Two separate incidents occurred on July 10th and July 12th 2026 involving adaptive cruise control. While adaptive cruise control was engaged my car slammed on the brakes for no reason. There was nothing in the roadway to cause abrupt braking. The second time it occurred my car was close to being rear ended by a vehicle driving behind me. I took the car which is still under warranty to Tesla who confirmed there is a known firmware issue. No time frame was provided as to when the issue will be fixed.

The horn on the steering wheel rarely works UNLESS it is pressed directly in the middle. I have tried to honk the horn when there is a safety event but it rarely works. This is definitely a safety issue. This has been going on since I bought the car June 2022. Full self drive is also unsafe and it will make sudden turns, accelerate too quickly, or have phantom braking. This can be unsafe for other drivers. I heard that this is improved with newer models, but if it is unsafe for older models like my 2022, it should not be available.

The contact owned a 2022 Tesla Model 3. The contact stated that while the vehicle was unoccupied, the vehicle independently started while parked in the driveway and caught on fire. No warning lights were illuminated prior to the failure. The contact stated that the fire spread to the residence and the residence burned down. The contact's daughter and her significant other were able to exit the residence safely. The local Fire Department arrived on the scene to extinguish the fire. The contact stated that the fire had started near the charging port. A Police Report was not filed. In addition, the contact stated that his daughter and her significant other suffered severe mental distress. Medical attention was not sought. A Fire Report was filed. The local dealer was not contacted. The vehicle was deemed a total loss by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 33,000.

On July 18, 2026, at approximately 10:00 AM EST, my son was manually driving a 2024 Tesla Model 3 at approximately 26 MPH when the steering suddenly became extremely stiff and difficult to turn. He attempted to steer away from a roadside barrier but was unable to turn the steering wheel sufficiently, resulting in a collision. There was no warning or indication observed on the dashboard before the steering suddenly became extremely stiff. Autopilot, cruise control, and Full Self-Driving were not activated; the vehicle was being driven manually. Fortunately, my son did not sustain any physical injuries. However, a sudden loss of normal steering control is a serious safety concern and could potentially cause severe injuries to the driver, passengers, pedestrians, or occupants of other vehicles. After the accident, the vehicle was towed to the insurance-designated facility and is currently being examined by the insurance adjuster. Within a few minutes of the accident, the vehicle also became completely locked, preventing us from retrieving personal belongings. I previously reported this incident to NHTSA under complaint number 11751698, but all required documentation was not available at that time. I am therefore submitting the complaint again with the additional information and documentation now available. I respectfully request that NHTSA investigate this incident and determine the root cause of the sudden steering stiffness and whether there is a safety-related defect or condition that could affect other vehicles.

My car randomly gave me alerts that my steering alignment fault then came the following two days later Automatic Emergency Braking is unavailable Feature may be restored on next drive 20:55 | DI_a246 Automatic vehicle hold disabled Use brake pedal when stopping 20:55 | DI_a223 Traction control disabled Drive with caution 20:55 | APP_W264 Lane Departure Avoidance features unavailable Features may be restored on next drive 20:55 | DI_a222 Stability control disabled. I take my car to a mechanic and they show me that my lower control arms are damaged. Tesla is more than 2 weeks out for something that has caused my car to act completely unsafe and not normal.

I purchased my car on 3/23/26. Since delivery I have noticed that the car bounces at low speeds 20-30mph and the steering wheel vibrates at highway speeds 65 - 80mph. I have had the car to tesla service center 6 times since march. They have replaced all 4 tires, found and replaced a bent wheel (rim) on the passenger side. Then this last visit they replaced the passenger right wheel hub. They claim that when comparing it to the 1 other car available to road test my car was the same even though it still has the wheel vibration at highway speeds. they claim it falls within specs and is safe. I do not feel this is acceptable because they dont know what else to do. Now if I want them to check the car they say I must pay for any diagnosis and repairs if needed because they say they fixed the car. I do not feel safe driving at highway speeds as I am not sure what may happen which could endanger myself or passengers

my 2021 tesla model 3 required replacement of both front upper control arms due to premature wear. the vehicle developed suspension noise, and an inspection determined both upper control arm assemblies with ball joints needed replacement. the repair cost was $1,845, including a wheel alignment. i am aware of numerous similar owner reports involving premature front suspension component failures in tesla model 3 vehicles and am submitting this complaint because i believe this may represent a broader safety-related issue.

Whilst driving on highways which feature multiple speed limits - eg. 70 cars, 65 Trucks, 60 Buses. If full self driving more or when in adaptive cruise control - the car systems fail to recognize the restriction linked to the lower speed limits. As a result, you can be driving on Full Self Drive at 75mpm, the car sees a 65 followed by a 60 speed limit (neither applicable to cars), but the car brakes hard dropping the speed from 75 to 62 almost instantly and, on more than one occasion, a closely following car has almost collided with me. Until such time as the Tesla system can recognize the 'sub text' on a speed limit - the FSD and Adaptive cruise should not be certified. I also drive a BMW i4 and this reads all signs correctly, only applying those applicable to a car to the vehicle.

I was driving down Interstate 635 Lane Departure Assist on my 2021 Model 3 pulls me into the lane it senses im departing into when system is set to "assist" (if vehicle sense im straying into the left lane as an example, it will pull me further towards the left lane markings and requires an abnormal amount of effort into recentering the vehicle within the lane I am in). Vehicle has since been driven in "warning" setting since this event with no further issues.

The adaptive cruise control on Tesla vehicles is dangerous. The model in question is a 2021 Model 3 but its an issue with many model years. The cruise control tends to get confused and applies the brakes during normal driving leading to very unsafe conditions for the Tesla and anybody behind it. The issue can happen in any environment and from 1-2 times per drive. There are no warnings or indications that the car is about to apply the brakes. The car has not been inspected by a service center but is in good working condition.

The horn system on the new Tesla are not effective. Other drivers cannot hear The unit is facing the ground, and a high speed. Other drivers. Can I hear it specially when it’s raining. The use of the original point was effective and never had a problem on my previous last Tesla, or any other car for that matter. Please help before a serious. Ask her that happens.

I own a 2022 Tesla Model 3 that is just past the 5-year/60,000-mile warranty. I bought it used from Tesla inventory and picked up my vehicle on June 1st, 2026 with ~32,000 miles on the odometer. The front passenger safety restraint system alert (RCM2_a636) first appeared on June 5, 2026. On June 15 at 64,008 miles, Tesla Service confirmed the airbag alert and front passenger OCS issue. They found the occupant classification system module damaged and exposed, replaced it, and noted the passenger-side module shorting was also causing a driver-side error. Both cleared after the repair and test drive. Cost: $363.23. The same alert came back. On July 30 at 66,475 miles they found an open in the passenger seat cushion self-capacitance trace wire and the driver occupancy sensor voltage out of range. They replaced the front passenger seat cushion trim and the driver seat occupancy sensor. Alerts cleared and the self-driving button now worked from park again which never did prior showing the issue was never fixed the first time. Cost: $930.36. No crash or injury, but this is a real safety risk. The restraint system may not work as designed and airbags could fail to deploy properly. This exact RCM2_a636/OCS fault keeps happening to other Model 3 owners both before and after warranty on forums, and Tesla has service bulletins on the same parts, yet people still end up paying for the repairs who are out of warranty and also have their safety and life at risk. Please investigate this as this is a severe safety defect.

While operating the vehicle at low speed (~35 MPH reducing to a stop) on August 1, 2026, at approximately 10:38 AM under normal road and weather conditions, the vehicle experienced a catastrophic and simultaneous loss of primary braking capability and steering responsiveness. > Detailed Description of Failure: > * Brake Failure: Upon pressing the brake pedal, the vehicle failed to decelerate or stop adequately despite continuous application, nearly causing a collision. > * Steering Loss / Disconnection: Concurrently, while the steering wheel could physically be turned, it offered no directional control—turning the wheel had no effect on the vehicle's path, feeling as if the steering linkage or assist was entirely disconnected or disengaged. > Vehicle Warnings / Diagnostic Codes: > The vehicle's onboard display immediately registered critical active safety and chassis failures, specifically displaying: > * "ABS (anti-lock braking system) disabled - Drive with caution" > * "Automatic Emergency Braking is unavailable - Feature may be restored on next drive" > Dashcam video and system screen captures documenting the exact speed, timestamp, and error messages have been saved and are available upon request. U.S. registered Tesla operating in [XXX]. Full Self-Driving engaged. Dark, nighttime conditions. Vehicle was traveling toward low traffic zones near the edge of [XXX] when the system made a severe mistake. The car went off the main road. Driver was forced to take control off-road after the system lost control. Damage to the rims occurred. At the time of this incident, manual speed control was still available. The driver could cap the speed. That capability has since been removed by a software update. The current version is more dangerous. If the same incident happened today, the driver would have no way to limit speed or prevent the system from repeating the error. The defect is the progressive removal of driver control from the Full Self-Driving system. Earlier versions allowed manual speed caps. Current software does not. This vehicle is U.S. registered and runs U.S. software on American roads. The same defect that caused this off-road incident in Mexico now exists without the safety net of manual speed control. Tesla software update 2026.20.6.11 v14 Lite NA-2025.44 removed manual speed control for Full Self-Driving. The driver can no longer set a maximum speed. The vehicle chooses its own speed and it is frequently wrong and dangerous. Curves are the worst failure. The car enters curves too fast. It accelerates through curves. It does not slow to a safe speed before the curve begins. The driver has no way to cap the speed. The scroll wheel is dead. There is no override. Posted speed limits are ignored. The vehicle uses bad map data instead of reading visible signs. It will do 65 in a 55. It will not slow down when the sign says 55. The driver cannot force it to obey the posted limit. Speed control is gone. The driver is a passenger watching the car make bad decisions. This is not safe. This is not driver assistance. This is loss of control. NHTSA must require Tesla to return manual speed control immediately. A human driver must be able to cap the speed at all times. The current software removes that ability and creates a clear safety hazard.

When cruise control is engaged on a Tesla (all models to my knowledge) the Auto Wipers automatically come on and then randomly wipe. There is no option to disabled the Auto Wipers, as you can only disable by manually selecting control panel and then turn off each time you engage cruise control. This creates a significant risk to driver safety as I have had numerous instances of wipers randomly starting (without any rain) and smearing the windshield. This forces me to look away from the road and go through a manual disable every time I use cruise control. NOTE: this is a software default that cannot be removed

Passenger side view mirror intermittently gets stuck in the "reverse" position while in drive gear. I have the side view mirrors settings to where they are in a certain position when in reverse gear, and then a different position when in drive gear. I will reverse out of a parking spot and then proceed on my drive. Every now and then, the passenger side mirror will still be in the "reverse" position while driving and I only notice it when making a lane change and looking at the mirror and its pointed down at the ground in its reverse position. This has distracted me a few times as I expect the mirror to be in the normal drive position and I'm unable to see any cars in my blind spot. The issue has never occured on the driver side mirror, and has happened for the past few months. I have to correct this each time it does this by going into the mirror settings and hitting the reset position button.
