A 2023 Tesla Model Y experienced a sudden electrical failure while parked/being prepared for use. Multiple warnings appeared on May 26, 2026, including high-voltage system service required, low-voltage electrical system power reduced/vehicle shutting down, cabin climate control limited, and steering/braking/parking brake functions degraded. The vehicle could not be driven normally and required flatbed recovery. The failure created a safety concern because the vehicle showed high-voltage and low-voltage faults together with steering/braking/parking brake warnings and could have left the vehicle disabled without reliable restart or parking brake function. The manufacturer service center inspected and repaired the vehicle after several days. The component should be available through manufacturer service records, and screenshots of the warnings, tow/recovery, and service records are available.

On XXX, at approximately 5:00 PM, my daughter was operating our 2024 Tesla Model Y on [XXX] near the [XXX] exit in [XXX]. The vehicle suddenly became uncontrollable. According to the driver, the steering wheel appeared to move on its own, the vehicle accelerated unexpectedly, braking was ineffective, and the vehicle zig-zagged across multiple lanes of traffic from the far left lane to the far right lane. The vehicle reportedly performed approximately half of a 360-degree spin before control was regained. Several motorists reportedly pulled over after observing the vehicle’s erratic behavior. Tesla Service later inspected the vehicle and advised that no fault was found and that the vehicle was operating within factory specifications. However, no explanation was provided for the loss-of-control event. Due to the seriousness of the incident and the risk of severe injury or death, I am requesting that NHTSA investigate this matter as a potential vehicle safety defect. Vehicle: 2024 Tesla Model Y Date of Incident: XXX Location: Southbound XXX Express Lanes near the [XXX]/[XXX] exit split in [XXX]. At the time of the incident, Tesla Full Self-Driving (FSD) Supervised was engaged. I was traveling in the Express Lanes and intended to remain in the Express Lanes. I was not attempting to take any exit. As the roadway approached a lane split/gore area, the vehicle began steering toward the striped boom barriers that prevent drivers from going into oncoming traffic, rather than remaining in the travel lane. There were not impending impact alerts or lane deviation alerts. There were no unusual weather conditions, and I did not observe any road conditions that would have made the lane path unclear to a human driver. As soon as I saw the vehicle veering into the boom barriers, I intervened by taking control of the steering wheel. The system disengagement alert/chime occurred only after I took over. Despite my intervention, the vehicle made contact with the boom barrier assembly. The impact caused significant damage to the windshield and driver's side door frame. Glass entered the passenger compartment. My infant child was in the vehicle at the time. Fortunately, no injuries were sustained. Tesla Dashcam footage of the incident was preserved and is available. A claim was opened with Tesla Insurance, and I was advised to submit a vehicle malfunction report for review of vehicle telemetry and FSD operation during the incident. After the collision I maneuvered the vehicle back into its lane, exited the toll highway and parked at a nearby shopping center to assess our injuries the vehicle. While there, another Tesla parked near me, and the driver had experience the same exact situation while his Tesla was in autodrive. I am requesting investigation into whether the vehicle's driver-assistance system improperly tracked toward the boom barriers rather than remaining within the intended travel lane. I was driving my Tesla in self drive mode, and ibecause it started raining. I reduce the speed to standard. I had a notification on the screen, an alert in red. That the tesla reached the maximum speed in that mode “standard”. I was surprised and decided ti canceled the FSD feature and took control of the vehicle. Then when I was driving in the expressway very close to my exit on the right lane, the car accelerated by itself and turn to the left unexpectedly. I tried to returned the car to my lane and I never had control again of the car. Not the steering wheel, not the breaks responded. I was spinning on the expressway until I hit the concrete barrier with the rear bumper. Hopefully I did not hit any car nor they didn’t hit me. The car shutdown after the impact. Tesla sent me the footage of the impact, ignoring the moment that caused that incident that was some seconds before. The brakes were automatically turned onz and the wheels did not move. I know something wrong happened. Specially because the incident some minutes before when I was driving in standard mode and accelerated with no reasons. I have safety concerns. Also I noticed that after the last upgrades the car was havung navigation issues that I reported immediately when the autopilot was removed and we have to select the options.

- #11743088June 9, 2026VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I am submitting this complaint to request an investigation into a serious safety defect involving my Tesla vehicle. On multiple occasions, the vehicle has experienced a significant and unexpected loss of acceleration when I attempted to enter or cross intersections. Despite pressing the accelerator, the vehicle failed to respond as expected, leaving me unable to accelerate normally while entering active traffic. This is not a minor hesitation or inconvenience. It is a serious roadway safety hazard. Drivers rely on immediate acceleration when crossing intersections, turning across traffic, or merging. A vehicle that unexpectedly limits acceleration can place occupants directly in the path of oncoming vehicles, creating a substantial risk of a severe collision, serious injury, or death. I reported this issue to Tesla Service and provided multiple timestamps documenting the events. Tesla was unable to identify any fault codes or provide a diagnosis and indicated that because no fault was recorded, the issue could not be confirmed. Through independent research, I found numerous reports from other Tesla owners describing substantially similar acceleration limitation events. Many reports specifically reference Obstacle Aware Acceleration as a possible cause, and some owners report that disabling the feature resolved the issue. If a safety system can falsely detect an obstacle and reduce acceleration while a driver is actively entering an intersection, the system itself may create a greater danger than the one it is designed to prevent. I respectfully request that NHTSA investigate whether Tesla vehicles are experiencing unexpected acceleration limitation or torque reduction during intersection entry and traffic merging situations, evaluate the role of Obstacle Aware Acceleration and related software systems, determine whether a broader pattern exists, and assess whether corrective action or a recall is warranted. This could cause serious injury or fatal crash

The heat pump failed after experiencing temperatures below 10 degrees fahrenheit. There was no warning until after the failure, when I received a notification that the climate system needed maintenance. All heating/cooling, including defrosting/defogging the windshield, is inoperable. The service center confirmed that this is a heat pump failure and requires a replacement of the entire system, and a Tesla employee indicated that this is a common issue in the first 6-9 months of this cooling system design. This is well documented across many forums, and there is a class-action lawsuit in Quebec, Canada related to this failure.

On 05/31/2026, while Tesla Full Self-Driving (FSD) was actively engaged, the vehicle was taking the La Brea Avenue entrance ramp onto the I-10 freeway in Los Angeles, CA. FSD suddenly accelerated aggressively and the vehicle drifted hard to the left without any driver steering input. The driver was forced to hit the brakes and manually override steering to prevent a serious accident. The behavior felt like a system glitch. Both driver-side wheels sustained severe impact damage and are confirmed cracked. Tesla Service Center confirmed the damage in writing on 06/08/2026 and quoted $2,325.74 in repairs. This dangerous FSD behavior matches the pattern currently under active NHTSA engineering analysis.

Tesla recently pushed a software update that introduces a major, unavoidable driver distraction right at the most critical moment of driving: the exact second you take manual control back from Full Self-Driving (FSD). Whenever you disengage FSD, a mandatory, multiple-choice survey pops up on the center touchscreen asking why you took over. Unlike previous versions where this would disappear after a few seconds, it now stays on the screen permanently. There is no close button, and it does not time out. This causes two clear safety hazards while driving: 1 It blocks the screen: The popup actively covers up part of the navigation map and basic screen controls. If you are trying to navigate a tricky intersection or see your route right after taking over, you can't see the map clearly because this giant survey is blocking it. 2 It forces you to look away from the road: Because the prompt never goes away on its own, it forces the driver to physically look down, read the small text options, and tap the screen to clear it. Forcing a driver to do a multiple-choice survey while actively managing a manual takeover in traffic is incredibly dangerous. Tesla should not be allowed to use a mandatory, permanent UI popup to collect data while a vehicle is actively moving. It is a severe visual and cognitive distraction at the exact moment a driver needs to be 100% focused on the road. Please investigate this software design and require Tesla to add an automatic timeout or a quick-dismiss button that doesn't require driver interaction.

I am writing to formally report a serious safety failure involving Tesla’s Full Self-Driving (Supervised) feature, which resulted in property damage and a severe safety hazard. While FSD (Supervised) was fully engaged, the vehicle failed to detect a large block of wood in the roadway. The vehicle struck the object, causing significant damage to the brand-new car's left-side tire and body, t touched two other cars hence I called 911 and reported it to ensure they remove the wood. Because I did not know how to manually trigger the dashcam save at the exact moment of the crash, video footage was not captured locally. However, the vehicle’s internal sensors, and Event Data Recorder (EDR) should have logged the active FSD status and sensor data at the time of impact and potentially save it.Immediately following the incident, I contacted emergency services via 911 to report the roadway hazard. I have since visited a Tesla Service Center, where staff informed me that I must wait for the formal complaint process to proceed.Due to recent publicly reported fatalities involving FSD, I strongly believe this software is not safe for public roads, and I have chosen not to renew the feature. I am requesting a comprehensive corporate review of my case. I expect Tesla to pull the vehicle's internal logs, take accountability for this hardware and software failure, provide a full repair of the vehicle, costs about $2000 as estimated by their collision center and issue a complete reimbursement for the damages.

I purchased a new 2026 Tesla Model Y Standard and took delivery on approximately 30 May 2026. Within the first week of ownership, the vehicle displayed multiple safety-related warnings, including: * Steering Assist Reduced * Power Braking Assist Reduced * Automatic Emergency Braking Unavailable * Lane Departure Avoidance Features Unavailable * Automatic Vehicle Hold Disabled The warnings have occurred more than once. They initially disappeared and then returned on a later drive. Both incidents occurred while the vehicle was operating with Full Self-Driving (FSD) engaged. During the most recent occurrence, while parking at less than 3 mph, the vehicle’s braking assistance appeared reduced and the brake pedal required significantly more effort than normal. As a result, I nearly struck a curb because the vehicle did not slow as expected. The warnings indicated that increased steering and braking effort may be required. Because the vehicle is brand new and these warnings involve steering, braking, and collision-avoidance systems, I believe this creates a significant safety risk. I no longer feel the vehicle is safe to drive until it is inspected and repaired. Tesla has been notified and a service request has been initiated.

While operating a 2026 Tesla Model Y on Full Self-Driving (FSD) Supervised, the vehicle's vision-based lane-keeping system failed on an unmarked residential road. At approximately 26 MPH, the vehicle approached a blind crest of a hill. Due to the lack of painted center lines, the FSD software miscalculated the road geometry and drifted significantly into the center/oncoming lane exactly as it reached the top of the hill. This lane departure created an immediate, severe safety hazard, as it placed the vehicle directly in the path of potential oncoming traffic with zero forward visibility over the crest. Due to the critical lane positioning error, immediate driver intervention was required. The driver took manual control of the steering wheel to force the vehicle back into the correct lane, causing the FSD system to instantly disengage. Simultaneously, the driver applied manual braking to reduce speed drastically down to 10 MPH to safely clear a resident working on the right shoulder and stabilize the vehicle. Prior to the failure, there were no warning lamps, error messages, or symptoms indicating the system would fail to maintain proper lane positioning. The system appeared to be functioning normally up until the crest of the hill. The vehicle and its Autopilot/FSD components have not yet been inspected by Tesla, police, or an independent service center following this specific event. The vehicle remains available for inspection upon request

A 2021 Tesla Model Y experienced steering/suspension warnings followed shortly by a steering-assist reduction event. On or around May 21, 2026, the vehicle displayed: “Steering alignment service recommended — OK to drive, schedule service soon.” The vehicle was brought to Tesla because the owner was concerned the car had detected a steering, alignment, suspension, or safety-related condition. The Tesla invoice from that visit appears inconsistent. It states that “a wheel alignment check was completed,” but also states “No Labor Performed — Customer Did Not Authorize Repairs or Inspection,” with 0.00 labor hours and no charge. A documented steering/suspension diagnostic inspection, alignment report, alert-log review, or safety assessment was not performed by Tesla or inspected. The owner was advised to replace tires and return later. Approximately 90 miles later, during a low-speed three-point turn, the vehicle made a loud pop/clunk from the front area and displayed: “Steering assist reduced — Steering may require increased effort.” A related alert also stated Lane Departure Avoidance features were unavailable. This created a safety concern because steering effort may unexpectedly increase during low-speed turns or parking maneuvers. Tesla later inspected the vehicle and documented torn front lower lateral/compliance link bushings and vehicle log data showing steering gear alerts. Tesla replaced the front lower lateral links and front lower compliance links and performed alignment work. After those repairs, Tesla stated a steering gear alert was still present and recommended steering gear/steering rack replacement with additional alignment work. The concern is that a steering/alignment warning occurred shortly before the physical front-end failure and reduced steering-assist event, but the earlier Tesla visit does not clearly document that the warning was meaningfully inspected or safety-assessed. Vehicle is currently unsafe to drive and is in Teslas possession

On XXX, at approximately 8:01:49 AM Eastern Time, my 2024 Tesla Model Y (VIN: [XXX]) was operating under Full Self-Driving (FSD) software version 14.3.3 when the system directed the vehicle into the outer wall of my residential garage without driver input. FSD was actively engaged at the time of the incident. The vehicle sustained significant damage to the front bumper fascia and wheel well trim. Additional damage includes a cracked front fog light/corner lens assembly with surrounding fascia stress, also caused by the same incident. The incident also caused paint transfer damage to the garage wall trim. I have preserved dashcam footage documenting the incident, which is consistent with FSD-directed vehicle behavior. I am referencing NHTSA Preliminary Evaluation PE25012 as this incident is consistent with the pattern of uninstructed FSD-directed maneuvers currently under federal investigation. The vehicle’s secondary Autopilot Processor / car computer is internally faulted and stuck in a continuous wake/shutdown loop. This causes severe standby battery drain (~20 miles of range lost per day while parked) and completely prevents all over-the-air software updates - including federally mandated safety recall remedies. On April 4, 2024, Tesla Service diagnosed the fault and linked it to the drain and inability to update. On the same visit, Tesla added two safety recalls (23V-838 Autosteer controls and 24V-051 Brake/Park/ABS indicators) to the work order. They then demanded ~$3,000 customer payment for computer replacement before the recalls could be performed. After I declined the paid repair, Tesla marked both recalls as “completed” via software update despite the same invoice stating the vehicle could not receive updates and remained on software version 2023.38.9 (which predates both remedy firmwares they claimed was installed). Tesla is effectively paywalling federally required safety recall remedies by requiring the customer to first pay for computer replacement first. Two additional recalls remain open (21V-389 seat belt fasteners and 24V-554 hood latch). The problem has been confirmed and documented multiple times by Tesla Service.

On the evening of June 3, 2026, while driving on the I-575 Express Lanes in Cobb County, Georgia, the vehicle’s autopilot/driver assistance system malfunctioned and steered the vehicle into the express lane boom barrier. The boom bar struck the windshield, causing catastrophic windshield failure with glass imploding into the cabin. Glass shards covered the driver’s seat, floor, and dashboard. The driver sustained glass puncture wounds to both forearms and forehead, as well as neck, back, and right arm soft tissue injuries. Component failed: Autopilot/ADAS system. Vehicle is available for inspection. Tesla was notified directly at time of incident. Insurance company has been notified. SR-13 filed with Cobb County. No warning lamps or messages appeared prior to the autopilot malfunction. The root cause of the windshield damage was the autopilot system failure, not driver error or external road debris. Photographic documentation of vehicle damage and driver injuries exists.

I am reporting a safety defect in Tesla’s Full Self-Driving (Supervised) system. On XXX, while using FSD, my 2024 Tesla Model Y (VIN: [XXX]) failed to recognize that the road terminated into the desert after a roundabout on the [XXX] road near [XXX]. The vehicle accelerated straight instead of making the sharp 90-degree left turn, left the roadway, traveled down an embankment, and crashed. Airbags deployed and the vehicle was declared a total loss. Vehicle Data Recorder (VDR) data confirms that Autosteer was active and available during the critical braking sequence. I applied very strong sustained braking (brake master cylinder pressure exceeded 129 BAR with ABS activation). The tow truck driver, [XXX], stated at the scene that this was the fifth Tesla he had recovered from this exact same location due to similar crashes. This documented cluster of at least five identical Tesla crashes at one hazardous spot indicates that Tesla had actual notice of a dangerous defect in its FSD mapping and road geometry recognition but failed to correct it. This incident aligns with NHTSA’s ongoing investigations into FSD performance issues (including EA26002). I am also pursuing a private product liability claim for damages. I can provide supporting documents including VDR screenshots, the Wickenburg Fire Department report, and a witness statement.
