seriously injured as a result. The vehicle, a brand-new 2024 Tesla Model Y, was in normal operating condition with no prior mechanical issues. I had not modified the vehicle in any way. Following the crash, Tesla provided data to attorneys regarding the incident which I believe to be inaccurate and inconsistent with my experience. I am requesting that NHTSA: 1. Open a formal investigation into this incident 2. Investigate the accuracy and completeness of data Tesla provides following Autopilot-related crashes 3. Preserve my complaint as part of any ongoing investigation into Tesla Autopilot/FSD systems Was I injured? YES Number of injuries: 2 Were there fatalities: NO Was there a police report? - #11743978June 14, 2026VISIBILITY/WIPER
This is a safety concern for visibility. How can a brand new, barely used windshield crack like this? There are no rock chips and it has barely been driven to fall as wear and tear on the vehicle. This is not okay and incredibly unexceptional for vehicle standards. This vehicle spends 90% of its time in a garage. I have contacted my insurance company.

- #11743977June 14, 2026UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: WARNINGS, VEHICLE SPEED CONTROL
Component/Issue Description: Following a recent over-the-air (OTA) software update to the vehicle's Full Self-Driving system, a dangerous user interface defect was introduced. Immediately upon an FSD disengagement—the critical transition window where the operator must resume manual driving control—the center touchscreen populates a mandatory, invasive feedback popup menu. This menu causes a severe driver distraction and creates an immediate safety hazard. The popup blocks vital driving visualizations and navigation data on the screen at a high-stress moment. Furthermore, the UI creates a functional touchscreen lockout, demanding manual and visual interaction from the driver to clear the box while the vehicle is actively in motion. Forcing a driver to divert their eyes from the roadway to navigate a touchscreen menu immediately following a system intervention creates an unreasonable and systemic risk of collision. The manufacturer has provided no setting to disable this dangerous distraction.

- #11743816June 12, 2026VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
The Tesla Autosteer (equivalent of adaptive cruise control) failed to brake on seeing the car ahead braking. In the 5 seconds before impact, the car ahead had started braking. The autosteer did not brake for the first 3 seconds and then It asked me to take over 2 seconds before collision. It did not apply automatic emergency braking causing me to rear end the car ahead of me on the freeway at around 30mph This is supposed to be a safety driver assist feature which instead was the primary cause of the accident. The take control was too late to avoid collision. This is a serious safety issue.

We were trying to drive home and the middle seat belt, where my youngest child was sitting, would not latch. it would 'click' in, but if you pulled on it, it would pop out. This was terrifying because we had to drive home on a 2-lane highway with oncoming traffic. I checked online and found that many others have reported this issue because the seat belt latch sits rigidly upright in a divot in the seat, which acts as a funnel to direct any crumbs or dropped items to go into the seat belt latch, which then apparently causes the issue. This is a death trap design for children, because of course children drop things and make messes all the time, and applies to all three of the seat belt latches in the back. What is even worse about this issue is that as a parent I listen for the Click to know they have buckled, and I also check the seat belt indicator on the dash screen. Even when this model Y seatbelt latch isn't going to hold the seatbelt in place, it makes the Click sound and also shows as latched on the dash screen, giving parents a false sense of security. It looks like no children have been injured or killed yet because of this design flaw, but please do not wait for that to happen. Please make them fix this issue immediately before tragedy strikes a family. Please watch the video I linked to in the PDF to see how the seatbelt clicks into place, but then can be pulled free by a firm tug on the seatbelt.

Model Y (VIN: [XXX]). On XXX, I brought my vehicle to Tesla Service for replacement of the front wheel hub assembly and paid approximately $1,000 for the repair. Shortly after picking up the vehicle, I noticed abnormal knocking and clicking noises while turning the steering wheel. Because I was concerned about the safety of the vehicle, I sought a second opinion from an independent repair facility. On XXX, the independent shop inspected the vehicle and found that two wheel hub mounting bolts had not been properly tightened and that one mounting bolt was completely missing. The repair invoice states that the vehicle was being operated in an unsafe condition and that continued operation could have resulted in wheel hub failure and a serious accident. The shop corrected the condition by replacing the missing bolt and properly tightening the wheel hub assembly. I incurred an additional expense of $284.47, which would not have been necessary had the original repair been completed properly.
