- #11537656August 10, 2023VEHICLE SPEED CONTROL, STEERING, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
While traveling on a busy interstate highway, my Tesla 3 presented four errors on the control screen: Automatic Emergency Braking disabled, Power reduced, Vehicle shutting down and Unable to drive. The left hand side of the screen flashed red and instructed me to pull over safely. The car was immediately slowing down, and as I was in heavy traffic, I was unable to pull over to the shoulder safely. I turned on my hazard lights and performed a soft reset of the car - which caused my hazard lights to turn off, all while cars were honking and whizzing around me. After the car reset, I was able to drive a short distance, with enough speed, to cross several lanes of traffic and come to a stop on the shoulder emergency lane of an exit ramp. The car is only 6 months old, with 4000 miles, but had to be towed to the Tesla service center. The problem has been diagnosed as a problem with the rear drive unit inverter and will be replaced under warranty. While I appreciate the resolution and that it will be fixed, it could have been a very serious situation and caused an accident. Thankfully, I was able to get to safety and wait to be towed.

The driver's seat automatically adjusts when I get into the car. Normally this is a good thing, right? But when my wife and I both get in the car, and I'm going to drive, the car often assumes my wife is the driver and adjusts the seats to her settings as I'm getting into the car. This is a serious problem that has the potential to injure me. Since I'm tall, and my wife is short, the seat starts moving forward as I get in, and it starts to crush me. It's too hard to get back out, and I'm not always able to select my seat configuration quickly enough. If I happen to be carrying something as I get into the car, then that object usually is compressed between me and the steering wheel. There have been times where I didn't have my legs in fully, and they started to get crushed by the seat pushing them forward into the steering wheel. I've talked to Tesla about this several times. Tesla employees have told me that they hear about this problem regularly, but they don't have a way to fix it. And when I ask how to contact Tesla, they tell me use Twitter. I think Tesla's service department should have a method to communicate customer complaints directly. No one has given me any feedback indicating that they intend to do anything, even though they acknowledge that this is dangerous. I also wonder if it is possible that this might happen while I'm driving! For example, what would happen if the battery on my phone drained? The bluetooth connection would be lost. Would the car then pair to my wife's phone and readjust the seats? Of course, there is a simple fix. Instead of doing the automatic but stupid and dangerous adjustment, it should just ask for my confirmation. And never automatically go into a mode of "let's crush the driver"! I only want a computer to think for me if it's going to reason properly, in every situation, every time. Clearly Tesla didn't think this one through.

- #11537335August 9, 2023FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: WARNINGS, VEHICLE SPEED CONTROL
I was driving on a rural, two-lane highway with no other cars around and with cruise control engaged. Without warning, the car decelerated suddenly and rapidly from about 60 MPH and continued to decelerate until I pressed on the accelerator pedal (at about 40 to 45 MPH) to stop the deceleration. It happened at least 5 times during a drive from Spring Grove, IL to Champaign, IL on July 7, 2023, between 10:30 AM and 2:00 PM. If there had been a car following close behind me it would have been very dangerous. I had the Basic Autopilot engaged with Traffic-Aware Cruise Control (but not Autosteer). Weather conditions were bright sunlight with a few clouds. The problem has occurred several times (at least 4 times) since then. The problem has not been reproduced or confirmed by a dealer or independent service center. I have not had the vehicle inspected by anyone. There were no warning lamps, messages, or other symptoms of the problem prior to the failures.

I was attempting to park my car at parking spaces inside a garage. The car was about half way into the spot, and I released the accelerator to slow down. Suddenly, while my foot was off the gas, the car accelerated on its own, lurching forward and hitting a barrier, which protected against falling off the side of the garage into the floor below. The car sustained minor damage to the hood, but it could have been far worse; if the car ran through the barrier, it would have fallen one story down to the floor below. I am now way too concerned to drive the car, and have not driven it since. I took the car to a Tesla service stations and they said that they cannot reproduce the same phenomena, and that the car passed their diagnostic check. Currently, this is the only person to inspect the car. During the sudden acceleration incident, the car did not make any warning sounds, which it typically does when driving into a parking spot and approaching a barrier or wall.

I took delivery of a Tesla Model 3 on July 7. On July 18, I received a digital message indicating that the software needed to be updated, and I complied. Later, when I got into the car to drive it, I got two error messages -- one indicating that the driver-assist steering might not work and the other relating to warning of approaching vehicles. I drove the car about 50 feet before realizing that I had no power steering and would not be able to control the car. Using all my weight to turn the steering wheel, I managed to pull over to the righthand shoulder of my street, where I left the car. A few hours later, I tried to drive the car again, and the problem had resolved itself. But I was nervous about driving it without an assurance from Tesla that it would not happen again, especially wile driving it, so I reported it to Tesla. Tesla's "remote team" did an analysis and reported six days later: "After reviewing your concern we have concluded that it can be chalked down to a known firmware issue that appears during the first start after completing the most recent update and goes away after leaving the vehicle and coming back. We are seeing no further issues with the steering assist, and no repair is needed at this time." Meanwhile, I had not driven my new car for six days out of concern that the steering wheel could freeze up while driving. When I search online, I found many, many reports from drivers about this problem, some of whom said they'd had to have their whole steering column replaced. I read this week that NHTSA had opened an inquiry, so I am submitting my experience so you can see the problem is not limited to 12 vehicles. Also, I suggest you do a Google search to amass more evidence of how common it is.

When performing a u-turn, at low speed, the steering wheel locked in position, only allowing the car to go straight. I was able to regain the ability to steer, after approximately 5 seconds. I didn't notice any alerts or warnings, as it was happening. It was in the middle of a busy intersection and I was fortunate I managed to regain control of the vehicle, in time. I have scheduled a service appointment with Tesla. the earliest available appointment is the 20th, in my area. I have since driven the vehicle, approximately 40 to 50 miles, without incident, but I am concerned that this might happen, again.

Was traveling on Interstate 94 near Steele, ND in broad daylight on July 30. The sky was mostly clear and it was not raining. There was no road construction on this stretch of road, . I was westbound in the right lane with no traffic approaching me or behind me: it was a rural roadway with no curves and mostly flat. At 7:39 PM Central time near mile point 198 the car braked without warning very aggressively. I continued traveling with the auto cruise and at 7:40 PM Central time near mile point 194 it again very aggressively braked with nothing but open straight highway with no vehicles, signs, construction or anything else that could have caused this to happen. I continued my journey and it happened several more time until I no longer felt safe using the adaptive cruise. This has happened before and Tesla Service asked me to track the event and report the time when it happened. I reported the above incident to Tesla and they scheduled a repair for August 8. Today they canceled this visit for repair and told me that this was something that occasionally happens with the cruise and that they were canceling my appointment. They told me that if I wanted to drive to the nearest service center that they too would not be able to fix it. I was lucky that this happened in North Dakota on a rural highway. The wheels locked on me and the tires even slightly screeched. I ascertain that had this happened where there would have been someone behind me there would have been an accident. Tesla clearly is not interested in fixing this and told me that if I did not feel safe that I should not use the cruise. I hope you can resolve this before there is a death or serious injury. I have talked to other Tesla Model 3 owners who have experienced this. Thank you for your consideration

Emergency braking applied when no other vehicles were in sight. Braking was hard enough to spill coffee. Seems to occur when adaptive cruise control is on. Happened about 10 times on recent road trip. Lucky no one was behind us! Felt very unsafe.

On August 5, 2023, my car charge got too low for me to make it to a charging station in Lafayette Louisiana. I had the vehicle towed on a flat bed. I put the car in Tow Transportation mode. While we were towing the car, I opened the car to check the battery level and somehow the frunk was activated (perhaps I accidentally hit it, I'm not sure). The frunk opened while we were going about 60 mph down the highway, the wind lifted the trunk up and it crushed my front windshield and the frunk and opening mechanics were damaged. Had we not immediately pulled over the frunk hood could have been ripped off the car by the wind and caused a major accident. This seems to be a flaw in the Tesla electronics/app system - you should not be able to open the Frunk once it is put into Tow Transportation mode.

2023 Tesla Model 3 with 3,000 miles. No accidents or impact with potholes or curbs. Started vehicle, put in drive, pressed accelerator, car went forward but would not turn. Steering wheel was stuck in straight position. I stopped the vehicle and put it in park, same symptom of stuck steering wheel. My wife (passenger) and I did not notice any error lights or messages. I exited the vehicle walked away to trigger automatic locking. Returned to the vehicle, started it , and tested steering. Steering was free. Took approximately 5 mile test drive. Steering was erratic, making small movements to the left and right. At times it felt the car would veer offroad or into oncoming traffic. Contacted support. Dealer dispached tow truck to return vehicle for inspection. Was informed by dealer that no loaner vehicles are available, they only offered Uber credits.

On May 22, 2023 I had an annual physical exam at the doctor's office at 9:00 am. Upon completion of the visit, I drove my model 3 from the parking spot (3rd level) towards the ground floor. At the final turn, at the ground floor, I felt that the car had an unusual resistance when I tried to navigate the turn. The steering wheel felt as if it were locked. As a result of not being able to turn the steering wheel, I hit seven cars that were parked at the ground floor and incurred substantial damage. The car is totaled unfortunately. I am hoping that the NHTSA may have a way to analyze the true cause of this accident before they send the car to the salvage market.

Just completed a 2week trip and the car attempted to stop on its own over 37 times (stopped counting) on the highway. 2times I had a semi within a block of my rear of the car. Since this occurrence is not anticipated it worries me that it's going to cause an accident. Reported to Tesla and it's days the feature is beta, I don't believe that for a second . This my 3td car with adaptive cruise control. I was not using the auto guidence feature.

On 19 July 2023 at approximately 4am in the morning, while with my wife traveling north on I-95, I was driving approximately 77 mph and my 2022 Tesla Model 3 SLAMMED ON THE BRAKES. I had several cars directly behind me that had to slam on brakes and perform other evasive driving maneuvers as well. Luckily my reaction time was quick enough for me to SLAM ON THE ACCELERTOR to avoid a very serious wreck and pile up. This is the 3rd time this has happened but thankfully the first 2 times I did not have traffic directly behind me. Although I paid an extra $6000 for the Enhanced Auto Pilot, I do not use it, as I am now scared to death of this car at highway speeds. On the night/morning of 19 July, I was on just regular cruise control. The very next day (20 July) I applied for an appointment with the local Tesla Service Center here in the Richmond VA area. After a couple days, and I had not taken the car in yet, Tesla sent me a message and informed "that I am NOT to bring the car in." I have not heard from them since then... and today is 5 August 2023. Respectfully request that you weigh in to either get me a Refund for this car, or in some way restore my confidence in it. I am scared to ever put this car on the highway again. THANK YOU.

The contact owns a 2023 Tesla Model 3. The contact stated that while decelerating to make a right turn, the steering wheel became difficult to maneuver as the lane shift avoidance reduced, and power steering assist reduced warning messages displayed. The contact stated that she pulled into a parking lot and parked the vehicle and after 20 minutes restarted the vehicle and resumed to normal functions. The manufacturer Tesla was contacted and advised her to reboot the computer as well as make an appointment with the Tesla service center. The computer was rebooted however several days later while making a left turn the steering wheel was difficult to maneuver with no warning messages displayed. The vehicle was towed to the dealer and the power steering rack was replaced. The failure reoccurred for a third time on July 7th while she was making a right turn with the message displayed steering assist reduced. The vehicle was towed back to the dealer where they replaced the steering rack and updated firmware however the failure reoccurred. The vehicle was towed back to the dealer they diagnosed the failure as a power steering harness that was replaced. The contact was relating the failure to a NHTSA Action Number: PE23014 (Steering) The failure mileage was 480.

When cruise control is activated at highways speeds, vehicle sometimes brakes abruptly for no reason whatsoever. Issue is phantom braking. No other cars in sight, no changes in speed limits. Happened repeatedly on CA Hwy 46 between Interstate 5 and US 101 on 7/31/23 and 8/2/23. Similar instances observed on other highways, too.

My vehicle constantly slows down and occasionally even slams on the brakes to slow down for nonexistent obstacles. I believe this is a serious safety concern if there is a vehicle behind mine, as the driver of that vehicle would not expect mine to rapidly slow down. The root cause seems to be that the camera vision system frequently mistakes certain signs and dark cars reflected in mirages as pedestrians.

I was stopped before turning left into a parking space. I was driving slowly. The car stopped but I did not engage the brake as a Tesla will stop once the foot is removed from the accelerator. At that point the car leaped forward at full speed hitting parts of a building and a fence. I did NOT step on the accelerator. No one was injured. The airbags did not deploy even though there was $17,000 damage to the front of the car. There were no warning signals. The person inside the building was waiting for me and saw the car come to a stop.
