- #11755663August 8, 2026SERVICE BRAKES, UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: WARNINGS
My 2025 Hyundai Tucson required the front brake rotors to be resurfaced at approximately 12,200 miles. The Hyundai dealership diagnosed the rotors as overheated and warped, despite the brake pads being in good condition. The repair occurred before Hyundai issued Recall 302 (NHTSA Recall 26V-316). Hyundai Recall 302 identifies a defect in the Forward Collision-Avoidance (FCA) system’s front-camera software that can cause the FCA system to engage earlier than expected and potentially result in sudden braking. Hyundai’s FCA system is capable of automatically applying the brakes. I am concerned that premature or unintended brake application associated with this recalled software condition may have contributed to excessive brake heat and premature overheating/warping of my front rotors. I am NOT claiming that this causal relationship has been established. Rather, I am reporting a potential safety defect and asking NHTSA to evaluate whether there may be a relationship. The dealership confirmed that the rotors were overheated/warped but characterized the condition as wear and tear. Hyundai Motor America subsequently denied my reimbursement request on the same basis. I have repeatedly asked Hyundai to evaluate whether Recall 302 could have contributed to the brake condition, but Hyundai has not acknowledged or technically addressed this potential relationship. The original rotors are no longer available for inspection because they were resurfaced during the repair. The dealership performed and documented the diagnosis and repair. I am submitting this complaint because I believe the combination of premature rotor overheating/warping at only 12,200 miles and a subsequently identified FCA defect involving unexpected braking warrants investigation.

- #11755583August 7, 2026ELECTRICAL SYSTEM
Under Hyundai Recall 290, the dealership permanently removed the functional trailer light wiring harness and replaced it with a nonfunctional rubber dust cap. A Hyundai case manager informed me that a replacement harness is "in development," but official NHTSA remedy filings show the cap is the final remedy, resulting in the permanent loss of a factory feature I paid for. The removal of towing functionality without a clear timeline for a safe replacement parts remedy constitutes a failure to properly correct the vehicle defect.

The contact owns a 2025 Hyundai Tucson. The contact stated that while driving at various speeds, the Forward Collision Avoidance Assist (FCA) warning light was intermittently illuminated. The dealer was notified of the failure and attributed the issue to inclement weather. The contact stated that while stopped, the vehicle was rear-ended because the brake lights failed to illuminate. The contact sustained lower back and shoulder injuries, was transported by ambulance to the hospital, and required ongoing injection treatments. A Police report was filed. The vehicle was towed to a dealer and repaired for rear-end and passenger-side damages; however, the cause of the brake lights' failure was not determined. Additionally, the contact stated that while attempting to accelerate from a complete stop at a traffic light, the vehicle stalled and was struck on the front driver's side by another vehicle. The crash caused the vehicle to spin, and the front end detached. The air bags failed to deploy, and a system failure message was displayed. The contact sought medical attention the following day. The contact sustained a dislodged left thumb bone which required surgery, a cast, and injections for left arm pain, as well as shoulder, neck, hip, and aggravated lower back pain. A Police report was filed. The second failure had occurred at [XXX]. The vehicle was towed to the auto body shop at the dealer and repaired; however, neither the dealer nor the insurance adjuster determined the cause of the vehicle stalling. The contact associated the failure with an unknown recall notification. The manufacturer was not notified of the failures. The failure mileage was unknown. While driving at night, the headlights on my 2022 Hyundai Tucson Hybrid shut off randomly, including when using the turn signal. The lights will turn off without warning at highway speeds, creating a serious safety hazard. Using the turn signal can cause the headlights to turn off, and sometimes using the turn signal will temporarily turn them back on. The headlight mode also changes by itself while driving (AUTO to OFF to ON to OFF). Additionally, when the vehicle is turned off, the headlights often stay on no matter what mode the switch is in. I have to restart the vehicle and manipulate the turn signal/headlight stalk multiple times to get the lights to turn off. This is a recurring and dangerous failure that affects nighttime visibility and safety for me and my family. No other electrical issues are present. This is available for inspection. Safety for my family and all others on the road put at risk as the headlights randomly shut off at night. The problem was reproduced by the dealer. The dealer quoted a $1,500 repair, but this a safety‑critical defect in the lighting control system.

Forward collision Warning and Automatic Emergency Braking systems failed on 5/13/2026. Driving at a low rate of speed, approximately 15mph in a roundabout traffic circle with no other vehicles in front of the vehicle, sunny dry weather, 740am. Proceeded into traffic circle and FCA engaged and hard braking occurred for instantaneous stop and my vehicle was hit in the rear by a school bus. Air bags did not deploy. No recall on the vehicle at time of accident. Recall issued 6 days after accident for FCA. After contacting insurance company, Hyundai was contacted and made aware of the incident and safety concerns. The vehicle was towed to the dealership that evening and the next day I was advised that they could not check out the vehicle prior to having the collision repaired. The vehicle was then towed to a collision repair shop and released from there and towed back to the dealership on 6/19/2026. The vehicle had two safety recalls at that time and according to the service department both have been addressed. The vehicle is presently sitting the dealership (8/4/2026) waiting for Hyundai to investigate further. There were a few incidents prior to the collision that occurred randomly with the warning going off when no other vehicles or structures were close by. I took possession of this vehicle on 11/22/2025 and it had 2,431 odometer miles when this accident happened.

My client lost her right eye and suffered a brain injury when an aftermarket rhinestone emblem decal from her steering wheel shot into her face during a low-impact crash with a deer. The crash left minimal damage to the front end of the vehicle, but caused the airbags to deploy, shooting the decal into my client’s head. She purchased this dangerous decoration on Amazon, and despite NHTSA notices in 2023 and 2024 telling consumers to “avoid purchasing these steering wheel decals for all vehicle makes and models” and to “remove any such decals that they have already applied to their steering wheels,” this warning was not passed on to my client, even though records are kept of individual customer purchases, and these warnings could have been easily shared. A recent check of Amazon’s Safety Recalls and Alerts webpage did not turn up the 2024 NHTSA notice. I am sure there are other Amazon customers out there, who, like my client, have no idea just how dangerous these things are.

While driving the car stalled and then the engine shutdown, caused by a misfired fuel injector. Myself and the passengers in the car were minorly safe, as this incident occurred while driving through the Yellowstone National park; having to pull over on the side of the road with no shoulder. The Woody Smith Hyundai Dealership in Idaho Falls confirmed that the feul injectors had misfired, and then replaced them. Prior to the failure, there was no warning until the car was actively stalling and failing (battery light and check engine light came on).

Unintentional and unexpected activation of emergency collision or adaptive cruise control, causing the vehicle to stop and become immobile in the lane of traffic. I was driving down the interstate with the adaptive cruse control on and the lane keeping assistance on. These both were on for about 20 minutes as I was driving down the highway. There were no vehicles in front of or behind me for a quarter mile or so. I as all alone with nothing around me. All of a sudden it seemed like the collision avoidance system activated. The car came to a complete stop in the right lane of the highway. I tried to proceed and could see the emergency brake was on, the engine was running the car was in drive but it would not move. Flipped on the hazard flashers and traffic caught up to me while sitting in the right lane of the highway waiting to get rear ended. Vehicles were barely getting by at 70 MPH. The car would I pressed the accelerator and the car move d forward enough to get off the highway, this took a minute or two. Once off he highway, I could see the emergency brake would not release. I turned the engine off went out to inspect all the forward sensors. There was no visible issue. I restarted the car after a minute or so and it proceeded normally. That was Saturday, this is Sunday, I am taking the vehicle to Hundai tomorrow for inspection.

Our dashboard went blank and said “No Signal” over two weeks ago. (recall 304). We took it in for service. They did a Software upgrade which is the stated remedy. Two days ago it did it again. This time we had it towed to the dealership. They have had it since Thursday but no word back on a solution

On 7/29/2026, the vehicle experienced a critical safety failure involving the electronic transmission shift-by-wire system, resulting in an unintended rollaway after the ignition was turned off. While parked on an incline, the driver depressed the brake pedal and pressed the Engine Start/Stop button while the vehicle was still positioned in Drive (D). The engine shut off completely. According to factory safety design and vehicle specifications, cutting the ignition in this state is engineered to automatically trigger an electronic command to shift the transmission into Park (P). However, the vehicle failed to execute this electronic fail-safe. The electronic parking pawl did not engage, and the vehicle effectively defaulted into an un-geared Neutral state. Upon the driver releasing the brake pedal, the unanchored vehicle immediately rolled down the incline with the engine completely powered off. No audible warning chimes or dashboard fault lights were activated prior to or during the event to alert the driver that the transmission had failed to secure the vehicle.

- #11753589July 29, 2026FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS, ELECTRICAL SYSTEM
On July 24, 2026, while driving on a marked city street with no vehicle, pedestrian, or obstruction ahead, the Forward Collision-Avoidance system on our 2022 Hyundai Tucson Limited AWD applied emergency braking on its own, three to four separate times, each lasting one to two seconds. Traffic behind us nearly struck our vehicle. My family was in the car. This is the latest event in a progressive electrical fault history. In June 2025, under warranty at 48,037 miles, a Hyundai dealer documented recurring multi-system warning cascades (forward safety, lane keeping, high beam assist, speed limit warning) and a failed auto start/stop system. The dealer's listed resolution was "battery voltage / replace battery"; their invoice records that no repair was performed. We replaced the battery at our own expense. The faults continued. In June 2026, the start/stop system shut the engine off at a red light and the vehicle would not restart, leaving us disabled in a live traffic lane. Power was only restored by cycling the hood release. Since then, TCM and ECU software updates and a battery harness replacement (our expense, approx. $1,200) have been performed. The faults persisted after these repairs and have now escalated to uncommanded emergency braking. Date- and time-stamped video and photographs exist for multiple events. Two Hyundai dealerships have not identified a root cause. Similar phantom braking reports exist from other owners of this model year. We no longer consider the vehicle safe to drive.

- #11753506July 29, 2026FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
This is not the first situation NOR the first time it is being "fixed". I took my car in early March for an oil change and informed the representative about the same thing. The time that made me say something is when I was switching lanes and the car must have sensed me being close causing me to stop completely and the car behind me having to hard break and swerve. Rick Case Hyundai in Duluth, Georgia had my car for a MONTH, just for me to experience the same thing over 3 times since then. I received my car in April, and the recall for this wasn't put out until May. I am constantly scared for my life and the passengers I may have lives, as well as the other drivers lives, considering I got this car due to being in a rear end collision. When I took it in April, they CONSTANTLY, drove my car to diagnose it but kept telling me they didn't see the problem, and it wasn't coming up for them on their internal tracking system for their cars(webDCS). Once they finally talked to the CORPORATE field technician, they advised they replace the whole front bumper, including sensors, for the solution. But like stated before, since then I have experienced near crash stops AT LEAST 3 times since then, two of them being in the same month (July 16 and July 28). If this is the same solution, I don't see it being beneficial and I would like to be bought out at this point. Especially if I have to be without my car for a long period of time again.

First time it happened I was driving on the freeway from [XXX] to [XXX]. The car did an instant jerk stop ON THE FREEWAY!! I thought my granddaughter touched something but then it did it again when I was driving by myself. So I took the car in for the recall that came out for this issue and the day after that as I parked into an In-N-Out parking space my collision light came on warning me that there was a collision ahead. I was in a parking space so in that moment, I kind of let it go. Has done it a few times in local traffic. Fast forward to yesterday XXX It was way more serious. I was doing a U-turn and my car jerked. Stop the car behind me almost hit me and I was stopped in oncoming traffic. It released and I got to move so I didn’t wreck, but it scared me. I was very shaken. This is now happened a few times and I have already done the recall at this point I feel very unsafe uncomfortable driving the car and feel like I need a refund full refund. I am now afraid to drive it. It’s gonna kill one of us. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
