- #11774338October 29, 2026LANE DEPARTURE: WARNING, LANE DEPARTURE: ASSIST, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I was driving steadily on the freeway in slow moving traffic at 20-30 miles and had a lot of distance between me and the car in front. There was a sudden jolt and it appeared the vehicle was attempting to brake unexpectedly. If my foot had not been on the accelerator, I believe the vehicle could have come to a sudden, complete stop, potentially causing the vehicle behind me to rear end my car. Several months, before this incident, the Collision Mitigation Braking System and Lane Assist warning alerts would sound while I was driving on the freeway. This happened at two separate times. The sudden alarms are startling, and I cannot safely read the dashboard messages while driving. I am forced to slow down or pull over to check what is wrong, creating additional safety risks. I have a scheduled appointment with the Dealer this weekend due to the sudden braking however, I've brought this issue up with the service department in the past with the warning lights and they don't seem to have a definitive answer as to what is causing it. I have less than 20,000 miles on this vehicle.

The vehicle exhibits physical steering wheel freeplay/backlash at dead center, noticeable at all speeds but most critical during highway driving. While parked, the steering wheel can be moved slightly in both directions with zero corresponding movement from the front tires. This mechanical slack requires constant micro-adjustments on the highway to keep the vehicle tracking straight in its lane, creating a constant driver distraction and safety hazard."

- #11772874October 22, 2026SERVICE BRAKES, ELECTRICAL SYSTEM
I am reporting multiple recurring safety concerns involving my 2025 Honda Accord, including apparent electronic parking brake malfunctions, Auto Brake Hold failures, electrical system warnings, violent engine restarts, and a possible battery or charging-system defect. While stopped in traffic, particularly when the Auto Idle Stop system shuts off and restarts the engine, my vehicle sometimes experiences an unusually violent shaking or jerking motion. The restart can feel like a sudden, forceful kick. During these episodes, the electronic parking brake appears to engage unexpectedly without my manually activating it. Multiple dashboard warning lights and messages intermittently illuminate, including Brake System Problem, Brake Hold warnings, and other braking-related alerts. I have also experienced braking-related warnings or abnormal operation when traveling on inclines. The warning indicators sometimes disappear afterward, but the problems recur. I am concerned that these malfunctions could interfere with normal braking, acceleration, or vehicle control, particularly in traffic or on hills, creating a collision risk. Additionally, an AAA roadside assistance technician advised me that my vehicle’s battery appeared to be leaking fluid and needed replacement. A subsequent battery check at an auto parts store did not identify the same concern. I have repeatedly noticed a strong rotten-egg, sulfur, or sewage-like odor near the front of the vehicle and entering the passenger compartment through the air-conditioning vents. Another occupant independently noticed the foul odor. I am concerned about possible battery overheating, charging-system malfunction, electrical faults, and potentially hazardous fumes entering the cabin. The exact cause of these recurring problems has not been determined. I am requesting that these braking, electrical, engine restart, and ventilation-related safety concerns be investigated. The vehicle is available for inspection upon request.

In XXX, the driver assistance and safety systems on my 2025 Honda Accord SE began failing repeatedly. While driving, warning messages would appear for the collision mitigation braking system, lane keeping assistance, adaptive cruise control, and automatic high beams, and these systems would become unavailable. This happened multiple times over XXX and XXX . I took the vehicle to Hugh White Honda in [XXX], four to five times for this problem. The dealer replaced the forward-facing camera twice under warranty, but the warnings and system failures continued after each repair. The dealer was never able to identify or fix the underlying cause. I filed a complaint with American Honda on XXX (case XXX). Because the safety systems on a nearly new vehicle could not be repaired, I traded the vehicle in on XXX, with approximately 17,000 miles. No crash or injuries occurred, but the vehicle repeatedly lost the use of its collision avoidance and lane keeping safety systems in normal driving, and the manufacturer could not repair the defect. My 2025 Honda Accord Hybrid Touring has an unresolved tire repair kit safety recall, NHTSA 26V-366, Honda campaign DOV. I contacted Bay Ridge Honda in [XXX], on XXX, to request a recall repair appointment. I was told the necessary parts were unavailable. I contacted the dealership again on XXX, and received the same response. As of XXX, 62 days have passed since my first request, and the recall remains incomplete. NHTSA’s recall information states that repair parts are available in limited supply, but I have been unable to obtain the repair. I have since relocated to [XXX], and need American Honda to arrange the free recall repair through an authorized Honda dealership near my current residence. I am concerned about the safety of the unrepaired tire repair kit and the continued delay. I request that NHTSA review the delay and that American Honda provide a written repair timeline, help locate the correct replacement parts, and confirm any complimentary tire-related roadside assistance available while I await repair. I was at a stoplight waiting to turn left. I got the green arrow light so I proceeded to follow the vehicle in front of me who also turned left, I was not following close to the vehicle and the vehicle in front of me did not brake. Then my cars automatic emergency brake engaged in the middle of the intersection while tuening left and casued the cars behind to crash and my back to be injured. I am very upset as this has happened on 2 other occasions where I did not track the mileage or insections. I called the dealer where I purchased the vehicle, Mungenast Honda in St Louis MO. They have yet to schedule me a service appointment, but I did ask for one.

- #11768365October 1, 2026LANE DEPARTURE: WARNING
Here is a condensed version that includes the engineer and should fit within a **2,000-character limit**: The driver-assistance, lane-mitigation, and collision-avoidance systems in my 2025 Honda Accord have malfunctioned. Warning lights and messages have appeared while driving. I have photographs of the warning lights, service records, and documentation of the dates and times the problems occurred. The most serious incident occurred in approximately mid-June 2026. The vehicle suddenly stopped because the system apparently believed another vehicle was turning in front of me. The other vehicle was properly traveling in its own lane and was not entering my path. Had someone been driving closely behind me, the unnecessary stop could have caused a serious rear-end collision. I feared for my safety and my life while driving this vehicle. I left the vehicle with a Honda dealership on multiple occasions for inspection and service. Honda also sent an engineer from California to examine the vehicle. Despite these inspections, the problem was not satisfactorily resolved. Earlier in 2026, I contacted American Honda and opened a formal case regarding these safety concerns. I requested that Honda repurchase the vehicle because I no longer felt safe driving it. Honda denied my buyback request and stated that it was within its rights not to repurchase the vehicle. I have documentation of the case, my communications with American Honda, my buyback request, and Honda’s denial. I purchased the vehicle brand new. The vehicle is available for inspection upon request. I am requesting an investigation because unexpected braking or stopping could cause a serious accident, injury, or death.

On August 29, 2026, I took my 2025 Honda Accord to Norris Honda for a persistent rotten-egg/sulfur-type odor. I paid $407.67 for a filter replacement and odor treatment, but the odor persisted. I returned the vehicle on September 9, 2026, at 5,640 miles. The odor was particularly noticeable while driving with the air conditioning operating. My adult daughter independently noticed the odor. There were no warning lamps or dashboard messages. On September 15, a Norris Honda technician reported detecting a different “moldy odor” when opening the vehicle before it was started. Norris Honda subsequently reported that the right sunroof drain was clogged and had overflowed, allowing water into the passenger compartment. The dealer stated that the carpet requires replacement and later reported mold in a “pan.” The vehicle and affected components remain at Norris Honda and are available for inspection. I am concerned about safety because water entered the passenger compartment and the full extent of moisture exposure has not yet been established. As of September 28, I was told the affected/moldy carpet was still in the vehicle and had NOT been removed to inspect underneath it. Therefore, whether water or moisture reached wiring, electrical connectors/modules, insulation, seat-related components, or other systems is UNKNOWN. The water intrusion and affected carpet have been confirmed by Norris Honda. However, the exact cause of the drain obstruction, why it occurred, the duration and full extent of the water intrusion, and whether the original sulfur odor is related to the subsequently identified water intrusion/mold condition remain UNKNOWN. The vehicle has remained continuously at Norris Honda since September 9. To my knowledge, it has not yet been inspected by an American Honda manufacturer representative, police, insurance representative, or independent service center. I have requested photographs and a complete inspection beneath the carpet before affected materials are r

This incidence occurred twice, one year apart, under the same driving and weather conditions and on exactly the same route. On XXX, after departing [XXX], we were driving north on [XXX] near [XXX], with the adaptive cruise control set to 70 mph. Suddenly the dashboard displayed the warning, “Some driver assist systems cannot operate: Clear area shown,” and the Honda emblem indicator illuminated. Following the vehicle’s instructions, we stopped and cleaned the Honda emblem by wiping and rinsing it, but the warning remained. After we continued driving north on [XXX] for approximately three to four hours, the warning cleared without further action. The weather was clear and sunny. An identical incident occurred one year earlier, on XXX, along the same section of northbound [XXX] and under the same driving and weather conditions. Accord was also set to 70 mph with adaptive cruise control engaged. That warning again cleared on its own after a few hours. In addition, while driving in the left lane on XXX with adaptive cruise control engaged, Accord braked aggressively while passing semi-trucks traveling in the right lane, even though there were no vehicles ahead. This happened several times. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11765100September 17, 2026ELECTRICAL SYSTEM, STEERING, SERVICE BRAKES
On August 23, 2026, the electronic power steering and braking systems on a 2025 Honda Accord Hybrid completely failed and locked up simultaneously while traveling at highway speeds. This caused an immediate and severe safety risk, resulting in a near-collision on a busy freeway with three occupants inside the vehicle. The vehicle is currently available for inspection upon request. Prior to and during the failure, widespread dashboard warning lights illuminated simultaneously, including the Collision Mitigation Braking System, Lane Keeping Assist System, Road Departure Mitigation, and Low Speed Braking Control. Additionally, 11 malfunction alerts were triggered via the HondaLink app. The vehicle was inspected by an authorized dealer from August 24 to September 11, 2026. As documented in the attached Honda dealer service report, technicians initially noted low voltage codes causing multiple system failures. However, instead of identifying and repairing the root cause of the voltage drop, the dealer merely cleared the codes, test-drove the vehicle, and closed the repair order stating they "could not duplicate concern." Furthermore, the attached text message conversation with the service advisor proves the service department explicitly admitted in writing that the software is up to date and that the failure could happen again if the vehicle encounters the same error. The vehicle has not been inspected by a manufacturer field engineer, police, or insurance representatives. American Honda Corporate refused requests to dispatch a field engineer and closed the case file. Note: Full video evidence of the dashboard warnings and loss of vehicle control during the incident exists, but cannot be attached here due to file format limitations. The manufacturer and dealer have returned this vehicle without identifying or repairing the underlying defect, leaving it highly susceptible to another catastrophic loss of steering and braking at highway speeds.

- #11764468September 15, 2026SERVICE BRAKES, AIR BAGS
The contact's son owns a 2025 Honda Accord. The owner stated that while coming to a stop, the brake pedal was depressed, but the vehicle failed to stop. No warning light was illuminated. The owner fully lifted the foot from the brake pedal and forcefully depressed the brake pedal while attempting to stop the vehicle. The vehicle failed to stop, causing the contact's son to rear-end a semi-truck. The contact's son stated that the vehicle powered off after the crash. The contact's son also stated that the air bags did not deploy during the crash. The vehicle was taken to the dealer, but the failure was not diagnosed or repaired. The dealer informed the contact that the failure could be duplicated. The manufacturer was made aware of the failure by telephone, and the contact was corresponding with a case manager via email. A case was opened. The failure mileage was approximately 14,000.

Recurring steering/front-end clicking/tapping noise occurs when turning the steering wheel left or right, including while the vehicle is stationary and at low speed. I first noticed the steering clicking noise at approximately 1,500 miles but initially overlooked it. The issue was previously confirmed by an authorized Honda dealer. In September 2025, the technician documented that the noise was easily duplicated while driving or stationary, used diagnostic equipment to locate it, identified the clicking/tapping as coming from the EPS steering rack assembly, and replaced the EPS rack under warranty. A later service record stated that the steering clicking problem was fixed. The same/similar clicking noise later returned. In August 2026, the dealer again documented the complaint but stated that the noise was a “normal characteristic” of the model/year rather than identifying and permanently correcting the cause. I have spent many personal hours testing, documenting, and comparing the vehicle with other similar Honda Accords. I obtained multiple comparison videos showing comparable vehicles tested under similar stationary steering conditions that do not produce the repeated clicking noise present in my vehicle. I provided this evidence and the service history to American Honda Corporate. American Honda also published a March 20, 2026 Tech Line Summary, “Clicking Sound While Turning the Steering Wheel,” applicable to 2023 and later Accord models, describing a clicking/light ticking/binding condition involving the steering gearbox/rack area. Because this recurring condition involves the steering system, I am concerned about safe vehicle operation if the underlying cause is not properly identified and repaired. My family is uncomfortable riding in the vehicle. The condition remains unresolved, and the vehicle is available for inspection. Supporting service records and additional documentation are being uploaded.
