On XXX, at approximately 12:30 PM, I was driving eastbound on XXX through [XXX]. The weather was mildly rainy, and I was traveling at approximately 65–70 mph. Without any input on my part, the vehicle suddenly steered sharply to the left, causing it to strike the center divider. The vehicle then crossed multiple lanes of traffic, slid toward the shoulder, and impacted barriers a second time. The collision resulted in the vehicle being declared a total loss, and all airbags deployed. Two days later, after reviewing the vehicle recordings, I observed that immediately after water splashed up from the vehicle in front of me, the Tesla appeared to make a sudden steering maneuver to the left as if attempting to avoid an object or hazard. Based on my review of the footage, I did not observe an actual obstacle in the roadway that would have required such an evasive action. As a result, I am concerned that the vehicle may have incorrectly detected a hazard and initiated an unintended steering maneuver. While I cannot determine the exact cause, the behavior appears consistent with a false or erroneous perception by the vehicle's automated driving or driver-assistance system. I have a 2024 Tesla Model Y and while parked, the rear windshield spontaneously shattered while defrosting my window. My kids were sitting in the back seat when the incident happened. This has not been confirmed by a dealer or service center but me, my wife and brother were standing outside our car when it happened and saw no signs or external impact or object. The vehicle has not been inspected by any external parties at this time. The vehicle presented no warning lamps, messages or other symptoms prior to the incident.

I am submitting this report based on my personal experience operating a Tesla Model Y equipped with Full Self-Driving (FSD). I have used FSD for approximately four years and have extensive experience with the system. During several recent road trips through heavily forested and mountainous areas, including Yosemite National Park and multiple locations throughout Oregon, I repeatedly observed similar unexpected behavior under the same environmental conditions. On roads where dense tree cover creates alternating bands of sunlight and shadow across the pavement, the vehicle occasionally performs abrupt deceleration, unnecessary braking, or jerky speed adjustments despite there being no visible hazards, obstacles, pedestrians, or animals in the roadway. I cannot determine the technical cause, but based on repeated observations at different locations, it appears that rapidly changing lighting conditions or high-contrast tree shadows may present a perception challenge for the vision-based driving system. This is only my observation and not a technical conclusion. My concern is that unexpected braking or sudden speed changes could surprise following drivers and increase the risk of rear-end collisions, particularly on two-lane highways or winding mountain roads. While I remained attentive and was able to supervise the vehicle, these unexpected reactions occurred multiple times under similar conditions. I respectfully request that NHTSA evaluate this type of real-world driving environment as part of its ongoing assessment of advanced driver assistance and automated driving systems. Forested roads and mountain highways are common throughout the United States, and ensuring reliable performance in these challenging lighting conditions is important before wider deployment of autonomous driving technology. I would be happy to provide additional information if needed.

"I have a Tesla Model Y that experienced what looks like a steering malfunction, with documented data and repair records already in hand. "While driving our 2024 Tesla Model Y when the steering wheel suddenly turned itself, hard left, and driver couldn't regain control. The car went up a hillside, between a utility/ electricity pole and an electrical box, coming to a stop just inches from the pole. No injury, but real vehicle damage." "I pulled Tesla's own vehicle data log for that exact moment. It shows the steering angle went from +362° to -358° — basically two full wheel rotations — in about 1.5 seconds, while the torque sensor barely registered any force. Then the wheel stayed pinned at full-left lock for about 8 seconds while the car was actively accelerating in Drive." There was a fault code 14 mention in the data file right before the incident. "Tesla told us their review of the data showed 'no malfunction.' But afterward, Tesla's own repair records show they replaced the steering gear assembly and the drive unit — after we complained the steering felt stiff once we got the car back from the first repair."

Safety features on vehicle stopped working after software update was pushed to my vehicle. GPS, all cameras including backup camera and ones used for blind spots, and cruise control have all stopped working. Time displays incorrectly due to GPS issue and night mode stays on so rear view & back window stay dark making it hard to see other vehicles around me during the day. I’ve followed all recommended steps to resolve and issue persists.

I had gotten the free trial of FSD (full self driving) from Tesla in 2024. I used it with a friend to take us to the zoo. It drove there perfectly with no issues & even backed itself in the parking spot. When time to leave, my friend suggested using the “summon” mode to have the car come to us- it was in view, maybe 10 or so yards away from the curb where we were. I held down the button to summon, the lights turned on & it began making the noise evs make when reversing to alert others that it was on the move. It started slowly creeping out of the spot, maybe a foot or two when it suddenly turned the wheel all the way to the left and accelerated, crashing into the parked car next to it. We were in a bit of a shock, as it was so slow- but it happened so fast? I didn’t even have time to take my finger off the button that made it move/ stop. It did everything in a split second. I have not used the feature since, and when I brought it up to a Tesla engineer they sort of downplayed it & said it was my fault for not properly supervising the vehicle. However, there was really nothing that could have been done in that situation to prevent that. At no point did my friend or I think the vehicle would randomly accelerate into the car next to it, and I would ask how a human would be able to predict that? The more I see incidents of faulty FSD online, the more I believe this incident should be investigated. Thank you for your time If you reach out regarding this case, I know the date of this incident was April 3, 2024 between window of 4-5pm

Power steering is giving difficulty while driving on the highway, stiffens up and hardens and makes it difficult to steer to the left or the right. Took it to the dealer, they confirmed needed a new steering rack, checked online quite some people with the same issue especially with their cars coming from the same year. No warning lamps doesn’t show up on the screen, took to Tesla dealership and they ran a diagnostics and then confirmed.

FSD / vision system , no warnings , the vehicle is turning left and right on a straight road sunny conditions. I have a video that it wouldn’t let me attach to the case. I had Tesla service review this and they told me it’s standard behavior, this never occurred before and is dangerous and unusable. Autopilot has an issue that Tesla themself can’t detect and when I get into an accident it’ll Be too late. I’ve been complaining about this for 3 months and Tesla keeps telling me it’s standard behavior, this never happened before the car used to drive straight, brand new tires front and back , blanket and aligned the car drives straight while not in autopilot, issue is directly related to FSD and Tesla autopilot.

While driving on the highway with Tesla FSD engaged, the vehicle activated the right turn signal and began steering right as if taking an exit. There was no exit ramp, gore area, or valid right-turn path at that location. I took manual control to prevent the vehicle from leaving the highway/entering the shoulder. I believe this could have caused a crash if I had not intervened. I have dashcam footage of the event.

Steering wheel vibration occurs at highway speeds between 70 and 75 mph on a 2026 Model Y with under 1,000 miles at time of complaint. The vibration was reported to the dealer, who performed a tire balance and alignment check and confirmed both were within specification. The vehicle was tested by the dealer during a service ride-along and no vibration was detected by the technician. However, vibration was independently confirmed by the owner in a same-model loaner vehicle on the same road, suggesting the issue may not be isolated to a single vehicle or tire set. Similar vibration issues have been reported by other owners of the same model year in online owner forums, with no documented fix currently available. Safety concern: reduced steering feedback at highway speeds. The vehicle remains available for inspection upon request.

Yesterday evening, my wife and I had friends over to our house, and we were excited to show them our new Tesla. We offered to take them on a short drive around town using the Full Self-Driving (Supervised) feature. At approximately 8:00 PM, we entered the address of our local Sam's Club as the destination. As the vehicle approached Sam's Club, it entered the parking lot through the gas station entrance, which appeared to be the expected route. However, instead of proceeding to a parking space, the vehicle unexpectedly continued driving and attempted to make another loop toward the gas station entrance. At that moment, an SUV was exiting the gas station. The Tesla steered toward the SUV and then continued toward a brick wall where there was no roadway. It appeared the vehicle was attempting to drive into an area that was not a valid path. I immediately disengaged Full Self-Driving, took manual control of the vehicle, and reversed to avoid what could have been a collision with both the SUV and the brick wall. The occupants of the other vehicles were understandably upset because the situation appeared unsafe. This was a very dangerous situation that placed my wife, our friends, and me at risk. Had I not intervened immediately, the incident could have resulted in a collision causing property damage or personal injury. I am reporting this event so your engineering team can investigate the behavior and improve the Full Self-Driving software. I hope this information helps identify and correct this issue to improve the safety of the system for all Tesla owners and the public.

My 2023 Tesla Model Y experienced a serious front suspension/control-arm fastener failure while I was driving on a road trip from Phoenix, AZ to Los Angeles, CA. The incident occurred in Riverside, CA. The vehicle has approximately 40,000 miles and is still within Tesla’s Basic Vehicle Limited Warranty. One front A-arm/control-arm bolt came completely out, and another related front suspension bolt was found loose. The tire and wheel are not damaged and are not flat. The vehicle became unsafe and undrivable. Tesla’s app initially categorized the issue as a flat tire, but this is not a tire issue. It is a front suspension fastener failure that could have caused loss of control, especially at highway speed. I am aware Tesla previously issued Service Bulletin SB-22-31-002, also associated with NHTSA Recall Campaign 22V895000, involving certain Model Y vehicles where front suspension lateral link fasteners may not have been properly attached. Tesla reportedly limited that campaign to a small affected population, and my VIN was not included. However, my vehicle appears to have suffered a very similar front suspension fastener failure. I am asking NHTSA to review whether this issue may affect vehicles beyond the originally identified population. Tesla should inspect the front suspension fasteners, control arm/lateral link mounting points, torque condition, threads, steering components, and alignment, and should determine whether the failure was caused by improper torque, defective fastener retention, assembly issue, or another safety defect.

I was driving down a city street near my home and put my Tesla Model Y in Full Self Driving and looked down for 1-2 seconds to retrieve something from my center console and the warning notifications started going off and before I had any time to react, the car drove into a parked car at approximately 39 mph. I have records showing the car was in Full Self Driving for 5 seconds before impact.

My Tesla has repeatedly exhibited dangerous behavior while Full Self-Driving (FSD) is engaged. On multiple occasions, the vehicle has stopped for a red light and then proceeded through the intersection while the traffic signal was still red. This has put my family and others at risk on a public roadway and creates a serious safety concern. One documented incident occurred on June 21, 2026, while my family was in the vehicle. During that event, the car stopped at a red traffic light and then proceeded to attempt to run intersection even though the light remained red. This was alarming and dangerous, and it required close driver supervision because the system did not behave in a safe or reliable manner. This was not an isolated incident. My Tesla has run red lights in this same manner on multiple occasions while FSD was engaged. In addition, the vehicle will suddenly stomp on its brakes on a public roadway, which is another serious and unpredictable safety issue. I have taken the vehicle to Tesla regarding these issues, and I was told that this behavior is considered normal and that nothing would be done about it. Tesla’s response was that the software is still being developed, and that this is essentially part of the risk of using the software. These repeated failures involve traffic signal compliance and erratic vehicle behavior that could easily result in a crash or injury. My family has been placed in danger by these events, and I believe this is a serious safety defect that needs to be investigated immediately. I am requesting that NHTSA investigate Tesla’s FSD system for repeatedly stopping at red lights and then proceeding through the intersection while the signal is still red, as well as for sudden unnecessary braking events on public roadways.

On Saturday, [XXX] at around [XXX] going west on [XXX] toward [XXX] stopped at the intersection of [XXX]. While driving my 2022 Tesla Model Y, Performance, I was stopped at a red light. When the light turned green and I began accelerating normally through the intersection, both passenger-side doors unexpectedly unlatched at the same time. The windows on both passenger doors lowered slightly, and the vehicle displayed a warning indicating the doors were open and to drive carefully. This created an immediate safety hazard, as the doors opened while the vehicle was in motion without any intentional action from the occupants(there were 0 occupants there). This is the second time this exact issue has occurred under similar circumstances, with the previous incident also happening as I accelerated from a red light. The vehicle should not allow multiple doors to unlatch while driving. I have reported the issue to Tesla for inspection and request that this incident be documented and investigated as a potential safety defect. I was going less that 10 mph when this occurred. Thankfully, my toddler was not sitting in the back seat and there were no passengers in the car. No injuries. This week, I was driving my Tesla at approximately 65 mph in the middle lane of a six-lane freeway. Autopilot was engaged. Traffic was moving normally, and I was not aware of any obstacle or hazard that would have required emergency braking. Without any warning, the vehicle suddenly applied the brakes and rapidly decelerated until it came to a complete stop in the middle travel lane of the freeway. I was not pressing the brake pedal. The event occurred unexpectedly and created a potentially life threatening safety hazard. my vehicle came to a stop in a high-speed traffic center lane! After I disengaged Autopilot and pressed the accelerator, the vehicle responded normally. Immediately following the incident, the display showed the message “Autopilot unavailable.” That message later disappeared, and no additional warning messages remained on the screen. I contacted Tesla to report the incident. Tesla reviewed the vehicle’s logs remotely and later discussed the results with me by telephone. During that conversation, I was told that the event was considered an “anomaly.” I was also told that the vehicle logs reflected the event approximately one hour from when it actually occurred and that the logs indicated the vehicle was not operating in Autopilot at the time of the incident. This information was inconsistent with the event, because Autopilot was engaged when the sudden braking occurred. I could not even move to berm. Tesla did not request that I bring the vehicle in for a physical inspection. Instead, I was told there was no reason to have the vehicle examined. During the same conversation, I was encouraged to consider subscribing to Tesla’s Full Self-Driving (Supervised) system, which requires an additional monthly subscription, rather than continuing to use autopilot as a result of a question I asked about two plans. Tesla closed my service ticket without performing an inspection indicating there was no benefit spending $$ to essentially receive no additional
